Abstract: gastroesophageal reflux (ger) is common in infants during the early months of life, as a result of a weak development of the regulatory mechanisms. nevertheless, ger tends to disappear as infants grow. nowadays, ger has become a common cause ofpediatric consultation in the clinical practice, especially if it is followed by vomits. sometimes, changing baby position while feeding seems to be a solution to reduce symptoms, while other times it is necessary manage the problem according to clinical practice guidelines. in these guidelines, the use of anti-regurgitation infant formulas are included, which has been formulated with thickening agents and/or whey/casein ratio modification. antiregurgitation milk products are available without pediatric prescription in chemists and other authorized establishments. published studies on antiregurgitant infant formulas show some contradictory conclusions about their use. a negative effect on mineral and micronutrient bioavailability has been proposed by some authors, in relation to the presence of thickening agents used as ingredients. different aspects which require more research, related to the use of antiregurgitation infant formulas, have been included in this review with the aim ofproducing more effective and safety products for an especially vulnerable population, where an optimum feeding will lead to an adequate development and good health status in the future.

Abstract: Las regurgitaciones gastroesofágicas en el lactante constituyen un proceso frecuente en los primeros meses de vida, fruto de la inmadurez de algunos mecanismos fisiológicos que regulan este proceso, con tendencia a desaparecer conforme avanza el desarrollo. Constituyen un motivo de consulta pediátrica frecuente, sobre todo cuando se acompa an de vómitos. En ocasiones, las recomendaciones posturales a adoptar durante la alimentación son suficientes para contribuir a mejorar la sintomatología. Cuando esto es insuficiente, existen protocolos de intervención en nutrición infantil como el dise o de fórmulas antirregurgitación, que contienen sustancias espesantes y/o modificaciones en la relación suero:caseína. Las fórmulas antirregurgitación se encuentran disponibles en farmacias y otros establecimientos de venta autorizados, sin que sea necesaria una prescripción médica para su adquisición. Estudios realizados sobre el empleo de fórmulas infantiles para el manejo pediátrico del reflujo gastroesofágico muestran ciertas contradicciones respecto a una recomendación sobre su uso. Algunos autores destacan que puede aparecer un efecto negativo sobre la biodis-ponibilidad mineral y de otros micronutrientes como consecuencia de los agentes espesantes utilizados como ingredientes. En la presente revisión se abordan los aspectos que, sobre el empleo de fórmulas infantiles antiregurgitación, precisan de mayor investigación para permitir elaborar fórmulas antirregurgitación eficaces y totalmente seguras para una población que es especialmente vulnerable, y en la que una correcta alimentación permite un adecuado desarrollo y buen estado de salud en la etapa adulta. Abstract: The present article shows the different factors intervening on the study and later setting into service of the internal ceramic liner for industrial chimneys. Firstly they are studied the diverse existing options and later decision as to the solution to be adopted. Also they are specified the physicochemical peculiar characteristics of the material as well as the criterion to folio w with regards to the adecuate dispositive for the material's quality control. En el presente artículo, se estudian los diversos factores que intervienen en el estudio y posterior puesta en servicio de los revestimientos internos cerámicos para chimeneas de uso industrial. Se estudian inicialmente las diversas opciones existentes y posterior decisión en cuanto a la solución a adoptar. También se especifican las características físico-químicas propias del material, así como los criterios a seguir en cuanto al dispositivo adecuado para el Control de Calidad del material.

Abstract: In this work, a numerical model is presented that describes the transfer of heat and mass inside a cylindrical regenerator of a solar adsorption refrigerator that uses the methanol/activated-carbon refrigerant pair. This model is based on the equations of mass conservation, energy conservation, Darcy’s law and the balance model between sorbate and sorbent given by the Dubinin-Astak- hov’s equation. On the other hand, the linear driving force (LDF) model is used to describe the rate of desorption. In the developed model, the spatial variation of methanol vapor pressure within the activated carbon bed is taken into account and, as one of the boundary conditions, the temperature is used at the external surface of the absorber measured experimentally along the day. Using the developed model, the temperature, pressure and concentration of methanol were calculated; both inside the grains of carbon and in the space between the grains, as a function of time. The algorithm was validated comparing the numerical results with the experimental data, obtaining a satisfactory concordance.
Estratificación socioeconómica y salud materno infantil en México

Abstract: El objetivo del artículo es el estudio de la salud de los ni os en México, mediante un análisis que tenga como eje la diferenciación social, la atención a la salud materno infantil y la estructura familiar. Se argumenta la necesidad de prestar mas atención al análisis de los determinantes que de los indicadores de salud, en virtud de las características de información disponible y de los niveles de mortalidad infantil. Un aspecto metodológico importante es que los estratos socioeconómicos actúan como variables intermedias tanto de las biodemográficas edad, paridad y espaciamiento como de la atención a la salud, por lo que el análisis de la mortalidad infantil no puede llevarse a cabo con las herramientas usuales, como las regresiones lineales, ya que se violaría el supuesto de la independencia de las variables explicativas. Al efectuar análisis por separado en cada estrato social vemos que los efectos de las variables consideradas son totalmente distintos. Si bien esto puede deberse al escaso número de eventos, ratifica nuestra hipótesis de que se debe proceder al análisis por separado en cada estrato, privilegiando la atención a la salud como la variable más importante.

In fact, there is an inverse linear relationship between renal function and prevalence of hypertension. Hypertensive patients with renal dysfunction exhibit a poor clinical profile, which markedly increases their risk for cardiovascular outcomes. This review considers the available evidence on the best therapeutic approach for optimizing renovascular protection in the hypertensive population. To effectively reduce or at least slow the establishment and progression of renal disease in the hypertensive population it is critical to reach blood pressure targets. Many studies have shown that angiotensin-converting enzyme inhibitors and angiotensin receptor blockers prevent or at least delay the development of microalbuminuria in patients with hypertension and type 2 diabetes, reduce the incidence of overt diabetic nephropathy, and are also beneficial in patients with nondiabetic renal disease. Therefore, renin-angiotensin system (RAS) inhibition plays a key role in the prevention of renal outcomes. As the majority of patients with hypertension will need at least two antihypertensive agents to achieve blood pressure goals, the use of RAS inhibitors is a mandatory part of antihypertensive therapy. The question of which antihypertensive agent is the best choice for combining with RAS blockers should be considered. Many studies have shown that diuretics and calcium channel blockers are the best choice. However, more studies are needed to clarify the subgroups of patients who will benefit more from a combination with a diuretic or from a combination with a calcium channel blocker. To date, RAS inhibitors recommended in this context are angiotensin-converting enzyme inhibitors and angiotensin receptor blockers. Aliskiren, the first oral direct renin inhibitor available, has shown promising results.

To attain these goals, however, many patients will need statins at high doses. The most frequent side effects related to the use of statins, myopathy, rhabdomyolysis, and increased levels of transaminases, are unusual. Although low and moderate doses show a favourable profile, there is concern about the tolerability of higher doses. During recent years, numerous trials to analyze the efficacy and tolerability of high doses of statins have been published. The majority of hypertensive patients need more than one antihypertensive agent to attain BP targets. Combination therapy is required when monotherapy fails to attain BP objectives and as a first-line treatment in certain situations, such as markedly elevated BP values, when lower targets are required in high or very high cardiovascular risk patients. The advantages of combination therapy are well documented, with an increased antihypertensive efficacy as a result of the simultaneous inhibition of different mechanisms of action and with a lesser incidence of adverse events, because of the possible compensatory responses and the lower doses used. Calcium channel blockers are effective drugs in the treatment of hypertension. The efficacy of lercanidipine has been evaluated in several noncomparative and in comparative studies showing a great efficacy with a good tolerability. On the other hand, the inhibition of the renin-angiotensin system appears to be very beneficial in the treatment of patients with hypertension. Enalapril is an effective and well tolerated angiotensin converting enzyme inhibitor. Although there are several fixed-combination drugs, the combination lercanidipine plus enalapril appears to be one of the most promising therapies in the treatment of hypertension. Estudio del polen atmosférico en San Sebastian.

Abstract: Con un Captador de esporas Volumétrico Burkard se ha estudiado el polen atmosférico en San Sebastián desde el 21 de marzo de 1983 hasta el 30 de septiembre de 1985. Usando los conteos diarios de polen se han confeccionado los calendarios polínicos y se han comparado las concentraciones polínicas con los datos climatológicos. Los granos clasificados y contados se han agrupado en 33 taxones. El polen más abundante es el de Pinaceae (20-35% del total), seguido del Gramineae (15%).
