Abstract: patients using obturator prostheses often present denture-induced stomatitis. in order to detect the presence of oral candida albicans in patients with oronasal communications and to evaluate the effectiveness of a topical antifungal treatment, cytological smears obtained from the buccal and palatal mucosa of 10 adult patients, and from the nasal acrylic surface of their obturator prostheses were examined. a therapeutic protocol comprising the use of oral nystatin (mycostatin?) and prosthesis disinfection with sodium hypochlorite was prescribed for all patients. seven patients were positive for c. albicans in the mucosa, with 1 negative result for the prosthetic surface in this group of patients. post-treatment evaluation revealed the absence of c. albicans on prosthesis surface and on the oral mucosa of all patients. the severity of the candidal infection was significantly higher in the palatal mucosa than in the buccal mucosa, but similar in the palatal mucosa and prosthesis surface, indicating that the mucosa underlying the prosthesis is more susceptible to infection. the therapeutic protocol was effective in all cases, which emphasizes the need for denture disinfection in order to avoid reinfection of the mucosa.

Abstract: the main objective of this study was to categorize the risk of suffering of cardiometabolic disease. for this, lipids and proteins metabolic biomarkers were used on sporting or sedentary students from universidad central de venezuela. the following parameters were determined by colorimetric methods: creatinine (cr), total protein (tp), albumin (al), triglycerides (tg), total cholesterol (t-cho) and high density lipoproteins (hdl-c). the low density lipoproteins (ldl-c) were calculated by using the friedewald formulae and globulins values were obtained by subtracting tp from al. the seric biomarkers levels were in the normal reference ranges. there were not significant differences on the hdl-c and globulins values between sporting and sedentary students. the sporting group showed very low tg levels, with high t-cho, due mainly to ldl-c. in this group, the conditions of the different components of the lipidic profile and t-cho/hdl-c rate were influenced by a transitory lipemic distortion, possibly due to inflammatory mechanism mediated by immune response, which is observed in physical activity of high intensity. low levels of hdl-c were obtained from both studied groups. only 2,1 % from the sedentary group and 1,0 % of total sample showed low levels of albumin, which is associated to high risk of suffering from cardiovascular disease. only a small part of the evaluated sample could be susceptible of suffering cardiometabolic disease.

Abstract: surgical replacement of the aortic valve is the therapy of choice for patients with severe symptomatic aortic stenosis. however, surgical risk increases significantly with age and with the association of comorbidities. in this clinical context, transcatheter aortic-valve implantation must be considered as a valid and effective therapeutic alternative. the purpose of this consensus is to provide a critical review of all the evidence and recent advances related to transcatheter aortic-valve implantation and summarize the current state of the art, in order to better guide therapeutic decision-making. patient selection for transcatheter aortic-valve implantation must meet the following criteria: 1) presence of severe symptomatic aortic valve stenosis, defined as aortic valve orifice area < 1 cm2 (or < 0.6 cm2/m2); 2) age > 75 years and a high probability of surgical morbidity and mortality due to pre-existing comorbidities; and 3) the presence of anatomical and morphological conditions favorable for transcatheter interventions. current scientific evidence indicates that this procedure improves symptoms and reduces mortality in patients with aortic stenosis who were not suitable candidates for surgery, as compared to standard therapy. additionally, in patients with severe aortic stenosis and high surgical risk, transcatheter aortic-valve implantation is an alternative effective treatment, able to alter the natural course of disease in a manner equivalent to conventional surgical treatment.

Abstract: background: surgical replacement of the aortic valve is the therapy of choice for patients with severe aortic stenosis. however, the surgical risk increases dramatically as age advances and with the association of comorbidities. recently, transcatheter aortic valve implantation has been introduced in brazil. therefore, it was important to create a national registry to determine the outcomes of this procedure in our country. the rationale and design of the brazilian transcatheter aortic valve implantation registry is described. methods: this is a multicenter prospective registry, including 200 patients with symptomatic aortic valve stenosis and high surgical risk submitted to transcatheter aortic valve implantation in brazilian institutions. the primary endpoint of the study is all cause mortality at 30 days, 12 and 24 months. secondary endpoints, among others, are: 1) technical success of transcatheter valve implantation; 2) cardiovascular mortality at 30 days, 12 and 24 months; 3) composite safety endpoint at 30 days; and 4) composite efficacy endpoint at 12 and 24 months. conclusions: the analysis of the results of this national registry will enable us to evaluate the outcomes of transcatheter aortic valve implantation in our country.
