Duplicidad intestinal en el adulto joven como causa de tumor abdominal recurrente

Abstract: Se presenta un paciente masculino, blanco, de 21 a os de edad con una duplicidad intestinal dependiente de colon transverso, malformación digestiva infrecuente nunca antes reportada en nuestro hospital. Se muestran el estudio clínico y los exámenes complementarios realizados, los que incluyeron, humorales, radiografías de colon por enemas, ecografía abdominal, laparoscopia y la laparotomía. Describimos la malformación encontrada, la técnica quirúrgica empleada y el estudio anatomopatológico de la pieza extraída, la evolución clínica, y las complicaciones aparecidas. A los 43 días fue dado de alta con tratamiento ambulatorio, evolucionando favorablemente. El resultado anatomopatológico fue: Duplicidad intestinal dependiente del colon transverso. Fue egresado vivo y se encuentra totalmente curado. Se realiza una revisión de la literatura y se exponen los criterios de mayor interés. A white male patient is reported, of 21 years of age with an intestinal duplication clerk of transverse colon, uncommon digestive malformation never before reported in our hospital. The clinical study and the carried out complementary exams those are shown that included, humorales, contrasted X-rays of colon, abdominal ultrasonography, laparoscopy, and the laparotomy. We describe the opposing malformation, the surgical technique employed and the histopathological conclusions of the extracted piece, the clinical evolution, and the complications. After 43 days he was discharged with ambulatory treatment, evolving favorably. The histopathological conclusions were: Intestinal Duplication clerk of the transverse colon. At present he is completely cured. A revision of the literature is carried out and the approaches of more interest are exposed.
Algunos aspectos clinicoquirúrgicos del neumotórax espontáneo

Abstract: an observational descriptive research was made to know the behaviour of some clinicosurgical aspects of the spontaneous pneumothorax, which was diagnosed in 180 patients attended at the service of general surgery of the "capitán roberto rodriguez fernández" provincial general hospital, in morón, from 1985 to 1999, including both. most of the patients were males and age groups were 15-44. 83.3% of the patients were smokers. 45.6% of the patients had no pathological history. pain and dyspnoea were the predominant symptoms. 50.6% of the studied individuals had a pneumothorax over 50%. minimum high pleurostomy and pleural puncture with aspiration were the most used procedures. the latter proved to be curative in 70.9% of the pneumothoraxes between 26 and 50%. pulmonary inexpansibility was the main complication. there was 2.8% of mortality

Abstract: an observational descriptive research was made in order to know the behavior of major surgery applied to outpatients from the service of general surgery of the "capitán roberto rodriguez fernández" general teaching hospital, in morón, from january 1st, 1996, to june 30th, 1998. most of the cases of both sexes were 15-44 years old. inguinal hernia was the most frequent affection and, therefore, the most performed operation. 10.5% of the operated on individuals had associated diseases. spinal anesthesia was used in 61.8% of those who underwent surgery. 2.2% had complications and just 0.4% needed hospitalization. 100% of the patients were followed up by the specialized surgery department and the family physician. the method had good acceptation in 99.6% of the investigated patients

Abstract: Se realizó una investigación observacional-descriptiva para conocer el comportamiento de la cirugía mayor aplicada a pacientes ambulatorios del Servicio de Cirugía General del Hospital General Provincial Docente Capitán Roberto Rodríguez Fernández , de Morón, durante el período comprendido entre el 1ro. de enero de 1996 y el 30 de junio de 1998. La mayor proporción de casos de uno y otro sexos, pertenecía a los grupos de edad de 15 a 44 a os. La hernia inguinal fue la afección más frecuente y por ende la intervención quirúrgica más realizada. El 10,5 % de los operados presentaba enfermedades asociadas. La anestesia espinal se utilizó en el 61,8 % de los intervenidos. El 2,2 % de los intervenidos tuvo complicaciones; sólo el 0,4 % de los pacientes necesitó hospitalización. El 100,0 % de los pacientes fue seguido por consulta especializada de cirugía y el médico de la familia. Algunos aspectos clinicoquirúrgicos del neumotórax espontáneo

Abstract: Se realizó una investigación observacional-descriptiva para conocer el comportamiento de algunos aspectos clinicoquirúrgicos del neumotórax espontáneo, el que fue diagnosticado en 180 pacientes atendidos en el Servicio de Cirugía General del Hospital General Provincial Docente Capitán Roberto Rodríguez Fernández , de Morón, desde 1985 hasta 1999, ambos inclusive. La mayor proporción de enfermos pertenecía al sexo masculino y a los grupos de edad de 15 a 44 a os. El 83,3 % de los pacientes era fumador. No existieron antecedentes patológicos demostrables en el 45,6 % de los enfermos. El dolor y la disnea fueron los síntomas predominantes. El 50,6 % de los investigados tenía un neumotórax mayor del 50 %. La pleurostomía mínima alta y la punción pleural con aspiración fueron los procederes mayoritarios, esta última curativa en el 70,9 % de los neumotórax entre 26 y 50 %. La complicación principal fue la inexpansibilidad pulmonar. Swift J1357.2-0933: the faintest black hole?

Abstract: Swift J1357.2-0933 is the first confirmed very faint black hole X-ray transient and has a short estimated orbital period of 2.8 hr. We observed Swift J1357.2-0933 for ~50 ks with XMM-Newton in 2013 July during its quiescent state. The source is clearly detected at a 0.5-10 keV unabsorbed flux of ~3x10^-15 erg cm-2 s-1. If the source is located at a distance of 1.5 kpc (as suggested in the literature), this would imply a luminosity of ~8x10^29 erg s-1, making it the faintest detected quiescent black hole LMXB. This would also imply that there is no indication of a reversal in the quiescence X-ray luminosity versus orbital period diagram down to 2.8 hr, as has been predicted theoretically and recently supported by the detection of the 2.4 hr orbital period black hole MAXI J1659-152 at a 0.5-10 keV X-ray luminosity of ~ 1.2 x 10^31 erg s-1. However, there is considerable uncertainty in the distance of Swift J1357.2-0933 and it may be as distant as 6 kpc. In this case, its quiescent luminosity would be Lx ~ 1.3 x 10^31 erg s-1, i.e., similar to MAXI J1659-152 and hence it would support the existence of such a bifurcation period. We also detected the source in optical at r' ~22.3 mag with the Liverpool telescope, simultaneously to our X-ray observation. The X-ray/optical luminosity ratio of Swift J1357.2-0933 agrees with the expected value for a black hole at this range of quiescent X-ray luminosities.

Abstract: the 92,1 % (210 patients) of the population over 40 years treated at a family physician's office in morón municipality took part in a glaucoma mass screening. it was found that 9,6 % (20 patients) showed figures of ocular tension exceeding normal values and that 3 patients had normal ocular tension but presented suspicious excavation of the optic disc. two patients having glaucoma family history were referred to the specialist at the provincial general teaching hospital of moron in order to confirm the diagnosis.
A Clean Sightline to Quiescence: Multiwavelength Observations of the High Galactic Latitude Black Hole X-ray Binary Swift J1357.2-0933

Abstract: We present coordinated multiwavelength observations of the high Galactic latitude (b=+50 deg) black hole X-ray binary (XRB) J1357.2-0933 in quiescence. Our broadband spectrum includes strictly simultaneous radio and X-ray observations, and near-infrared, optical, and ultraviolet data taken 1-2 days later. We detect Swift J1357.2-0933 at all wavebands except for the radio (f_5GHz < 3.9 uJy/beam). Given current constraints on the distance (2.3-6.3 kpc), its 0.5-10 keV X-ray flux corresponds to an Eddington ratio Lx/Ledd = 4e-9 -- 3e-8 (assuming a black hole mass of 10 Msun). The broadband spectrum is dominated by synchrotron radiation from a relativistic population of outflowing thermal electrons, which we argue to be a common signature of short-period quiescent BHXBs. Furthermore, we identify the frequency where the synchrotron radiation transitions from optically thick-to-thin (approximately 2-5e14 Hz, which is the most robust determination of a 'jet break' for a quiescent BHXB to date. Our interpretation relies on the presence of steep curvature in the ultraviolet spectrum, a frequency window made observable by the low amount of interstellar absorption along the line of sight. High Galactic latitude systems like Swift J1357.2-0933 with clean ultraviolet sightlines are crucial for understanding black hole accretion at low luminosities.
