Una aproximación al estado de la partería tradicional en Colombia

Abstract: in colombia there are two systems of care delivery: the formal and traditional. the second is still in use because of the low health coverage associated with pregnancy, childbirth and the postpartum period provided by the proper system, and other cultural factors. however, despite the consolidation of traditional midwifery and existence of the midwife figure in remote rural areas and urban marginal in the country, the lack of scientific knowledge persist of this event; who takes current impact in the presentation of mortality and maternal and perinatal morbidity. this paper aims to discuss about the need for research addressing the issue of midwives and traditional birth attendant. it presents the few results of the research progress on this subject, we present some outstanding balances to respond from scientific research and the possible contribution that could perform the traditional midwives to improve the health of women.

Abstract: madeleine leininger in her theory of multicultural infirmary emphasizes coherence, participation, system of values and patient?s treatment as well. for this reason infirmary must find strategies allowing the concept of sensibility and the awareness people have of their health problems in order to deal with their own urban context and their needs for self care through a biographic narration. the patients have the opportunity to express their true reality and mainly their own experience about health and illness - process. the purpose of this article is to reveal experiences narrated by the protagonist (an urban midwife) from the magdalena region, "a colombian area". this true story is full of believes and unique cultural details from the rural region of the department (state) of antioquia where they set methods for woman?s care during delivery and after it.

Abstract: this 2008 study described the concepts of health and disease in rural communities in the remote rural region of valle del río cimitarra, colombian magdalena medio. qualitative descriptive study with triangulation techniques for gathering information: group discussion, graphical representation, participant observation and semi-structured depth interviews. the information provided by farmers of all ages was analyzed under the hermeneutic interpretive method. emerging categories were "emotional and physical well-being" versus "unrest" were determined by the development of social and armed conflict and constructed as opposite from a collective and comprehensive speech in which privileged social determinants of health over the biology. in addition, they were permeated by the history of colonization of the jungle area, the relationship with nature and processes of peaceful resistance peasant. the cherished dream of health is not alienating and constructed within the framework of organizational processes by which they improve their living conditions and demanding respect for their rights. the disease is a reflection of reality lived.
Con el poder de Dios y el saber campesino: curando al picao de culebra en el Valle del Río Cimitarra

Abstract: the cimitarra river valley is a rural area forest landscape and forest, where the ophidic accident is common.there is a limited access to antivenom serum, due to its distance from the municipal area, the deplorable conditions of roads, lack of health care facilities and the lack of safety and public nuisance resulting from the development of social conflict and armed. because of this, people bitten by snakes are attended mostly by traditional health agents. manuel sabana is a healer of the region experienced in treating people who are bitten by venomous snakes. this article narrates through biographical narrative the way he cures. he explains that he feels as an "instrument of god," which combined with the power of the word using the 'secret' and the peasant knowledge, he removes the "benumb" caused by the poison.

Abstract: the experience of a research proposal of medium term that is currently developing is presented. the purpose, is trying to know the health care in the dynamic, social, economical and political context in which they have determined them in order to contribute to the construction of the health empowering and consideration of the identity of rural population, that in its conformation, different cultural backgrounds and socio-political and economic conditions, product of forced relocation produced by violence have influenced them, and the social and armed conflict that is been developed in the valley of the cimitarra river - colombian medium magdalena. the research context is approached and the "route" following is: the construction of the health problems experience, description of the care practices, the traditional agents, traditional resources used for care and the communities’ health expenses. an attempt of triangulation by the complexity of the study phenomenon is set out. the theoretical referents used are the universality theory and the diversity of the cultural care, the medical sociology and the critical medical anthropology. methodologically, the quantitative approach is assumed, and for the collection of information, ethnography techniques and action-research. the interpretative-hermeneutic method is proposed for the analysis of the information.

Abstract: Estudio realizado en 2008 que describió las conceptualizaciones de salud y enfermedad de las comunidades rurales de la apartada región rural del Valle del río Cimitarra, Magdalena Medio colombiano. Estudio cualitativo descriptivo con triangulación de las técnicas de recolección de la información: discusión de grupo, representación gráfica, observación participante y entrevistas semi-estructuradas. La información brindada por campesinos de todas las edades se analizó bajo el método interpretativo hermenéutico. Las categorías emergentes fueron el "Bienestar físico y emocional" versus "Intranquilidad", estuvieron determinadas por el desarrollo del conflicto social y armado y construidas como opuestas, desde un discurso integral y colectivo en el cual se privilegiaron los determinantes sociales de la salud sobre lo biológico. Además, se encontraban permeadas por la historia de colonización del territorio selvático, la relación con la naturaleza y los procesos de resistencia pacífica campesina. El sue o anhelado de la salud no es enajenante y se construye en el marco de los procesos organizativos mediante los cuales mejoran sus condiciones de vida y reclaman el respeto de sus derechos. La enfermedad es el reflejo de la realidad vivida. Abstract: El Valle del río Cimitarra es una zona rural dominada por el paisaje selvático y boscoso por lo cual son frecuentes los accidentes ofídicos. Sin embargo, producto de su lejanía de las cabeceras municipales, las deplorables condiciones de las vías de comunicación, la inexistencia de centros de atención sanitaria y las condiciones de inseguridad y alteraciones del orden público producto del conflicto social y armado, es bajo el acceso al suero antiofídico. Por lo anterior, las personas mordidas por serpientes son atendidas, en su mayoría, por agentes tradicionales de salud. Manuel Sabana es un curandero de la región experto en tratar a los habitantes que son picados por serpientes venenosas y narra, mediante un relato biográfico, como los cura. Lo hace sintiéndose un "instrumento de Dios", combinando el poder de la palabra mediante el "secreto" y del saber campesino para sacar el "pasmo" que causa el veneno. Abstract: Madeleine Leininger en su teoría enfermería transcultural, pone énfasis en la coherencia entre las intervenciones de enfermería y el sistema de valores así como el contexto cultural que rodea al sujeto de cuidado. Por tanto la enfermera debe buscar estrategias que le permitan ser sensible a la concepción que tienen las personas de sus problemas de salud con el fin de mediar entre el contexto cultural y las necesidades de cuidado de la persona. Por medio de un relato biográfico se da la oportunidad a los sujetos de cuidado de expresar la complejidad de su realidad y sobre todo la experiencia de la persona frente sus procesos de salud - enfermedad. El presente artículo tiene como objetivo narrar en la voz de su protagonista, la experiencia de vida de una partera tradicional de la región del Magdalena medio colombiano. En la narración, se entrecruzan las creencias y acervos culturales propios de la región rural del departamento de Antioquia y que determinan las prácticas de cuidado para la atención de la mujer durante el parto y el puerperio. Abstract: En Colombia coexisten dos sistemas de atención del parto: el formal y el tradicional. El segundo sobrevive dada la baja cobertura de la atención de los eventos relacionados con la gestación, el parto y el puerperio por el sistema formal, y por factores de tipo cultural. Sin embargo, a pesar de la consolidación de la partería tradicional y la figura de la partera en las zonas rurales apartadas y las urbanas marginales, en el país persiste un desconocimiento científico de este evento que cobra trascendencia actual ya que es uno de los que interviene en la presentación de la mortalidad y morbilidad materna y perinatal. El presente artículo se propone discutir acerca de la necesidad del abordaje investigativo en el tema de la partería y la partera tradicional. Así, se presentan los escasos resultados de los avances investigativos en este tema, se plantean algunos saldos pendientes a responder desde investigación científica y el posible aporte que pudiera realizar la partería tradicional al mejoramiento de la salud de la mujer. Abstract: Se presenta la experiencia de una propuesta investigativa a mediano plazo que se desarrolla en la actualidad. En ésta se pretende comprender los cuidados de la salud en el contexto de las dinámicas culturales, sociales, económicas y políticas que los han determinado para aportar a la construcción de los procesos de empoderamiento de la salud y consolidación de la identidad de una población rural que en su conformación han influido diferentes acervos culturales, y condiciones sociopolíticas y económicas producto del desplazamiento forzado por la violencia y el conflicto social y armado que se desarrolla en la región del Valle del río Cimitarra - Magdalena Medio colombiano. Se aborda el contexto investigativo y la "ruta" que se sigue: la construcción de la experiencia de los problemas de salud, descripción de las prácticas de cuidado, de los agentes tradicionales, los recursos tradicionales utilizados para el cuidado y el gasto en salud de las comunidades. Se plantea una triangulación por la complejidad del fenómeno de estudio. Los referentes teóricos utilizados son la Teoría de la universalidad y diversidad de los cuidados culturales, la Sociología médica y la Antropología médica crítica. Metodológicamente, se asume el enfoque cualitativo y para la recolección de la información, técnicas de la etnografía y la investigación-acción.
