Efecto de la temperatura del ba?o de hemodiálisis en diabéticos

Abstract: during the dialysis procedure, arterial hypotension is one of the most common problems and it has been object of many studies. in hemodialysis, changes are produced in body volume through ultrafiltration that generate an increase in the production of thermic energy, which is removed during the treatment. the hypovolemia resulting from the removal of volume activates the sympathetic system, avoiding in this way heat loss and increasing body temperature that promotes vascular vasodilatation and interferes with the compensatory constrictive response to volume fall with consequent arterial hypotension. patients with autonomic neuropathy would be the most affected by volume depletion and they are usually the ones that show the highest frecuency of hypotension episodes, typical of patients with diabetes. it has been proved before that the use of a cold bath does not decrease the efficiency of the dialysis treatment and improves the cardiovascular stability as well, mostly in patients proned to it, such as diabetics, elderly, and patients with cardiac failure. in this study, it was observed that patients showed low basal temperatures before dialysis treatment and that the use of bath temperature of 35.5 °c increased the temperature post dialysis less than with the standard bath at 37 °c. the bath at 35.5 °c decreased the episodes of arterial hypotension, with an improvement in patient's welfare, and lower requirement of attention and treatment costs.

Abstract: Durante el procedimiento dialítico la hipotensión arterial es uno de los problemas más comunes y ha sido objeto de numerosos estudios. En la hemodiálisis se producen cambios en el volumen corporal a través de la ultrafiltración, que generan aumento en la producción de energía térmica, la cual se remueve en el transcurso del tratamiento. La hipovolemia resultante de la remoción de volumen activa el sistema simpático impidiendo de esta manera la pérdida de calor originando aumento de la temperatura corporal que promueve vasodilatación vascular e interfiere con la respuesta constrictiva compensatoria a la caída del volumen con la consecuente hipotensión arterial. Los pacientes que padecen neuropatía autonómica serían los más afectados por la depleción de volumen y habitualmente son los que presentan mayor cantidad de episodios hipotensivos, típico caso de la población con diabetes. Se comprobó anteriormente que el uso de ba o frío no sólo no disminuye la eficiencia del tratamiento dialítico sino que mejora la estabilidad cardiovascular sobre todo en las poblaciones con mayor predisposición a padecerla: diabéticos, a osos, cardiópatas. En este trabajo se observó que la población con diabetes presentó temperaturas basales bajas antes del tratamiento dialítico, que el uso de ba o a 35.5 °C aumentó menos la temperatura post diálisis que con el ba o estándar de 37 °C. El ba o a 35.5 °C disminuyó los episodios de hipotensión arterial desarrollando un mejor bienestar en los pacientes y modificando las características en el tratamiento por generar menor requerimiento de atención y eventualmente observar repercusión en los costos del tratamiento. Abstract: fostering translational research of advanced therapies has become a major priority of both scientific community and national governments. advanced therapy medicinal products (atmp) are a new medicinal product category comprising gene therapy and cell-based medicinal products as well as tissue engineered medicinal products. atmp development opens novel avenues for therapeutic approaches in numerous diseases, including cancer and neurodegenerative and cardiovascular diseases. however, there are important bottlenecks for their development due to the complexity of the regulatory framework, the high costs and the needs for good manufacturing practice (gmp) facilities and new end-points for clinical experimentation. thus, a strategic cooperation between different stakeholders (academia, industry and experts in regulatory issues) is strongly needed. recently, a great importance has been given to research infrastructures dedicated to foster translational medicine of advanced therapies. some ongoing european initiatives in this field are presented and their potential impact is discussed.
Structural and Functional Properties of Venous Wall: Relationship between Elastin, Collagen, and Smooth Muscle Components and Viscoelastic Properties

Abstract: The aims of this work were (1) to analyze the viscoelastic behavior of different venous segments and their differences, considering the structural characteristics (elastin, collagen, and smooth muscle content) of the venous wall; (2) to analyze the venous biomechanical behavior by means of the histological characteristics of the veins. Nine healthy male Corriedale sheep were included. One vein was selected from each animal to evaluate its biomechanical properties: (a) anterior vena cava, (b) right jugular vein, and (c) right femoral vein. Each selected vein was instrumented with pressure and diameter sensors. After excision, a small ring-shaped sample was set apart from each segment for histological analysis. The amounts of elastin, collagen and smooth muscle were correlated to calculated biomechanical parameters (high- and low-pressure compliance and viscosity). Conclusions are the following: (1) the viscoelastic behavior of the venous wall varies depending on the vascular territory, and it is associated with the variation of the histological structure. These differences involve muscle (both smooth and striated), elastin, and collagen contents. (2) In addition, the quantity of collagen was negatively correlated with high- and low-pressure compliances, and (3) the smooth muscle content was higher in peripheral veins and is positively correlated with venous wall viscosity. 1. Introduction The use of veins as grafts has been introduced in clinical practice with several purposes: (a) aortocoronary saphenous vein grafts in myocardial direct revascularization [1]; (b) to perform arteriovenous fistulae in hemodialyzed patients [2]; (c) to perform peripheral revascularization, including the confection of interposing vein cuffs in expanded polytetrafluoroethylene bypasses [3]. For the aforementioned reasons, the use of veins has been extensively indicated in both ischemic and hemodialyzed patients. However, there are several complications, such as intimal hyperplasia, which has been considered to be produced by a mechanical mismatch between the native arteries and the grafts [4]. Consequently, the functional parietal biomechanical behavior of blood vessels has an essential role determining the performance of the cardiovascular system, both in physiological and pathological conditions. Abstract: este artículo se organiza alrededor de la pregunta por la autonomía de las provincias argentinas en contextos de inestabilidad política. para abordar la temática, se consideran dos coyunturas críticas que ocurrieron en san juan en el último período democrático, las cuales fueron resueltas por una vía alternativa a la intervención federal: el juicio político al gobernador. el artículo se estructura en dos grandes partes, la primera presenta un breve repaso histórico sobre el modo de relación entre escalas de gobierno en argentina, y las transformaciones recientes que presenta. la segunda parte indaga sobre las características centrales de las situaciones de crisis que se registraron en la provincia de san juan en 1992 y 2002, considerando centralmente las tomas de posición de los distintos actores.
