Pleuresia inflamatoria en la infancia: Conducta quirúrgica

Abstract: pneumonia complicated due to pleurisy has always been a process affecting morbidity and mortality at the pediatric age and on ocassions some surgical procedure has been necessary. the initial decision wether to perform only a pleural puncture or to add a minimum pleurotomy may be controversial. that's why it was decided to conduct a study of all the children that were admitted at the pediatric teaching hospital of san miguel del padrón with pneumonia complicated due to inflammatory pleurisy (pleurisy during an inflammatory pulmonary process) and that underwent surgery to resolve the effusion from january 2000 to january 2001.the medical histories of the 10 patients were studied. tables were made and figures were obtained. the age group 1-5 was the most affected with 40 %. no sex preponderance was observed and the most frequent localization of the inflammatory process (90 %) was the right lung and, within this, the right basis with 40 %. 50 % of the patients underwent only pleural puncture and in the other half pleurotomy was performed, too. 30 % of this last group had more than 150 ml in the initial puncture. a minimum pleurotomy was necessary in 5 patients. la neumonía complicada por pleuresía ha sido siempre un proceso que ha afectado la morbilidad y mortalidad en la edad pediátrica y en ocasiones necesita de algún proceder quirúrgico para su solución. la decisión inicial de si realizar solo una punción pleural o agregarle una pleurostomía mínima, puede ser controversial, por lo que se decidió realizar un estudio de todos aquellos ni?os que fueron ingresados en el hospital pediátrico universitario de san miguel del padrón con neumonía complicada por pleuresía inflamatoria (pleuresía durante un proceso inflamatorio pulmonar), a los cuales hubo que realizarles algún proceder quirúrgico para la solución de su derrame, en el período de enero de 2000 a enero de 2001; se estudiaron los expedientes clínicos de 10 pacientes; se les conformaron tablas y figuras. el grupo d

Abstract: La neumonía complicada por pleuresía ha sido siempre un proceso que ha afectado la morbilidad y mortalidad en la edad pediátrica y en ocasiones necesita de algún proceder quirúrgico para su solución. La decisión inicial de si realizar solo una punción pleural o agregarle una pleurostomía mínima, puede ser controversial, por lo que se decidió realizar un estudio de todos aquellos ni os que fueron ingresados en el Hospital Pediátrico Universitario de San Miguel del Padrón con neumonía complicada por pleuresía inflamatoria (pleuresía durante un proceso inflamatorio pulmonar), a los cuales hubo que realizarles algún proceder quirúrgico para la solución de su derrame, en el período de enero de 2000 a enero de 2001; se estudiaron los expedientes clínicos de 10 pacientes; se les conformaron tablas y figuras. El grupo de edad más afectado fue el de 1 a 5 a os con un 40 %. El sexo no tuvo preponderancia y la localización más frecuente del proceso inflamatorio (90 %) fueron el pulmón derecho, y entre estas la base derecha con un 40 %. Al 50 % de los pacientes se le realizó solamente una punción pleural y a la otra mitad se le a adió una pleurostomía, y tenía el 30 % en este último grupo más de 150 mL en la punción inicial; a 5 pacientes hubo que realizarles una pleurostomía mínima. Con estos resultados se consideró que en el momento de la punción pleural inicial el hallazgo macroscópico de aspecto fibrino purulento es indicativo de pleurostomía, así como este aspecto en el líquido de una segunda punción de ser necesario ésta y una celularidad mayor de 175 ′ 106, y una glucosa menor de 1 mmol/L en el microscópico del líquido de la primera, si se hace necesario realizar una segunda punción o los mismos resultados del líquido proveniente de esta segunda o cualquier otra punción. Abstract: ambulatory major elective surgery has imposed itself in the world. this method began to be developed at the pediatric teaching hospital of san miguel del padrón, in october, 1988, and it has been enriched and transformed since then. to know the results of this method, it was decided to study what was done with this type of procedure in 2000 and 2000. different variables were also studied. a total of 1 335 ambulatory elective major operations were performed during these 2 years, accounting for 65 % of the total elective major surgery in 2000 and 60 % in 2001. there was a saving in the expenses by operation and an increase in the population's satisfaction as regards short-stay major surgery, which makes this method recommendable to be used in 60 % of the cases in all the pediatric hospitals with surgery service.

Abstract: the recurrent abdominal pain is very common in childhood, and on some ocassions it is not possible to find the cause. to make clear at least in part this problem, it was decided to study the motor disorders of the biliary tract in childhood as the possible causes of these clinical pictures. a prospective study was conducted at the "san miguel del padrón" pediatric teaching hospital from 1989 to 1994 with all those children suffering from recurrent abdominal pain and with no specific cause. ultrasonography and oral cholecystography with timed emptying were used and 5 patients were found on whom these diagnoses could be made. the most frequent was the hypertonic and hypercinetic gallbladder in 2 patients who underwent cholecystectomy; the rest had a satisfactory evolution without surgical procedure. according to the results obtained, it is suggested that these disorders must be taken into account in the study of patients with recurrent abdominal pain.

Abstract: En la infancia el dolor abdominal recurrente es muy frecuente, y en algunas ocasiones no es posible encontrar la causa. Con el propósito de elucidar en parte este problema, se decidió estudiar los trastornos motores del sistema biliar en la infancia como posibles causas de estos cuadros. Se realizó un estudio prospectivo en el Hospital Pediátrico Docente "San Miguel del Padrón", de 1989 a 1994 de todos aquellos ni os con dolor abdominal recurrente y sin una causa específica. Se usó ultrasonografía y colecistografía oral con vaciamiento cronometrado y se encontraron 5 pacientes en quienes se pudo realizar esos diagnósticos. El más frecuente fue la vesícula biliar hipertónica e hipercinética en 2 pacientes a quienes se les efectuó una colecistectomía; el resto tuvo una evolución satisfactoria sin intervención quirúrgica. Se sugiere debido a los resultados obtenidos, que estos trastornos deben ser tenidos en cuenta en el estudio de pacientes con dolor abdominal recurrente. Variational approach to vortex penetration and vortex interaction

Abstract: A variational calculation for vortex penetration is presented. Variational trial functions for the Meissner state are combined with variational functions for a vortex near the surface. The latter is based on Clem's trial solutions for a vortex in bulk, which were adapted to include surface effects through consideration of an image vortex. Three variational parameters are considered, corresponding to the effective coherence length of the vortex, the effective penetration length for the Meissner currents, and the value of the order parameter at the surface. The results show that the last two variational parameters are independent of vortex position. Explicit calculations are presented for several $\kappa$ values. The energy barrier for vortex penetration is shown to be in good agreement with full numerical calculations of the Ginzburg--Landau equations. We consider the variation of the magnetic flux carried by a vortex as it gets inside the superconductor and agreement with known experimental and theoretical results is obtained. The model was extended to calculate the force between two vortices, and the results show that the force goes to zero as the pair comes close to the surface. This result can be of interest for the study of the melting of the vortex lattice and for vortices confined in mesoscopic superconductors. The variational approach can be very helpful for intermediate $\kappa$ values when numerical calculations become computationally demanding because it provides manageable expressions for all physically relevant quantities.

Abstract: site index is the most commonly used method in mexico and in several countries to estimate the productivity of forest lands, since it is a numerical expression easy to understand and density-free. the objective of this work was to adjust model groups to predict the site index of pinus durangensis from data of a the remeasurement of 12 permanent plots settled in a natural forest in the ejido vencedores at san dimas, durango. thirteen mathematical models were adjusted through the mean square procedure, ten of which are linear and three non-linear. were adjusted three groups of models of linear simple regression, logarithmic,quadratic and a mixed model. in regard to the three non-linear, the chapman-richards and schumacher functions as well as the weibull model were tested. the best models were selected according to the degree of adjustment or pseudo r2, the f test, the mean square error (mse), the test of hypothesis of the estimators and the sign of the parameter. results show that provided the best site index estimation was the logarithmic 6 (ln[e/h] = β0 + β1[1/e]) since it revealed the highest values for the residual r2 test, which was 0.7419; mse was 0.01608 and 221.304 in the f test.

Abstract: the streptococcus pneumoniae, the main causal agent of community pneumonia, leader in the etiology of the otitis media and the meningitis, during the past three decades has increase in a significant way its resistance to the more used therapeutic agents including the beta-lactamase, macrolides, azalides and fluroquinolones. adaptive versatility of the microorganism allows it to create mechanisms able to overcome to any of these therapeutical aggressions with a variable degree of effectiveness. authors made a review of the more important mechanisms involved in acquisition of the antimicrobial resistance by s. pneumoniae and some of risk factors involved in the infection due to resistant s. pneumoniae are specified.

Abstract: El Streptococcus pneumoniae, principal agente causal de la neumonía comunitaria, líder en la etiología de la otitis media y la meningitis, en las últimas 3 décadas ha incrementado, de manera importante, su resistencia a los agentes terapéuticos más utilizados, como los betalactámicos, macrólidos, azálidos y fluroquinolonas. La versatilidad adaptativa del microorganismo le ha permitido crear mecanismos capaces de sobreponerse a cualquiera de estas agresiones terapéuticas con un grado variable de eficacia. Se realiza una revisión de los mecanismos más importantes implicados en la adquisición de resistencia antimicrobiana por S. pneumoniae, y se precisan algunos de los factores de riesgo implicados en infección por S. pneumoniae resistente.
