Abstract: background: systemic lupus erythematosus (sle) patients have higher rates of morbility and mortality than general population because of inflammatory activity and high infections frequencies. the infections have been associated with activity index of the disease and the use of immunosuppressant drugs. objective: to determine the factors associated with nosocomial infections in sle patients who required in hospital treatment. methods: we performed a prospective study of sle patients who met the acr criteria and who were hospitalized between january 2001 and december 2004. exclusion criteria were community acquired infections, onset of infections in the first 48 hours of hospitalization, hiv and patients with neoplasm. the sle activity was quantified with the sledai index and infection was defined according to microorganism isolated or cdc criteria for those without an isolated microorganism. we studied this variables: age, sex, time from onset of sle, hospital stay, use of immunosuppressant drugs, type of infection, and cause of death. results: one hundred and twenty three patients were included who had 155 hospital admissions, 106 were women (82.2%). mean age was 27.7 +/- 12.6 years (7-73) with a mean time from diagnostic of 27.8 +/- 49.4 months (0-300), 56 patients (45.5%) had novo diagnosis, 25 patients had chronic renal insufficiency including 10 in hemodialysis. respect of previous treatment, 58 (47.1%) patients did not have received steroids, 36 (29.3%) have been receiving 20 mg of prednisone or less, and 29 (23.6%) more than 20 mg of prednisone, 31 (25.2%) patients pulses of ciclophosphamide and 13 others immunosuppressant drugs. the sledai median and mean were 14 (iqr 7-20) and 14.7 +/- 9.9 respectively. nosocomial infection was reported in 38 (30.9%), 31 patients just one episode of infection and 13 more than one, with a total of 69 events. in univariate analysis was noted that these factors were associated with nosocomial infections: de novo diagnosis, longe

Abstract: this paper presents an introductory discussion on the relationship between electoral democracy and the quality of governments. it briefly reviews the main conceptual dilemmas faced by the contemporary literature when trying to define the term "quality of government", and it proposes a definition that underlines the impartiality of governmental processes, the responsiveness of governments, and their effectiveness. based on such definition, the paper analyzes the complex relationship between democratic elections and quality of government from a theoretical perspective. finally, it discusses the case of municipal governments in mexico to illustrate the problems of governmental quality at the local level.

Abstract: this paper analyzes the influence of the electoral environment on the development of institutional capacities at the level of local government in mexico. drawing on the assumption that establishing professional, merit-based and impersonal administrative systems entails political costs to government leaders and their electoral activists, the article investigates whether electoral competition and party alternation have stimulated the development of necessary institutional capacities in municipal governments. the empirical evidence seems to support both hypotheses. it also suggests the possibility that strong local political machines deter the ability of incumbent leaders to reduce clientelism in the operation of public bureaucracies

Abstract: Este trabajo analiza la influencia del entorno electoral sobre la creación de capacidades institucionales en el nivel local de gobierno en México. Partiendo del supuesto de que el establecimiento de sistemas administrativos profesionales, meritocráticos e impersonales conlleva costos políticos para los gobernantes y sus activistas electorales, el artículo investiga si la competencia electoral y la alternancia partidista han promovido el desarrollo de capacidades institucionales básicas entre los gobiernos municipales. La evidencia empírica parece apoyar ambas hipótesis. Evaluación inicial de la fertilización con roca fosfórica en tres especies del género brachiaria

Abstract: an experiment was carried out in yaracal municipality cacique manaure, state falcon, to measure the effect of the fertilization with four phosphorus levels (0 [control], 50, 100 and 200 kg p2o5/ha, on the biomass (dm production), height and relationship leaf/tiller(l/t) of brachiaria humidicola, b. decumbens and b. brizantha. the soil used shows a franc-oozy texture, acid ph and very low phosphorus content. a block design was used in with sub-divided plots; main plots: phosphorus, sub-plots: species and sub sub-plots: cut date. the phosphoric source comes from: phosphate rock coming found in riecito. the fertilization was completed with 100 kg n/ha and 100 kg k2o/ha. four cuts were carried out to the fourth and sixth week of the regrowth. the dm in p1, p2 and p3 was similar, but it registered significant differences (p<0.01) with the control (p1:2116, p2:2233, p3:2404 vs p0:1544 kg dm/ha/cut to the fourth week and p1:3495, p2:3744, p3:4107 vs p0:2,551 kg dm/ha/cut to the sixth week). the b. humidicola was superior (p<0,01) to that of b. brizantha and b. decumbens (2384, 1945 and 1895 kg dm/ha/cut respectively to the fourth week and 3919, 3246 and 3261 kg dm/ha/cut to the sixth week). likewise the cut date affected significantly (p<0.01) the dm. the height presented a similar behavior to the dm and the relationship l/t it diminished significantly (p<0.05) with the treatments with fertilization in relation to the control. in conclusion, the amount of phosphorus content in the fertilizer did not show a significant effect, the differences within the control are attributable mainly to the effect of the nitrogen content.

Abstract: Factores de riesgo para infección nosocomial en pacientes con lupus eritematoso sistémico en un Hospital universitario en Medellín, Colombia. Estudio prospectivo: 2001-2004 Introducción: los pacientes con lupus eritematoso sistémico (LES) tienen mayor morbilidad y mortalidad que la población general y ello está determinado por la actividad inflamatoria y la frecuencia de infecciones, las cuales participan en la presentación bimodal de la mortalidad de los pacientes con LES. Las infecciones se han asociado con la actividad de la enfermedad y con el uso de inmunosupresores. Objetivo: determinar los factores asociados con infección nosocomial en pacientes con LES que requirieron tratamiento intrahospitalario. Específicamente buscamos establecer asociación con actividad al ingreso y posterior desarrollo de la infección. Métodos: es un estudio de cohorte prospectivo y descriptivo cuya población está constituida por pacientes con LES de acuerdo a los criterios del ACR y que se hospitalizaron entre enero de 2001 y diciembre de 2004. Se excluyeron pacientes con infección adquirida en la comunidad o que la manifestaran dentro de las 48 horas siguientes al ingreso, pacientes con neoplasias y HIV. La actividad se cuantificó con la aplicación del SLEDAI y la infección fue definida con base en el aislamiento del germen y según los criterios del CDC de Atlanta, para los casos sin confirmación microbiológica. Se consideraron variables como edad, género, tiempo de diagnóstico, tiempo de estancia, uso de corticoides, ciclofosfamida (CFM), otros inmunosupresores, tipo de infección, agente y causa de mortalidad. Resultados: ingresaron al estudio 123 pacientes que tuvieron 155 hospitalizaciones, de ellos 106 (82,2%) mujeres, con un promedio de edad de 27,7+-12,6 a os (7-73) con tiempo de diagnóstico de 27,8+-49,4 meses (0-300), 56 pacientes (45,5%) con diagnóstico de novo, 25 pacientes estaban en insuficiencia renal crónica (IRC) con 10 de ellos en terapia dialítica. Con respecto al tratamiento 58 (47,1%) no habían recibido corticoides, 36 (29,3%) recibían una dosis menor de 20 mg/d de prednisona y 29 (23,6%) una dosis mayor, 31 (25,2%) habían recibido bolos de CFM y 13 (10,6%) otros inmunosupresores. El promedio y mediana de SLEDAI fueron de 14,7+-9,9 y 14 (IQR 7-20) respectivamente. Comparison between logistic regression and neural networks to predict death in patients with suspected sepsis in the emergency room

Abstract: The study population comprised patients with suspected bacterial infection as their main diagnosis for admission to the emergency room at two University-based hospitals. Mortality within the first 28 days from admission was predicted using logistic regression with the following variables: age, immunosuppressive systemic disease, general systemic disease, Shock Index, temperature, respiratory rate, Glasgow Coma Scale score, leucocyte counts, platelet counts and creatinine. Also, with the same input and output variables, a probabilistic neural network was trained with an adaptive genetic algorithm. The network had three neurone layers: 10 neurones in the input layer, 368 in the hidden layer and two in the output layer. Calibration was measured using the Hosmer-Lemeshow goodness-of-fit test and discrimination was determined using receiver operating characteristic curves.A total of 533 patients were recruited and overall 28-day mortality was 19%. The factors chosen by logistic regression (with their score in parentheses) were as follows: immunosuppressive systemic disease or general systemic disease (2), respiratory rate 24–33 breaths/min (1), respiratory rate ≥ 34 breaths/min (3), Glasgow Come Scale score ≤12 (3), Shock Index ≥ 1.5 (2) and temperature <38°C (2). The network included all variables and there were no significant differences in predictive ability between the approaches. The areas under the receiver operating characteristic curves were 0.7517 and 0.8782 for the logistic model and the neural network, respectively (P = 0.037).A predictive model would be an extremely useful tool in the setting of suspected sepsis in the emergency room. It could serve both as a guideline in medical decision-making and as a simple way to select or stratify patients in clinical research. Abstract: this paper presents the analysis of the problems in social security and health system due to the relations of the system with its users; problems that have been neglected in the studies and in the analyses on this subject. the determining criteria and aspects that define the election of health promotion entities (eps) and health providing institutions (ips ) by the users of bogota are analyzed whereby free election and an informed decision are pointed out. the analysis of the qualitative and quantitative results allows concluding that the available information and the exercise of the free election are limited.
The Legislative Power Participation in Mexican Foreign Policy in a Democratic Opening Scenario

Abstract: For most of the twentieth century, the environment in which the Mexican foreign policy (PEM) was developed was characterized by the existence of rigid structures –internationally the Cold War and at internal level the one-party system-. In this context, a "tradition" in the way of conducting the international relations of Mexico was forged, not only in regard to the principles that guide it, but also in its formulation, review and analysis. However, since the late eighties, that context began to change it is therefore relevant to ask what were the consequences of the increasing electoral competition in Mexico for the PEM? This article aims to answer that question by analyzing how the new political scenario has allowed the participation of new actors in the PEM. Emphasis is placed on the participation of the legislative branch through initiatives of law and points of agreement.
