Controle de injúrias sob a ótica da pediatria contextual

Abstract: objective: to describe the relationship between injury control and contextual pediatrics. sources of data: quasi-systematic review of medline, scielo and lilacs databases, using combinations of the words contextual, community, injury, accident and violence; and non-systematic review of book chapters and classic articles. summary of the findings: safety depends on the interaction of family habits, cultural patterns and surroundings. contextual pediatrics sees the child, the family, and the community as a continuum; health diagnosis (sequential observation of problems and assets) is one of its cornerstones. changing intrapersonal factors for injuries requires the use of both passive and active strategies. family and cultural risk factors for injury: home overcrowding, moving, poverty, and young, illiterate and unemployed parents. the main neighborhood factors: material deprivation and traffic. cultural factors: illiteracy, unsafe products, lack of mass transportation, handguns, workplaces without safety rules, faulty community organization, lack of communication between social sectors, inadequate legislation, low priority for safety among government actions, lack of economic resources, and low academic commitment with the field of safety. conclusions: the pediatrician's roles include strengthening of the longitudinal relationship with families, integrated interdisciplinary work, constructive intervention, partnership with community, counseling on injury risks pertaining to each developmental stage, by using lists with explicit processes and contents, and by handing out written materials. active advocacy for safety promotion in different environments, besides the clinical setting.
A puericultura hoje: um enfoque apoiado em evidências

Abstract: objective: health promotion is one of the chief activities of pediatricians, although still lacking enough scientific bases. this article reviews the scientific support for the main preventive interventions, as well as who should make them, when and how they should be made. sources of data: systematic review of recent literature, through the search of medline and lilacs databases, using the terms well-child care, health supervision and health promotion (both in english and portuguese); non-systematic review of reference lists of book chapters; non-systematic internet search of organizations that make recommendations on health supervision; selection of classic articles within this field. summary of the findings: pediatricians must seek integration with other professionals in providing preventive services, as well as in establishing effective partnerships with all community sectors. it is essential to improve communication skills. the ideal number of health supervision visits has never been established; interventions must be individually adapted according to family and community contextual risk factors. there is scientific evidence of the effectiveness of prenatal visits, preventive guidance, metabolic screening, immunization, and vision and hearing screening. growth monitoring and development screening must be rationalized. the systematic repetition of complete physical examinations and laboratory tests is not warranted. conclusions: pediatricians play a fundamental role in child and adolescent health promotion, but their actions regarding well-child care must no longer be totally empirical. there are innumerous evidence-based resources to guide pediatricians as to the most effective interventions.

Abstract: the modern physician must intensify his human attributes, adapt to varied contexts, and keep continued education. the formation of doctors usually does not take place in the context of real clinical practice but in the hospital, with emphasis on disease. the national curricular guidelines for medical graduation courses call for an articulation between the university and the health system and foster the development of personal skills in continued education. thus, the settings for the medical practice should be the outpatient units of the health services, and the learning process should be focused on the health needs of the community. nowadays there is a strengthening of clinical academic medicine, which combines health care with research, education, and administration. the student must assimilate the modern view of a doctor-patient relationship with respect to shared decision- making, a task that requires developing communication skills. teaching institutions and health care services as well as the media play a key role in this process. the introduction of the student to the scenery of clinical practice must be integral part of all disciplines of the curriculum, allowing the student to turn his attention to the health needs of the community. for establishing an innovative medical curriculum, educators need to understand the weariness of the traditional education model based on disease and transmission of knowledge, which turns the student away from the practical view of medicine. the modern pedagogical approach emphasizes formative aspects. its chief domains are: patient behavior, physician's attitude and role, doctor-patient interactions and sociocultural factors related to health care.
Anticuerpos ANTI-HTLV-I/II en una población del Amazonas colombiano

Abstract: En Colombia ya son varias las regiones donde se ha informado la presencia de la infección por los retrovirus linfotrópicos humanos tipos I y II (HTLV-I y HTLV-II). La principal región endémica para HTLV-I es el sur de la costa pacífica. El HTLV-II se ha encontrado en grupos indígenas que viven en diferentes regiones rurales como en la Guajira, la Orinoquía, en la costa pacífica y en una familia en Cali (urbana). Hasta el momento los estudios hechos en el Amazonas colombiano no habían confirmado la presencia de estos retrovirus. Se pretende estudiar la presencia de anticuerpos contra HTLV-I y/o HTLV-II utilizando una prueba comercial de Western blot que confirma y diferencia la infección por cada uno de los dos retrovirus mencionados, en caso de encontrar pruebas reactivas en pruebas de tamización. Luego de aceptar la participación en este estudio, se buscó la presencia de anticuerpos contra los retrovirus HTLV-I/II con pruebas de tamización en un total de 401 muestras de suero de personas que atendieron al Hospital de Leticia y a la consulta médica en Puerto Nari o. La muestra correspondió a 238 personas de raza mestiza y 163 de origen amerindio. Se realizaron pruebas de ELISA y/o aglutinación de partículas. Las muestras con pruebas de tamización reactivas, se sometieron a un estudio confirmatorio de la infección mediante el estuche comercial de Western blot Problot 2.4 de GeneLabs. En una mujer amerindia se demostró anticuerpos contra el HTLV-II, enun hombre mestizo contra el HTLV-I y en otro hombre mestizo contra el HTLV-II. No había enfermedad asociada en las personas estudiadas. La prevalencia de infección por los HTLV en la muestra estudiada fue 0.75%. Aunque este porcentaje no es muy elevado, debe alertar a las autoridades de salud de la presencia de estos retrovirus en la población y tomar las medidas pertinentes para evitar su contagio, así como estar atentos a la presencia de enfermedades asociadas con estos retrovirus.

Abstract: In the Tsarist, Soviet, and Russian military tradition, doctrine plays a particularly important role. Doctrine is supposed to represent an official view or views about the character of contemporary war, the threats to Russia, and what policies the government and armed forces will initiate and implement to meet those challenges. Since 2002 President Vladimir Putin has regularly called for and stated that a new doctrine, to meet the challenges of the post September 11 strategic environment will soon appear. However, no such doctrine has yet appeared or is in sight. Coherent planning and policy-making are still bedeviled by multiple threats that haunt senior military leaders. Today, if anything, we see a continuing inclination to turn back the strategic clock towards quasi-Cold war postures and strategies.

Abstract: OBJETIVO: a promo o da saúde é atividade essencial do pediatra, mas ainda pouco fundamentada em evidências científicas. Este artigo estuda o apoio científico para as principais interven es preventivas, quando, como e quem deve realizá-las. FONTES DOS DADOS: revis o sistemática da literatura recente, por meio de busca nos bancos de dados Medline e Lilacs, usando as palavras puericultura, supervis o de saúde e promo o de saúde (em inglês e português); revis o n o sistemática das referências bibliográficas de capítulos de livros; busca n o sistemática na Internet de organiza es que emitem recomenda es sobre supervis o de saúde; e sele o de artigos clássicos na área. SíNTESE DOS DADOS: o pediatra deve integrar-se com outros profissionais na presta o de servi os preventivos, bem como estabelecer parcerias efetivas com todos os setores da comunidade. é essencial aprimorar as habilidades em comunica o. O número ideal de consultas de supervis o de saúde nunca foi estabelecido; os procedimentos devem ser adaptados individualmente, segundo fatores de risco contextuais familiares e comunitários. Há inúmeros recursos, apoiados em evidências científicas, para guiá-lo quanto aos procedimentos mais efetivos.
