Patogénesis de la infección por Helicobacter pylori

Abstract: infection due to helicobacter pylori affects to more than 50% of world population and it is the leading cause of development of affections like gastric and duodenal ulcers and the gastric cancer. even so, it is known that most of persons infected don't developed any of these diseases because of the pathogenesis induced by this microorganism depends of different factors including host features, the environment where it is developed and the infectious strain. in present paper the subject on pathogenesis of infection due to h. pylori was reviewed and updated to expand the knowledge of the main factors leading to the development of severe affections and thus, contribute to a better treatment of this infection in our population, due to the high prevalence of h. pylori.

Abstract: Con el propósito de evaluar la utilidad de la serología, reacción a la cadena de la polimerasa e histología para el diagnóstico de Helicobacter pylori utilizando como prueba de oro el cultivo se realizó un estudio descriptivo prospectivo en pacientes mayores de 18 a os de edad que acudieron al servicio de Gastroenterología del Centro de Investigaciones Médico-Quirúrgica (CIMEQ) con indicación de endoscopia digestiva superior durante el período comprendido entre julio y diciembre del 2010. La muestra quedó constituida por 89 pacientes que cumplieron todos los criterios de inclusión, de ellos 52 pertenecían al sexo masculino y 37 al femenino, el promedio de edad fue de 49,6 ± 18.0 a os. El 60,7 % fueron casos positivos para la bacteria; la serología fue el método más sensible y la reacción a la cadena de la polimerasa el más especifico para el diagnóstico del Helicobacter pylori. In order to evaluate the usefulness of serology, reaction to polymerase chain and histology for the diagnosis of Helicobacter pylori using as a gold farming was conducted a prospective study of patients over 18 years of age who presented serving Gastroenterology Research Center Medical Surgical (CIMEQ) with indication of upper gastrointestinal endoscopy during the period between July and December 2010. The sample was composed of 89 patients who fulfilled all inclusion criteria, of whom 52 were male and 37 were female, average age was 49.6 ± 18.0 years, these patients underwent upper gastrointestinal endoscopy and sampling for detection of Helicobacter pylori. 60.7 % were positive for bacteria cases, serology was the most sensitive reaction to polymerase chain the most specific for diagnosis of Helicobacter pylori.

Abstract: La infección por Helicobacter pylori afecta a más del 50 % de la población mundial y constituye la causa principal del desarrollo de afecciones como úlceras gástricas y duodenales, y el cáncer gástrico. Aún así, se conoce que la mayoría de las personas infectadas no desarrollan ninguna de estas enfermedades porque la patogénesis inducida por este microorganismo depende de diferentes factores que incluyen características del hospedero, el ambiente en el que se desarrolla y la cepa infectante. En este trabajo se revisó y actualizó el tema de la patogénesis de la infección por H. pylori para ampliar el conocimiento de los principales factores que conducen al desarrollo de las afecciones severas y, por tanto, coadyuvar al mejor tratamiento de esta infección en nuestra población, que tiene alta prevalencia de H. pylori. Abstract: helicobacter pylori is a bacterium affecting gastric mucosa in more than 50% of world population, and has been recognized as the most important etiologic factor in the development of many gastric pathologies including gastritis, ulcer, gastric cancer, and the gastric mucosa-associated lymphoid tissue lymphoma (malt lymphoma). due to pathogenic potential of this bacterium, it is necessary to have effective methods for its detection. techniques used for helicobacter pylori infection detection are divided into two groups: invasive techniques, requiring a gastric endoscopy for biopsy obtention, and non-invasive techniques which are less aggressive for patient. this review is an updating of main techniques used in diagnosis of helicobacter pylori infection, which are in a continuous improvement.

Abstract: Helicobacter pylori es una bacteria que infecta la mucosa gástrica de más del 50 % de la población mundial y ha sido reconocida como el factor etiológico más importante en el desarrollo de diversas afecciones gástricas como gastritis, úlcera, cáncer gástrico y el linfoma del tejido linfoide asociado a la mucosa gástrica (linfoma MALT). Por el potencial patogénico de esta bacteria, resulta necesario contar con métodos eficaces para su detección. Las técnicas empleadas para el diagnóstico de la infección por Helicobacter pylori se dividen en 2 grupos: técnicas invasivas, que requieren una endoscopia gástrica para la toma de biopsias y técnicas no invasivas que son menos agresivas para el paciente. Esta revisión constituye una actualización de las principales técnicas empleadas en el diagnóstico de la infección por Helicobacter pylori, las cuales continúan en constante perfeccionamiento. Embolización superselectiva de angiomiolipoma renal. Presentación de un caso: A case presentation Overselective embolization of renal angiomyolipoma

Abstract: INTRODUCCIóN: Los angiomiolipomas renales son tumores benignos infrecuentes. Tanto la observación periódica como el tratamiento quirúrgico y la embolización arterial selectiva son modalidades válidas para el manejo de dichas lesiones, teniendo en cuenta su tama o y la sintomatología del paciente en cuestión. OBJETIVOS: Presentar un caso de angiomiolipoma renal, así como el tratamiento utilizado. PRESENTACIóN DEL CASO: Paciente masculino de 65 a os de edad, quien asistió a consulta por dolor lumbo-abdominal intenso del lado derecho. En el ultrasonido se constató imagen tumoral hiperecogénica con densidad grasa en la cara anterior y parte media del ri ón derecho. La lesión fue corroborada mediante tomografía axial computarizada, y presentaba las características típicas del angiomiolipoma, con aproximadamente siete centímetros de diámetro. Se realizó la embolización superselectiva mediante una técnica mixta de gelfoam y alcohol absoluto. El paciente no presentó ninguna complicación. Se ha mantenido asintomático y en la evolución tomográfica, realizada a los seis meses de la embolización, se constató la disminución del tama o de la lesión y de su porción angiomiogénica. CONCLUSIONES: La embolización superselectiva de los angiomiolipomas renales sintomáticos constituye una opción de tratamiento electivo segura y mínimamente invasiva, que permite la conservación de la unidad renal. As much the periodical observation like a surgical treatment as the selective arterial embolization are valid modalities for the management of such lesions, taking into account its size and the symptoms of patient as regards. OBJECTIVES: To present a case of renal angiomyolipoma, as well as the treatment applied. CASE PRESENTATION: A male patient aged 65 who came to consultation due to intensive lumboabdominal pain in right side. In ultrasound (US) it was possible to verify a hyperechogenic tumor image with fat density in the anterior face and in the middle part of right kidney. The lesion was corroborated by computed axial tomography with the typical features of an angiomyolipoma of approximately seven centimeters of diameter. A Overselective embolization was carried out by means of a mixed technique of Gelfoam and absolute alcohol. There was not complication and the patient has remained asymptomatic and in the tomography course carried out at 6 months of embolization, there was a decrease in the lesion size and of its angiomyogenic. Oxigenación hiperbárica en el tratamiento de la enfermedad de Legg-Calvé-Perthes

Abstract: bearing in mind that legg-calvé-perthes is an ischemic bone process and that oxygen is required for bone viability, cicatrization, and remodelling, it was determined to use hyperbaric oxygen in the treatment of the disease to shorten time of natural development of the disease, to avoid surgery and other complications. from the clinical, imaging and scintigraphic viewpoints, 210 patients aged 6 years as an average were studied. thirty five hyperbaric oxygen therapy sessions distributed into 3 cycles of 15, 10 and 10 sessions were administered at 7 week intervals, with 2 absolute atmosphere pressure and 50min isopressure time in a single chamber. healing time was under 24 months in 92 % of patients, 24 to 36 months in 5 % whereas 3 % did not recover after 36 months and the use of supplementary cycles. it was underlined that 87 % of patients classified in catteral iii-iv recovered in less than 24 months and all those classified in catteral i-ii recovered, except for one who did after that period of time. there was neither complication nor relapse. in general, 87 % of the treated patients recovered. it was concluded that this therapy substantially shortens the normal development time of the disease, leaves no sequel and avoids surgery, all of which results in economic, psychical and social benefits for the patients and their relatives.

Abstract: Microorganims of the bovine rumen fluid biocatalyzed the reduction of nitro-compound substrates to yield the respective amines. This enzymatic process, using ruminal contents, has rarely been reported in associa- tion with the bioreduction of nitro groups. The biotransformation reactions catalyzed by this system were de- pendent of both the electronic characteristics and the area/volume of the nitro-substrates confirming the processes are enzymatic. The semi-preparative scale biotransformation went by in good yield showing the rumen fluid may be employed in the synthesis of amines under very mild conditions and, moreover, it may have application in the bioremediation of nitro-compounds.

Abstract: we present the analysis of vla archive observation made in the year 2000 at 8.46 ghz toward m17. in the region of the stellar cluster ngc 6618 we detect seven compact radio sources, of which four have chandra x-ray counterparts. two of the radio+x-ray sources coincide positionally with the two o4 stars that form the binary systen cen 1. these stars are the main ionizing sources of the m17 h ii region and the brightest x-ray objects in the zone. the observed 8.46 ghz flux densities exceed by an order of magnitude the values expected from free-free emission from a wind. an additional archive observation made in 1988 at 4.86 ghz suggests variability in the radio sources associated with cen 1.
