Abstract: objective: to estimate the lifetime prevalence of physical marital violence among women from a low-income urban community and to investigate help-seeking behavior among victims. methods: this is the brazilian pilot cross-sectional study for an international multicenter study conducted in 1999, and is based on a probabilistic cluster sample from the municipality of embu, s?o paulo state. we considered as eligible women aged 15 to 49 years, living with children under age 18 years, who had lived with a husband or partner in lifetime. information was collected using standardized questionnaires (n=86), administered by trained interviewers. we investigated three types of physical violence: severe (kicking, hitting with fist, beating, and/or use/threat to use weapon), non-severe (slapping in the absence of severe violence), and any type (severe and/or non-severe and/or other physical aggressions spontaneously referred), as well as the type of help sought by the victim (from people or institutions). we calculated frequency and 95% confidence intervals for each type of violence. results: subjects reported slapping (32.6%), hitting with fist (17.5%), beating (15,2%), use/threat to use weapon (13.9%), and kicking (10.6%). prevalence of marital violence was high: 22.1% (13.3-30.9) for severe violence, 10.5% (4.0-17.0) for non-severe violence, and 33.7% (32.7-34.7) for any type of violence. victims of severe violence were more likely to seek help from the police (36.8%) or from traditional healers (21.1%) than from health care facilities (5.3%), despite the availability of these services in the area. conclusions: physical marital violence is frequent and severe among the population studied, and help was sought preferentially from the police or traditional healers rather than from health care services.

Abstract: OBJETIVO: Estimar a prevalência de violência conjugal física ao longo da vida em mulheres de comunidade urbana de baixa renda e identificar os tipos de ajuda procurados pelas vítimas. MéTODOS: Trata-se de estudo-piloto brasileiro de corte transversal, vinculado a projeto multicêntrico internacional conduzido em 1999, com amostra probabilística de conglomerados no município de Embu, Estado de S o Paulo. Foram considerados elegíveis os domicílios com mulheres de 15 a 49 anos, que residissem com filho/filha <18 anos e tivessem vivido com algum marido/companheiro ao longo da vida. Entrevistadoras treinadas aplicaram questionários padronizados (n=86). Três tipos de violência conjugal física sofrida ao longo da vida foram investigados: grave (chute, soco, espancamento e/ou uso/amea a de uso de arma), n o grave (tapa na ausência de violência grave) e algum tipo (grave e/ou n o grave, além de outras formas de agress o física espontaneamente referidas) e os tipos de ajuda procurada (pessoas e institui es). Foram calculadas as freqüências dos tipos de violência e respectivos intervalos de confian a de 95%. RESULTADOS: As entrevistadas referiram tapa (32,6%), soco (17,5%), espancamento (15,2%), uso/amea a de arma (13,9%) e chute (10,6%). Foram altas as taxas de prevalência de violência conjugal: grave 22,1% (13,3-30,9), n o grave 10,5% (4,0-17,0) e algum tipo 33,7% (32,7-34,7). Vítimas de violência grave procuraram ajuda mais freqüentemente da polícia/delegacia (36,8%) ou de curandeiros/benzedeiras/pais de santo (21,1%) que de centros de saúde (5,3%), apesar da disponibilidade desses servi os na regi o. CONCLUS ES: A violência conjugal física ao longo da vida é freqüente e grave na comunidade estudada, sendo que a procura de ajuda foi direcionada mais freqüentemente à polícia/delegacia ou a curandeiros/benzedeiras/pais de santo do que a centros de saúde.
Measuring child exposure to violence and mental health reactions in epidemiological studies: challenges and current issues

Abstract: this paper examines challenges and current issues involved in measuring exposure to different types of violence which are associated mental health problems in children and adolescents. standardized measures suitable for epidemiological studies, selected based on their relevance in the current literature, are briefly described and commented. the assessment of child's exposure to violence may focus on a specific event (e.g., kidnapping), a specific context (e.g., war) or even of a certain type of exposure (e.g., intrafamilial physical violence). the assessment of child mental health after exposure to violence has traditionally focused on posttraumatic stress disorder (ptsd) - most frequently measured through non-diagnostic scales. however, other mental health reactions may be present and screening as well as diagnostic instruments which may be used to assess these reactions are also described. two issues of emerging importance - the assessment of impairment and of traumatic grief in children - are also presented. availability of culturally appropriate instruments is a crucial step towards proper identification of child mental health problems after exposure to violence.
Severe physical punishment and mental health problems in an economically disadvantaged population of children and adolescents

Abstract: objective: to estimate the prevalence of severe physical punishment of children/adolescents in a low-income community, and to examine child mental health problems as a potential correlate. method: this study is a brazilian cross-sectional pilot study of the world studies of abuse in family environments. a probabilistic sample of clusters including all eligible households (women aged 15-49 years, son/daughter < 18 years) was evaluated. one mother-child pair was randomly selected per household (n = 89; attrition = 11%). outcome (severe physical punishment of children/adolescents by mother/father) was defined as shaking (if age < 2 years), kicking, choking, smothering, burning/scalding/branding, beating, or threatening with weapon. three groups of potential correlates were examined: child/adolescent (age, gender, physical/mental health); mother (education, unemployment, physical/mental health, harsh physical punishment in childhood, marital violence); father (unemployment, drunkenness). severe marital violence was defined as kicking, hitting, beating or use of /threat to use a weapon. the following standardized questionnaires were applied by trained interviewers: world studies of abuse in family environments core questionnaire, child behavior checklist, self-report questionnaire. results: outcome prevalence was 10.1%. final logistic regression models identified two correlates: maternal harsh physical punishment in childhood (total sample, or = 5.3, p = 0.047), and child/adolescent mental health problems (sub-sample aged 4-17 years, n = 67, or = 9.1, p = 0.017). conclusions: severe physical punishment of children/adolescents is frequent in the studied community. the victims have a higher probability of becoming future perpetrators. when intrafamilial violence occurs, child/adolescent mental health may be compromised.

Abstract: objective: to verify the prevalence and identify the risk factors related to mental health problems among schoolchildren and its possible association with the beliefs and educational attitudes of parents/caretakers. methods: cross-sectional study with a stratified probabilistic sample (n=454) of first to third-graders from public and private schools in southeastern brazil. standardized instruments were administered to parents/caretakers by trained interviewers, including screening questionnaires for mental health problems among children and parents/caretakers; a questionnaire on beliefs and attitudes; and a questionnaire for socio-economic status. chi-square tests and logistic regression models were used for statistical analysis. results: we found 35.2% prevalence of clinical/borderline cases among students. parents/caretakers that believed in corporal punishment as a child-rearing method used physical aggression towards their children more frequently (64.8%). logistic regression models showed that the act of hitting the child with a belt was associated to conduct problems and to overall mental health problems among schoolchildren in the presence of other risk factors: child gender (male), parents/caretakers with mental health problems, and adverse socioeconomic conditions. conclusions: the high prevalence of mental health problems among schoolchildren and its association with child-rearing methods and mental health problems among parents/caretakers indicate the need for psycho-educational interventions aimed to reduce physical abuse and mental health problems in childhood.
