Color Image Enhancement by an Integral Mask-filtering Approach Employing Nonlinear Transfer Function  [PDF]

Abstract:

We demonstrate a brand-new method to sharpen a color image by using an integral mask-filtering technique. The derivatives between the target pixel and its neighbors are transferred by the cubic root function instead of the traditional linear one. The obtained final image has clearer fine characteristics along with much less overshooting.

Training gastroenterology fellows to perform gastric polypectomy using a novel ex vivo model

Abstract: AIM: To evaluate the effect of hands-on training of gastroenterology fellows in gastric polypectomy using an ex vivo simulator. METHODS: Eight gastroenterology fellows at Mackay Memorial Hospital, Taipei were evaluated in gastric polypectomy techniques using a pig stomach with artificial polyps created by a rubber band ligation device. The performance of four second year (year-2) fellows who had undergone one year of clinical training was compared with that of four first year (year-1) fellows both before and after a 4-h workshop using the ex vivo simulator. The workshop allowed for hands-on training in the removal of multiple artificial polyps and the placement of hemoclips at the excision site. Evaluation included observation of technical skills, procedure time, and the fellows’ confidence scale. RESULTS: One week after the workshop, the year-1 fellows were re-evaluated and had significantly improved mean performance scores (from 17.9 ± 1.8 to 22.5 ± 0.7), confidence scale (from 4.5 ± 1.0 to 7.8 ± 0.5) and procedure time (from 615.0 ± 57.4 s to 357.5 ± 85.0 s) compared with their baseline performance. After 4 h of training using the ex vivo simulator, the skills of the year-1 fellows were statistically similar to those of the year-2 fellows. CONCLUSION: Use of this ex vivo simulator significantly improved the endoscopic gastric polypectomy skills of gastroenterology fellows who had not had previous clinical training in gastric polypectomy.

Abstract: AIM: To investigate acute transient hepatocellular injury in patients with cholelithiasis and cholecystitis but no evidence of choledocholithiasis.METHODS: The medical records of patients with cholelithiasis who underwent cholecystectomy between July 2003 and June 2007 were retrospectively reviewed. Imaging studies to detect common bile duct (CBD) stones were performed in 186 patients, who constituted the study population. Biochemical liver tests before and after surgery, and with the presence or absence of CBD stones were analyzed.RESULTS: In 96 patients with cholelithiasis and cholecystitis without evidence of CBD stones, 49 (51.0%) had an alanine aminotransferase level elevated to 2-3 times the upper limit of normal, and 40 (41.2%) had an elevated aspartate aminotransferase level. Similar manifestations of hepatocellular injury were, as would be expected, even more obvious in the 90 patients with CBD stones. These markers of hepatocellular injury resolved almost completely within 2 wk to 1 mo after cholecystectomy. Compared to 59 patients with histologically less severe cholecystitis in the group undergoing urgent surgery (total 74 patients), the 15 patients with a gangrenous gallbladder had a higher mean level of total bilirubin (1.14 ± 1.27 mg/dL vs 2.66 ± 1.97 mg/dL, P < 0.001) and white cell count (9480 ± 4681/μL vs 12840 ± 5273/μL, P = 0.018).CONCLUSION: Acute hepatocellular injury in cholelithiasis and cholecystitis without choledocholithiasis is mild and transient. Hyperbilirubinemia and leukocytosis may predict severe inflammatory changes in the gallbladder.
Psychiatric disorders of patients seeking obesity treatment

Abstract: Subjects were retrospectively recruited from an obesity treatment center in Taiwan. The obesity treatments included bariatric surgery and non-surgery treatment. All subjects underwent a standardized clinical evaluation with two questionnaires and a psychiatric referral when needed. The psychiatric diagnosis was made thorough psychiatric clinic interviews using the SCID. A total of 841 patients were recruited. We compared the difference in psychiatric disorder prevalence between patients with surgical and non-surgical treatment.Of the 841 patients, 42% had at least one psychiatric disorder. Mood disorders, anxiety disorders and eating disorders were the most prevalent categories of psychiatric disorders. Females had more mood disorders and eating disorders than males. The surgical group had more binge-eating disorder, adjustment disorder, and sleep disorders than the non-surgical group.A high prevalence of psychiatric disorders was found among ethnic Chinese seeking obesity treatment. This is consistent with study results in the US and Europe.Obesity is becoming an important issue for health promotion. The World Health Organization estimated that around 1.5 billion adults were overweight (body mass index, BMI ≧ 25 kg/m2) and about 500 million people were obese (BMI ≧ 30 kg/m2) in 2008. Oral Glutamine Supplement Inhibits Ascites Formation in Peritoneal Carcinomatosis Mouse Model

Abstract: Background. Peritoneal carcinomatosis (PC) accompanied with ascites formation causes several distressing symptoms, resulting in poor quality of life. Methods. Twenty BALB/c nude mice generated by direct orthotopic injection of human pancreatic cancer PANC-1 cells were randomized to receive either a stock laboratory diet or a stock diet supplemented with glutamine. Half of the mice were sacrificed at day 76 to measure the amount of ascitic fluid and pancreatic tumor volume. The remaining mice were subject to survival analysis. Serum albumin levels were estimated every 2 weeks. Results. At day 76, the average amount of ascitic fluid measured in the control group was ?mL compared to ?mL from the glutamine-supplemented mice ( ). The volume of pancreatic tumor was ?cm3 in the control group and ?cm3 in glutamine-supplemented mice ( ). The mean survival time of glutamine-supplemented mice was prolonged from to days ( ). Mean serum albumin levels were higher in the glutamine-supplemented group. Conclusions. This preclinical study showed that oral supplementation of glutamine may provide ascites-reducing activity in pancreatic cancer patients with PC, via a cell-mediated immunity-independent mechanism. 1. Introduction Peritoneal carcinomatosis (PC) is well established as a terminal feature of advanced primary or secondary neoplasms involving the peritoneum. PC is a challenging complication associated with a poor prognosis and limited treatment options [1]. Locally advanced pancreatic cancer is one of the most common diseases causing PC and subsequent ascites formation. Terminal stage cancer patients with PC have an estimated median survival of 3–6 months [2]. At this terminal stage, quality of life, rather than the prolonging of survival, is considered the most crucial issue in palliative care. Nevertheless, the importance of life-prolonging palliative care is gaining recognition in the new era of cancer management. Because PC-associated ascites develops due to hydrostatic pressure factors rather than osmotic factors, the current management of the condition includes abdominal paracentesis, diuretics administration, and salt restriction. Nutrition support represents an alternative strategy to improve the general well-being after management for ascites. However, the value of nutrition support in patients with PC remains controversial with inconclusive survival benefits [3] and concerns that it may accelerate tumor growth [4, 5]. Abstract: A new image sharing method, based on the reversible integer-to-integer (ITI) wavelet transform and Shamir's (r,m) threshold scheme is presented, that provides highly compact shadows for real-time progressive transmission. This method, working in the wavelet domain, processes the transform coefficients in each subband, divides each of the resulting combination coefficients into m shadows, and allows recovery of the complete secret image by using any r or more shadows (r≤m). We take advantages of properties of the wavelet transform multiresolution representation, such as coefficient magnitude decay and excellent energy compaction, to design combination procedures for the transform coefficients and processing sequences in wavelet subbands such that small shadows for real-time progressive transmission are obtained. Experimental results demonstrate that the proposed method yields small shadow images and has the capabilities of real-time progressive transmission and perfect reconstruction of secret images.
