Current management of gastric cancer Situación actual en el tratamiento del cáncer gástrico

Abstract: Gastric cancer is a disease with high incidence and mortality in our population. The prognosis of patients with this disease is closely related to the neoplasm stage at diagnosis, including the following characteristics of the tumor: extension into the gastric wall thickness, spread to locoregional lymph nodes and the ability to generate distant metastases, as described by the TNM classification. For localized tumors characterized only by invasion of mucosa or submucosa at diagnosis, survival at 5 years is between 70 and 95% with exclusive surgical management; however, when extension into the gastric wall is higher and/or there is locoregional nodal involvement, survival decreases to 20-30% at 5 years. Currently, at high-volume centers, the extent of gastrectomy is individualized based on several parameters, which in an increasing number of cases allows a total gastrectomy with D2 lymphadenectomy and preservation of the spleen and pancreas. This improved procedure increases the chance of R0 surgery and improves the relationship between resected and affected lymph nodes, resulting in a decreased risk of the long-term locoregional recurrence. To improve these results, different therapeutic strategies combining chemotherapy or chemoradiotherapy with surgery have been tested. Previously, the Intergroup 0116 clinical trial, published in 2001, which changed clinical practice in the United States, showed that adjuvant chemoradiotherapy improved survival (from 26 to 37 months overall survival) of these patients. In Europe, perioperative chemotherapy has been considered the standard treatment, since the publication of two randomized phase III trials showed an increase at 5 years survival in the group treated with chemotherapy. El cáncer gástrico es un tumor de alta incidencia y mortalidad en nuestro medio, y su pronóstico está íntimamente relacionado con la situación neoplásica al diagnóstico, que incluye su extensión en el grosor de la pared gástrica, sobre los ganglios linfáticos locorregionales y su capacidad de generar metástasis a distancia, extensión basada en la clasificación TNM. En aquellos tumores localizados al diagnóstico, caracterizados por la invasión únicamente de mucosa-submucosa, la supervivencia a 5 a os se establece entre el 70 y el 95% con manejo quirúrgico exclusivo, sin embargo, cuando la extensión en la pared es mayor y/o existe afectación ganglionar locorregional, la supervivencia disminuye al 20-30% a 5 a os. Abstract: hygiene, health and education have been connected since the origins of mankind. this very close relationship offers different forms and intensities. the apparition of the hygienist movement about the middle of the 19th century, and the social and educational changes linked to the schooling process that took place in the mentioned century and in the first half of the 20th, as well as the scientific study of the childhood, developed in the intermediate period between both centuries, favoured the creation of new societies and professions or professional specialities, the proliferation of magazines, leaflets and books, the organization of congresses, the passing of several laws about those matters, and the public and private initiatives. in this prefatory text all of this is accounted for. likewise the rest of the texts of this monographic issue are introduced; also some observations about the so called "education for health" are made, and a chronology of more relevant events, publications and laws, related to the subject, is annexed.

Asimismo, la creación de museos y sociedades interesadas por la conservación y el estudio del patrimonio educativo ha experimentado un crecimiento notable en las dos últimas décadas. Abstract: In this article the relationships and interactions among memory, educational inheritance and history are tackled. First, in a specific way, the relationships and interactions between memory and history are addressed through a special analysis of the commemorations as a link between both subjects and as an institutional tool of the policies of oblivion and memory. Then, the relationships and interactions between memory and educational inheritance are studied (exhibitions, researches, scientific societies, conferences, groups and projects of research, international statements and recommendations as well as pedagogical museism). Finally, some of the issues arising from the safeguard and cataloguing of the educational inheritance are set out: the need to connect them with its study, the diversity of its uses and values, the problems related to the changes occurring in the technological supports of memory, and the intergenerational preservations and transmission of knowledge, namely of the cultural and educational legacy or inheritance.

Abstract: Neste artigo se analisam as rela es e intera es entre memória, patrim nio educativo e história. Primeiro, de um modo específico, entre memória e história, mediante uma análise especial das comemora es como nexo entre ambas e instrumento institucional das políticas do esquecimento e da memória. Depois, se estudam as rela es e intera es entre memória e patrim nio educativo (exposi es, investiga es, sociedades científicas, congressos, grupos e projetos de pesquisa, declara es e recomenda es internacionais e museísmo pedagógico). Por último, se exp em algumas das quest es relacionadas à preserva o e cataloga o do patrim nio histórico e educativo: a necessidade de conectar ambas a seu estudo, a diversidade de usos e valores do mesmo e os problemas relacionados com as mudan as que aconteceram nos suportes tecnológicos da memória e da conserva o e transmiss o intergeracional do saber e do conhecimento, ou seja, da heran a do patrim nio cultural e educativo.

Abstract: Este texto pretende analizar, a partir del caso espa ol, las relaciones existentes entre la historia de las disciplinas (y, dentro de ella, de los manuales escolares), el proceso de profesionalización docente y la formación de profesores. Primero se exponen las causas que explican la ausencia legal, en el caso espa ol, de la historia de las disciplinas en la formación de profesores, así como algunas iniciativas aisladas en relación con este tema. Seguidamente se da cuenta de algunos aspectos o cuestiones en los que fundamentar la presencia de la historia de las disciplinas en la formación de profesores, en especial de la necesidad de insertar los estudios sobre la manualística escolar en el ámbito, más amplio, de dicha historia. Por último, se exponen algunos conceptos e ideas clave para estructurar la historia de las disciplinas. This text tries to analyze, through a Spanish case, the relationships existing among the history of school subjects (and, inside it, the history of textbooks), the process of the professionalization of teaching and teachers' training. First, the causes explaining the legal absence, in Spanish, of the history of school subjects in teachers' training, as well as some isolated initiatives on this theme are presented. Then, some aspects and issues are dealt with in order to justify the presence of the history of school subjects in teachers' training, especially the need to include the analysis of textbooks in the wider field of such history. Finally, some key concepts and ideas with the aim of shaping the history of the school subjects are also presented.
Los programas europeos para la igualdad de oportunidades entre mujeres y hombres: una valoración crítica.

Abstract: A principios del a o 2000 realizamos una evaluación de un programa llevado a cabo por el Ayuntamiento de Leganés, co-financiado por la Iniciativa Comunitaria Empleo- NOW de la Unión Europea, cuyo objetivo principal era fomentar la igualdad de oportunidades entre hombres y mujeres en el empleo. Durante nuestro trabajo, constatamos que existía muy poca información sobre esta actuación de la Unión Europea, y que lo poco escrito sobre la misma estaba en documentos institucionales, de escasa difusión. Por ello, nos pareció oportuno escribir, para publicar, una descripción de esta Iniciativa Comunitaria, con un breve análisis. Por ello, este trabajo presenta un balance de los proyectos cofinanciados dentro de la Iniciativa Comunitaria Empleo-NOW en Espa a, destinada a fomentar la igualdad de oportunidades entre hombres y mujeres en el mundo del empleo y la formación profesional. Este balance se articula sobre dos aspectos: por un lado, los productos o realizaciones de los proyectos y, por otro, sus resultados e impactos. Este análisis de resultados se aborda en relación con los principios que inspiran las Iniciativas Comunitarias: innovación, efecto multiplicador, transnacionalidad, enfoque de abajo-arriba , complementariedad y transregionalidad. Al hilo de dicho balance se apuntan algunas líneas de reflexión y perspectivas de futuro de la política europea de igualdad de oportunidades en el empleo.
