Outcome of Pediatric Cataract Surgery in Patients Who Have Undergone Bone Marrow Transplantation  [PDF]
Ariel Chen, Kimberly G Yen
Open Journal of Ophthalmology (OJOph) , 2018, DOI: 10.4236/ojoph.2018.81008
Abstract:
Purpose: Bone marrow transplantation (BMT) and pre-treatment conditioning increases the risk of developing pediatric cataracts. We present the outcome of cataract surgery in children who have had BMT. Methods: We conducted a retrospective chart study with 15 BMT patients (28 eyes) who underwent cataract extraction between 2002 and 2012. Outcome measures include change in best corrected visual acuity (BCVA) and complications. Results: 7 (47%) patients had acute lymphoid leukemia, 3 (20%) had acute myeloid leukemia, 2 (13%) had myelodysplastic syndrome, 1 (7%) had Fanconi anemia, 1 (7%) had juvenile myelomonocytic leukemia, and 1 (7%) had adrenoleukodystrophy. Patients received BMT at a mean age of 3.9 ± 1.6 years. 12 (80%) patients received total body irradiation (TBI) and 3 of these 12 received cranial irradiation in addition to TBI; one (7%) received only cranial irradiation. Total body irradiation included head and eye exposure. Mean age of cataract surgery was 9.1 ± 2.3 years; mean follow-up was 55.9 ± 45.1 months. All cataracts were of posterior subcapsular subtype. Mean BCVA improved from 0.7 ± 0.4 logMAR to 0.3 ± 0.5 logMAR (p < 0.001). 23/28 eyes (80%) had cataract extraction with intraocular lens placement; 5/28 (20%) of the eyes had cataract extraction with primary posterior capsulotomy and anterior vitrectomy (PC/AVx). 23/23 (100%) of the eyes without primary PC/AVx developed PCO an average of 2.3 ± 6.9 months after surgery. No eyes with primary PC/AVx eyes developed PCO. Conclusions: Children with history of BMT have a predisposition of developing posterior subcapsular cataracts and have a high rate of visually significant PCO if the posterior capsule is left intact at the time of cataract surgery.

Competing interests7. ReferencesThe etiology of reptilian viral diseases can be attributed to a wide range of viruses occurring across different genera and families. Thirty to forty years ago, studies of viruses in reptiles focused mainly on the zoonotic potential of arboviruses in reptiles and much effort went into surveys and challenge trials of a range of reptiles with eastern and western equine encephalitis as well as Japanese encephalitis viruses [1-3]. In the past decade, outbreaks of infection with West Nile virus in human populations and in farmed alligators in the USA have seen the research emphasis placed on the issue of reptiles, particularly crocodiles and alligators, being susceptible to, and reservoirs for, this serious zoonotic disease [4-7]. Although there are many recognised reptilian viruses, the evidence for those being primary pathogens is relatively limited. Transmission studies establishing pathogenicity and cofactors are likewise scarce, possibly due to the relatively low commercial importance of reptiles, difficulties with the availability of animals and permits for statistically sound experiments, difficulties with housing of reptiles in an experimental setting or the inability to propagate some viruses in cell culture to sufficient titres for transmission studies. Viruses as causes of direct loss of threatened species, such as the chelonid fibropapilloma associated herpesvirus and ranaviruses in farmed and wild tortoises and turtles, have re-focused attention bac
Gravano,Ariel;
Intersecciones en antropolog?-a , 2006,
Abstract: the circular management model is characterized by planned actions and continuous evaluation by those involved. organizational culture, consisting of both practices and imaginaries shared between actors and agents, plays a key role. confronting the challenge of building a region with medium-sized cities, these imaginaries, which create their own local identities, cover any practice and social management in their heterogeneity as well as in their necessary conjunction of interests. for fifteen years there has been a vain attempt to create a partnership of territorial productive development among four cities in the province of buenos aires (tandil, olavarría, azul and rauch: toar). the challenge consists of creating a strategic plan that finally manifests this reality. the aim of this paper is to enrich the plan's management model, facilitating its organization, and to study the regional imaginaries. our hypothesis focuses on the methodological dimension of organizational management and establishes that the failures have been due to lineal and conductivist approaches, based on the theory of social homeostasis as is most strategic development. we propose a circular approach to management, based on the dialectic theory of conflict. in addition, we show how local imaginaries could be incorporated into regional planning.

Abstract: according to the actual knowledge of the ephemeroptera order in chile, it is concluded that there are 57 species included in 25 genera and 7 families. a 40% of the species are recognized through adults and larvae, 12% only for the larvae and 47% only for male and/or female adults. about its geographical distribution, the 56% of the species are endemics, the 33% are native to chile and argentina and 11% are of widespread distribution.
Gravano, Ariel;
Horizontes Antropológicos , 2009, DOI: 10.1590/S0104-71832009000200004
Abstract: the objective of this work is to contribute to the organizational facilitation on participation processes of urban planning through the projection of an ethnographical approach. the problem at hand is the one that is portrayed in the social scene of real processes as well as the one that underlying in the wings of the methodological, theorical and ideological reflection. the point in common between both levels of analysis is that the actors involved in the scenes and the analysts themselves act according to a shared component that we place here as the core and objective of our proposal: the reflixibility, in terms of a cultural otherness built and objectifying that nurtures the practices towards the transformation as a circular and effective method of organizational achievements.
Wainer,Ariel;
Subjetividad y procesos cognitivos , 2010,
Abstract: the objective of this study consists in contrasting the results obtained on two levels of analysis of the discourse of a patient: the level of narration and the level of the words. the sample used corresponds to the transcription of a first interview of an analytic session. the instrument with which the discourse analysis was tackled is the david liberman algorithm (dla). as regards the procedure: - on the level of narration, the material is split into 22 narrative sequences in which the drives and defenses are considered, the main ones as well as the complementary ones. regarding the defenses their state is also considered. apart from carrying out a study on a micro level of each sequence, the global results are evaluated, insofar as drives and defenses are concerned. concerning the first, statistical as well as logical prevalences are evaluated. - on the level of the words the computerized dla dictionary is used. this instrument is applied in its two modalities: automatic and interactive. in both cases the whole of the sample is considered. the results obtained correspond to the drives which are present in the patient′s discourse. in the last point, concerning the discussion and the conclusions, the results obtained on the two levels are compared. coincidences and divergences between them are analyzed.
