Invaginación intestinal recurrente secundaria a apéndice cecal invaginado: Caso clínico Recurrent intussusception secondary to invaginated cecal appendix: Case report

Abstract: Introducción: La invaginación intestinal es una patología de causa idiopática en el 95% de los casos, encontrándose en el 5% restante algún factor desencadenante que la justifique. Las recurrencias se presentan en un 7 a 12% de los casos y se asocian a múltiples causas. El objetivo de este trabajo es presentar el caso de un paciente con invaginación intestinal recurrente y analizar la causa de la recurrencia. Caso clínico: Paciente de 1 a o y 4 meses de edad, sexo masculino, con antecedente de primer episodio de Invaginación intestinal operado a los 4 meses de edad. Presenta segundo episodio de invaginación a los 9 meses de edad, la cual se trató con desinvaginación neumática y a los 16 meses de edad presentó su tercer episodio, manejandose con desinvaginación hidrostática. Ocho horas post procedimiento el paciente presentó nueva invaginación; se realizó reducción manual quirúrgica. Se encontró una invaginación ileocólica con un remanente apendicular intracecal actuando como cabeza invaginante. Se reseco remanente y se envió a biopsia. Conclusiones: La invaginación intestinal recurrente se asocia a factores anatómicos desencadenantes, por lo cual al presentar recurrencias se debe tener en consideración una posible causa orgánica, ante lo cual se recomienda el tratamiento quirúrgico. Introduction: Intussusception is an idiopathic pathology that accounts for 95% of cases found; the remaining 5% is caused by some triggering events. Recurrences occur in 7 to 12% of cases and are associated with multiple causes. The aim of this research is to present the case of a patient with recurrent intussusception and analyze the cause of the recurrence. Case report: The patient was a 16 month-old male infant with a history of first episode of intussusception surgery at 4 months old. His second intussusception episode took place at 9 months of age, which was treated with pneumatic reduction process; at 16 months of age he presented his third episode, which was treated with hydrostatic reduction process. 8 hours post procedure, the patient had a new intussusception condition and surgical manual reduction was performed. Ileocolic intussusception was found with an appendiceal intrathecal remnant acting as the invaginating head. Remnant was removed and sent for biopsy. Conclusions: Recurrent intussusception is associated with triggering anatomical factors; therefore, in the case of recurrences, a possible organic cause should be considered and surgical treatment is recommended.

Abstract: Antecedentes: En la etiopatogenia de las várices ha cobrado importancia la teoría parietal que relaciona fibras de elastina y colágeno de la pared venosa en el desarrollo de esta patología. Objetivos: Cuantificar la expresión de fibras de elastina, colágeno tipo I y tipo III en la pared de Vena Safena Magna varicosa y no varicosa Métodos: Estudio de casos y controles, participaron 21 pacientes por cada grupo. Intervalo de confianza 95%; Poder estadístico 80%; participaron sujetos de ambos sexos, mayores de 30 a os, safenectomizados por enfermedad varicosa y no varicosa en establecimientos de salud públicos y privados de la Provincia de Cautín; Consentimiento informado, se excluyeron aquellos que tuviesen patologías locales y/o sistémicas que pudiesen alterar el resultado de las mediciones. Analisis de imágenes: Digitalización y cuantificación de densidades colorimétricas (Program Metamorph Software); Estadístico: Medidas de tendencia central; T- test; Test de Fisher; Regresión lineal múltiple; Cálculo de Odds Ratio. Resultados: Fibras Elastina: Casos 3,1±1,99 (Unidades colorimétricas); Controles 5,1±3,2; p=0,018. Colágeno Tipo I: Casos 6,9±13,10; Controles 25,3±25,82; p=0,0048. Colágeno Tipo III: Casos 19,7±10,92; Controles 26,7±14,55; p <0,0001. Conclusión Existe una disminución en la expresión de fibras de elastina, de Colágeno Tipo I y Tipo III en las paredes de Vena Safena Magna de pacientes varicosos.

Abstract: this work is a reflection about the construction of imaginaries that took form along the xix century in the south american countries: chile, argentina, uruguay, and brazil. these countries intended to install a modern and attractive appearance consistent with the so-called "civilized world." we try to establish an understandable frame for posing again different issues of our common history; in this case, the creation of an imaginary of prosperous countries, so powerful as to hide other possible imaginaries.

Abstract: El cómo abordar el peso de lo étnico y/o cultural dentro de un proceso de mediación, es sin duda una deuda del actual dise o de la mediación familiar en Chile. No existe oferta pública que aborde estos casos de una forma pertinente, ni existe por parte del Ministerio de Justicia, en las recientes bases de licitación para la contratación de servicios de mediación familiar, consideración alguna en cuanto a exigir a los mediadores familiares que viven en las regiones con mayor población indígena, conocimientos específicos en torno al tema. Lo anterior es preocupante si consideramos que existe una obligación estatal en dicho sentido, conforme a lo dispuesto en el Convenio 169 de la Organización Internacional del Trabajo sobre pueblos indígenas y tribales en países independientes, ratificado por Chile el 15 de septiembre de 2008. El presente trabajo propone instalar la temática en el debate académico, reflexionando acerca de su necesidad y analizando formas de intervención por parte de los mediadores y mediadoras familiares frente a casos en que una o ambas partes pertenezca a un pueblo originario y en especial al pueblo mapuche.

Abstract: El artículo se centra en una pieza fundamental del entramado teórico de la filosofía de laciencia de Thomas Kuhn, el conocimiento tácito. La importancia de un tipo de conocimiento que no esposible (ni necesario) reducir a reglas ni definiciones y que no es adquirido con recurso exclusivo a lalógica es esencial para Kuhn dentro de la Ciencia. El artículo indaga en los orígenes de esta idea en elpensamiento kuhniano, rastreando las influencias ejercidas en este aspecto por la psicología de la Gestalty el segundo Wittgenstein. Examina la relación de esta idea con los aportes filosóficos de Kuhn másconocidos y divulgados, a saber, el concepto de paradigma (ejemplar) y la caracterización de las fases dedesarrollo científico conocidas como ciencia normal y revolución científica; confirmando que sin lautilización de un tipo de conocimiento de esta naturaleza dichas ideas no hubiesen tenido el fundamentonecesario para ser enunciadas. Para finalizar, se revisa la importancia del conocimiento tácito a través deluso de la metáfora y de los experimentos imaginarios en la ciencia, como vehículos mediante el cual elcientífico puede ponerse en contacto con esta dimensión de su conocimiento. The article focuses on a fundamental piece of the theoretical framework where the work ofThomas Kuhn develops, tacit knowledge. The importance of such a kind of knowledge, that is notpossible (nor necessary) to reduce into rules and definitions, and that is not acquired with exclusiverecourse to logic is essential for Kuhn within Science. The article investigates the origins of this idea inkuhnian work, searching for influences exerted by Gestalt psychology and the second Wittgenstein. Itexamines the relation between this idea and Kuhn’s most known and important philosophicalcontributions, the concept of paradigm (exemplar) and the incommensurability of scientific theories;confirming that without the utilization of a kind of knowledge of this nature such ideas would not havehad the necessary support to be stated. At the end of the article, the importance of tacit knowledgethrough the use of metaphor and thought experiments is revised, as vehicles through which the scientistcan put himself in touch with this dimension of knowledge.
The pass-through from depreciation to inflation: Chile 1986-2001

Abstract: Utilizando un modelo microfundado de competencia oligopolística a la Cournot, se deriva una relación explícita entre el nivel de precios agregados y el tipo de cambio nominal. De esta forma se obtiene un precio cuyo margen sobre el costo marginal varía en forma endógena con el ingreso real de los consumidores, abandonando los modelos ad hoc usuales en la literatura de traspaso. Usando este marco teórico el coeficiente de traspaso estimado, esto es, el impacto de la devaluación cambiaria en la inflación doméstica, alcanza entre 9% y 11% en el corto plazo, y entre 21% y 32% en el largo plazo para Chile en el período 1986-2001. Contrario a la creencia generalizada no se encontró evidencia de un traspaso procíclico. Los datos apoyan la existencia de un cambio estructural en 1991, después del cual los coeficientes de traspaso se reducen significativamente desde valores cercanos al 20% hasta estimaciones que se ubican en el tramo entre 5% y 7%.

Abstract: We hope that PDIdb will be useful to many researchers working in fields such as the prediction of transcription factor binding sites in DNA, the study of specificity determinants that mediate enzyme recognition events, engineering and design of new DNA binding proteins with distinct binding specificity and affinity, among others. Finally, due to its friendly and easy-to-use web interface, we hope that PDIdb will also serve educational and teaching purposes.The ability of some proteins to bind selectively to DNA constitutes the basis of key cell processes such as RNA transcription, DNA packing, DNA replication, DNA recombination and DNA repair. The understanding of the molecular recognition process that mediates the specific protein-DNA binding selectivity is one of the most interesting challenges in structural biology. To date, there are several hundreds of protein-DNA complexes that have been solved by X-ray crystallography. These experimental structures, deposited at the Protein Data Bank (PDB) [1] and publicly available to the scientific community, constitute a rich source of information to study the different binding modes and the determinants of protein-DNA binding specificity.To facilitate the investigation about the mechanisms involved in the protein-DNA recognition process, several databases of protein-DNA complexes and associated software have been developed (Additional file 1). Among these resources we find AANT [2], which has statistical information on aminoacid-nucleotide interactions; ProNuC [3], a database that provides a list of atomic contact pairs between proteins and DNA; ProNIT [4], which gathers experimental binding data of protein-nucleic acid complexes that have been described in the literature; NPIDB [5], a database that contains a description of hydrogen bonds and hydrophobic interactions between proteins and nucleic acids; BIPA [6], a database containing several physicochemical features of protein-nucleic acid interfaces and multiple structu
Variabilidad y estructura genética en dos poblaciones de Vicugna vicugna (Camelidae) del norte de Chile

Abstract: two populations of vicugna vicugna from regions first and second of chile were studied on the basis of the electrophoretic determination of 28 presumptive loci. due to the phenotypic differences between the populations mentioned above, this kind of study is required to determine some parameters like taxonomic status, genetic variability and population structure in order to help in conservation management of the specie. the percentage of polymorphism and the level of heterozigosity of vicu?as from the first region were 17.8 % and 0.078, respectively. the corresponding figures of vicu?as from the second were 14.3 % and 0.045, respectively. these two parameters show that the two populations have a high degree of genetic variability. high level of demic substructuring (fst = 0,324) and significant degree of genic and genotypic differentiation between populations was found. genetic drift and polygamy mating system are suggested like promoters of the observed genetic differences. the sub specific status suggested by morphological differences was not confirmed by the genetic distance value (0.097).

Abstract: RESUMEN Se estudiaron dos poblaciones de Vicugna vicugna pertenecientes a la Primera y Segunda regiones del país, en base a la determinación electroforética de 28 loci presumtivos. Las diferencias fenotípicas existentes entre las vicu as de estas poblaciones hace necesario este trabajo con el fin de re-estudiar su posición taxonómica y obtener antecedentes de variabilidad genética y estructura poblacional que podrían resultar útiles en su manejo de conservación. En la población de vicu as de la Primera Región se detectó un polimorfismo de 17,8 % y un nivel de heterocigosidad de 0,078. En aquellas de la Segunda Región, el polimorfismo y la heterocigosidad se estimaron en 14,3 % y 0,045, respectivamente. Ambos parámetros revelan un alto grado de variabilidad genética poblacional. Se encontró un nivel de subestructuración démica alto (F ST = 0,344) y un grado de diferenciación génico y genotípico significativo entre las poblaciones. Se infiere que la deriva genética y el sistema social poligámico tendrían un rol importante como promotores de las diferencias genéticas observadas. El valor de distancia génica (D = 0,097) no confirmó el estatus subespecífico atribuido en base a sus diferencias morfológicas. Boron dynamics related to fruit growth and seed production in kiwifruit (Actinidia deliciosa, cv. Hayward)

Abstract: the effect of foliar boron treatments on fruit growth and seed development in actinidia deliciosa, cv. hayward, was studied. the distal leaves (bh) or fowers (bf) of a shoot were sprayed with 500 mg l-1 boric acid during the fowering period. the mobility of boron from the leaves to the fowers through the phloem was determined by spraying only the leaves. from 0 to 96 h after treatment, leaves and fowers were sampled and total boron was assessed. at harvest time, fruit weight and diameter plus seed weight and number were measured for the different treatments. the weight of fruits growing from shoots with boron-treated leaves were 14.1% higher than the control, while the weight of fruits derived from boron-treated fowers was 17% higher than that of fruits from untreated fowers. regarding fruit length, signifcant differences between boron treatments and the control were observed, with an 8.2% and 8.5% increase for bh and bf treatments, respectively. no signifcant differences were found in fruit diameter. fruits grown from shoots with boron-treated leaves had 43% more seeds than fruit grown from untreated control shoots, while fruits resulting from boron-treated fowers had 44% more seeds. this demonstrates the positive effect of boron treatment in seed production. no differences were observed in seed weight between treatments. the correlation coeffcient between seed number and fruit weight was r2 = 0.1294 for bf, r2 = 0.1298 for bh and r2 =0.0002 for the control. there was no variation in non-treated leaves and fowers with respect to boron concentration for a 96 h time period. on the contrary, in sprayed leaves boron levels reached up to 16 mg kg-1 between 24 and 48 h and slightly decreased after 96 h, demonstrating the boron absorption capacity of leaves. in the fowers from the shoots with boron-treated leaves, there was a signifcant 14 mg kg-1 increase in boron concentration from 24 to 96 h, showing boron mobility from leaves to fowers.
