Metástasis orbitaria como primera manifestación de adenocarcinoma pulmonar

Abstract: introduction: metastasis of primary tumours can involve the orbit via the blood stream. in 19% of cases this type of metastasis is the first oncologic symptom. case report: a 42-year-old woman with a hard, well-defined nodule situated anteriorly in the right orbit and with mild associated inflammation. mr showed an oval image with homogeneous gadolinium uptake. on excision-biopsy a ductal cancer was found and determined on cat scan to be of pulmonary origin. chemotherapy was given and no recurrence occurred in the orbit. conclusion: ophthalmologic examination was the first step in the diagnosis of a metastasis of adenocarcinoma of the lung.

Abstract: Introducción: Las metástasis de tumores primitivos invaden la órbita generalmente por vía hemática y en el 19% de los casos representan la primera manifestación de enfermedad neoplásica. Caso Clínico: Mujer de 42 a os con un nódulo de crecimiento rápido en región orbitaria anterior derecha, de consistencia pétrea, bien delimitado y asociado a leves signos inflamatorios. En la resonancia magnética se observó una imagen ovoidea con captación homogénea de gadolinio. La exéresis-biopsia del tumor mostró un carcinoma ductal y el estudio sistémico evidenció su origen pulmonar. Se inició tratamiento quimioterápico sin objetivarse en estudios posteriores de imagen signos de recidiva orbitaria. Discusión: La exploración oftalmológica ha constituido el inicio del proceso diagnóstico de un adenocarcinoma pulmonar diseminado. Abstract: positron emission tomography with (18f)2-fluoro-2-deoxy-d-glucose (pet-fdg) is an imaging metabolic technique. pet-fdg is more precise than ct for the evaluation of mediastinal affectation of patients with non-small cell lung cancer, offering a high negative predictive value. it can detect occult metastases in 11 % of the patients, although the etiology of the extra-thoracic isolated receptions needs confirmation. theoretically, pet-fdg can influence the planning volume of radiotherapy, mainly of patients with atelectasis. the quantification of the metabolic activity by means of pet-fdg is influenced by the size of the lesion, the glucose levels, and the time elapsed since the isotope injection. more clinical assays are needed to standardize pet execution procedures, its use as a prognostic factor and the evaluation of treatment response.
Genetic relationship of discrete-time survival with fertility and production in dairy cattle using bivariate models

Abstract: Bivariate analyses of functional longevity in dairy cattle measured as survival to next lactation (SURV) with milk yield and fertility traits were carried out. A sequential threshold-linear censored model was implemented for the analyses of SURV. Records on 96 642 lactations from 41 170 cows were used to estimate genetic parameters, using animal models, for longevity, 305 d-standardized milk production (MY305), days open (DO) and number of inseminations to conception (INS) in the Spanish Holstein population; 31% and 30% of lactations were censored for DO and INS, respectively. Heritability estimates for SURV and MY305 were 0.11 and 0.27 respectively; while heritability estimates for fertility traits were lower (0.07 for DO and 0.03 for INS). Antagonist genetic correlations were estimated between SURV and fertility (-0.78 and -0.54 for DO and INS, respectively) or production (-0.53 for MY305), suggesting reduced functional longevity with impaired fertility and increased milk production. Longer days open seems to affect survival more than increased INS. Also, high productive cows were more problematic, less functional and more liable to being culled. The results suggest that the sequential threshold model is a method that might be considered at evaluating genetic relationship between discrete-time survival and other traits, due to its flexibility.

Abstract: Hay una frase que bien podría resumir la aportación de la imagen diagnóstica en el campo de la Oncología: La PET ve las lesiones y la CT y/o RM ayudan a especificar su origen. Aunque esto no es del todo cierto, es el punto de partida que impulsó el desarrollo de los equipos híbridos PET-CT. Lógicamente, la obtención de la imagen de fusión anatomo-metabólica en tiempo real debería permitir a priori la caracterización de los hallazgos de la CT y la localización precisa de las captaciones de la PET, con lo cual, aparte de reducir dos pruebas a una con el consiguiente ahorro de recursos y confort para el paciente, aumentaría de forma considerable la exactitud diagnóstica.

Abstract: Mujer de 75 a os de edad con antecedentes de carcinoma folicular de células de Hürthle, con dosis acumuladas de 131I de 527 mCi. Tiroglobulina en ascenso (395 ng/ml). PET previo y CT de hace un a o con nódulos subcentimétricos en ambos lóbulos inferiores pulmonares. Rastreo con 131I sin hallazgos patológicos.

Abstract: Mujer de 50 a os de edad intervenida hace 3 a os mediante cuadrantectomía CSE de mama izquierda y linfadenectomía axilar izquierda con diagnóstico A.P. de carcinoma infiltrante tipo medular (RE-; RP-; Ki67: positividad nuclear del 10-20% de las lesiones neoplásicas; C-erb-B2: positividad nuclear (3+); micrometástasis en 1 gl.linfático). Se realizó asimismo cuadrantectomía CSE de mama derecha con diagnóstico de carcinoma in situ. En CT se apreció lesión sugestiva de metástasis en segmento VIII y masa adrenal izquierda de 2 cm. Recibió posteriormente tratamiento quimioterápico, con aparente respuesta completa
Pseudotumor inflamatorio pulmonar múltiple y esplenomegalia

Abstract: Historia clínica: Mujer de 70 a os de edad, diagnosticada en 1992 de pseudotumor orbitario (sin confirmación histológica). Cuadro inespecífico de disnea. CT: múltiples nódulos pulmonares, esplenomegalia. Broncoscopia sin alteraciones realizándose biopsia transbronquial y cepillado con resultado anatomopatológico de tejido inespecífico. Ante la falta de diagnóstico se realiza toracotomía, apreciándose múltiples nódulos pulmonares. Histología: pseudotumor inflamatorio múltiple. Ecografía: gran esplenomegalia con varios nódulos ( linfoma?).Motivo del estudio PET-CT: Pseudotumor inflamatorio pulmonar múltiple con esplenomegalia. Descartar linfoma esplénico.

Abstract: this article deals with one of the public school issues that cause most of the social concern and public opinion impact in our country: violence in schools. specifically, the paper presents partial of a quantitative and qualitative study developed in primary and secondary public schools, during 2009 in the region of valparaíso, concerning the influence of school management on the phenomenon of violence in schools. the results represent key findings related to perceptions teachers and students regarding violence in schools its relationship with the challenges this brings in at the level of school management and public policy.

Abstract: this work is part of an integrated study of leaf architecture of myrtaceae native species of argentina. seventeen native species of native flora were studied to find patterns of leaf architecture to differentiate the argentinean species of the informal subtribal: "group pimenta" and "group myrteola", tribu myrteae (family myrtaceae). the leaves studied in this work are characterized by being simple, with apex and base variable, membranaceous to coriaceous in texture, and with entire margin or slightly crenate. its venation pattern is characterized by having the first vein category pinnate, simple, and straight, without agrophic veins; venation of secondary category is brochidodromous-camptodromous veins (except in myrteola nummularia that there was a pattern acrodromous-brochidodromous), with one or none paramarginal vein and intramarginal vein; the third and fourth vein category are percurrent alternate, percurrent alternate and opposite, random reticulate or branched, the areolae vary from undeveloped to fully developed and venules branching one, two, or more times.
