Confidencialidad e historia clínica: Consideraciones ético-legales

Abstract: confidentiality is a key aspect in the relationship between health professionals and patients. it involves the cession by the patient of a reserved part of herself and the ethical principles of autonomy and non-maleficency are closely linked to its preservation. there are also numerous legal regulations that make professional secrecy obligatory, in some cases imposing hard sanctions that are often not known about. as the medical record is the document where the relationship with the patient is recorded, it requires extraordinary protection due to the especially sensitive nature of the information it contains. it should be compulsory that the professional who works with it should know some basic aspects of the fundamental laws that regulate it in different aspects. this article reviews questions that at times are only known in the abstract, but that are contained in the legal regulations, such as ownership of the medical record, subjective annotations, its custody and rectification or who can have access to it and why.

Abstract: La confidencialidad es un aspecto clave de la relación entre profesionales sanitarios y pacientes. Supone la cesión del paciente de una parte reservada de sí mismo y los principios éticos de autonomía y no maleficencia están íntimamente ligados con su preservación. También numerosas normas legales obligan al secreto profesional, imponiendo en algunos casos duras sanciones a menudo desconocidas. Al ser la historia clínica el documento donde la relación con el paciente queda reflejada, requiere de una protección extraordinaria por la naturaleza especialmente sensible de la información en ella contenida. Es preceptivo para el profesional que la elabora conocer algunos aspectos básicos de las leyes fundamentales que la regulan en diferentes aspectos. En este artículo se hace un repaso por cuestiones que a veces resultan conocidas en abstracto, pero que están recogidas en la normativa legal, como son la propiedad de la historia clínica, las anotaciones subjetivas, su custodia y rectificación o quién y para qué se puede acceder a la misma. Registros de enfermería sobre el sue?o y percepción de los pacientes en una unidad psiquiátrica: Estudio comparativo

Abstract: background. insomnia is a frequent problem in people who suffer from mental illnesses. the nursing staff control and register whether or not patients sleep. the aim of this article is to analyze the concordance between the perceptions of the patients of a psychiatric unit on the quality of sleep, and the notes in this respect contained in the nursing records. methods. a comparative study between the answers given by the patients of our unit to 126 questions on insomnia on specific nights, and what was reflected in their nursing records. a reduced version of the athens insomnia scale (ais-5) was used. results. the average value obtained according to the patients answers was higher (worse sleep) than that given by the nurses, both globally and by item. the nursing records showed much lower values than those given by the patient; pearsons correlation analysis gave a coefficient of 0.26. analysis of the degree of agreement provided an index of 0.13 that does not differ significantly from nil agreement. conclusions. there is a difference between the nursing observations and the subjective sensations of the patients about how they sleep. they indicate that they sleep worse than the nursing records reflect. external observation and self-perception are not antagonistic aspects, but two complementary aspects of patients sleep.

Abstract: Fundamento. El insomnio es un problema frecuente en las personas que sufren enfermedades mentales. El personal de enfermería controla y registra si los pacientes duermen o no. El objetivo de este trabajo es analizar la concordancia entre las percepciones de los pacientes de una unidad psiquiátrica de agudos sobre la calidad de su sue o y las notas recogidas en el evolutivo de enfermería al respecto. Material y métodos. Estudio comparativo entre las respuestas dadas por pacientes de nuestra unidad a 126 cuestionarios sobre insomnio en noches concretas y lo reflejado en sus historias de enfermería. Se utilizó una versión reducida de la Escala Atenas de Insomnio (EAI-5). Resultados. El valor medio obtenido según las respuestas de los pacientes es superior (peor sue o) al dado por las enfermeras, tanto de manera global como por ítem. La historia de enfermería refleja valores mucho más bajos que los otorgados por el paciente, el análisis de correlación de Pearson proporcionó un coeficiente de 0,26. El análisis de grado de acuerdo proporcionó un ICC de 0,13 y no difiere de manera significativa al acuerdo nulo. Conclusiones. Hay discordancia entre las observaciones de enfermería y las sensaciones subjetivas de los pacientes sobre cómo duermen. Refieren dormir peor de lo que refleja la historia de enfermería. La observación externa y la autopercepción no son aspectos antagónicos, sino dos aspectos complementarios del sue o de los pacientes. Background. Abstract: Multiferroic BiFe1-xCrxO3 (x = 0.2 and 0.4) ceramics were synthesized in a single phase. The effects of Cr3+ substitution on the crystal structure, dielectric permittivity and leakage current were investigated. Preliminary X-ray structural studies revealed that the samples had a rhombohedral perovskite crystal structure. The dielectric constant ε' significantly increased while the dielectric loss tanδ was substantially decreased with the increase in Cr3+ substitution. The temperature effect on the dielectric properties exhibited an anomaly corresponding to magneto-electric coupling in the samples and was shifted to lower temperatures with the increase in Cr3+ substitution. The leakage current density also reduced in magnitude with the increase in the Cr3+ substitution.

Abstract: We investigate the FFT (Fast Fourier Transform) model and G-CSF (granulocyte colony-stimulating factor) treatment of CN (Cyclical Neutropenia). We collect grey collies and normal dog’s data from CN and analyze the G-CSF treatment. The model develops the dynamics of circulating blood cells before and after the G-CSF treatment. This is quite natural and useful for the collection of laboratory data for investigation. The proposed interventions are practical. This reduces the quantity of G-CSF required for potential maintenance. This model gives us good result in treatment. The changes would be practical and reduce the risk side as well as the cost of treatment in G-CSF.

Abstract: In scaled CMOS processes, the single-event effects generate missing output pulses in Delay-Locked Loop (DLL). Due to its effective sequence detection of the missing pulses in the proposed Error Correction Circuit (ECC) and its portability to be applied to any DLL type, the ECC mitigates the impact of single-event effects and completes its operation with less design complexity without any concern about losing the information. The ECC has been implemented in 180 nm CMOS process and measured the accuracy of mitigation on simulations at LETs up to 100 MeV-cm2/mg. The robustness and portability of the mitigation technique are validated through the results obtained by implementing proposed ECC in XilinxArtix 7 FPGA.
