Angioplastia con stents liberadores de rapamicina en diabéticos tipo 2: Experiencia del Hospital DIPRECA Angioplasty stenting with Sirolimus (Rapamycin) eluting stents in type 2 diabetic patients

Abstract: Background: Drug-eluting stents have been developed to reduce the rates of restenosis after coronary angioplasty. Several studies have demonstrated that rapamycin eluting stents are reliable and effective. Aim: To report the experience in our Health Centre with rapamycin-eluting stents. Patients and methods: Forty two stents with rapamicine were implanted to 32 diabetic patients, between June 2002 and December 2004. After the procedure, subjects were clinically followed-up for an average period of 19.9+9.9 months, evaluating functional capacity, angina pectoris, dyspnea, need for hospital admission, acute coronary events and cardiac death. In those subjects clinically suspected to have restenosis, a coronary angiography was performed. Results: Twenty-nine subjects (90.6%) remained asymptomatic, two subjects (6.3%) developed angina pectoris but restenosis was ruled out, and one subject (3.1%) died. Conclusions: The use of rapamycin-eluting stents in these patients was safe and successful with no evidence of clinic restenosis. These positive results are similar to those reported in the Diabetes Study
Evaluación de la microcirculación sublingual en un paciente en shock séptico refractario tratado con hemofiltración de alto volumen

Abstract: microcirculation is severely compromised in sepsis, with a reduction of capillary density and flow impairment. these alterations have important prognostic implications, being more severe in non-survivors to septic shock. today microcirculation may be assessed bedside, non-invasively usingpolarized light videomicroscopy a technique known as sdf (side dark field). we report a 54 year-old man with an extramembranous nephropathy that developed a necrotizing fascitis associated to septic shock, in whom microcirculation was periodically assessed during his management. the patient was treated with buids, vasoactive drugs, antibiotics and was operated for exploration and debridement. as the patient persisted in refractory shock despite treatment, high-volume hemofiltration was started. before hemofiltration the patient had severe microcirculatory alterations that improved during and after the procedure. physiologic endpoints of high-volume hemofiltration in septic shock remain unknown, but it has the capacity to clear in?ammatory mediators. since microcirculatory alterations are in part secondary to these mediators, their removal is beneficia! like other authors, we found no relation between microcirculation and other haemodynamic and perfusión variables.

Abstract: We report a 58 years old male that developed a bone marrow aplasia associated to the use of ticlopidine, prescribed after coronary artery stenting. The patient developed a pneumonia as a complication. He was admitted to the Intermediate Treatment Unit, receiving wide spectrum antimicrobial therapy and a granulocyte colony stimulating factor (Neupogen(r)) with favourable response. Ticlodipine is an effective anti-platelet agent, but has serious hematological and other side effects. Its prescription requires a close follow up and search for complications (Rev Méd Chile 2002; 130: 798-802).

Abstract: Acute liver failure has a mortality rate in excess of 80%. Most deaths are attributed to brain edema with intracranial hypertension and herniation of structures, where ammonium plays a major role in its generation. We report an 18 year-old female with a fulminant hepatic failure caused by virus A infection. The patient developed a profound sopor and required mechanical ventilation. A CT scan showed the presence of brain edema and intracranial hypertension. A Raudemic catheter was inserted to measure intracranial pressure and brain temperature. Intracranial hypertension became refractory and intravascular hypothermia was started, reducing brain temperature to 33°C. Seventy two hours later, a liver transplantation was performed. After testing graft perfusion, rewarming was started, completing 122 hours of hypothermia at 33°C. The patient was discharged in good conditions after 69 days of hospitalization.

Abstract: Desde el campo de los estudios urbanos sociotécnicos, el texto analiza la controversia vender o no vender la Empresa de Telecomunicaciones de Bogotá (ETB), una empresa pública de la capital colombiana. La primera sección rese a el surgimiento, desarrollo y potencial crítico de dicho campo. La segunda sección exponeel estudio de caso: el primer apartado presenta la ETB y su carácter sociotécnico; el segundo explora algunas controversias sucedidas en la interacción ETB - ciudad y contextualiza la controversia analizada; el terceroexpone el dise o metodológico empleado que incluyó: i) conformación de un corpus de prensa escrita sobre la controversia vender o no vender la ETB sucedida en el a o 2011, ii) adaptación de las categorías analíticas del protocolo de investigación SCOT1; y iii) aplicación del análisis de contenido al corpus textual con soporte en el software ATLAS/ti (versión 7). El cuarto apartado expone los resultados del análisis que permitió identificar los actores y argumentos que dominaron el debate y el marco tecnológico o conjunto deconceptos empleados para discutir y resolver el dilema vender o no vender : por un lado, el conjunto mercado de telecomunicaciones y por otro, el conjunto patrimonio público de la ciudad . La tercera sección deconsideraciones finales critica la polarización que sustenta el dilema vender o no vender , pues bloquea lacomprensión de la interacción sociotécnica ETB - ciudad y el desarrollo de sus potencialidades.

Abstract: This article deals with the different stages of production and implementation of the decentralization policy in Chile. Three phases or sequences were established: the decade that goes between 1960 and 1970, in which the epistemic community will be identified to the origin of the policy; then, the decade 1980-1990 in which will be analyzed how the community manages to consolidate relations and how it manages to become emancipated from the original organizations and, finally, in the third period that goes between 1990 and 1993, it will be studied how the community joined to the new democratic structures in order to intervene in the project of law of decentralization in Chile.This method of sequences allows to extend the focus of analysis in order to observe not just the actors who integrate the policy network in its beginnings but also it allows to depict the evolution of the network through the integration of new actors across the time and how these new members could have influenced the course that certain events have taken.
