PLACE OF 5α-REDUCTASE IN THE THERAPY FOR BENIGN PROSTATIC HYPERPLASIA

Abstract: Twelve years ago, finasteride, the first 5α-reductase inhibitor, was introduced as drug therapy for benign prostatic hyperplasia, and more recently dutasteride has emerged as an alternative. The efficacy, safety and ability of these 5α-reductase inhibitors to reverse the natural progression of benign prostatic hyperplasia have been convincingly demonstrated and both drugs are now welt established in the medical armamentarium against the disease. Given the multifactorial etiology of benign hyperplasia, the usefulness of 5α-reductase inhibitors in combination with a adrenergic blockers has also been investigated and justified in select patients. Wider applications of 5α-reductase inhibitors are also emerging, though their perhaps most important new role as chemopreventive agents remains unclear.
CROSSING VESSELS IN THE URETEROPELVIC JUNCTION OBSTRUCTION: ROLE OF THE IMAGING METHODS IN SURGICAL TREATMENT PLANNING

Abstract: The knowledge of asssociation of the crossing vessels in the ureteropelvic junction obstruction is important in planning the therapeutic strategy (endoscopic procedure, laparoscopic or open pyeloplasty). Next to the well-known imaging methods (ultrasonography-US, intravenous urography-IVU and retrograde pyelography-RP) is added the Doppler colour ultrasonography (US) which in the hands of a well-trained uroradiologist is reliable enough in the preoperative diagnosis of the vessels crossing at the ureteropelvic junction. But there is a controversy regarding the functional significance of this vessels in the ureteropelvic obstruction, because the information brought by Doppler colour US relyes on topographical data. Crossing vessels are important for two reasons: they are a potential source of hemorrhage following endoscopic incision and they may play an etiologic role in ureteropelvic junction obstruction and therefore may be a reason for failure of endoscopic techniques. OBJECTIVE: This article reviews the reliability of the imaging methods, especially colour Doppler US, in the detection of crossing vessels in patients with surgically proven ureteropielic junction obstruction. METHODS: The files of all 117 patients who were diagnosed with ureteropelvic junction obstruction in our clinic between 2002 and 2005 were reviewed. There were selected and analysed the cases of 21 patients surgically treated and intra-operative diagnosed with crossing vessels. The indication for surgery was according to the usual criteria. RESULTS: Usual imaging methods (US, IVU) were used in all cases, and RP only in 9 selected cases. The results cleared up the three goals of imaging for ureteropelvic junction obstruction: to determine the presence and degree of renal obstruction, to assess renal function, and to determine the cause of the problem. For this final objective, Doppler colour US proved to have a sensibility of 79,6% in detecting the crossing vessels. CONCLUSION: Colour Doppler US is reliable in the detection of crossing vessels at the ureteropelvic junction guiding the strategy to open dismembered pyeloplasty.

Abstract: According to many of its traditional critics, the main weakness in Kantian moral-political philosophy resides in its impossibility of admitting exceptions. In nuce, all these critical positions have converged, despite their reciprocal heterogeneity, in the so called accuse of moral rigorism (unjustly, I would say) directed against Kant’s moral and political perspective. As such, basically, I will seek to defend Kant against this type of criticism, by showing that any perspective attempting to evaluate Kant’s ethics on the grounds of its capacity or incapacity to admit exceptions is apriorily doomed to lack of sense, in its two logical alternatives, i.e. either as nonsense (predicating about empty notions), or as tautology (formulating ad hoc definitions and criteria with respect to Kant’s system and then claiming that it does not hold with respect to them). Essentially, I will try to show that Kantian ethics can organically immunize itself epistemologically against any such so called antirigorist criticism.
HOBBES’ POLITICAL PHILOSOPHY

Abstract: This is basically an attempt at an original conceptual reconstruction of Hobbes’ philosophy as set in Lehiathan, namely one in the view of which Hobbes was neither an atheist nor an absolutist, as the standard interpretation holds, but rather what we could call an agnostical pragmatist (fact which, quite surprisingly, places Hobbes in the company of Burke). More to the point, my basic claim within this paper is that Hobbes was not such an ‘enemy of individual freedom’ as we traditionally hold him to be and that his thought was just as attached to the notion of individual freedom as the later contractualist views. The difference however, arises from the fact that Hobbes, unlike Locke, Rousseau or Kant, was what we could call a voluntaristic determinist and consequently viewed human freedom not so much as ‘unhindered action derived from reflective choice’, but rather as what we could call ‘reasonable fulfillment of the basic human inclinations’ (self-interest). As such, I will analyze the three main focal points of Hobbes’ thought, namely (i) human nature, (ii) the principle of association and (iii) the principle of authority. More specifically I will try to offer a perspective on the link between his voluntaristic determinism, his notion of legitimate absolute coercion (sovereignity) and his political theology (the view that any form of political authority rests on a religious legitimacy) in trying to demonstrate how all these were Hobbes’ specific way of seeking to find individual freedom a place under the sun.
Semi-invariant submanifolds of (g, F)-manifolds

Abstract: We introduce (g,F)-manifolds and initiate a study of their semi-invariant submanifolds. These submanifolds are generalizations of CR-submanifolds of Kaehler manifolds. We obtain necessary and sufficient conditions for the integrability of distributions on a semi-invariant submanifold and study the geometry of foliations defined by these distributions. In particular, for a large class of (g,F)-manifolds we prove the existence of a natural foliation on their semi-invariant submanifolds.
Normal Anti-Invariant Submanifolds of Paraquaternionic K hler Manifolds

Abstract: We introduce normal anti-invariant submanifolds of paraquaternionic K hler manifolds and study the geometric structures induced on them. We obtain necessary and sufficient conditions for the integrability of the distributions defined on a normal anti-invariant submanifold. Also, we present characterizations of local (global) anti-invariant products.

Abstract: : Among the specimens of Mytilus galloprovincialis Lmk. from the Black Sea, function of the color of the ostracum (brown, dark blue or brown with blue stripes), one can differentiate several forms. These colors are genetically determined (Stolbova, Pirkova, Ladyghina, 1996; Scherban, 2000; Shurova, 2001). The present paper analyzes the situation of these phenotypic groups, using specimens sampled at different depths in the area of Agigea dike.
