Abstract:

The Background: Tramadol, is a central acting analgesic that possesses weak affinity for the μ-opioid receptor and modifies transmission of nociceptive impulses through inhibition of monoamine reuptake. This study was designed to determine the effect of tramadol on blood glucose levels and also to investigate whether or not alpha-2 adrenergic receptors were responsible for this effect. Methods: Twenty-five Wistar male rats were assigned to four groups to receive: Group I: saline; Group II: tramadol (1 mg·kg-1); Group III and Group IV: pretreatment with a2-receptor antagonist drugs yohimbine (1 mg·kg-1) or idazoxan (1 mg·kg-1), 30 min before administration of tramadol (1 mg·kg-1). Samples for plasma glucose measurement were withdrawn at 0, 30, 60, 90 and 120 minutes of the experiment. Results: A significant rise in blood glucose levels was observed following administration of i.v. tramadol. Pretreatment with both yohimbine and idazoxan (1 mg·kg-1) significantly attenuated tramadol-induced hyperglycemia. Conclusion: The results of the study indicate that, tramadol administered at an analgesic dose of 1 mg·kg-1 produces hyperglycemia in diethyl ether anesthetized rats. Reversal of this effect with a2-adrenoceptor blocking agents suggests that monoaminergic pathways which contribute to the analgesic action of tramadol, may have a role in the hyperglycemic action of the drug.

Abstract: A 68-year-old woman who had a history of progressive systemic sclerosis, pulmonary fibrosis, kyphoscoliosis and decreased oral apertura underwent total hip arthroplasty. This operation was performed successfully under combined spinal epidural anesthesia.Systemic sclerosis is a complex disease that involves multiple organ systems. Every aspects of anesthetic care may be altered or hindered by the pathogenesis of disease. Although the choice of regional or general anesthesia is unclear, to choose combined spinal epidural anesthesia may be useful.Systemic sclerosis, also termed 'scleroderma' is a multsystem- connective tissue disease, characterized by excessive fibrosis, vascular abnormalities, and immune dysfunction. A relatively rare syndrome, scleroderma has an annual incidence of 1 or 2 cases per 100 000 and 3 times more frequent in women than men. The usual onset is between 30 to 50 years of age. Muscles, bones, mucous membranes, heart, lungs, intestinal tract and other internal organs may be involved [1]. Kidney, lung and heart involvement is associated with poor prognosis; moreover visceral involvement and immunological findings have been considered as prognostic factors by some authors [2]. Sixty percent of deaths are secondary to heart or lung involvement and (or) to pulmonary hypertension[3].It is reported that, anesthesiologists managing patients with SSc intraoperatively must have experience and background about the pathogenesis, clinical manifestations, systemic involvement and anesthetic considerations. One should never forget that every aspects of anesthetic care may be altered or hindered by the pathogenesis of this disease [4].In this report we present a very rare case of combined spinal-epidural anesthesia in a patient with SSc whose pulmonary function tests were decreased with low exercise capacity.A 68 year-old woman, 150-cm in height, weighing 45-kg with traumatic left femur fracture admitted to the hospital for total hip arthroplasty. She had been

Abstract: Post-intubation tracheal stenosis is usually caused by pressure necrosis at the cuff. Although the best results are obtained by tracheal reconstruction, not all patients are able to undergo this operation for either medical or personal reasons. A 67-year-old male was admitted to our hospital with progressive dyspnea unrelieved by medical management. Bronchoscopy revealed stenosis of the trachea 2 cm below the vocal cord. In this case, as respiratory failure accompanied by COPD constituted a contraindication for tracheal surgery. It was decided to dilate the tracheal stenosis with APC. A 20-year-old female was admitted to our hospital with dyspnea and stridor. Bronchoscopy revealed a stenosis of the trachea 3 cm below the vocal cord. Because surgery was refused it was decided to dilate the tracheal stenosis with APC. APC has been successfully employed in the treatment of post-intubation tracheal stenosis in two surgery-refused and inoperable patients. The patients immediately experienced a relief of symptoms after APC. APC appears to be an effective alternative procedure for endobronchial dilatation in selected patients who are inoperable for either medical or personal reasons.

Abstract: Background: Despite scientific and technological developments on dentistry, man is still losing their teeth due to several reasons. Tooth loss may result in esthetical and functional discrepancies. Although beginning of dental transplantation is quite old, it started to take attraction on scientific basis recently. In recent years, oral implants are being used nearly in all fields of dentistry. Purpose: The purpose of this study was to evaluate 5 year follow-up of a patient with an impacted maxillary canine transplantation and a single-tooth implant. Materials and Methods: In our study an impacted canine was removed and transplanted immediately. Thereafter the missing premolar was replaced by an implant retained cemented crown. The patient was evaluated clinically and radiographically. Clinical periodontal parameters and implant mobility was evaluated, marginal bone level was measured on radiographs. Results: No significant changes have been observed in these parameters and implant mobility between year one and year five of the study. Conclusion: This study has demonstrated that stable long-term results can be achieved with replacement of single teeth with MIS Implant, and cemented crown on abutment with adjacent transplanted canine, with cumulative success rate for five years.

Abstract: The purpose of this study was to determine how both 2000 and 2004 science curricula were evaluated by prospective teachers. For this purpose, prospective teachers were asked to compare the content in both science curricula. Comparison criteria were not provided for the prospective teachers; it was deemed that they could determine their own criterion in the context that they can express their own thoughts freely and unrestrictedly. In this study, third year university students in the science education department, were selected as a sample (n=60) who was deemed to have sufficient knowledge about how to evaluate and how to use a curriculum since they had attended Science Laboratory Applications-I-II, Subject Methodology-I, School Experience-I courses respectively. Another reason for selection of this sample was that they were willing to make such an evaluation. The results obtained from the comments of prospective teachers were important since their views reflected their thoughts. When the thoughts of prospective teachers are taken into consideration, it can be expressed that they have more positive thoughts about 2004 curriculum.

Abstract: Background and Design: Topical drugs are an important group of contact allergens. The present study aimed to evaluate contact sensitivity to topical drugs in patients with contact dermatitis. Materials and Methods: Between 2003 and 2008, 129 patients were followed up at the Department of Dermatology at Ankara University School of Medicine with clinically suspected contact sensitivity to topical drugs. In this study, the patch test reactions to the European Standard Battery and topical drugs used by the patients and medicament patch test results were evaluated. Results: Positive patch test reaction to one or more allergens was found in 80 (62.0%) of 129 patients included in the study. Sixty-one of the 80 patients (61/129, 47.3%) had positive patch test reaction to medicaments. Medicament sensitivity was detected in 37.9% (49/129) of subjects. Nitrofurazone was found to be the most common allergen (18.6%). Discussion: The present study showed that topical drugs are a frequent cause of allergic contact dermatitis. Therefore, the probability of contact sensitivity to topical drugs should also be considered in patients with the clinical diagnosis of allergic contact dermatitis and, suspected cases should be evaluated further with patch testing in order to find the responsible allergens.

Abstract: Objective: To investigate the clinical performance of implants with chemically modified surfaces in irradiated bone in a period of five-years. Patients & Methods: 15 (6 females, 9 males, 50.2 years with a range of 38 - 60 years) patients who had been operated for oral tumors and had undergone radiotherapy were enrolled and 40 SLActive surface implants were placed (24 in the maxilla, 16 in the mandible). Implants were allowed to integrate for a period of 90 days and the stability of the implants was measured with Resonance Frequency Analyzer/Osstell? Mentor (Integration Diagnostics, Savedalen, Sweden) at implant placement, 30 days later and at the end of the 90th day. Patients follow up periods after the implant placement varied from 20 months to 60 months (mean: 45 months). Results: Two implants were lost in maxilla in healing period as a result of osseointegration failure. The survival rate was 95%. During the observation period, totally 4 implants were lost. The overall success rate was 90%. 3 of the lost implants were in maxilla and one was in mandible. The initial ISQ values of the implants differed from 20 - 71; the second values were between 24 - 71 and the last values were between 30 - 89. The implants which were lost could not show a value greater than 35. Conclusion: There was no any complication on the SLActive surface implant placed bones due to irradiation. Within the limitations of this study, it may be concluded that osseointegrated implants can be placed in irradiated bones, unless a careful patient selection and treatment planning is performed.
