Abstract: chronic obstructive pulmonary disease (copd) is a leading cause of morbid-mortality world wide, leading not only to pulmonary damage but also to multisystemic impairment, with repercussions on skeletal muscles and the ability to undertake effort, as measured in the six-minute walk test (6-mwt). aims: to correlate the level of obstruction in copd with lactate concentration and heart rate (hr) at rest, and distance walked. to correlate distance walked with blood gas analysis and correlate desaturation in 6-mwt with post 6-mwt lactate concentration and heart rate. methods: copd patients underwent spirometry, blood gas analysis and 6-mwt to evaluate distance walked, heart rate, capillary lactate (cl) concentration pre- and post 6-mwt, and desaturation with 6-mwt. results: 91 patients with all levels of obstruction were evaluated. hr and cl increased significantly post 6-mwt. the decrease in peripheral saturation of haemoglobin to oxygen observed with 6-mwt was also significant. the distance walked was shorter the greater the obstruction. the correlation analysis was significantly positive between fev1 and distance walked, negative between fev1 and hr at rest and negative between distance walked and paco2, and not significant for the other variables. conclusions: increased obstruction in copd and higher paco2 values contribute to a reduction in distance walked in 6-mwt. the level of obstruction in copd leads to a haemodynamic impairment with increased hr at rest of these patients.

Abstract: The article presents the normative framework of alternatives to sanctions and the mechanisms of procedural diversification in the Brazilian Law, more specifically in laws 8.069/90, 9.099/95 and 9.714/78. In a comparative approach, it evaluates both the penal law system and the juvenile law system of response to offenses, highlighting the penalties applicable to young offenders in conflict with the law. The study analyzes the process of community service, problematizing the existing tension between alternative sanctions to incarceration and the expansion of the punitive power in the contemporary world (punitivism).Resumo: O artigo apresenta o quadro normativo de alternativas às penas e às medidas (socioeducativas e de seguran a) e os mecanismos de diversifica o processual no direito brasileiro no Estatuto da Crian a e do Adolescente (Lei n. 8.069/90), na Lei dos Juizados Especiais Criminais (Lei n. 9.099/95) e na Lei das Penas Alternativas (Lei n. 9.714/78). A partir de uma abordagem comparativa, avalia os sistemas de direito penal e de direito juvenil de resposta ao ilícito, enfatizando as san es aplicáveis ao adolescente em conflito com a lei. Concretiza o estudo na análise da presta o de servi o à comunidade, problematizando a tens o entre as san es alternativas ao encarceramento e a amplia o da rede do poder punitivo na contemporaneidade (punitivismo).

Abstract: O presente trabalho objetiva analisar o crescente papel do Judiciário no controle das políticas públicas e sua repercuss o sobre o tema da eficácia dos direitos fundamentais sociais. A Constitui o Federal de 1988 assegura a aplica o imediata dos direitos fundamentais, independentemente de sua fei o e mesmo que n o estejam expressamente consignados em seu texto. A despeito disso, é inegável a dificuldade enfrentada quando se trata de direitos sociais, caracterizados por necessitarem de presta es positivas por parte do Estado. Diante desse panorama, analisa-se se o princípio da reserva do possível pode ser aplicado ao ordenamento jurídico brasileiro como forma de justificar determinadas escolhas trágicas, além de sua liga o com o conceito de mínimo existencial. Por fim, é estudada a importante discuss o travada acerca da possibilidade e dos limites da atua o do Judiciário no controle destas políticas públicas desenvolvidas pelo Executivo e pelo Legislativo. Abstract: fine-needle aspiration (fna) biopsy is considered the routine initial and the most cost-effective procedure in the evaluation of thyroid nodules. notwithstanding the advantages of the procedure, it depends heavily on the pathologist's experience and on technical skills to obtain adequate smears. the aim of this paper is to evaluate the procedure efficacy in a general teaching hospital. smears obtained from 754 fna were examined over a 3-year period. each palpable nodule was aspirated by an endocrinology resident with a fine-needle 4 to 6 times. material were stained by giemsa and papanicolaou techniques. histopathologic examination was done on 93 patients. fna biopsy was performed in 663 patients (613f/50m), mean age of 50±14 years (range 12 - 93). fna results were: colloid nodule (58.5%), follicular neoplasm or suspicious (8.2%), hashimoto's thyroiditis (5.3%), papillary carcinoma (2.3%), anaplastic carcinoma (0.4%), medullary carcinoma (0.1%), and unsatisfactory (25.2%). neoplasias were detected in 41 (44%) of 93 surgeries undertaken, and most of the carcinomas were of papillary type (65.6%). there were 24% follicular carcinomas among the follicular neoplasms. the fna data analysis revealed: sensitivity 92% (ci= 78.6 to 98.3%), specificity 80% (ci= 65.4 to 90.4%), positive predictive value 79.5% (ci= 64.7 to 90.2%), negative predictive value 92.3% (ci= 79.1 to 98.4%), and accuracy 86% (ci= 76.1 to 92.3%). fna established a diagnosis in 66.6% of the patients, decreasing costs and the number of surgeries. as a result of better patient selection, the yield of thyroid carcinomas in excised nodules has increased from 19.4% to 31.2% in a period of 11 years in our hospital.

Abstract: the observation of generalized gm1 gangliosidosis type 1 (norman-landing disease) is reported. the case is typical, featuring all the main clinical and biological signs of the disease. diagnosis was established by the demonstration of a severe deficit in beta-galactosidase activity in leucocytes, by the demonstration of oligosacarides in the urine, and by the histological examination after the fatal outcome before the age of two with severe respiratory distress.
