Abstract: objective: femoral fixation in acl reconstruction with flexor tendon grafts can vary greatly according to the provision of materials and the surgeon's experience. but studies comparing fixation systems are most often performed on cadavers or animals, without evaluating the clinical results, which affects their comparison with live human patients. this article evaluates the clinical and subjective outcomes of two methods of graft fixation to the femur (titanium interference screw and titanium transcondilar device) to determine whether there is any difference between these methods. methods: forty patients with acl injury were selected. of these, 20 had their graft fixed to the femur with interference screw and 20 with the transcondylar device. all the patients were reevaluated at least two years postoperatively by the anterior drawer test, pivot shift test and lachman test, as well as obtaining the lysholm and ikdc (international knee documentation committee) scores. results: the results were not statistically different for the criteria evaluated, which leads to the conclusion that. conclusion: both forms of fixation are effective for this technique, within the parameters established. level of evidence ii, prospective comparative study.

Abstract: purpose: the authors intend to evaluate the eyelid position before and after the external prosthesis adaptation, using digital image processing techniques. methods: eighteen patients were evaluated at the "faculdade de medicina de botucatu - universidade estadual paulista - unesp", with and without the external prosthesis. qualitative and quantitative analysis were done using the scion image program. the superior eyelid sulcus distance, margin eyelid distance and eyelid medial and lateral canthus angles were evaluated. results: pseudostrabismus and high eyelid sulcus were the most observed alteration. the superior eyelid sulcus distance had shorter values with the prosthesis and higher values were seen analyzing the other parameters when the external prosthesis was in place. conclusion: all patients had some abnormalities, which show us how difficult it is to provide a normal aspect for anophthalmic cavity carriers. the image processing allowed objective evaluations of the measures contributing to the follow-up of the patients.

Abstract: purpose: to evaluate the eyelash alterations in anophthalmic cavity patients. methods: an observational study was done looking for eyelash alterations in anophthalmic cavity patients at "faculdade de medicina de botucatu". results: eyelash alterations were observed in 70.7% of the anophthalmic patients. the most frequently observed eyelash alterations were eyelash ptosis (24.4%), reduction in the quantity (19.5%), trichomegaly (19.5%), trichiasis (14.6%) and entropium (17.0%). conclusion: the anophthalmic cavity patients have eyelash alteration and ptosis, reduction in quantity, trichomegaly, trichiasis are the most common observed situations. further studies are needed to know more about it.

Abstract: purpose: to evaluate eyelid margin alterations in ectropion carriers. methods: an observational study was done involving 53 eyelid ectropion patients and 25 individuals with dermochalasis (control group). eyelash position and margin inflammation were observed using digital images. the data were submitted to statistical analysis. results: patients with ectropion showed a decreased number of eyelashes, loose convexity, trichiasis and distichiasis as compared with the control group. conclusion: patients with eyelid ectropion have eyelid margin alterations probably due to the chronic inflammatory process in this region.

Abstract: purpose: to evaluate the eyebrow position in children and elderly using angle measurements. methods: the eyebrow position was evaluated by digital images in 4 to 6-year-old children (children group) and in patients with 50 years or more (elderly group), separated by age range. the images were taken in the primary gaze position with a sony lithium camera and then transferred to a macintosh g4 computer and processed by the nih 1.58 program. the eyebrow position was evaluated using angle measures. comparisons were done according to age, sex and laterality and the data were submitted to statistical analysis. results: the comparison of the angular measures showed that there was a significant difference in the position of eyebrow position between the studied groups with the same age. but comparing children and elderly there were differences in all performed measurements. conclusion: the eyebrow external angle positions showed differences between children and elderly with a positive association between eyebrow position and age.

Abstract: purpose: to evaluate the possibility of epiphora relief in congenital nasolacrimal obstruction according to the age of the beginning of epiphora complaint and treatment effectiveness. methods: forty children with congenital nasolacrimal obstruction underwent treatment between 1997 and 1999 and were evaluated according to: age of start of epiphora, the results of the treatment and the possibility of successful outcome. the data were submitted to statistical analysis by the binomial proportional test between and within populations. results: the success rate was worst when the epiphora started after the age of 4 months and results were similar on massage and probing, cure occurring even beyond 3 years of age. conclusion: according to our results, the relief of epiphora was worst when starting at age over 4 months. massage and probing may have good results even in children older than 3 years.

Abstract: purpose: to evaluate the characteristics of the essential blepharospasm and hemifacial spasm patients and the feasible treatment with botulinum toxin a. methods: thirty-four essential blepharospasm or hemifacial spasm patients were evaluated according to gender, ocular complaint, time of disease, treatment outcome and complications. results: age median was 63 years and the mean was 61 years, with no difference regarding sex; 66.66% of the patients had hemifacial spasm and 33.33%, essential blepharospasm. many patients complained of dry eye associated with involuntary spasm. botulinum toxin a showed a positive outcome in 91.30% of the treated patients and complications observed after treatment were eyelid ptosis (8.33%) and buccal angle deviation (8.33%). conclusion: essential blepharospasm and hemifacial spasm occurred in the elderly, of both sexes. treatment with botulinum toxin a was useful, with very low complication rates.

Abstract: purpose: to determine the characteristics of patients with conjunctival pterygia in the botucatu region (s?o paulo state). methods: a retrospective study was conducted to evaluate the gender, occupation, complaints and lesion characteristics (primary or recurrent lesion, size, involutive or pulpous) of patients with pterygium. the data were submitted to statistical analysis. results: approximately half of the patients were female and over 40 years old. the main complaint was the esthetic effect of the lesion. the majority of the lesions were primary (77.08%), grade 2 (69.6%) and pulpous (86.7%). pterygium grade 4 was present in the 1.4% of the patients. conclusion: most patients were female, over 40 years old. primary, grade 2 and pulpous lesion was the most frequently observed. blindness caused by pterygium is still present in brazil.
Inhaled medication for asthma management: evaluation of how asthma patients, medical students, and doctors use the different devices

Abstract: asthma results from a combination of three essential features: airflow obstruction, hyperresponsiveness of airways to endogenous or exogenous stimuli and inflammation. inadequacy of the techniques to use different inhalation devices is one of the causes of therapeutic failure. the main purpose of this study was to evaluate how 20 medical students, 36 resident physicians of internal medicine/pediatrics, and 40 asthma patients used three devices for inhalation therapy containing placebo. all patients were followed at the pulmonary outpatient service of botucatu medical school and had been using inhaled medication for at least six months. the following devices were evaluated: metered dose inhalers (mdi), dry powder inhalers (dpi), and mdi attached to a spacer device. a single observer applied a protocol containing the main steps necessary to obtain a good inhaler technique to follow and grade the use of different devices. health care professionals tested all three devices and patients tested only the device being used on their management. mdi was the device best known by doctors and patients. mdi use was associated with errors related to the coordination between inspiration and device activation. failure to exhale completely before inhalation of the powder was the most frequent error observed with dpi use. in summary, patients did not receive precise instruction on how to use inhaled medication and health care professionals were not well prepared to adequately teach their patients.

Abstract: purpose: to evaluate the characteristics of patients with anophthalmic cavity who developed sphere extrusion. methods: a retrospective observational study was done evaluating 37 patients with anophthalmic cavity and sphere extrusion at the "faculdade de medicina de botucatu-unesp". results: extrusion was observed in enucleated and eviscerated cavities. the majority of the patients had the eye removed because of phthisis bulbi or trauma and the extrusion happened 1 or 2 years after the surgery. extrusion was preceded by conjunctival dehiscence and exposure of the sphere and occurred with all used implants. conclusion: complications after orbital implant placement are a possibility. dehiscence and sphere extrusion may happen and another surgery would be necessary. the patient and the ophthalmologist have to be prepared for this.
