Abstract: the present paper does a review and analyses the models which have been developed for the analysis of determinants and explanatory factors of the health-disease process as an elementary input for designing educational programs in the training of health manpower. establishing the hypothesis, variables, advantages, disadvantages, time and authors of each model, the object of the analysis includes the models' methodological and/or scientific aspects: magic-religious, sanitary, unicausal, multicausal, epidemiological, ecological, social, social-historical, geographical, economic and interdisciplinary. the discussion and conclusions focus on the critical and reflexive characteristics of the models, contradictions and compatibilities among them, and their relevance for the training of human resources in health.

Abstract: this article includes the results of a multi-level conceptual approach review, considering four models of analysis, delving into the determinants and the explanatory factors in the process of search and use of health services. such models are: psychosocial, epidemiological, sociological, and socio-economic. the implications, determinants, and characteristics of utilization of health care services have been subject to scrutiny since the eighties. research on this matter has not been independent of the process of reform, which has been carried out simultaneously in virtually every country in the world. in fact, establishing adequate health services for patients who need them most has become a generally accepted priority. for the purpose of this study, and as a preamble to understanding the process of search and use of health services, the concept of need, along with its applications in the different models of service use, must be defined. in the literature, there are four types of need: a) normative need, that which is expert-defined as such in a given situation, b) felt need, which corresponds to a perceived lack of something beneficial to the individual, c) expressed need, one not only experienced, but also articulated by the individual, and d) comparative need, i.e. a standard that meets the same characteristics of another receiving certain coverage or service. this paper will refer to the expressed need, which may or may not correspond exactly to the use of health care services. along the same lines, the concept of demand, applied from an economic perspective, will be utilized as a synonym for use. the implementation of reform processes and the evaluation of the possible implications of such reforms require extensive knowledge regarding the determinants of use of health care services. a main determinant can be identified, for example, when an individual uses such services, sometimes repeatedly, because he is in a situation of need (experienced), understood as the

Abstract: El presente artículo incluye los resultados de una revisión y reflexión sobre diferentes niveles de aproximación conceptual que plantean cuatro modelos de análisis para el estudio de los condicionantes, determinantes y factores explicativos del proceso de búsqueda y utilización de servicios de salud. Dichos modelos son: el psicosocial, el epidemiológico, el sociológico y el socioeconómico. Se parte de la premisa central de que en el contexto de reformas del sector salud en países de América Latina, y particularmente en México, los usuarios de servicios de salud pasan a ocupar un rol preponderante en el establecimiento de las necesidades de salud que deberán responder los sistemas de atención médica. En efecto, los principales hilos conductores y de ajuste estructural de los sistemas de salud lo constituyen los cambios en el establecimiento de necesidades de salud de acuerdo con cambios epidemiológicos, cambios en la percepción de da os a la salud, así como cambios en las variables de equidad, eficiencia, cobertura y accesibilidad de los sistemas de salud para diferentes grupos poblacionales. Desde esta nueva perspectiva, los cambios del sistema de salud proponen a su vez cambios relevantes en la interacción entre los dos principales actores sociales de este proceso de reforma: por un lado, el comportamiento de los proveedores (toda institución que provee servicios de salud a la población) y, por otro, el comportamiento y percepción de los usuarios (población que busca y solicita servicios de salud a determinada institución).

Abstract: In the present paper we consider biologically motivated reduction operations on double occurrence words. Then we define the nesting index of a double occurrence word to be the least number of reduction operations it takes for a word to be reduced to the empty word. We use chord diagrams and circle graphs as tools to study the nesting index of double occurrence words.
A New Era in the Management of the Hepatitis C  [PDF]

Hepatitis C is an infection caused by the hepatitis C virus that attacks the liver and leads to inflammation. The current standard-of-care regimens include a protease inhibitortelaprevir or boceprevirin combination with pegylated interferon and ribavirin. Hepatitis C treatment options on the horizon hold promise for better viral clearance with less toxicity than current regimens. There are new data about new drugs, both direct-acting antivirals while minimizing intolerable side effects or adverse events. Developed new data from 4 phase 3 trials with the hepatitis C drug sofosbuvir and ribavirin show that a 12-week regimen is effective in treating HCV genotypes 1 through 6. In the Annual Scientific Meeting and Postgraduate Course of the American College of Gastroenterology, different research was presented that was drawn from 4 phase 3 studies: NEUTRINO, FISSION, POSITRON and FUSION which enrolled different types of patients, who received Sofosbuvir with Peginterferon Alfa 2a and Ribavirin for 12 or 24 weeks in treatment; for all studies, the primary end point was sustained virologic response at 12 and 24 weeks posttreatment. In all studies, sofosbuvir was well tolerated, with a low incidence of adverse events. In conjunction with the suggested brief duration of this regimen, this indicates that drug combinations should improve treatment adherence compared with IFN-based treatment. In conclusion, 2 novel direct-acting antiviral agentssofosbuvir and simeprevirtarget various components of the HCV genome.

Abstract: a description is given of both the male and female of a new species of phlebotomine sandfly cought in tree trunks in yacambú nacional park, state of lara, venezuela. the new species belongs to the subgenus helcocyrtomyia, barretto 1962 or group vexator, series peruensis of theodor (1965) and of young and fairchild (1974). the male is characterized by the presence of 5 spines on the dististyle and a basal patch of long bristles on the basistyle, implanted on queratinized, strong tubercules. female presents cibarial armature with 4 large teeth, and a segmental spermatheca similar to another species of the series peruensis.
