Abstract: A través de la teoría del vínculo de Enrique Pichón Riviére nos proponemos rescatar una visión totalizadora acerca de la huída del método clínico de los principales escenarios de formación. Un mal ejercicio del método clínico parte de una resistencia al depósito. Abstract: an epidemiological, analytical and cross-sectional investigation was undertaken among 327 patients admitted with criterion of acute myocardial infarction with supradepression of st selected from a group of 363 patients with ami in the polyvalent intensive care unit of “joaquín albarrán” hospital from september 2004 to september 2006 in order to determine the factors associated with intrahospital mortality in this entity. the percentage statistical method and the chi square test were applied. mortality was higher in patients of advanced age (35.9 %). a significant association was observed between age (p=0.0159), sex (p=0.0232), diabetes (p=0.0012), dyslipidemia (p=0.0041) and arterial hypertension (p=0.0189) with the state on discharge. mortality was higher in patients with anterior localization of ami (18 %), electrical complications (29.2 %) and class iv killip-kimbal (83.3 %). a marked association was found between the non carrying out of thrombolysis (p=0.0499) and therapeutic retardation (p=0.0013) with the mortality of the patients under study.

Abstract: Se realizó una investigación epidemiológica, analítica y transversal en la Unidad de Cuidados Intensivos Polivalente (UCIP) del Hospital ¨Joaquín Albarrán¨ entre Septiembre de 2004 y Septiembre de 2006 a 327 pacientes ingresados con criterio de Infarto Agudo del Miocardio (IAM) con supradesnivel del ST, de un universo de 363 pacientes con IAM, con el objetivo de determinar los factores asociados a la mortalidad intrahospitalaria en esta entidad Se aplicó el método estadístico del porcentaje y chi cuadrado. La mortalidad fue mayor en pacientes de edad avanzada (35,9%). Se encontró asociación significativa entre la Edad (p=0,0159), el Sexo (p=0,0232), la Diabetes (p=0,0012), la Dislipidemia (p=0,0041) y la Hipertensión Arterial (p=0,0189) con el estado al egreso. La mortalidad fue mayor en los pacientes con localización anterior del IAM (18%), complicaciones eléctricas (29,2%) y clase Killip-Kimbal IV (83,3%), encontrándose asociación significativa entre la no realización de trombolisis (p=0,0499) y el retraso terapéutico (p=0,0013) con la mortalidad de los pacientes en estudio. Abstract: it has been stated that dementia is the fourth basic cause of death in first world countries. dementia with lewy bodies (dlb) is the second cause of dementia in current necropsic studies, only surpassed by alzheimer's disease. the first cuban case of dementia with lewy bodies was reported. a female white patient aged 89 with clinical diagnosis of dementia that died after 5 years of progressive deterioration. in the neuropathological study, the presence of lewy bodies at the level of the black matter, locus ceruleus and transentorhinal cortex was evidenced by using the haematoxylin/eosin technique. the neurofibrillar pathology assessed by bielschowsky technique corresponded to ii stage braak. a focal espongiform change was identified at the hippocampal level. the diagnosis of lbd variety with predominance of the brain stem was made. literature and the main differential diagnoses were reviewed.

Abstract: left ventricular pseudoaneurysm is an uncommon clinical entity mainly from a acute myocardial infarction (ama), this a severe complication and potentially mortal. however, in exceptional cases, rupture is content by an organized hematoma, pericardium and fibrous tissue. the early diagnosis is essential owing to high probability of a spontaneous rupture; echocardiography is the standard diagnosis for this entity. here is the case of a large pseudoaneurysm diagnosed in a patient with recent infarction.

Abstract: some studies have obtained vast information on psychosocial specific domains including depression, anxiety, character traits, and personality disorders and have demonstrated a clear evidence on the significant contribution of these psychosocial factors to expression and pathogenesis of coronary diseases. the objective of present review was to examine the new epidemiological evidences on the association between these psychosocial risk factors and the coronary arterial disease and to identify the main pathophysiological mechanisms present among these two entities. this approach is from the rise of a emergent field in behavioral cardiology, based on the understanding that these psychosocial and behavioral risk factors in coronary disease not only are closed related, but that also they require a team work among cardiologists, psychiatrists and other specialists of primary care.

Abstract: Objectives To determine the association of several parameters of arterial stiffness withdiastolic dysfunction and arterial hypertension in patients under 60 years.Methods: We conducted a cross-sectional study that included 83 patients (healthy,n=43; hypertensive, n=40) assisted in the external consultation of the NationalCardiology and Cardiovascular Surgery Institute, from April to October,2009. We included clinical (age, sex, personal antecedents of smokinghabit, obesity, hypercholesterolemia, arterial hypertension) and echocardiographic(diastolic function, arterial stiffness index [β], pressure strain elasticmodulus [Ep], arterial compliance, local pulse wave velocity [LPWV]) variables.Results Arterial stiffness index [β], Ep and LPWV were significantly greater in thegroup of hypertensive patients vs healthy subjects (p<0,001) and hypertensivepatients with diastolic dysfunction vs hypertensive with normal diastolicfunction (p<0,001). We demonstrated a proportional and significant increasein the presence of diastolic dysfunction in patients withLPWV≥5m/seg and more than 5 years of hypertension, when compared tothose with LPWV≥5m/seg and less than 5 years of hypertension, andLPWV<5m/seg independently of the years of hypertension (30, 0% vs 20,0% vs 12, 5% vs 2, 5%, p=0,036, respectively).Conclusions Arterial stiffness showed a significant association with the presence of arterialhypertension and diastolic dysfunction.
