Xantogranulomatosis orbitaria: Enfermedad de Erdheim-Chester

Abstract: case report: a patient was studied because of upper lid bilateral edema and xanthelasmae-like lesions after three years of evolution. during the ophthalmologic examination orange-yellowish lesions and two symmetrical tumours were observed on the temporal part of both upper lids. corticoid-therapy was undertaken which reduced the size of the tumours, however the size increased again after the discontinuation of treatment. a biopsy was performed and lid xanthogranulomatosis was diagnosed. other systemic examinations were normal. discussion: erdheim-chester disease is a xanthogranulomatosis that can affect ocular and periorbital structures. combination of xanthelasmae-like lesions and bilateral orbital masses should make us consider this process and try to locate any associated systemic conditions.

Abstract: Caso clínico: Paciente con edema bilateral de párpados superiores y lesiones similares a xantelasmas de tres a os de evolución. La exploración oftalmológica mostraba dos tumoraciones temporales simétricas en ambos párpados superiores con lesiones naranjo-amarillentas. Se pautaron corticoides que hicieron disminuir el tama o de las tumoraciones, con recidiva de los mismos al suspender dicha medicación. La biopsia reveló una xantogranulomatosis palpebral. El resto de las exploraciones sistémicas fueron normales. Discusión: La enfermedad de Erdheim-Chester es una xantogranulomatosis que puede afectar a las estructuras oculares y periorbitarias. La combinación de lesiones similares a xantelasmas y masas orbitarias bilaterales deben hacernos pensar en este proceso y buscar la posible afectación sistémica. Abstract: Caso clínico: Hombre de 52 a os que acudió a urgencias por disminución progresiva de su visión en el ojo derecho. Como antecedentes había tenido nefrectomía derecha por carcinoma renal 7 a os antes y neumonectomía derecha por metástasis 3 meses antes. En la funduscopia se observaba una masa coroidea en ojo derecho, compatible con metástasis. Se realizó enucleación. Durante un a o permaneció sin progresión pero en los últimos seis meses ya ha aparecido. Discusión: Las metástasis coroideas de carcinoma renal son infrecuentes y pueden ocurrir varios a os después de la curación del primario. Su sintomatología es variable y su aspecto poco característico por lo que puede dificultar su diagnóstico diferencial. Penitentiary Treatment in Mexico: A Comparison between Men’s and Women’s Situation  [PDF]

Abstract: This work presents results of a comparative study between penitentiary treatment for men and women in Mexico. Firstly, a documental review was made to estimate the differences in operation between men’s and women’s penitentiaries in Mexico; right away, we expose reactions of a focus group integrated by penitentiary personnel working in a women’s correctional center. It was found that there exists a differentiated treatment between men and women in prison that does not seem naturally justified, nor juridically pertinent. Starting from the administration, a distinct treatment persists between male and female inmates. Even if men correctional facilities have been improving recently, omissions, anti-constitutional practices and misstatements are still often seen in women correctional centers. Besides this, a comparative discourse is maintained among correctional officers, being more negative for those who work in women’s penitentiaries, and transmitting a generalized feeling of abandon due to the lack of government support; correctional officers also believe that the main expressions of government behavior to inmates are those that show power and authority and very little that of balanced exchanges.

Abstract: the case management program of chamartin mental health center in madrid is presented to illustrate with an experience outside the anglo-saxon context, where such programs arise, the theory advanced in the first part about the emergence, development and philosophy of continuity of care programs. finally, some risks in the development and maintenance of continuity of care programs are pointed out. the health, social and staff resources are described, and also the population to be targeted, the form of intervention and the type of actions that are performed. finally, we point out some risks in the development and maintenance of continuity of care programs.

Abstract: the case management programs arise in the late 70's in the u.s., in response to problems identified during the process of deinstitutionalization of psychiatric hospitals. since then, the case management and the assertive community treatment programs have spread throughout the world, with variations and particularities in different regions, becoming the cornerstone of the community care for people with severe and persistent mental illness. this article reviews the origins of these programs in u.s., their development for over thirty years around the world, the philosophy and objectives that have guided its evolution, as well as successes and failures in their development.

Abstract: this document presents a review of the role played by the security council (s.c.) of the united nations before those situations in which at an international level its involvement have been required, in what can be called “major cases of the use of armed force” from 1990. after a brief review of the relevant facts, the article questions the legality of the of the (s.c.) statements after the disturbances to the international peace and security entailed by the situations of iraq, kosovo and afghanistan, stressing the damage done to international law and the application of the united nations charter; this being a result of unilateral actions taken by countries or organisations. consequently, the essay ends highlighting the need to go back to the multilateral mechanisms to face contemporary threats to international peace and security.

Abstract: cervantes induces us to re-examine the problem of reality vs. fiction/fantasy. indeed the quijote leads into an extraordinary play of novelistic fiction. however, the boundary lines between fantasy and reality have no precise definition, and this is because cervantes himself wished it so the literary fictions in the quijote (fiction out of reality-reality out of fiction- fiction out of fiction) is a game played by the author to introduce an apparent lunatic who will not dicriminate between the limits of his reality (reality out of fiction) and the fictional in this own reading (fiction out of fiction). this clinical quixotism is but a cervantine play (ludic quixotism) to lead us to a symbolic quixotism: agony after an ideal and trascendental values, and to a search for a project of heroic humanity, a road to liberty and to an illusion of an individual conscience sustained by mystery and by the miracle of a subjective perception of an imaginary world: in other words, of the eternal and immutable
