A Web Clustered System for Achieving Higher Performances through Load Balancing Mechanism  [PDF]

In this paper, we study a methodology of distributing client requests in the LVS cluster system. The basic WLC algorithm is studied intensively. A load distributing algorithm which assigns its weight into each real server is devised by considering the state of the network resources on real servers. Network simulation is executed to estimate balanced usage of the resource using web load generating software. In order to analyze the dynamics of server performance due to the workload, we model a system software to evaluate the level of load balancing in the LVS, and perform actual experiments using web agenda. It is shown that the correction potentiality of the suggested algorithm is somewhat better than the WLC algorithm in terms of balanced resources usage.

Abstract: In this paper, a balanced assignment methodology is proposed in case of entity with more than 2 attributes. The Mahalanobis distance is a basic idea of MTS method as a distance indicator considering the correlation of entities in quality control techniques. This can be applied to ensure the balanced assignment, and utilized the Signal to Noise Ratio as a measure of classifying large groups with entities into many subgroups. In order to analyze the balanced assignment, the simulation is performed for a given example. Validations against simulation data establish the effectiveness of this approach.
Hospice and Palliative Care Services in South Korea Supported by the National Health Insurance (NHI) Program  [PDF]

Abstract: Previous main body of research on end-life-care in South Korea has focused on developing services quality in hospital settings or service payment system in National Health Insurance Program. The delivery system of hospice and palliative care services has evolved in diverse ways but there is little research on reviewing the past history of development and whole picture of them so far. So, the aim of this study is to review the old hospice and palliative care system and also to introduce the current one supported by the National Health Insurance Program in South Korea. The palliative care or hospice services in South Korea have been available in diverse settings and provided by different organizations (i.e. catholic hospitals or charity organizations). Finally, it was set up in 2004 that the hospice team or official Palliative Care Units (PCUs) was established in hospitals, in order to meet the end-of-life care for the patients with terminal cancer under the Cancer Control Act. The current hospice and palliative care services such as pain management, bereavement services, and counselling can be reimbursed by National Health Insurance program since 2008. Nevertheless hospice and palliative care services are available to dying patients, yet the utilization rate of hospice and palliative care services or the length of stay in the palliative care unit (PCU) is still relatively short compared to other country systems. South Korea is undergoing several efforts to expand the services in PCU along with the development of quality indicators for PCU. Hospice and palliative care services are still new in the health care system and unfamiliar to the public so it requires raising awareness for medical professionals and the public as well as further research.
Comparison and Validation of Distance on the Balanced Assignments of Group Having Entities with Multiple Attributes  [PDF]

In this paper, the balanced assignment is studied in classification of a group with multiple attribute into many subgroups without losing its similarity. The similarity or closeness in clustering is often measured as a distance. The Mahalanob distance is considered as one of the tools for measuring its closeness. The comparison between the distance criterion is shown by changing a specific assignment standard, and finally comparing it against the MTS method.
Risk factors for acute respiratory distress syndrome during neutropenia recovery in patients with hematologic malignancies

Abstract: We studied critically ill patients with hematologic malignancies with the dual objectives of describing patients with ARDS during neutropenia recovery and identifying risk factors for ARDS during neutropenia recovery. A cohort of consecutive neutropenic patients with hematologic malignancies who were admitted to the intensive care unit (ICU) was studied. During a 6-year period, 71 patients recovered from neutropenia, of whom 38 (53.5%) developed ARDS during recovery.Compared with non-ARDS patients, patients who experienced ARDS during neutropenia recovery were more likely to have pneumonia, be admitted to the ICU for respiratory failure, and receive mechanical ventilator therapy. The in-ICU mortality was significantly different between the two groups (86.8% versus 51.5%, respectively, for patients who developed ARDS during neutropenia recovery versus those who did not during neutropenia recovery). In multivariate analysis, only occurrence of pneumonia during the neutropenic episode was associated with a marked increase in the risk of ARDS (odds ratio, 4.76).Patients with hematologic malignancies complicated by pneumonia during neutropenia are at increased risk for ARDS during neutropenia recovery.Over the past two decades, the survival of patients with a hematologic malignancy has substantially improved as a result of new and intensive chemotherapeutic regimens, which may be followed by hematopoietic stem cell transplantation (HSCT) [1]. Abstract: We analyzed MS status and whole blood lead, mercury, cadmium, manganese, and creatinine-adjusted urine arsenic concentrations in 1,405 subjects, ? 20 years of age, who were registered for the Korea National Health and Nutrition Examination Survey, 2008.Various demographic and biochemical parameters were associated with MS and blood heavy metal status. Investigation of Responsiveness to Thyrotropin-Releasing Hormone in Growth Hormone-Producing Pituitary Adenomas

Abstract: Objective. The aim of this study was to investigate how the paradoxical response of GH secretion to TRH changes according to tumor volumes. Methods. Patients with newly diagnosed acromegaly were classified as either TRH responders or nonresponders according to the results of a TRH stimulation test (TST), and their clinical characteristics were compared according to responsiveness to TRH and tumor volumes. Results. A total of 41 acromegalic patients who underwent the TST were included in this study. Between TRH responders and nonresponders, basal GH, IGF-I levels, peak GH levels, and tumor volume were not significantly different, but the between-group difference of GH levels remained near significant over the entire TST time. during the TST were significantly different according to the responsiveness to TRH. Peak GH levels and during the TST showed significantly positive correlations with tumor volume with higher levels in macroadenomas than in microadenomas. GH levels over the entire TST time also remained significantly higher in macroadenomas than in microadenomas. Conclusion. Our data demonstrated that the paradoxical response of GH secretion to TRH in GH-producing pituitary adenomas was not inversely correlated with tumor volumes. 1. Introduction Abnormal responsiveness of growth hormone- (GH-) producing pituitary adenomas to hypothalamic hormones has been previously described [1]. This paradoxical response of GH to thyrotropin-releasing hormone (TRH) in GH-producing pituitary adenomas was first reported in 1972 [2, 3] and observed in 50~75% of untreated acromegalic patients [4]. However, this response is not specific to acromegaly and was also found in various pathologic conditions such as severe hepatic failure [2], chronic renal failure [5], diabetes mellitus [6], and anorexia nervosa [7]. There have been many studies of the predictive value of the paradoxical response for treatment outcome and prognosis in acromegaly [4, 8–10]. However, the detailed mechanism of the paradoxical response of GH to TRH in acromegaly remains unknown despite a number of possible hypotheses: local production of TRH by adenoma cells [11, 12], TRH-induced release of GH [13], TRH production by anterior pituitary gland [14], and inappropriate expression of TRH receptors at tumor cells [15]. Efficiency of Selecting Important Variable for Longitudinal Data  [PDF]

Variable selection with a large number of predictors is a very challenging and important problem in educational and social domains. However, relatively little attention has been paid to issues of variable selection in longitudinal data with application to education. Using this longitudinal educational data (Test of English for International Communication, TOEIC), this study compares multiple regression, backward elimination, group least selection absolute shrinkage and selection operator (LASSO), and linear mixed models in terms of their performance in variable selection. The results from the study show that four different statistical methods contain different sets of predictors in their models. The linear mixed model (LMM) provides the smallest number of predictors (4 predictors among a total of 19 predictors). In addition, LMM is the only appropriate method for the repeated measurement and is the best method with respect to the principal of parsimony. This study also provides interpretation of the selected model by LMM in the conclusion using marginal R2.

A Review of the Scaling Study of the CANDU-6 Moderator Circulation Test Facility  [PDF]

Following the previous relevant works [1]-[3], a scaling analysis is performed to derive a set of scaling criteria which were thought to be suitable for reproducing the major thermal-hydraulic phenomena in a scaled-down CANDU moderator tank similar to that in a prototype power plant during a full power steady state condition. The objective of building a scaled-down moderator tank is to generate the experimental data necessary to validate the computer codes which are used to analyze the accident analysis of CANDU-6 plants. The major variables of interests in this paper are moderator flow velocity and temperature of the moderator which is D2O inside the moderator tank during a steady state and transient conditions. The reason is that the local subcooling of the moderator is found to be a critical parameter determining whether the stable film boiling can sustain on the outer surface of the calandria tube if the contact of overheated pressure tube and cold calandria tube should occur due to pressure tube ballooning during LBLOCA with ECC injection failure [4]. The key phenomena involved include the inlet jet development and impingement, buoyancy force driven by the moderator temperature gradient caused by non-uniform direct heating of the moderator, and the pressure drop due to viscous friction of the flow across the calandria tube array. In this paper, the previous researches are reviewed, some concerns or potential problems associated with them implied by comparing CFD analyses results between the CANDU-6 moderator tank and 1/4 scaled-down test facility are described, and as a way to examine the assumption of the scaling analysis is true an order-of-magnitude analyses are performed. Abstract: In order to obtain all the properties of fermented ginseng, we fermented ginseng using Bacillus subtilis (B. subtilis) isolated from Cheonggukjang. A sterilized ginseng medium was made with 4-year-old ginseng powder and distilled water (300% ginseng powder [w/w]), and the ginseng was fermented by B. subtilis (1% ginseng medium) followed by incubation at 37°C for 3, 5, 7, and 10 days. The growth of B. subtilis in the ginseng medium significantly increased up to 9 log CFU/g, but no significant difference was observed after 3 days.
