Saúde mental e trabalho: uma reflex?o sobre o nexo com o trabalho e o diagnóstico, com base na prática

Abstract: seven cases of workers selected among 150 who had undergone psychiatric treatment at the workers' health reference centers in santo amaro and andré gabois from 1994 to 1997 were presented for a detailed analysis characterizing work situations and discussing definition of the diagnosis and causal connections with work. the qualitative case study provides a thematic analysis of the patient history and files. work situations are characterized by: unhealthy working conditions, problems related to work organization, inadequate human resources management, and urban violence. symptoms included: fear, anxiety, depression, nervousness, tension, fatigue, malaise, loss of appetite, sleep disorders, and psychosomatic disorders (gastritis, hypertension); in addition, workers were unable to forget about work while off the job. diagnosis varied. three cases involved post-traumatic syndrome. two cases involved organic psychosis linked to accidents or exposure to neurotoxic chemical products. cases also included neurotic syndromes of fatigue, depression, and paranoia, as well as adaptation and reaction to acute stress. in all of the cases it was possible to relate the clinical picture to the work situation.

Abstract: S o apresentados sete casos de saúde mental, dentre os 150 atendidos nos Centros de Referência em Saúde do Trabalhador de Santo Amaro e André Gabois, no período de 1994 a 1997. Com base na análise das anamneses e prontuários, buscou-se caracterizar as situa es de trabalho, discutir o estabelecimento do diagnóstico e do nexo causal com o trabalho. As situa es de trabalho caracterizaram-se por: condi es de trabalho nocivas, problemas relacionados à organiza o do trabalho, gest o inadequada de pessoal e violência. Os quadros clínicos mostraram a existência de medo, ansiedade, depress o, nervosismo, tens o, fadiga, mal-estar, perda de apetite, distúrbios de sono, distúrbios psicossomáticos (gastrite, crises hipertensivas), além disso, ocorreu contamina o involuntária do tempo de lazer, ou seja, os trabalhadores sonhavam com o trabalho, n o conseguiam "desligar-se". Os diagnósticos foram variados. Em três casos tivemos síndromes pós-traumáticas ligadas a assaltos. Dois casos referem-se a quadros psicóticos organicos ligados a acidentes ou exposi o a produtos químicos. Apareceram ainda síndromes neuróticas de fadiga, depressivas, paranóides, de adapta o e de rea o ao estresse grave. Em todos os casos foi possível relacionar o quadro clínico com a situa o de trabalho.

Abstract: Insulinomas ocultos s o tumores n o detectáveis aos exames convencionais. O estímulo superseletivo arterial com cálcio e coleta venosa hepática (EACV) é um exame que pode ser utilizado para localizar o segmento pancreático acometido pelo tumor. Relatamos o caso de uma paciente com insulinoma oculto detectado por meio do EACV. Além disso, s o discutidos aspectos técnicos da realiza o do exame.
Análise da tendência da mortalidade por acidente vascular cerebral no Brasil no século XXI

Abstract: background: although it is the second leading cause of deaths worldwide, the cerebrovascular accident (cva) has shown a significant reduction in mortality rates in recent decades. objective: to evaluate the trend of cva mortality rate in brazil, in both sexes, older than 30 years old, between 2000 and 2009. methods: population data were obtained from the database of the brazilian institute of geography and statistics (ibge) and deaths through the mortality information system of the health surveillance secretariat of the ministry of health, and included codes i60 to i69 according to 10th international classification of diseases. we calculated the incidence of deaths/1,000 inhabitants, gross and standardized mortality rates /100,000 inhabitants. the modeling of the trend of rates was performed using regression models. results: there was an increase in mortality until 2006, followed by a decline until 2009, when the incidence was the lowest. comparing the years 2000 and 2009, there is a downward trend in standardized mortality rate in both sexes (male = -14.69% and female = -17%) and total (-14.99%), with fluctuations during the period. between 30 and 49 years in both sexes, there was a trend of continuous and linear decrease in mortality rate, while the other age groups showed a curvilinear function, leading to an effective decrease in values. conclusion: there was a downward trend in mortality in all age groups and both sexes. the reduction in gross mortality rate was more pronounced in males, while the standardized mortality rate showed a greater reduction in females.
A percep??o da mulher sobre o exame preventivo do cancer cérvico-uterino: estudo de caso

Abstract: cervical-uterine cancer is among the most feared chronic degenerative diseases, due its high mortality and morbidity rates. this study describes women's feelings before a preventive examination for cervical-uterine cancer, through a case study based on observations and semi-structured interviews as data collection techniques. a total of 24 women were interviewed, between eighteen and sixty years old, who attended the cancer prevention unit at a basic healthcare clinic in fortaleza, ceará state between april and august 2004. this study shows that women generally request these preventive examinations only when symptoms appear. they view this procedure with apprehension and fear, due to the possibility of a positive diagnosis of cervical-uterine cancer; they feel embarrassed at exposing their bodies for examination, particularly by male physicians; and they are poorly informed about their own bodies and even their own sexuality. these findings highlight the need for healthcare practitioners to conduct educational activities for women, in order to enhance professional relationships between healthcare providers and users, helping lower the rate of this type of cancer through more frequent preventive examinations.

Abstract: occult insulinomas are tumors which are not identified by conventional tests. selective arterial calcium stimulation and hepatic venous sampling (asvs) can be used to identify the pancreatic segment where the tumor is located. we report the case of a patient with clinically proven insulinoma detected only by asvs. the details of the procedure are also discussed.

Abstract: this study approached the health problems which were involved with stress using self reporting questionnaires (srq-20) in order to evaluate mental disorders. the sample was constituted of 258 trucker drivers in brazilian roadway (37,5±10,0 years old, 55% drink alcohol, 37% with arterial hypertension e 57% used drugs to get alert). the drivers informed they were nervous, tense and worried (56%), disturbed sleep (47%), headaches (37%), difficulties in making decisions (38%) and difficulties in thinking clearly (20%). the results of srq-20 showed that 33% were probably with common mental disorders and an association (p<0.05) with fatigue, diminished concentration, being nervous and worried, personal problems at work, and trucker transportation. as conclusion, the presence of common mental disorders was considered probably as consequence of stresseful conditions at work.
Insulin autoimmune syndrome: case report

Abstract: context: insulin autoimmune syndrome (ias, hirata disease) is a rare cause of hypoglycemia in western countries. it is characterized by hypoglycemic episodes, elevated insulin levels, and positive insulin antibodies. our objective is to report a case of ias identified in south america. case report: a 56-year-old caucasian male patient started presenting neuroglycopenic symptoms during hospitalization due to severe trauma. biochemical evaluation confirmed hypoglycemia and abnormally high levels of insulin. conventional imaging examinations were negative for pancreatic tumor. insulin antibodies were above the normal range. clinical remission of the episodes was not achieved with verapamil and steroids. thus, a subtotal pancreatectomy was performed due to the lack of response to conservative treatment and because immunosuppressants were contraindicated due to bacteremia. histopathological examination revealed diffuse hypertrophy of beta cells. the patient continues to have high insulin levels but is almost free of hypoglycemic episodes.
O ensino em grupo do processo de aplica o de insulina

Abstract: O presente estudo teve por objetivo analisar a habilidade da pessoa com diabetes mellitus (DM) em realizar o processo de aplica o de insulina antes e após o ensino em grupo desse procedimento. A coleta de dados se deu pela aplica o de um instrumento em forma de check list, que avaliou a habilidade inicial da pessoa com DM em rela o ao processo. Posteriormente, foi realizada uma interven o educativa em grupo, sendo aplicado novamente o mesmo instrumento para avalia o da habilidade final. Os dados obtidos foram armazenados em um banco de dados e submetidos à análise estatística. Os resultados mostraram que a maioria das pessoas apresentou dúvidas em quase todas as etapas do processo de aplica o de insulina, n o realizando algumas etapas essenciais ou executando-as de forma errada. Após a interven o educativa houve aumento dos acertos, evidenciando que a atividade contribui para o autocuidado no tratamento insulinoterápico.

Abstract: the chemical composition of two specimens of esenbeckia grandiflora, collected in the south and northeast regions of brazil, was investigated. in this study, three β-indolopyridoquinazoline alkaloids from the leaves (rutaecarpine, 1-hydroxyrutaecarpine) and roots (euxylophoricine d) were isolated for the first time in this genus. in addition, the triterpenes α-amyrin, β-amyrin, α-amyrenonol, β-amyrenonol, 3α-hydroxy-ursan-12-one, and 3α-hydroxy-12,13-epoxy-oleanane, the coumarins auraptene, umbelliferone, pimpinelin, and xanthotoxin, the furoquinoline alkaloids delbine and kokusaginine, and the phytosteroids sitosterol, stigmasterol, campesterol and 3β-o-β-d-glucopyranosylsitosterol were also isolated from the leaves, twigs, roots and stems of this species. structures of these compounds were established by spectral analysis.
