Pancreatitis aguda secundaria a hipertrigliceridemia: presentación de dos casos clínicos

Abstract: the acute pancreatitis is a reversible inflammatory process. hypertriglyceridemia as etiology of the acute pancreatitis reaches frequencies you enter 1.3 11% according to literature when the triglycerides levels of reach values over 1000 mg/dl; nevertheless hypertriglyceridemia is observed in 12 39% of the acute pancreatitis like factor associate. the objective of the medical treatment is to increase the activity of lipoproteinlipasa and to increase the degradation of chylomicrones; diminishing therefore the serum triglycerides values of at levels smaller to 500 even to less of 200mg/dl if it is possible with different strategies, among them the insulin. in the present article, we presented/displayed two clinical cases of severe pancreatitis induced by hypertriglyceridemia, handled with therapy of insulin infusion with suitable evolution and clinical answer given by significant diminution of the levels of triglycerides, 48 hours post treatment.

Abstract: La pancreatitis aguda es un proceso inflamatorio reversible. La hipertrigliceridemia como etiología de la pancreatitis aguda varía entre 1,3 a 11% de acuerdo a la literatura cuando los niveles de triglicéridos alcanzan valores por encima de 1000 mg/dl; sin embargo, la hipertrigliceridemia se observa en un 12 a 39% de las pancreatitis agudas como factor asociado. El objetivo del tratamiento médico es aumentar la actividad de la lipoproteinlipasa y aumentar la degradación de los quilomicrones; disminuyendo así los valores plasmáticos de triglicéridos a niveles menores a 500 mg/dl incluso a menos de 200 mg/dl si es posible con diferentes estrategias, entre ellas la insulina. En el presente artículo, presentamos dos casos clínicos de pancreatitis severas inducidas por hipertrigliceridemia, manejadas con terapia de infusión de insulina con adecuada respuesta clínica y paraclínica con disminución significativa de los niveles de triglicéridos, 48 horas postratamiento. Prevalencia de sensibilización a látex en personal de pabellones quirúrgicos del Hospital Clínico de la Universidad de Chile

Abstract: background: health care workers (hcw) are a high risk group for developing natural rubber latex (nrl) hypersensitivity and allergy. some studies showed a correlation between time and frequency of exposure to nrl gloves and hypersensitivity, but a recent meta-analysis showed no clear evidences for such assumption. aim: to determine the prevalence of nrl hypersensitivity and allergy in a group of hcw at the university of chile clinical hospital. materials and methods: ninety five hcw (aged 37±10 years, 59 females) were interviewed about time of exposure, atopic diseases and latex-related allergy symptoms. different nrl extracts and seven nrl gloves brands were tested by the prick test method. results: twenty four workers (25%, 95% ci = 16.9%-35.2%) were sensitized. no gender differences were found. no symptomatic cases were found in the sensitized group. in the workplace, six and two non sensitized subjects had respiratory symptoms or contact urticaria, respectively. sensitivity to bananas, avocadoes, kiwi and chestnut was not significantly more common among latex sensitive individuals. no differences between sensitized and non sensitized subjects were observed for the presence of atopic diseases or the mean number of years at the workplace. the sensitization rate to nrl increased along with years of work, from 18.6% in patients working less than 10 years, to 28.1% in the group between 10 and 20 years and to 35% in those working more than 21 years (p=0.693). sensitization was also related to the weekly hours of exposure: 1-10 hours, 17%; 11-20 hours, 23.5%; 21-30 hours, 50% and 31-40 hours, 28.5% (p=0.036). conclusions: sensitization increased proportionally to the years and weekly hours wearing nlr gloves. we did not find symptomatic patients in the sensitized group

Abstract: since 2003 the school of medicine at the universidad del rosario in bogotá, colombia, offers to the students a program named peer tutors. the program's objectives are: to generate a social interaction mechanism that favors knowledge construction side by side with advanced peers; to promote the development of ethical, moral and coexistence oriented values through an academic experience, and to generate a space to explore specific academic interests and teacher′s potential. this article presents the theoretic frame that supports the importance of social interaction in knowledge construction, as well as some indicators that allow a first appraisal of the program. several achievements that account for the synergic value of an experience that not only fulfills the function of initiating a process of teaching formation and academic support, but that also builds a proactive attitude before learning, are highlighted.
Los Jóvenes de Familias Tareferas de los Barrios Periurbanos: ?La Generación del Cambio?

Abstract: from the mid-90's in the province of misiones there was generated a process of expulsion of agricultural workers living in the countryside. this population was concentrated in the periurban areas of intermediate cities in the province, and today they depend on the temporary work in the yerba mate harvest ?tarefa- and occasional jobs in town. as part of these processes, young people are the first generation who was socialized in such territories. this study aims to understand and explain the young people contributions to the family reproduction strategies, in the case of families from the suburban areas of the oberá city, whose members work as yerba mate harvesters. in this regard, we observe substantial transformations between current contributions and those young people made when they resided in the country side -generation of their fathers and mothers-, which vary by gender and family types. to assess these processes, we adopt a triangular approach, combining data from a inquest of peri-urban households in oberá and qualitative information from ethnographic fieldworks.

Abstract: En este artículo presento un análisis de los procesos de constitución del self e jóvenes de familias de cosecheros de yerba mate -tareferos y tareferas- que residen en los barrios periurbanos de la provincia de Misiones (Noreste de Argentina), indagando sobre las dimensiones corporal, situacional y emocional. Reflexiono particularmente sobre los procesos de objetivación de identidades juveniles, considerando el lugar protagónico que tiene la emoción del sufrimiento en su conformación. Al respecto, observo que los sujetos jóvenes trazan diversas estrategias ligadas a la resistencia por asumir la identidad tarefera sufrida de sus padres y madres. Tales hallazgos demuestran el lugar protagónico de la emoción corporizada en la objetivación de ciertas relaciones sociales. Para evaluar estas cuestiones, adopto una metodología etnográfica.

Abstract: La provisión de los servicios de acueducto y alcantarillado en Colombia ha sido sometida, desde 1994, a una terapia consistente en la promoción de un modelo empresarial que se abre paso a costa de la estigmatización del municipio como mal prestador y como asignador ineficiente de unos recursos escasos. a quince a os de operación del modelo empresarial y en desarrollo del Acto Legislativo 04 de 2007, el Gobierno nacional ha introducido otra reforma, en esta oportunidad al Sistema General de Participaciones, esgrimiendo en la exposición de motivos los mismos argumentos de anta o, cuando en verdad mediante la prestación directa de los municipios sólo se provee de estos servicios a una porción minoritaria de la población urbana del país. En la primera parte se discute la noción del agua potable como bien mayor y en la segunda se presenta una breve síntesis de la evolución institucional de la provisión del agua potable en colombia. en la tercera parte se discuten algunos rasgos centrales del proceso de urbanización de la población colombiana a la que se sirve con el servicio de acueducto mientras que en la última se analiza la coyuntura reciente con énfasis en la crítica a la reforma introducida por el acto Legislativo 04 de 2007. este artículo propone otros puntos de vista para criticar, además de la reforma en curso, las fallas de un esquema de provisión cifrado en un enfoque empresarial sometido a una inocua regulación, que no logra conciliar los intereses de ampliación de las ganancias de las empresas de servicios públicos con las aspiraciones sociales en materia de universalización y calidad de los servicios de acueducto y alcantarillado.
Factores asociados al rendimiento académico en estudiantes de medicina

Abstract: objectives: to determine and evaluate factors that affect academic performance in first semester medicine students. methodology: the population was characterized in a search for factors that would later be studied in order to determine statistical association and prediction of final academic performance, using a logistic regression model. results: eighty students, aged between 17 and 18, were analysed. the majority were women, from bogotá, from mixed (i.e. not single-sex), private and monolingual schools. the group was homogeneous in terms of demographic, social and cultural factors, and of schooling and motivation. features of interfamilial violence, alcohol and cigarette consumption (but not psychoactive drug use) were detected. the results obtained in the differential and general learning aptitude tests (badygs) were low. factors associated with poor academic performance were: no interest in reading as a pastime; interfamilial violence; a history of smoking marijuana; mixed educational background; no history of premedical studies; economic dependence on one of the parents. grades in biology and biochemistry and the quarterly average grades were the best predictors. the best predictor of poor academic performance when controlled by other factors included in the model was the quarterly average, and the best predictor of failing the academic semester was the biochemistry laboratory grade. conclusions: although prior factors may explain academic performance before admission to medical school, it is important to evaluate the performance during the first semester to identify cases in which rapid intervention is required in order to avoid academic failure.
Factores asociados al rendimiento académico en estudiantes de medicina Factors associated with academic performance in medical students

Abstract: Objetivos: Determinar y evaluar los factores que estuvieran incidiendo en el desempe o académico de nuestros estudiantes de primer semestre de medicina. Metodología: Se caracterizó la población en búsqueda de factores que se analizaron posteriormente para determinar asociación y predicción, a través de un modelo de regresión logística, del rendimiento académico final. Resultados: Se analizaron 80 estudiantes de edades comprendidas entre 17 y 18, la mayoría mujeres, procedentes de Bogotá, de colegios mixtos, privados y monolingües. El grupo fue homogéneo por factores sociodemográficos, culturales, escolaridad y de motivaciones. Se detectaron rasgos de violencia intrafamiliar, de consumo de alcohol y cigarrillo pero no de drogas psicoactivas. Los resultados obtenidos en la prueba de aptitudes diferenciales y generales (BADyGs) del aprendizaje fueron bajos. El análisis mostró que el no leer como pasatiempo, la presencia de violencia intrafamiliar, el haber fumado marihuana, el provenir de un colegio mixto, el no haber realizado estudios profesionales, de quien se depende económicamente, las notas de biología, bioquímica y del promedio trimestral fueron los factores que se asocian con fracaso académico o pérdida de cupo. La variable que predice fracaso académico cuando se controla por los otros factores incluidos en el modelo es el promedio trimestral y la que determina pérdida de cupo es la nota del laboratorio de bioquímica. Conclusiones: Aunque existen factores previos al ingreso que puedan explicar el desempe o académico, es importante evaluar el rendimiento durante el semestre para intervenir rápidamente y evitar el fracaso. Abstract: the consumption of remittances can be related to socio-demographic structures that reveal social vulnerability such as high rates of demographic and economic dependency. this article describes the socio-demographic and economic characteristics of households located in layers two and three of cali (middle low-income), according to the destination that give the remittances. the results show that poorer households are usually characterized by the use of remittances in basic needs like food, house rent and utility payments. consumption on health and education are related with high rates of childhood and aging. households that spend the remittance for investment and leisure finance the basic needs from their own income
