Abstract: a descriptive observational study of a 36-year-old patient that suffers from dyspnoea, easy fatigue, and chest pain on exertion was conducted aimed at presenting a rare disease in which the clinical data of right heart failure should be taken into consideration. it was evaluated and analyzed the radiological study carried out 10 years ago that helped to make the diagnosis of chronic calcified constrictive pericarditis and which was not observed at that time.

Abstract: Se exponen criterios de los autores sobre los cambios que impone la municipalización de la ense anza a la asignatura Propedéutica Clínica. Cambios en el contenido dados por una morbilidad desplazada al período preclínico y de riesgo de los síndromes, por la edad más amplia del paciente en la atención primaria con respecto al de los hospitales de adultos, que tampoco incluyen embarazadas. Se plantearon retos a afrontar en la dirección del proceso docente y algunas vías para conciliar las ventajas de la municipalización con las dificultades de un período de cambio. Se trazan, como estrategias principales, el trabajo metodológico y el aprendizaje propedéutico y clínico como estrategia curricular. The author's criteria on changes required by teaching municipalization to Propedeutics Clinic as subject are exposed as well as the changes in content due to a shifted morbidity to preclinical period and the syndrome risks by a broader patient's age in primary care regarding that of adult hospitals without inclusion of pregnants. Challenges to be faced in Teaching Process Management and some ways to reconcile the municipalization advantages and the difficulties of a change period are posed. The main strategies were: the methodological work and propedeutic and clinical learning as a curriculum strategy.

Abstract: Se realizó un trabajo descriptivo observacional de un paciente de 36 a os de edad que desde hace unos 13 a os padece de disnea al realizar esfuerzo físico, fatiga fácil y dolor torácico, con el objetivo de presentar una enfermedad poco común y en la cual hay que pensar ante los datos clínicos de insuficiencia cardíaca derecha. Se evaluó y se analizó el estudio radiológico de hace 10 a os que ayudaba al diagnóstico de pericarditis constrictiva crónica calcificada, el cual pasó inadvertido en aquel momento. Abstract: Con el propósito de explorar la correspondencia entre los procederes técnicos programados, los patrones de utilización práctica y los criterios sobre cuáles deben ser objeto de entrenamiento para el internista durante la residencia, se encuestó un grupo de residentes, especialistas y docentes, durante el mes de noviembre del 2002. Los resultados del estudio permitieron identificar procederes con diferentes grados de abordaje en la residencia. Hubo pobre correspondencia entre los procederes programados, los patrones de utilización práctica y los criterios sobre los procederes que deben ser objeto de entrenamiento para el internista, excepto en los casos de la punción lumbar, la lectura de electrocardiograma, la paracentesis, la pleurocentesis, el fondo de ojo y la resucitación cardiopulmonar, el resto de los procederes mencionados deben ser objeto de un análisis particularizado. In order to explore the relation existing among the technical programmed procedures, the patterns of practical utilization and the criteria on which of them should be object of training for the internist during the residency, a group of residents, specialists and professors were surveyed in November 2002. The results of the study allowed to identify procedures with different approach degrees in the residency .There was a poor correspondence beetween the programmed procedures, the patterns of practical utilization and the criteria on the procedures in which the internist should be trained, excepting those cases of lumbar puncture, the reading of the electrocardiogram, paracentesis, pleurocentesis, fundus of the eye, and cardiopulmonary resuscitation. The rest of the mentioned procedures should be particularly analyzed.

Abstract: Se exponen criterios de los autores, obtenidos tras revisión de la literatura y su experiencia docente, sobre las conferencias, en forma de orientaciones metodológicas. De forma secuencial se exponen como ejemplo, elementos del Síndrome de inflamación articular en Propedéutica Clínica, excluyendo los aspectos propios del estudio de entidades nosológicas. Authors' criteria are expounded obtained after the review of literature and their teaching experience on the lectures in the shape of methodological guiding. In a sequential way as example authors expose the elements of the articular inflammation syndrome in Clinical Propedeutics ruling out the features typical of Nosology entities study.

Abstract: in order to explore the relation existing among the technical programmed procedures, the patterns of practical utilization and the criteria on which of them should be object of training for the internist during the residency, a group of residents, specialists and professors were surveyed in november 2002. the results of the study allowed to identify procedures with different approach degrees in the residency .there was a poor correspondence beetween the programmed procedures, the patterns of practical utilization and the criteria on the procedures in which the internist should be trained, excepting those cases of lumbar puncture, the reading of the electrocardiogram, paracentesis, pleurocentesis, fundus of the eye, and cardiopulmonary resuscitation. the rest of the mentioned procedures should be particularly analyzed.

Abstract: the medical interview is the most powerful diagnostic tool for physicians, since it is the most important source for obtaining primary data. the aim of this paper was to determine the most frequent deficiencies found in the medical interview made by students from the second semester of the 3rd year of the medical career. a descriptive study was conducted during the academic courses 1999-2000 and 2000-2001. of a total of 99 students, the medical interview was recorded to 50 of them. the collected data were compared with what was witten in the history of the present disease they were evaluated by a survey that classified the imperfections as regards the history of the present disease, types of questions and general aspects. 383 difficulties were found (17.0 % of a possible total of 2 250). 168 (43,8 %) were connected with the history of the present disease, 153 (40.0 %) with the general aspects of the development of the interview and there were 62 mistakes (16.2 %) in relation to the type of question. many deficiencies were observed in the medical interview made by students not only concerning the domain of semiological abilities but also in the interpersonal communication.

Abstract: La entrevista médica es la herramienta diagnóstica más poderosa con que cuenta el médico, constituye la fuente más importante de la obtención de datos primarios. El objetivo del trabajo fue determinar las deficiencias más frecuentes encontradas en la entrevista médica por alumnos del segundo semestre de tercer a o de la carrera de medicina. Se realizó un trabajo descriptivo durante los cursos académicos 1999-2000 y 2000-2001, de un total de 99 alumnos se les grabó la entrevista médica a 50. Se compararon los datos recogidos con lo transcripto en la historia de la enfermedad actual, estos fueron evaluados mediante una encuesta que clasificó las imperfecciones con respecto a la historia de la enfermedad actual, tipos de preguntas y aspectos generales. Se encontraron 383 dificultades (17,0 % de un total posible de 2 250); en relación con la historia de la enfermedad actual fueron 168 (43,8 %); en relación con los aspectos generales del desarrollo de la entrevista, 153 (40,0 %) y con el tipo de pregunta, 62 errores (16,2 %). Se presentaron numerosas deficiencias en el interrogatorio médico realizado a pacientes por los alumnos y estas fueron tanto en el dominio de las habilidades semiológicas como en las de comunicación interpersonal.
