Abstract: the cerebrovascular disease is a health problem for cuba being the third cause of death and the first one of adult's inability with serious repercussions for the patients, its family and society. the aim of present review is to update on the quality indicators and the economic assessment studies of disease. in health field some different quality definitions are reviewed, approaching on the continuous assessment of quality by appropriate indictors and the hospital care efficiency with its social and economic implications as an essential feature for this field in cuba. from the principle that the health has not price, although to recover and to maintain it be necessary to incur in high costs, the subject of economic assessment is approached with the premise that the economy and health are essential for this field in cuba. from the principle that health has not price, although to recover and to maintain it is necessary incur in high costs, the subject of economic assessment is approached with the premise that the economy and the health must to be an inseparable binomial feature.

Abstract: the drawing up of health situation analysis at community level is a common element in study plans of health sciences careers. the medical information technology is a structured discipline also in all health careers may and must to support effectively the carrying out of this methodological scientific tool. thus, a qualitative study was conducted to determine the potentials in the information generated in the new training scenarios, the linking of medical information with the health situation analysis and to identify the contents present in this discipline supporting its carrying out. the study was framed in the "victoria de girón" basic and preclinical sciences institute in the academic course 2009-2010. the procedures used were the bibliographic review and the documentary analysis, deep interviews aimed to the key informers and the methodological triangulation alternative. present paper concludes with the identification of information goodness generated in the new scenarios (university polyclinic) for the learning of medical information technology and its link with the analysis of health situation as a subject, as well as the identification of the main contents of this discipline leading to the development of this tool. authors recommended that the competent authorities must to assess the use of results from present study in the improvement of medical information technologies education and to continue in the deepening of this subject matter emphasizing the multidisciplinary and interdisciplinary approach of the analysis of health situation content.

Abstract: the clinical method, which principles are based on the scientific method is the application of this method to the individualized attention of patients. the importance of clinical abilities for the diagnosis such as clinical background and physical exam has been quoted by numerous authors through time. a descriptive study was designed from january to september 2004 to determine the conferred validity of family medicine specialists to the clinical background, the physical exam and the diagnosis tests. these criterions were explored in ten diseases or syndromes in the current clinical practice. at the same time, patient's preferences were established when being attended by these means. an ordinal qualitative scale was utilized as an instrument of evaluation. out of 492 graduated specialists of less than six years gave a 55 % of value to interrogation in their current practice, this number increased to an 85% when added the physical exam. less than 20% of all the questioned doctors consider the diagnostic tests as fundamental to diagnosis. of all the diseases and syndromes that were explored, a 60% of their diagnosis is made by interrogation and physical exam, being of a 70 % in 8 of them. concerning the patients, an 80% of them conferred more importance to interrogation and physical exam.

Abstract: El método clínico cuyos principios se desarrollan a partir del método científico no es más que la aplicación de este método a la atención individual de enfermos. La importancia que para el diagnóstico tienen las habilidades clínicas, a saber, el interrogatorio y el examen físico se ha demostrado a través de los a os por muchos y renombrados autores. Se dise ó un estudio descriptivo entre los meses de enero a septiembre de 2004, mediante el cual se determinó el valor conferido por los especialistas en Medicina General Integral, a la anamnesis, el examen físico y los complementarios en el diagnóstico. Se exploraron estos criterios en diez enfermedades o síndromes y en la práctica médica en general. Paralelamente, se establecieron las preferencias de los enfermos a la hora de ser atendidos en relación con los citados elementos del método clínico. Se utilizó una escala cualitativa ordinal como instrumento de evaluación. De un total de 492 especialistas evaluados, los de menos de seis a os le confirieron más de 55% de valor al interrogatorio en su práctica habitual, cifra que ascendió a 85% cuando se le a adió el examen físico. Menos de 20 % de todos los médicos encuestados consideraron los exámenes complementarios como el elemento más importante para el diagnóstico. En todas las enfermedades o síndromes explorados se consideró que alrededor de 60% de los diagnósticos se realiza por la anamnesis y el examen físico llegando a 70% o más en 8 de ellas. En cuanto a los pacientes, 80% de los entrevistados le confirió mayor valor al interrogatorio y el examen físico. Abstract: La confección del Análisis de Situación de Salud, a nivel comunitario, es elemento común en los planes de estudio de las carreras de Ciencias de la Salud. La Informática Médica, disciplina estructurada también en todas las carreras sanitarias, puede y debe apoyar más eficazmente la realización de este instrumento científico metodológico. Por esta razón, se realizó el presente estudio de corte cualitativo, para determinar las potencialidades en información generada en los nuevos escenarios de formación, para la vinculación de la Informática Médica con el Análisis de Situación de Salud, e identificar contenidos presentes en esta disciplina que deben apoyar su realización. El estudio se enmarcó en el Instituto de Ciencias Básicas y Preclínicas, "Victoria de Girón", curso académico 2009-2010. Los procedimientos utilizados fueron la revisión bibliográfica y el análisis documental, entrevistas a profundidad dirigidas a informantes clave, y la alternativa de triangulación metodológica. El estudio culmina con la identificación de las bondades en información, generada en los nuevos escenarios (Policlínico Universitario), para el aprendizaje de la Informática Médica y su vinculación con el tema Análisis de Situación de Salud, así como la identificación de los principales contenidos de la disciplina que tributan al desarrollo de este instrumento. Se recomendó valorar, por las autoridades competentes, la utilización de los resultados de este estudio en el perfeccionamiento de la ense anza de la Informática Médica y continuar en la profundización de esta temática, con énfasis en el enfoque multidisciplinario e interdisciplinario del contenido Análisis de Situación de Salud. Abstract: Introducción: los desajustes de la evaluación son frecuentes en la práctica docente y las publicaciones suelen referirse a lo considerado como normal. Objetivo: ofrecer opiniones como punto de partida para reflexiones en los colectivos docentes como parte del trabajo metodológico. Métodos: se parte del análisis individual de la secuencia del proceso evaluativo y se enriquece con la revisión de la literatura y discusión grupal. Resultados: se refieren desajustes asociados a la selección de contenidos, elaboración y aplicación de los instrumentos de evaluación, calificación, retroalimentación y reinicio del proceso. Conclusiones: aunque se prioriza el aprendizaje, las enfermedades de la evaluación pueden impactar en el éxito de este. El enfoque nosológico aportado es un recurso expresivo para destacar la importancia del proceso evaluativo y establecer puntos de mira para la reflexión. Introduction: Imbalances of evaluation are frequent in the teaching practice and the publications considered this issue as normal. Objective: To provide several opinions as a starting point for further reflections on the part of the teaching staff, as part of the methodological work. Methods: Individual analysis of the sequence of the evaluation process, as a basis, reinforced with the literature review and the group discussion. Results: Imbalances associated to the selection of contents, the wording and application of evaluation, the grading and feedback instruments and the restart of the process. Conclusions: Although learning is given priority, the imbalances in the evaluation may hinder the success of this process. The offered nosological approach is an expressive resource to underline the importance of the evaluation process and to set some targets for reflection.

Abstract: the results of the analysis of the literature and the experience of the authors on the approach of the evaluation of the quality of the medical interview are dealt with. the following theoretical foundations are defined: 1) the points of view on the dimensions to be evaluated in donabedian's medical care quality; 2) the triangulation in the objectives of the anaylisis of quality (aids, methods and results); 3) the priority of the model of interview centered in the patient, a model that is more compatible with engel's biopsychosocial paradigm; 4) the priority given to the educative function and the taking of the own student as a pattern, in addition to the objectives emerging from the social demands. all this is expressed in the evaluative objectives and in the selection of the evaluation method. it is recommended the direct observation with the utilization of other techniques guaranteeing a better evaluation of the internal medicine resident.

Abstract: our medical and teaching practice lacks specific methodological instructions that materialize in an evaluative instrument of communication skills, of formative value fundamentally, to assume the evaluative criteria by tendencies. objective: to develop a methodological guideline for evaluating the development of communication skills under direct observation. methods: surveys administered to 13 internal medicine professors, who worked at "carlos j. finlay" central military hospital, with a view to finding out their opinions on an evaluative methodological guideline for communication skills. results: the suggested changes were made in terms of elements of expressive skills and the idea of evaluation by tendencies was accepted conclusions: the submitted methodological guideline has more validity in contents since it covers new aspects that better reflect the cognitive and behavioral construct of physician in the course of interviews to patients. the functional validity of the guideline is greater as it succeeds in attaining better order of expressive skills, more clarification in the enunciation of skills and better implementation of the evaluation by tendency approach.
