Relationship between dental fluorosis and quality of life: a population based study

Abstract: the aim of this study was to verify the prevalence of dental fluorosis in schoolchildren aged 6 to 15 and its possible association with the impacts on their daily activities. this study is observational, cross-sectional and analytical. a total of 513 schoolchildren from the city of pinheiro preto, sc, took part in this study. the children were examined by three calibrated dentists, after obtaining a kappa > 0.80. to assess the prevalence of fluorosis, clinical examinations were performed according to the methodology set forth by the 4th edition of the who. to assess the impact of fluorosis on their daily activities, a modified oidp (oral impacts on daily performance) was adopted. the statistical analysis used was the chi-squared test with a 5% significance level. of the total number of children examined, 262 (51.1%) were of the female gender and 251 (48.9%) were of the male gender. in regard to the prevalence of fluorosis, 94 (18.3%) of the children presented this condition, while 419 children (81.7%) presented a normal condition. in regard to the severity of fluorosis, few children presented severe alterations. no association was found between dental fluorosis and gender (p = 0.646), between fluorosis and socioeconomic status (p = 0.848) or between fluorosis and access to public water supply system (p = 0.198). the activities that most affected children's daily performance were: oral hygiene (40.9%) and food intake or enjoying food (40.4%). none of the daily activities could be associated with the occurrence of dental fluorosis. the prevalence of dental fluorosis was consonant with the standards found for locations with optimum fluoride content in the water supply. the questionable and very slight levels of fluorosis were the most frequently found, without influence in the quality of life of the schoolchildren participating in the study.

Abstract: Introduction and objective:The objective of this study was to verifythe professional activities profile of dental surgeons that worked in a private enterprise. Material and methods: A descriptive cross-sectional and analytic study was carried out, through self-administered questionnaire given to the professionals who worked in the company’s units located in different cities of S o Paulo State. The research was approved by the Research Ethics Committee of School of Dentistry of University of S o Paulo. Data were organized and analyzed at STATA 10.0 program.Results:194 dental surgeons that were working in the company in 2008 participated in the research. Regarding sociodemographic characteristics, 68.04% were female and 31.96% were male, with 25 to 54 years(average of 35.78 years).Aboutprofessional formation, 64.43% had studied in public colleges and35.57% in private colleges; 116(59.79%)had finished specializationcourse.The average years since graduation was 12.85 years, and theaverage working hours per week was 41.08 hours.Most of them workedalso in another private setting (78.35%),and between them 32.99%attended dental insurance patients in those places.In relation to other professional activities, 30.93% worked also in public service and 7.22% were college professors. Conclusion:The majority of professionals who participated in this study was female, had studied in public universities,was specialist and worked in another private setting.

Abstract: Objective: The objective of this study was to verify the continuing education activities profile of dentists who worked in a private company of S o Paulo State in 2008. Methods: A descriptive cross-sectional and analytic study was carried out, using a self-administered questionnaire distributed to dentists who worked in the company’s units located in different cities of S o Paulo State. The research project was submitted to and approved by the Ethical Committee of the School of Dentistry of the University of S o Paulo (FOUSP). Data were organized and analyzed using the STATA 10.0statistical package. Results: One hundred and four dentists participated in the study and all were working in the private company in 2008. Regarding sociodemographic characteristics, 68.04% were female and 31.96% were male, with ages between 25 and 54 years (mean 35.78 years). In relation to professional formation and performance, 64.43% studied in public colleges and 35.57% in private colleges, 116 (59.79%) were specialists, 26 (13.40%) had Masters degree and 10 (5.15%) were PhDs. With regards to access to scientific updates, 79.38% reported having participated in ascientific congress in the last two years, 87.63% had access to scientific journals and 30.41% were members of a scientific society. Conclusion: The professionals used to participate in a variety of continuing education activities, such as scientific congresses, and had access to scientific journals and specialization courses, although few of them have Masters degree or PhD.

Abstract: the current study assessed trends in oral cancer mortality in the city of s?o paulo, brazil, from 1980 to 2002. the official mortality information system supplied data on deaths whose underlying cause was classified as oral cancer, stratified by sex, age, and anatomic site. death rates were estimated and adjusted by the direct method, using population data supplied by national censuses from 1980, 1991, and 2000 and a population count performed in 1996. there was an upward trend in overall cancer mortality, at a yearly rate of 0.72%. accounting for more than one third of these deaths, tongue cancer was the main mortality category. labial, gengival, and retromolar cancer showed a downward trend, while oropharyngeal cancer and cancer in unspecified parts of the mouth and oropharynx showed increasing mortality. monitoring the magnitude and trends in cancer mortality can assist the planning of health initiatives aimed at reducing the disease burden from oral cancer in brazil.

Abstract: the scope of this paper is to discuss the evolution of mortality due to cervical cancer in the state of paraná, brazil, between 1980 and 2000 and analyze the socioeconomic differentials in each region of the state. mortality data were gathered from the system for information on mortality by age and town of residence. age-adjusted death rates were calculated for 22 regions of the state in each year. comparative analysis evaluated socioeconomic indicators associated with regions that showed either stationary or increasing mortality trends. cervical cancer deaths increased in the state of paraná, with an annual percentage increase of 1.68% (1.20 to 2.17, 95% confidence interval). most of the regions presented a stationary trend of cervical cancer deaths. the comparison of regions presenting an increasing trend indicated poorer socioeconomic indices for the former set: regions with an increase in cervical cancer mortality had a significantly higher illiteracy rate (p<0.001), percentage of individuals older than 15 years with less than 4 years schooling (p=0.001), and lower per-capita income (p=0.025) and human development index (p=0.023). an increasing mortality trend was thus observed throughout the state and areas with higher mortality rates had poorer socioeconomic indices.
Scheduling delay in suspected cases of oral cancer

Abstract: the objective of the study was to evaluate scheduling delay of dental exams in the city of s?o paulo of patients suspected of having oral cancer. a cross-sectional study was performed in which telephone conversations simulated clinical situations that represented two types of patients: one presenting symptoms suggestive of oral cancer (ca), and another one suggesting the need for prostheses (pr). the scheduling delay was evaluated by the days until an appointment for care; and among public offices, by type of schedule (emergency or routine). negative binomial regression was used (95% statistical significance). five hundred and seventy-five public and private dental offices participated in the study. the mean scheduling delay for the ca group was 2.88 days, and for the pr group, 4.34 days (p = 0.01). the mean scheduling delay was shorter in private dental offices (2.59 days) than in offices that accepted health insurance (2.74 days) (p = 0.01); the delay was shorter when performed by the dentist rather than by the dental assistant, 2.45 versus 4.21 days (p = 0.01). in public services, 69% of patients in the cancer group were sent to the emergency service. dental services were accessible for scheduling clinical examinations among patients suspected of having oral cancer.

Abstract: introduction: the dmf-t index is the most widely used instrument to estimate prevalence of dental caries on permanent teeth in epidemiological surveys. since the 60's, the world health organization has been recommending standards for oral health surveys to improve validity and reliability of epidemiological data. during this period, different criteria for diagnosis and coding the observations have been used. objective: to compare data from the same population using different standardizations for dmf-t and dmf-t indexes in order to verify if changing dental caries criteria redounds to different population estimates. material and method: data from three epidemiological surveys carried out in the city of s?o paulo in 1996 and 1998 were analyzed. these surveys had different methodological aspects as sample design and calibration of examiners. dmf-t averages and value distribution related to schoolchildren from 6 to 12 years of age were analyzed comparing averages and proportions. results: no significant difference was observed between averages. there were no significant differences in percentages of caries- free individuals in relation to deciduous dentition, unlike permanent teeth. conclusion: different criteria used to define dental caries did not lead to different averages and percentages in these three surveys.

Abstract: monteiro lobato's opinions with regard to the health and disease themes were not restricted to jeca tatu's condition; he went beyond, and his opinions and reports about the illness experience can be observed in several excerpts of his adult work. this paper highlights the health and illness conceptions observed in monteiro lobato's writings. through the analysis of his adult work, we could learn about his opinions and reports on his beloved ones' experience of being sick, the possibility that the disease offers of socializing individuals, medical corporativism, his participation in the pro-sanitation league, some paternalistic health practices and the vulnerability of those with unfavorable social conditions, among other topics, in some occasions with comical characteristics, in others with resignation and even anger. therefore, this study analyzed his adult work, discussing the excerpts that deal with health and illness experienced in the first decades of the 20th century.
Evaluation of patient access to medical specialty of Endodontics in two health units in the city of Curitiba (PR)

Abstract: Introduction: In Brazil dental treatment occurs mainly through the Unified Health System (SUS), which can direct to various services in its basic health units. Whenever needed, other specialties including Endodontics are recommended to patient at the dental visit. Nevertheless, oral health national inquiries indicate that a part of Brazilian population has never been attended by a dental surgeon. Objectives: To evaluate the access of patients enrolled in the waiting line to Endodontics specialty in two health units in Curitiba-PR and a Basic health Unit (UBS) and the other strategy’s Family Health (ESF) Material and methods: A qualitative, quantitative, descriptive and explorative study was performed, with data collection from reports. Furthermore, the variation of people enrolled in Endodontics in a particular health unit was observed, while an intervention in the waiting line was performed in a different health unit. Results: From april to July 2008, there was a 95% reduction in the total number of enrolled patients in the waiting line for Endodontic treatment in the Basic Health Unit of Pilarzinho, while an 88% reduction of the same aspect occurred in the Basic Health Unit of S o Domingos (Family Health Program), which had suffered intervention in the waiting line. Conclusion: There is a need to improve the professionals working care in basic health units (validation), as well as it is necessary to optimize the periodical intervention in the waiting line, the screening, the booking to special attendance and the decentralization based on geographical proximity from the Reference Center.
Tuberculose e leite: elementos para a história de uma polêmica

Abstract: Pode o ser humano contrair tuberculose pela ingest o do leite de vacas tuberculosas? S o efetivos o diagnóstico e o controle da tuberculose bovina para a prote o da popula o humana? Quest es como estas estiveram em pauta no pensamento médico paulistano no início do século XX. O presente estudo procurou reconstituir elementos da polêmica travada em S o Paulo, com o intuito de incentivar a divulga o de dados suplementares sobre a matéria. Aqui s o apresentadas indica es sintéticas das posi es em confronto: de um lado, as preocupa es em intensificar as a es de controle do gado tuberculoso; de outro, o cuidado de n o prejudicar os criadores nacionais com a demanda de precau es adicionais, as quais se temia serem lesivas à comercializa o do produto. O acompanhamento das interven es sobre o tema na imprensa médica permitiu identificar a preponderancia da primeira posi o, com conseqüências para o controle sanitário de alimentos.
