A 49 year old gentleman presented with intermittent bilateral knee swelling for the past 37 years and was eventually diagnosed as primary hypertrophic osteoarthropathy after secondary forms were excluded. This case illustrates that the disease can present during childhood with an insidious onset and progressively become more severe and that is when medical attention is sought. Furthermore, the patient did not voluntarily report his skin changes as he considered the slow progressive changes as normal. We report a case of primary hypertrophic osteoarthropathy, its clinical presentation, imaging findings and therapeutic treatment. It is a rare differential diagnosis for chronic joint pain and swelling.
A Survey of Mental Health of New Patients at Traditional Chinese Medical Clinic  [PDF]

Abstract: In Chinese societies, traditional Chinese medical clinics are popular and play a significant role in primary healthcare. The current study aimed to examine the mental health of new patients at Chinese medical clinic. A cross-sectional questionnaire survey was conducted. Subjects were new patients of a Chinese medical clinic recruited through 100% consecutive sampling during a 5-week period. A response rate of 84.2% and a sample size of 367 were achieved. Sample included 111 (30.2%) men and 256 (69.8%) women. Mean age was 44.0 (SD = 17.1). Presenting problems were all somatic, with 68.3% lasted over 1 month and 37.1% lasted over 1 year. In the month prior to first consultation, 26.4%, 33.2% and 19.3% patients consulted other public, private and Chinese medical clinic on average 1.5, 1.9 and 3.0 times respectively. Besides, 7.4% and 6.5% patients self-administered Western and Chinese drugs on average 1.3 and 2.4 times respectively. Using a cut-off of ? 5 on GHQ-12, prevalence of common mental disorders was estimated at 21.0%. In conclusion, findings revealed that common mental disorders were prevalent among new Chinese medicine patients al-though presenting problems of subjects were all somatic. Many of these presenting medical conditions were chronic, with 68.3% lasting over 1 month and 37.1% lasting over 1 year. Over 70% patients had been seek-ing various forms of treatment in the month prior to first consultation. Mental health and illness behavior of Chinese medicine patients warrants further research and attention.
Theory of Excitons in Insulating Cu-Oxide Plane

Abstract: We use a local model to study the formation and the structure of the low energy charge transfer excitations in the insulating Cu-O$_2$ plane. The elementary excitation is a bound exciton of spin singlet, consisting of a Cu$^+$ and a neighboring spin singlet of Cu-O holes. The exciton can move through the lattice freely without disturbing the antiferromagnetic spin background, in contrast to the single hole motion. There are four eigen-modes of excitons with different symmetry. The p-wave-like exciton has a large dispersion width. The s-wave-like exciton mixes with p-state at finite momentum, and its dipole transition intensity is strongly anisotropic. The model is in excellent agreement with the electron energy loss spectra in the insulating Sr$_2$CuO$_2$Cl$_2$.
Utility-Based Wireless Resource Allocation for Variable Rate Transmission

Abstract: For most wireless services with variable rate transmission, both average rate and rate oscillation are important performance metrics. The traditional performance criterion, utility of average transmission rate, boosts the average rate but also results in high rate oscillations. We introduce a utility function of instantaneous transmission rates. It is capable of facilitating the resource allocation with flexible combinations of average rate and rate oscillation. Based on the new utility, we consider the time and power allocation in a time-shared wireless network. Two adaptation policies are developed, namely, time sharing (TS) and joint time sharing and power control (JTPC). An extension to quantized time sharing with limited channel feedback (QTSL) for practical systems is also discussed. Simulation results show that by controlling the concavity of the utility function, a tradeoff between the average rate and rate oscillation can be easily made.

Abstract: The tin(IV) atom in the title compound, [Sn(C8H9)2Cl2(C12H8N2)], is chelated by the N-heterocycle and bonded to two C atoms and two chloride ions in an SnC2Cl2N2 octahedral coordination environment with the C atoms trans to each other. The dihedral angles between the heterocycle ring system and the benzene rings of the 4-methylbenzyl groups are 21.20 (12) and 28.71 (11)°.

Abstract: The two Sn—O—C—C—C—O chelate rings in the title compound, [Sn(C15H11O2)2Cl2]·0.5C7H8, adopt envelope conformations, with the Sn atom deviating from the least-squares plane passing through the C and O atoms by 0.626 (1) in one ring and by 0.690 (1) for the other. The two planes are aligned at an angle of 59.6 (1)°. The Cl atoms occupy cis positions in the octahedral SnCl2O4 coordination environment. The solvent molecule is disordered about a center of inversion.

Abstract: The Sn atom in the title compound, [Sn(C7H6Cl)3(C8H14NO2S2)], is coordinated by three chlorobenzyl ligands and one carboxylate O atom of the substituted acetate ligand in a distorted tetrahedral environment. Three of the C atoms of the n-butyl group are disordered over two sites with equal occupancies.
