Entomological investigation of a sylvatic yellow fever area in S o Paulo State, Brazil

Abstract: Following reports of two autochthonous cases of sylvatic yellow fever in the State of S o Paulo, Brazil, in 2000, entomological surveys were conducted with the objective of verifying the occurrence of vector species in forest environments close to or associated with riparian areas located in the western and northwestern regions of the State. Culicidae were captured in 39 sites distributed in four regions. Haemagogus leucocelaenus and Aedes albopictus were the most abundant species and were captured in all the regions studied. H. leucocelaenus was the most abundant species in the municipalities of Santa Albertina and Ouroeste, where the two cases of sylvatic yellow fever had been reported. Mosquitoes from the janthinomys/capricornii group were only found at eight sites in the S o José do Rio Preto region, while Sabethes chloropterus was found at one site in Ribeir o Preto. H. leucocelaenus showed its capacity to adapt to a secondary and degraded environment. Our results indicate a wide receptive area for yellow fever transmission in the State of S o Paulo, with particular emphasis on the possibility of H. leucocelaenus being involved in the maintenance of this sylvatic focus of the disease.

Abstract: the study used environmental parameters associated with standardized coefficients incidence of american tegumentary leishmaniasis (atl) in the state of sao paulo, brazil, between1986 and 1995. in the 140 municipalities with leishmaniasis transmission, the entomological collection of sandflies showed the most frequent species collected in the domestic environment: lutzomyia intermedia was present in 87.7% of the surveyed municipalities, l. whitmani in 53.6%, l. migonei in 49.7%, l. pessoai in 28.5% and l. fischeri in 53.6%. it was found that the topographical relief and natural vegetation cover significantly influenced the mean values of standardized coefficients of accumulated incidence of atl in the state (p < 0.001). multivariable linear regression analysis showed that incidence of the disease was significantly associated with the presence of l. migonei (p = 0.029), in the municipalities of the atlantic plateau geomorphological region (p = 0.005) and those in which predominant vegetation cover was type v forest (p < 0.001). this analysis reinforces the hypothesis that l. migonei is a vector for leishmania in s?o paulo state.

Abstract: O estudo utilizou parametros ambientais associados aos coeficientes padronizados de leishmaniose tegumentar americana (LTA), no período de 1986 a 1995. Nos 140 municípios com transmiss o da doen a a pesquisa entomológica realizada mostrou que as espécies mais freqüentes capturadas no ambiente domiciliar, foram Lutzomyia intermedia em 87,1% dos municípios, 53,6% L. whitmani, 49,7% L. migonei, 28,5% L. pessoai e 53,6% L. fischeri. Verificou-se que as variáveis tipos de relevo e de cobertura vegetal natural influíram, significativamente (p < 0,001), nas médias dos coeficientes padronizados de incidência média acumulada da LTA no estado. A análise de regress o linear múltipla mostrou que a incidência da doen a esteve associada, significativamente (p = 0,029), à presen a de L. migonei nos municípios situados na regi o geomorfológica do Planalto Atlantico (p = 0,005) e, naqueles cuja cobertura vegetal predominante foi Tipo V - mata (p = 0,000). Esta análise resgata a discuss o sobre o papel vetorial de L. migonei no Estado de S o Paulo.

Abstract: O controle da Leishmaniose Visceral Americana (LVA) está fundamentado no combate ao vetor e na elimina o do reservatório doméstico da área de foco - 200 metros em torno do caso humano ou canino. O presente trabalho tem como objetivo discutir a utiliza o de técnicas de análise espacial na vigilancia epidemiológica da LVA no Município de Ara atuba, S o Paulo, buscando estabelecer um modelo de vigilancia epidemiológica em base territorial, redirecionando as estratégias de controle atualmente adotadas. Verificou-se que a transmiss o da LVA n o foi homogênea no município: a transmiss o humana ocorreu nas áreas com maiores taxas de prevalência canina. A dispers o do vetor parece restrita a poucos domicílios, embora n o tenha sido possível estabelecer um perfil da densidade vetorial. Visando estudar a distribui o do vetor e variáveis correlacionadas está sendo realizado estudo de campo por amostragem de domicílios, que permitirá o desenvolvimento de novas ferramentas de análise espacial e, possivelmente, permitirá redefinir as propostas de controle da endemia em ambiente urbano.

Abstract: the control of american visceral leishmaniasis (avl) is based on combating the vector and eliminating the domestic reservoir of the focus area - defined as 200 meters around human or canine cases. this paper discusses the use of spatial analysis techniques in the epidemiological surveillance of avl in ara?atuba, s?o paulo state, in order to propose a model for territorial epidemiological surveillance, reformulating current control strategies. the results showed that avl transmission was not homogeneous; human cases were more frequent in areas with higher canine prevalence rates. vector dispersion appeared to be restricted to a few houses, although it was not possible to model the vector density. in order to study the vector distribution and correlated covariates, a field study based on house sampling is being conducted. the results will aid the development of new spatial analysis tools and possibly redefine protocols and routines for the control of this endemic disease in urban areas.
IgM antibodies to Schistosoma mansoni gut-associated antigens for the study of schistosomiasis transmission in Ribeir o Pires, S o Paulo

Abstract: The potential of an immunofluorescence test for detection of IgM antibodies against Schistosoma mansoni gut-associated antigens (IgM-IFT) was evaluated as a tool for studying aspects related to the schistosomiasis transmission in Ribeir o Pires, in the metropolitan area of the capital of the State of S o Paulo, Brazil. Children from a school with about 400 students, 6 to 18 years, were followed-up for two years. In the five surveys, carried out at 6-month intervals, from October 92 to October 94, serological (IgM-IFT) prevalence indices of 5.3%, 5.8%, 6.2%, 2.9% and 3.3% were obtained. These indices were 7 to 10 times higher than the parasitological prevalence indices of 0.5%, 0.5%, 0.7%, 0.4% and 0% determined by the Kato-Katz method. Seroconversion from IFT negative to positive was indicating possible newly acquired S. mansoni infection in three children. But confirmation of infection by fecal examination was possible in only one child. The IgM-IFT can constitute a valuable tool for the improvement of the vigilance program in low endemic areas for schistosomiasis, better characterizing the S. mansoni transmission in such areas.
American visceral leishmaniasis: disease control strategies in dracen microregion in alta paulista, SP, Brazil

Abstract: despite measures adopted to control american visceral leishmaniasis (avl), the disease is spreading in a fast and worrying way throughout western s?o paulo state. the aim of this work was to study the variables involved in the disease cycle as well as the effectiveness of controlling measures. the study was carried out in the microregion of dracena, which is composed of twelve cities and belongs to alta paulista, a region of western s?o paulo. the necessary data were provided by the superintendence for endemic disease control and adolfo lutz institute, regional laboratory of presidente prudente. from august 2005 to january 2008, the following factors were observed: detection of phlebotomine sandflies in the cities and periods in which dogs or humans were diagnosed; number of human deaths; prevalence of suspected dogs tested by serology; percentage of euthanasia in suspected dogs; a possible correlation between positive dogs and cases of the disease in humans; and the disease prevalence among municipalities from the studied region. it was verified that, despite the strategies adopted in dracena microregion to control avl, the disease continues to rise. thus, some procedures of the avl monitoring and control program should be reviewed, to grant the initiative more credibility and effectiveness.

Abstract: the multiplicity of factors involved in the transmission of american cutaneous leishmaniasis (acl) make it difficult to formulate an efficient strategy for the control of the disease. on the basis of the number of notified cases per location, the state of s?o paulo detemines the application of ddt in houses in those areas where two or more human cases have occurred. this procedure, however, impairs part of the program because 40% of the counties report only one human case each. these and other factors are analyzed in the light of the increase in the incidence of the disease in the state, its advance in the extra-forest environment and the delay in the decision-making process about the efficacy of the program. it is recommended that the control strategy be reviewed.

O Estado de S o Paulo, considerando o número de casos notificados por localidade, estabelece aplica o de DDT em área com ocorrência de dois ou mais casos. Com isto, parte do programa fica afetado, pois em 40% dos municípios tem-se registrado apenas um caso humano. Esses e outros fatores s o analisados diante de um crescimento de distribui o da incidência no Estado e da evolu o da doen a em ambiente extraflorestal. Procedimentos administrativos com retardamento na tomada de decis o afetam a eficácia do programa. Daí, a necessidade de uma revis o na estratégia.
