Granuloma reparativo de células gigantes agresivo en región mandibular

Abstract: the giant cells reparative granulomas (gcrg) is a reactive and aggressive process appearing more frequently in anterior sector of mandible and maxilla in children and young adolescents accounting for the 1 % of tumor bone lesions. there is a considerable controversy if they are benign or reactive lesions from the point of view of its origin, from its clinical and histological features as well as therapeutical. this is the case of a patient seen in external consultation of maxillofacial surgery of the "juán manuel márquez" children and university hospital at october, 2007 in whom an extent lesion diagnosed as an aggressive gcrg provoked a facial deformity and osteolysis of mandibular body. complementary, physical and iconopathographic examinations were made as well as surgical treatment and analysis of this sample. patient has neither aesthetic alterations nor functional. there was no relapse after 30 months of follow-up. we made a review of more update literature in med line, lilac and google websites with the giant cells reparative granulomas as key words in english and spanish languages, to compare our procedures and results with other reports.

Abstract: El granuloma reparativo de células gigantes (GRCG) es un proceso reactivo agresivo, que aparece con mayor frecuencia en los sectores anteriores de la mandíbula y el maxilar, en ni os y adolecentes jóvenes. Constituye el 1 % de las lesiones óseas tumorales. Existe una considerable controversia acerca de si son lesiones benignas o reactivas; también desde el punto de vista de su origen, de sus características clínicas e histológicas, así como su terapéutica. En octubre de 2007 acudió un caso a consulta externa de Cirugía Maxilofacial del Hospital Pediátrico Universitario "Juan M. Márquez", con una lesión diagnosticada como GRCG agresivo, la cual provocó gran deformidad facial y osteólisis del cuerpo mandibular. Se realizaron exámenes físicos, complementarios e iconopatográfico. Se ejecutó tratamiento quirúrgico y análisis de la pieza. La paciente no tuvo alteraciones estéticas ni funcionales. Hubo ausencia de recidiva, luego de 30 meses de seguimiento. Se revisó la literatura más reciente en los sitios Med Line, Lilac, Google, con las palabras clave granuloma reparativo de células gigantes, en inglés y espa ol, para comparar nuestros procederes y resultados con otros reportes. Abstract: objectives: to show the results from the first patients presenting with sleep obstructive apnea syndrome (soas) of skeletal peripheral type treated in our country with osteogenesis by mandibular distraction. methods: nine patients were treated with bilateral mandibular osteogenic distraction using leibinger and synthes distraction devices. latency period was of 72 hours. mandibular distraction rate at 1,0 mm each 12 hours and restraint was of 8 weeks. then, postsurgical orthodontics treatment was started. results were assessed by polysomnography and cephalometry studies before intervention, after it and at a year of treatment. results: the 89 % of cases had a good recovery, the 11 % showed an improvement. complications included: a temporary limitation of mouth opening in 55 % disappearing with physiotherapy and two patients needed tracheostomy for anesthetic purposes. conclusions: osteogenesis by mandibular distraction was an effective therapeutical method for treatment of peripheral skeletal soas.
Presentación de un modelo antropométrico para la evaluación del paciente sometido a distracción osteogénica mandibular

Abstract: aim: to propose a anthropometric model allows in a homogenous way to assess the course of patients underwent to mandibular osteogenic distraction. methods: a prior study was carried out which allow us to confirm the lack of a international protocol generalizing the study and the evaluation of results achieved with the mandibular osteogenic distraction and also allows to assess the long-term skeletal relapse. measurements elements include the mcnamara and steiner's angular and linear cephalometry studies as well as the simoes panography. a mathematical model was designed with formulae allowing assessing the distraction achieved in branch and body as well as the effectiveness of treatment and long-term stability. authors propose also a valuation of occlusal changes obtained. results: the anthropometry model proposed is a tool to apply the scientific method in the study and the assessment of patients underwent mandibular osteogenic distraction. conclusions: the anthropometry model proposed is a suitable method to assess in a homogenous way the course of patients underwent mandibular osteogenic distraction.

Abstract: the stomatology history in each region of our country must to be rescue. thus, in the province of camagüey authors carried out present research in the historical period of viv century from may, 1993 to june, 1994, reviewed and compared in 2010. the primary source of information were historical documents of xix century looked up in the provincial library of camagüey including the capitular minutes from the puerto principe town hall, as well as the newspapers la gaceta de puerto principle, la gaceta, el pueblo, el camagüey and history books gathering the study period. in a documentary way it was possible to demonstrate the progress of stomatology during xix century in this province going from empirical multiple works of the barber-phlebotomist-dentist up to appearance of surgeon-dentist profession dentists participation of this province was significant in the guerra de los diez a?os arising a general and a captain of liberating army and together with other patriotic dentist of this province were sent to prison sheded their blood in the battlefields of this province. the training of the first of camagüey was in the dental schools of united states until the teaching as a discipline began in cuba after the guerra de los diez a?os. at the end of xix the basis were created to found the camagüey dental college in 1907.
Las concepciones de curriculum y su importancia en la elaboración de un doctorado curricular en estomatología

Abstract: the curricular design is an essential stage of teaching-learning process. depending of the theoretical and epistemologic concept on curriculum, will be the structuring of teaching process and of the type of pedagogy to be applied. in cuba, the stomatology faculty has a broad experience of 110 years in the training of human resources; however it is considered that the postgraduate training in this science must to be strengthened. that is aim of present paper was to assess the curricular concepts applicable to doctoral training of postgraduate in stomatology and to propose a critical location regarding the concept of curriculum, creating theoretical bases for the design of a contributing curricular doctorate in this profession. the curriculum conceptual position of present study was totally coincidental with the concept of the improvement of higher education center of university of la habana, since emphasize the structural-formal plane, where the worked out project is specified expressed in documents, regulations, rules of the official curriculum and at the same time the process-practical plane, that is, the curricular carrying out process identified with the postgraduate educational practice.

Abstract: Introducción: las clasificaciones sobre las alteraciones cefalogénicas presentan una gran dispersión debido a la multiplicidad de especialidades médicas que intervienen en la región de la cabeza. Objetivo: describir las clasificaciones más relevantes de las malformaciones craneofaciales y proponer una nueva clasificación. Métodos: se realizó un estudio mixto, hermenéutico y cuantitativo descriptivo, durante el cual se analizó la producción de clasificaciones sobre este tema en la Web of Sciences en el periodo comprendido desde 1992 hasta 2012. Luego de seleccionar 263 artículos que trataban este tema se estudiaron 22 de ellos en los que proponían clasificaciones o modificaciones al respecto. Se estudió la región anatómica, el tipo de clasificación, el tipo de malformación que describe y la revista de publicación. Resultados: hubo predominio del tipo de clasificación morfogénica en el 60 %, el tipo de malformación mayormente clasificada fue por defecto (fisuras e hipoplasias) en 72 % y la revista con mayor representación de publicaciones sobre clasificaciones de alteraciones cefalogénicas fue TheJournal of CraniofacialSurgery donde se publicaron el 44 % de ellas. Se propuso una clasificación integradora morfo-fisiopatológica y se analizó su validación interna y externa. Conclusiones: se demostró la ausencia de una clasificación que agrupara todas las malformaciones cefalogénicas, que incluyera a todas sus regiones anatómicas y tipos de malformaciones. La clasificación propuesta podría ser de utilidad para las especialidades médicas y estomatológicas afines a esta región anatómica. Introduction: classifications on cephalic alterations have great dispersion secondary to diversity of medical specialties involved in head region. Objective: to describe relevant classifications in craniofacial malformations and recommend a new classification. Method: was carried out a mixed study: hermeneutic and quantitative descriptive, to analyze the production of this kind of classifications on the Web of Sciences during the years 1992 to 2012. As result 263 papers was find on this topic and 22 papers were selected because provided classifications or modifications in this respect. This research analyzed the anatomical region, the classification type, the type of malformation and the magazine of publication. Abstract: La historia de la estomatología en cada región del país debe ser rescatada. Para ello se realizó esta investigación en la provincia de Camagüey, Cuba, en el periodo histórico del siglo XIX. Esta investigación fue realizada desde mayo de 1993 a junio de 1994 y revisada y cotejada durante el a o 2010. La fuente primaria de investigación fueron documentos históricos del siglo XIX consultados, en la Biblioteca Provincial de Camagüey, como las Actas capitulares del ayuntamiento de Puerto Príncipe, así como los periódicos: La gaceta de Puerto Príncipe, La gaceta, El pueblo, El Camagüey y libros de historia que recogen la etapa objeto de estudio. Se demostró documentalmente el progreso que tuvo la Estomatología durante el siglo XIX en dicha provincia, que transitó desde el empirismo multioficio del barbero-flebotomista-dentista, hasta el surgimiento de la profesión del cirujano dentista. Resultó significativa la participación de los dentistas camagüeyanos en la Guerra de los diez a os, de los que surgieron un general, un coronel y un capitán del Ejército libertador, que junto a otros patriotas camagüeyanos dentistas, sufrieron prisiones, y regaron con su sangre la llanura camagüeyana. La formación de los primeros dentistas camagüeyanos ocurrió en los colegios dentales de los Estados Unidos, hasta que se comenzó la docencia de esta disciplina en Cuba, después de la guerra de los diez a os. A finales del siglo XIX ya se consolidaban las bases para la fundación del Colegio Dental de Camagüey, que surgiría en 1907. The Stomatology history in each region of our country must to be rescue. Abstract: Propósito del estudio: proponer un modelo antropométrico que permita evaluar de forma homogénea la evolución de los pacientes sometidos a distracción osteogénica mandibular. Métodos: se realizó un estudio previo en el que se constató la ausencia de un protocolo internacional que estandarice el estudio y evaluación de los resultados obtenidos con distracción osteogénica mandibular y que permita evaluar la recidiva esqueletal a largo plazo. Se tomó como elementos de medidas los estudios cefalométricos angulares y lineales de McNamara y Steiner y se incluyó el panorograma de Simoes. Se dise ó un modelo matemático con fórmulas que permiten evaluar la distracción obtenida en rama y cuerpo, así como la efectividad del tratamiento y la estabilidad a largo plazo. Se propuso además una valoración de los cambios oclusales obtenidos. Resultados: el modelo antropométrico propuesto constituye un instrumento útil para implementar el método científico en el estudio y evaluación de los pacientes tratados con distracción osteogénica mandibular. Conclusiones: el modelo antropométrico propuesto es un método idóneo para evaluar, de forma homogénea, la evolución de los pacientes sometidos a distracción osteogénica mandibular. Propuesta de teorías integradoras para la cefalogénesis y sus malformaciones Proposal of integrative theories for cephalogenesis and their malformations

Abstract: En las teorías embriogénicas de las malformaciones del macizo craneofacial es frecuente encontrar explicaciones simplistas, que no aluden a la biología celular, ni molecular de sus fisiopatologías. El propósito de esta investigación fue describir las teorías de la cefalogénesis y sus malformaciones craneofaciales, como requisito para proponerteorías integradoras que favorezcan su mejor comprensión. Se realizó una revisión biobliográfica sobre el tema de la cefalogénesis y las concepciones científicas actuales concernientes a las teorías de las malformaciones congénitas craneofaciales. Se realizó una revisión en Internet, en el periodo comprendido entre diciembre de 2011 y enero de 2012. Se analizaron los artículos de revistas de la Web of Sciences que trataron este tema desde el a o 2006 hasta el 2012 en los buscadores Google academico, Hinari, Scopus, Scielo y Medline. Como resultado se obtuvieron 131 artículos que fueron tamizados; pero el estudio se ci ó solo a 43 artículos que enfocaron estas temáticas de manera más integral. Se consideraron reduccionitas o incompletas las teorías hasta ahora expuestas. Por tal motivo se propusieron teorías holístico-sinérgicas de la cefalogénesis y sus malformaciones. Frequently is possible to find simplistic explanations regarding the embriogenics theories of craniofacial malformations, without any explanation about cellular or molecular biology of this physiopathology process. The purpose of this investigation was to provide elements to allow the understanding of cephalogenesis as to value theories of cephalogenesis and craniofacial malformations and to suggest an integrative theory. A bibliographic study was carried out about cephalogenesis, and current scientific conceptions on theories of craniofacial congenital malformations. An Internet exploring was made during December of 2011 to January of 2012. Was evaluated the Web of Sciences reports related those topics published since 2006 to 2012 in searching as: Academic Google, Hinari, Scopus, Scielo and Medline. Was obtained 131 reports as result they wich were sifted and bounded to 43 papers focused this topic in an integral way. As conclusion was brought up to date cephalogenesis, and their alterations since the knowledge of differents biological levels that participate in cranial grown. It was considered simplistic or incomplete the theories used until now and intended holistic-sinergic theories of cephalogenesis and their malformations.
