Valida??o do "King's Health Questionnaire" para o português em mulheres com incontinência urinária

Abstract: objective: to translate into portuguese and evaluate the condition-specific quality of life king's health questionnaire (khq) for female urinary incontinence. methods: two brazilian translators, aware of the aim of the project, prepared two versions of the khq into portuguese, which were back-translated into english by two other english translators. the differences were harmonized and pre-tested in a pilot study. the final version of the khq and the "short-form health survey" (sf-36), which has already been translated and validated into portuguese were simultaneously administered to 156 and 119 women respectively. khq's psychometric properties such as reliability (internal consistency and retest) and construct validity were tested. a retest was performed within 2 weeks from the start date. results: the cultural adjustment process resulted in no changes in the khq portuguese version, although for low schooling patients the questionnaire had to be read by the researcher during face-to-face interview. for all other patients, the khq was self-administered. khq's standardized cronbach's alpha was 0.87 and when assessed by domains ranged from 0.49 to 0.92. reliability measured by intraclass correlation (icc) was considered moderate to strong for all domains and the severity measure scale ranged from 0.53 to 0.81. pearson′s correlation coefficient between khq and sf-36 was considered weak to moderate in the majority of the related domains, ranging from -0.27 to -0.53. conclusions: the khq portuguese version was translated and adjusted for brazilian women with urinary incontinence complaints. it represents an important tool for the assessment of incontinent women in clinical trials.
MALE INCONTINENCE: A CRITICAL REVIEW OF THE LITERATURE

Abstract: El objetivo fue identificar y analizar estudios sobre la calidad de vida y experiencias de hombres con incontinencia urinaria. Para la revisión crítica de la literatura, los datos fueron tratados con la técnica de análisis de contenido y analizados en las referencias psicodinámicas. Los resultados se agruparon en dos temas: Aspectos psicosociales vividos por hombres con incontinencia urinaria; La experiencia de los hombres en manejar la incontinencia urinaria. Los hombres incontinentes viven con sentimientos de estigma asociados con imagen de un cuerpo deteriorado. Buscan tratamiento cuando la incontinencia es peque a y buscan mecanismos psicológicos y sociales para adaptarse a incontinencia. El estudio muestra que los hombres incontinentes tienen bajas expectativas cuanto a tratamiento debido a la falta de conocimiento sobre las terapias existentes y las estrategias para controle de la pierda de orina.

Abstract: O objetivo deste estudo foi traduzir e adaptar à cultura brasileira os instrumentos The O'Leary-Sant e PUF, utilizados no diagnóstico de cistite intersticial. Foram realizadas as etapas metodológicas recomendadas pela literatura internacional para a adapta o cultural. As etapas de tradu o, síntese das tradu es e retrotradu o foram realizadas satisfatoriamente, e a avalia o das vers es sintéticas pelo comitê de especialistas resultou em algumas altera es, assegurando as equivalências entre as vers es originais e traduzidas. O PUF foi pré-testado entre 40 sujeitos e The O'Leary-Sant em uma amostra de 50 indivíduos, devido à necessidade de ajustes em decorrência da baixa escolaridade da popula o. O processo de tradu o e adapta o foi realizado com sucesso e os instrumentos, após as modifica es, demonstraram ser de fácil compreens o e rápido preenchimento. Entretanto, este é um estudo que antecede o processo de valida o e será premente o emprego do instrumento em novas pesquisas para que sejam avaliadas suas propriedades psicométricas. Se objetivó traducir y adaptar a la cultura brasile a los instrumentos The O'Leary-Sant y PUF, utilizados para diagnosticar cistitis intersticial. Fueron efectuadas las etapas metodológicas recomendadas por la literatura internacional para adaptación cultural. Las etapas de traducción, síntesis de traducciones y retrotraducción se realizaron satisfactoriamente, la evaluación de las versiones sintetizadas por parte del comité de especialistas derivó en algunas alteraciones, asegurando las equivalencias entre versiones originales y traducidas. El PUF fue pre-testeado con 40 sujetos y The O'Leary-Sant en muestra de 50 individuos, por la necesidad de ajustes derivados de la baja escolarización de la población. El proceso de traducción y adaptación se efectuó con suceso y los instrumentos luego de las modificaciones demostraron ser de sencilla comprensión y rápido completado. Sin embargo, este estudio es previo al proceso de validación, será imprescindible el uso del instrumento en nuevas investigaciones para evaluar sus propiedades psicométricas. Abstract: OBJETIVO: Traduzir e adaptar para o português o questionário de qualidade de vida denominado "King's Health Questionnaire" (KHQ) em mulheres com incontinência urinária. MéTODOS: Cientes dos objetivos da pesquisa científica, dois tradutores brasileiros prepararam duas vers es do KHQ para o português, as quais foram retro-traduzidas por outros dois tradutores ingleses. As diferen as foram harmonizadas e pré-testadas em um estudo piloto. As vers es finais do KHQ e de outro questionário, o "Short-Form Health Survey" (SF-36), já vertido e publicado em português, foram simultaneamente administradas a 156 e 119 pacientes, respectivamente. Foram testadas as propriedades psicométricas do KHQ como confiabilidade (consistência interna e teste-reteste) e validade de constructo. O reteste foi realizado em um período de duas semanas, a partir da primeira entrevista. RESULTADOS: O processo de adapta o cultural n o alterou a vers o em português do KHQ comparado ao original, exceto no modo de administra o para pacientes com baixo grau de alfabetiza o. Neste caso, o questionário mudou de auto-avalia o para ser lido para as pacientes durante entrevista com o pesquisador. Para as outras pacientes, o KHQ foi auto-administrado. O alfa de Cronbach padronizado do KHQ foi de 0,87 e avaliado por seus domínios variou de 0,49 a 0,92. A confiabilidade, medida pelo índice de correla o intraclasses (ICC) foi considerada de moderada a forte em todos os domínios e na escala de medidas de gravidade, variando de 0,53 a 0,81. O coeficiente de correla o de Pearson entre o KHQ e o SF-36 foi considerado de fraco a moderado na maioria dos domínios afins, variando de -0,27 a -0,53. CONCLUS ES: A vers o para o português do KHQ, traduzida e adaptada para seu uso em mulheres brasileiras com queixas de incontinência urinária. Representa um importante instrumento para a avalia o de mulheres incontinentes em pesquisa clínica.

Abstract: OBJETIVO: Traduzir e validar para a língua portuguesa o questionário de qualidade de vida condi o-específico denominado International Consultation on Incontinence Questionnaire - Short Form (ICIQ-SF) em pacientes com incontinência urinária. MéTODOS: Duas tradu es independentes do ICIQ-SF foram feitas por brasileiros, fluentes na língua inglesa. Após harmoniza o das mesmas, a tradu o resultante foi retrotraduzida independentemente por dois nativos de países de língua inglesa. As diferen as foram harmonizadas e pré-testadas em um estudo piloto. A vers o final do ICIQ-SF para o português, bem como a vers o em português do King's Health Questionnaire (KHQ) foram aplicadas simultaneamente em 123 pacientes consecutivos com queixa de incontinência urinária (29 homens e 94 mulheres) que procuraram o laboratório de uroginecologia e o servi o de urodinamica de um hospital universitário, localizado em Campinas. Foram testadas as propriedades psicométricas do questionário, como confiabilidade e validade de constructo. RESULTADOS: A idade mediana foi de 53 anos (intervalo de 16 a 86 anos). O período médio de reteste para o ICIQ-SF foi de 14,37 dias (intervalo de seis a 41 dias). Nenhuma altera o do formato original do ICIQ-SF foi observada no final do processo de tradu o e adapta o cultural. A consistência interna foi alta, como demonstrado pelo coeficiente alfa de Cronbach (0,88). O resultado do teste-reteste foi considerado de moderado a forte, como indicado pelo índice Kappa ponderado, cujos valores variaram de 0,72 a 0,75, e o coeficiente de correla o de Pearson que foi de 0,89. A correla o entre o ICIQ-SF e o KHQ foi considerada de moderada a boa para a maioria dos itens, variando de 0,44 a 0,77. A avalia o das validades de constructo e concorrente foi também satisfatória e estatisticamente significante. CONCLUS ES: A vers o para o português do ICIQ-SF foi traduzida e validada com sucesso para aplica o em pacientes brasileiros de ambos os sexos, com queixa de incontinência urinária, apresentando satisfatória confiabilidade e validade de constructo.
Administration of BAY 41-2272 prevents bladder dysfunction in nitric-oxide deficient rats

Abstract: Objective: to evaluate the protective effects of BAY 41-2272, asoluble guanylate cyclase activator, on changes in cystometricparameters in rats deficient in nitric oxide (NO). Methods: Ratswere divided into the following groups: (a) control; (b) DMSO; (c)L-NAME; (d) BAY 41-2272 alone; (e) L-NAME + BAY 41-2272.The NO synthase blocker L-NAME (20 mg/rat/day) was given indrinking water concomitantly or not with BAY 41-2272 (10 mg/kg/day, given by gavage). Results: Chronic L-NAME treatmentmarkedly increased the mean arterial blood pressure, and cotreatmentwith BAY 41-2272 nearly reversed L-NAME-inducedrise on mean arterial blood pressure. Non-void contractions weresignificantly increased in L-NAME group (0.90 ± 0.1 number/minute) compared with either DMSO or control group (0.49 ± 0.1number/minute), which were prevented by co-treatment with BAY41-2272 (0.56 ± 025 number/minute; p < 0.05). The thresholdand peak pressure increased by 70 and 44%, respectively, afterchronic L-NAME treatment, while co-treatment with BAY 41-2272largely attenuated both effects (27 and 22% increase, respectively).The frequency of micturition cycles decreased by about of 50%in L-NAME-treated rats compared with control animals, andco-treatment with BAY 41-2272 normalized this parameter.Conclusions: Our data show that long-term oral administrationof BAY 41-2272 counteracts the bladder dysfunction seen inNO-deficient rats, indicating that restoration of the NO-cGMPpathway by this compound may be of beneficial value to treatbladder symptoms.
Impact of treadmill gait training with neuromuscular electrical stimulation on the urodynamic profile of patients with high cervical spinal cord injury

Abstract: Objective: To evaluate the impact of gait training with neuromuscular electrical stimulation on urodynamic parameters of patients with neurogenic bladder. Methods: Eight male quadriplegic patients with complete cervical injury level ranging from C4 to C7 comprised the study population. They underwent treadmill gait training with neuromuscular electrical stimulation for six months, only after having their quadriceps and tibialis anterior muscles stimulated for five months in order to support at least 50% of their body weight (pre-gait training). Urodynamic testing was performed before the treadmill gait training and six months after. Results: The mean time after cervical lesion was 74.63 months. The urodynamic parameters before and after neuromuscular training by electrical stimulation did not show significant difference. Conclusion: This study demonstrated that neuromuscular training with electrical stimulation can benefit the urinary tract. This promising minimally invasive field requires further and more complete studies to confirm a possible benefit to the low urinary tract.
Laparoscopic treatment of lymphoceles after renal transplantation

Abstract: objective: lymphocele formation following renal transplantation is a frequent complication and may affect as many as 49% of patients. operative treatment of symptomatic post transplant lymphocele (ptl) consists of wide drainage of the fluid collection into the abdominal cavity by excising its wall, connecting the lymphocele cavity to the intraperitoneal space. laparoscopic fenestration seems to be the best treatment as it combines satisfying success rates with a minimally invasive approach. the aim of the study was to review a single center experience on the laparoscopic treatment of symptomatic ptl and detail relevant aspects of the surgical technique. materials and methods: the data of 25 patients who underwent laparoscopic surgical treatment for a symptomatic lymphocele following kidney transplantation were retrospectively reviewed. demographic data and surgical results were assessed. detailed surgical technique is provided. results: between 1996 and 2008, 991 patients received a kidney transplant at our institution. twenty-five patients (2.52%) developed a symptomatic lymphocele and laparoscopic drainage was performed. the indications for surgical drainage were graft dysfunction (84%), local symptoms (16%) or both (32%). the mean time until surgical therapy was 14.2 ± 6 weeks. mean hospital stay was 1.5 ± 0.2 days. postoperative complications occurred in only 2 patients (8%) (one ureteral injury and one incisional hernia) and required reoperation. after a mean follow-up of 36.2 ± 4 months, only 1 patient had a symptomatic recurrence. conclusions: laparoscopic fenestration is an effective surgical technique to treat symptomatic lymphocele following kidney transplantation with low recurrence rate and long standing results.

Abstract: objective: to translate into and validate for portuguese the "international consultation on incontinence questionnaire - short form" (iciq-sf), a condition-specific quality-of-life questionnaire for patients with urinary incontinence. methods: two brazilians independently translated the original iciq-sf into portuguese. these two translations were harmonized, and then checked by independent back-translation by two native english speakers. the harmonized translation was pre-tested in a pilot study on 20 patients. the final version of the iciq-sf in portuguese was applied to 123 consecutive patients aged 16 or over (29 males and 94 females) with a complaint of urinary incontinence, who had sought the department of urogynecology and uroneurology of the school of medical sciences of unicamp. the portuguese version of the king's health questionnaire (khq) was also applied to the same group. the psychometric properties of the questionnaire, such as reliability and construct validity were assessed. results: the median age was 53 years (range: 16 to 86). the mean retest interval for the iciq-sf was 14.37 days (range: 6 to 41). no changes from the original format of the iciq-sf were observed at the end of the process of translation and cultural adaptation. the internal consistency was high (0.88), as measured by the cronbach alpha coefficient. the test-retest value was considered moderate to strong, as measured by the weighted kappa index (range: 0.72 to 0.75) and pearson correlation coefficient (0.89). the correlation between the iciq-sf and khq was considered to be moderate to good for most items (range: 0.44 to 0.77). the evaluation of the construct and concurrent validity was also satisfactory and statistically significant. conclusions: the "international consultation on incontinence questionnaire" (iciq-sf) was successfully translated into portuguese and validated for application to brazilian female and male patients complaining of urinary incontinence, with satisfactor
A comparative study of pelvic floor muscle training in women with multiple sclerosis: its impact on lower urinary tract symptoms and quality of life

Abstract: objective: to compare pelvic floor muscle training and a sham procedure for the treatment of lower urinary tract symptoms and quality of life in women with multiple sclerosis. methods: thirty-five female patients with multiple sclerosis were randomized into two groups: a treatment group (n = 18) and a sham group (n = 17). the evaluation included use of the overactive bladder questionnaire, medical outcomes study short form 36, international consultation on incontinence questionnaire short form, and qualiveen questionnaire. the intervention was performed twice per week for 12 weeks in both groups. the treatment group underwent pelvic floor muscle training with assistance from a vaginal perineometer and instructions to practice the exercises daily at home. the sham group received a treatment consisting of introducing a perineometer inside the vagina with no exercises required. pre- and post-intervention data were recorded. results: the evaluation results of the two groups were similar at baseline. at the end of the treatment, the treatment group reported fewer storage and voiding symptoms than the sham group. furthermore, the differences found between the groups were significant improvements in the following scores in the treatment group: overactive bladder questionnaire, international consultation on incontinence questionnaire short form, and the general quality of life, and specific impact of urinary problems domains of the qualiveen questionnaire. conclusions: the improvement of lower urinary tract symptoms had a positive effect on the quality of life of women with multiple sclerosis who underwent pelvic floor muscle training, as the disease-specific of quality of life questionnaires demonstrated. this study reinforces the importance of assessing quality of life to judge the effectiveness of a treatment intervention.
