Electrogastrography associated with symptomatic changes after prokinetic drug treatment for functional dyspepsia

Abstract: AIM: To evaluate the effect of prokinetic drugs on electrogastrography (EGG) parameters according to symptomatic changes in patients with functional dyspepsia (FD). METHODS: Seventy-four patients with FD were prospectively enrolled in this study between December 2006 and December 2010. We surveyed the patients using a questionnaire on dyspeptic symptoms before and after an 8-wk course of prokinetic drug treatment. We also measured cutaneous pre-prandial and post-prandial EGG recordings including percentage of gastric waves (normogastria, bradygastria, tachygastria), dominant frequency (DF), dominant power (DP), dominant frequency instability coefficient (DFIC), dominant power instability coefficient (DPIC), and the ratio of post-prandial to fasting in DP before and after the 8-wk course of prokinetic drug treatment. RESULTS: Fifty-two patients (70%) achieved symptomatic improvement after prokinetic drug treatment. Patients who had normal gastric slow waves showed symptom improvement group after treatment. Post-prandial DF showed a downward trend in the symptom improvement group, especially in the itopride group. Post-prandial DP was increased regardless of symptom improvement, especially in the itopride group and mosapride group. Post-prandial DFIC and DPIC in the symptom improvement group were significantly increased after the treatment. The EGG power ratio was increased after treatment in the symptom improvement group (0.50 ± 0.70 vs 0.93 ± 1.77, P = 0.002), especially in the itopride and levosulpiride groups. CONCLUSION: Prokinetics could improve the symptoms of FD by regulating gastric myoelectrical activity, and EGG could be a useful tool in evaluating the effects of various prokinetics.
Preparation and biodistribution of 188Re-labeled folate conjugated human serum albumin magnetic cisplatin nanoparticles (188Re-folate-CDDP/HSA MNPs) in vivo

Reaction time and optimal conditions of labeling were investigated. The particles were intravenously injected into mice, which were sacrificed at different time points. Radioactivity per gram of tissue of percent injected dose (% ID/g) was measured in vital organs. The biodistribution of 188Re-folate-CDDP/HAS magnetic nanoparticles was assessed.Results: Optimal conditions for 188Re-labeled folate-conjugated albumin combined with cisplatin magnetic nanoparticles were: 0.1 mL of sodium gluconate solution (0.3 mol/L), 0.1 mL of concentrated hydrochloric acid with dissolved stannous chloride (10 mg/mL), 0.04 mL of acetic acid buffer solution (pH 5, 0.2 mol/L), 30 mg of folate-conjugated albumin combined with cisplatin magnetic nanoparticles, and 188ReO4 eluent (0.1 mL). The rate of 188Re-folate-CDDP-HSA magnetic nanoparticle formation exceeded 90%, and radiochemical purity exceeded 95%. The overall labeling rate was 83% in calf serum at 37°C. The major uptake tissues were the liver, kidney, intestine, and tumor after the 188Re-folate-CDDP/HSA magnetic nanoparticles were injected into nude mice. Uptake of 188Re-folate-CDDP/HSA magnetic nanoparticles increased gradually after injection, peaked at 8 hours with a value of 8.83 ± 1.71, and slowly decreased over 24 hours in vivo.Conclusion: These results indicate that 188Re-folate-CDDP/HSA magnetic nanoparticles can be used in radionuclide-targeted cancer therapy. Abstract: Resolution of the Nile waters question has proved, once again, to be an elusive task. Identifying the major hurdle which has bedeviled past cooperative initiatives and rendered current efforts mere Sisyphean ones is thus of paramount importance. The main thrust of this article is to identify this challenge which has thus far stifled almost all efforts at resolution of the Nile waters question in a fair and equitable manner. The consistently obstinate position Egypt has taken over the years to maintain its poignantly inequitable “share” of Nile waters forever is the heart of the problem which makes any settlement of the Nile waters question a virtual impossibility. Relying on its status as the basin’s hydro-hegemon, Egypt has so far been able to not only defend the indefensible but has also been able to effectively hoodwink and contain the non-hegemonic riparians by engaging them in “cooperative initiatives” and a “benefit sharing” scheme it effectively is using as stalling tactics while aggressively pursuing giant hydraulic projects as instruments of resource capture. A real transformation and a breakthrough in this stalemate requires, of necessity, a change in the malign, oppressive nature of Egyptian hydro-hegemony into a benign, cooperative one, at least. The non-hegemonic riparian states have thus to adopt effective counter-hegemonic strategies in order to force Egypt back to the negotiation table, developing, in the mean time, the resource and technical capability that would enable them to resist and overcome the multifaceted pressure and influence the hydro-hegemon will inevitably exert to keep them in line; failure to do so would surely condemn them to live, ad infinitum, with the grotesquely inequitable status quo.

Methods: Magnetic Fe3O4 nanoparticles were prepared using a coprecipitation method. The appearance, structure, phase composition, functional groups, surface charge, magnetic susceptibility, and release in vitro were characterized by transmission electron microscopy, x-ray diffraction, scanning electron microscopy-energy dispersive x-ray spectroscopy, and a vibrating sample magnetometer. Blood toxicity, in vitro toxicity, and genotoxicity were investigated. Therapeutic effects were evaluated by MTT [3-(4, 5-dimethyl-2-thiazolyl)-2, 5-diphenyl-2H-tetrazolium bromide] and flow cytometry assays. Results: Transmission electron microscopy revealed that the shapes of the Fe3O4 nanoparticles were approximately spherical, with diameters of about 26.1 ± 5.2 nm. Only the spinel phase was indicated in a comparison of the x-ray diffraction data with Joint Corporation of Powder Diffraction Standards (JCPDS) X-ray powder diffraction files. The O-to-Fe ratio of the Fe3O4 was determined by scanning electron microscopy-energy dispersive x-ray spectroscopy elemental analysis, and approximated pure Fe3O4. The vibrating sample magnetometer hysteresis loop suggested that the Fe3O4 nanoparticles were superparamagnetic at room temperature. MTT experiments showed that the toxicity of the material in mouse fibroblast (L-929) cell lines was between Grade 0 to Grade 1, and that the material lacked hemolysis activity. The acute toxicity (LD50) was 8.39 g/kg. Micronucleus testing showed no genotoxic effects. Pathomorphology and blood biochemistry testing demonstrated that the Fe3O4 nanoparticles had no effect on the main organs and blood biochemistry in a rabbit model. MTT and flow cytometry assays revealed that Fe3O4 nano magnetofluid thermotherapy inhibited MCF-7 cell proliferation, and its inhibitory effect was dose-dependent according to the Fe3O4 nano magnetofluid concentration. The endogenous hydrogen sulfide producing enzyme cystathionine-β synthase contributes to visceral hypersensitivity in a rat model of irritable bowel syndrome

Abstract: In rat DRG, CBS-immunoreactivity was observed in approximately 85% of predominantly small- and medium-sized neurons. Colon specific DRG neurons revealed by retrograde labeling DiI were all CBS-positive. CBS-positive colon neurons co-expressed TRPV1 or P2X3 receptors. Western blotting analysis showed that CBS expression was significantly increased in colon DRGs 8 weeks after neonatal AA-treatment. Furthermore, the CBS inhibitor hydroxylamine markedly attenuated the abdominal withdrawal reflex scores in response to colorectal distention in rats with CVH. By contrast, the H2S donor NaHS significantly enhanced the frequency of action potentials of colon specific DRG neurons evoked by 2 times rheobase electrical stimulation.Our results suggest that upregulation of CBS expression in colonic DRG neurons and H2S signaling may play an important role in developing CVH, thus identifying a specific neurobiological target for the treatment of CVH in functional bowel syndromes.Hydrogen sulfide (H2S), a gas synthesized by sulfate reducing colonic bacteria and the endogenous enzymes cystathionine-β-synthetase (CBS) and cystathionine-γ-lyase (CSE) [1-5], is increasingly recognized as a biologically important signaling molecule in various tissues and processes including pain and inflammation [6,7]. Its putative role as a neurotransmitter is supported by recent reports on its effects on hippocampal neurons as well as capsaicin-sensitive peripheral sensory neurons [8-10]. Abstract: AIM: To investigate the effect of two-channel gastric electrical stimulation (GES) with trains of pulses on gastric emptying and slow waves.METHODS: Seven dogs implanted with four pairs of electrodes and equipped with a duodenal cannula were involved in this study. Two experiments were performed. The first experiment included a series of sessions in the fasting state with trains of short or long pulses, each lasted 10 min. A 5-min recording without pacing was made between two sessions. The second experiment was performed in three sessions (control, single-channel GES, and two-channel GES). The stimulus was applied via the 1st pair of electrodes for single-channel GES (GES via one pair of electrodes located at 14 cm above the pylorus), and simultaneously via the 1st and 3rd channels for two-channel GES (GES via two pairs of electrodes located at 6 and 14 cm above the pylorus). Gastric liquid emptying was collected every 15 min via the cannula for 90 min.RESULTS: GES with trains of pulses at a pulse width of 4 ms or higher was able to entrain gastric slow waves. Two-channel GES was about 50% more efficient than single-channel GES in entraining gastric slow waves. Two-channel but not single-channel GES with trains of pulses was capable of accelerating gastric emptying in healthy dogs. Compared with the control session, two-channel GES significantly increased gastric emptying of liquids at 15 min (79.0% ± 6.4% vs 61.3% ± 6.1%, P < 0.01), 30 min (83.2% ± 6.3 % vs 68.2% ± 6.9%, P < 0.01), 60 min (86.9% ± 5.5 % vs 74.1% ± 5.9%, P < 0.01), and 90 min (91.0% ± 3.4% vs 76.5% ± 5.9%, P < 0.01).CONCLUSION: Two-channel GES with trains of pulses accelerates gastric emptying in healthy dogs and may have a therapeutic potential for the treatment of gastric motility disorders.

Abstract: In the early middle ages the customary practice of killing wifes who did not conformed to the accepted norms (by breaking sexual code, plotting against her husband, practicing witchcraft), or were only suspected of doing it, was generally considered legal and tolerable, as testified the legal sources from different parts of Europe. During the 8th and 9th centuries growing influence of the Church on the matrimonial matters and the consolidation of the doctrine about spiritual nature of christian marriage and marital relations inclined the ecclesiastical authorities to make efforts to eradicate this sinful practice from social life. The different texts of this period cast light on the tension between traditional morality based on the economy of honour” and christian teaching. This tension disturbed the stability of value systems of early medieval society. Writings of Hincmar of Reims dealing with the theory of christian marriage and the limits of husband’s authority over his wife (De coercendo ed exstirpando raptu viduarum, puellarum ac sanctimonialium; De divortio Lotharii regis et Theutbergae reginae) are here analysed in order to reveal that canonists and moralists of this period were aware of these conflicts, and tried to harmonize the contradictory norms. Such practices as mediation between the spouses, public penance and monastic exclusion of the adulterous wife can be considered as a model of the strategies proposed by ecclesiastical authorities to protect the woman’s life and at the same time to preserve honour of her husband. However, in spite of the Church condemnation of uxoricide and dissemination of the Christian concept of the marriage, the several cases from the narrative and diplomatic records from the 9th century clearly demonstrate that killing the wife in the name of family’s honour was not only the accepted tradition, but it was also considered as the moral obligation of the husband or his male relatives. The refusal to fulfill such obligation, even in the name of Christian principles, could be dangerous not only for the social position of the husband but also for his male identity.
