Endoscopic Staging and Treatment of Early Gastric Cancer  [PDF]

Abstract:

Gastric cancer is the most common cancer worldwide and it is often diagnosed in an advanced stage. In countries where screening endoscopy is performed widely, early detection is possible. In fact, early gastric cancer incidence is increasing during the last years worldwide and screening could be a major factor in such increase. In the past, the standard treatment of gastric cancer was surgical resection; however, the endoscopic treatment has increased due to advances in the instruments available and clinician experience. In fact, endoscopic resection has become one of the greatest advances in EGC treatment. It is the standard treatment in most of the cases because early gastric cancer is associated with a low rate of lymph node metastasis and a high survival rate. Endoscopic Mucosal Resection and more recently Endoscopic Submucosal Dissection are the two main developed procedures. Endoscopic Submucosal Dissection achieves a higher rate of en-bloc resection, complete resection, curative resection and lower local recurrence compared with Endoscopic Mucosal Resection group. The disadvantages associated with Endoscopic Submucosal Dissection, such as higher perforation rates and longer procedure time, will probably improve as the endoscopists experience increases and new endoscopic tools are developed. The aim of this paper is to review the management of EGC with a special focus on endoscopic detection, staging, therapy, surveillance, and prevention.

CagA-positive Helicobacter pylori infection is not associated with decreased risk of Barrett's esophagus in a population with high H. pylori infection rate

Abstract: We studied 104 consecutive patients, residents in an area with a high prevalence of H. pylori infection, with BE and 213 sex- and age-matched controls. H. pylori infection and CagA antibody status were determined by western blot serology.H. pylori prevalence was higher in patients with BE than in controls (87.5% vs. 74.6%; OR. 2.3; 95% CI: 1.23–4.59). Increasing age was associated with a higher prevalence of H. pylori (p < 0.05). The prevalence of CagA+ H. pylori serology was similar in patients with BE and controls (64.4% vs. 54.5%; NS). Type I H. pylori infection (CagA+ and VacA+) was similar in patients with BE and controls (44.2% vs. 41.3%; NS). Logistic regression analysis identified alcohol (O.R. 7.09; 95% CI 2.23–22.51), and H. pylori infection (OR: 2.41; 95%CI: 1.20–4.84) but not CagA+ serology as independent factors.Neither H. pylori infection nor H. pylori infection by CagA+ strains reduce the risk of BE in a population with high prevalence of H. pylori infection.The incidence of adenocarcinoma of the esophagus has dramatically increased during the last 2 decades [1-3] and so has the incidence of Barrett's esophagus (BE), one of the most important risk factors for esophageal adenocarcinoma [4,5]. Despite the fact that BE is a predisposing factor, only 0.5% of the patients with BE will develop adenocarcinoma per year [6], meaning that factors that contribute to this development are unknown.Helicobacter pylori is the major cause of peptic ulcer disease, and it is also implicated in the pathogenesis of adenocarcinoma of the distal stomach and gastric lymphoma, especially the mucosa-associated lymphoid tissue-type lymphoma (MALT lymphoma). The relationship between corpus gastritis, H. pylori infection, reflux oesophagitis and Barrett's esophagus is complex and not fully understood. Abstract: this paper analyzes the accumulation pattern of spanish capitalism during the last half a century, based on the evolution of the capital return and the behavior of its main determinants: distribution and technical change. first, it explains the accumulation logic of the theory of work-value and argues that the so-called “law of the falling tendency of the rate of profit” (ltdtg) provides a suitable frame to explain the movements of the capitalist development in the long term. it then shows how this dynamic has materialized in the development of world accumulation since the end of the second world war, and concludes that the evolution of the spanish capitalism in this period follows the same world pattern, emphasizing its main singularities.

Abstract: En este escrito se analiza la pauta de acumulación del capitalismo espa ol en el último medio siglo examinando la evolución de la rentabilidad del capital y de sus principales determinantes, la distribución y el cambio técnico. Primero describe la lógica de la acumulación de la teoría del valor-trabajo y defiende la tesis de que la ley de la tendencia descendente de la tasa de ganancia es un marco adecuado para interpretar los movimientos del desarrollo capitalista a largo plazo. Luego muestra cómo se ha manifestado esa dinámica en la acumulación mundial desde finales de la segunda guerra mundial. Abstract: Television is one of the most important elements in the construction of the musical and audible reality of children and teenagers. It offers a selected and edited version of a musical environment, with an indisputable level of communicative and patrimonial influence. This paper is part of a research project that sets out to know what children are listening to in children’s television programming, in a sample of Latin American television coverage. The general objective has been to establish the parameters of everyday listening experiences and to create an instrument which is able to determine the objective elements of listening environments through their most notable medium: the soundtrack of children’s television programmes. Here we present the creation and validation process of such an instrument. An important element is the consensus process, since we attempt to validate the instrument within the Latin-American context; decision-making because of the inherent characteristics of the music as variable analysed, since sound develops over time, and this makes an accurate classification of it very difficult. La televisión es un elemento destacado en la construcción de la realidad musical y sonora del ni o y del adolescente que ofrece, como totalidad, una versión seleccionada y editada del entorno musical. El artículo que presentamos forma parte de un proyecto de investigación que trata de conocer qué escuchan los ni os en la programación infantil de televisión en una muestra latinoamericana. El objetivo general ha sido establecer los parámetros de la escucha televisiva y determinar sus elementos objetivos. Lo que presentamos en este artículo es el proceso de elaboración y validación de dicho instrumento. El proceso ha tenido como elementos generadores el consenso por el ámbito de aplicación con validez latinoamericana. la toma de decisiones por la naturaleza de la música como variable de análisis, y las características del sonido, cuyo desarrollo en el tiempo requiere formas específicas de clasificación
Indigenous participation and representation in Venezuelan electoral processes Participación y representación indígena en los procesos electorales venezolanos

Abstract: This article examines the Venezuelan regional elections of 2008 as a contextual event for the analysis of electoral strategies and results associated with the indigenous representation. Three factors intertwined in the electoral moment are analyzed: 1. the existence of minimum guaranteed representation for indigenous population in legislative organs; 2. the participation of indigenous candidates and electors; 3. the maneuvers of political parties and civil organizations that attempt to channel and/or benefit from such indigenous representation and participation. The description of the electoral context facilitates the identification of factors that, beyond the normative structure of the State, condition the agency of individuals and parties involved in electoral processes. Among those factors are the symbolic value of indigeneity in the current process of national identity re-definition, the interest of political parties in controlling the vote of the indigenous representation and the tendency towards the consolidation of professionalized elites within the indigenous activism. Este artículo examina los comicios regionales de 2008 en Venezuela como evento contextual para el análisis de estrategias y resultados electorales vinculados a la representación indígena. Se analizan tres factores interrelacionados en el momento electoral: 1. la existencia de representación mínima garantizada a la población indígena en los órganos legislativos; 2. la participación de candidatos y electores indígenas en los comicios; 3. las maniobras de partidos políticos y organizaciones civiles que intentan canalizar y/o beneficiarse de dicha representación y participación. La descripción del contexto electoral permite identificar factores que, más allá de la estructura normativa estatal, condicionan la agencia de individuos y partidos involucrados en los comicios. Entre esos factores se encuentran el valor simbólico de la indigenidad en el actual proceso de redefinición de identidad nacional, el interés de los partidos convencionales en controlar el voto de la representación indígena en el polarizado escenario político del país y la tendencia a la consolidación de élites profesionalizadas en el activismo indígena.

Abstract: RESUMEN: En esta nota confirmamos la presencia de Arabis soyeri subsp. soyeri en el Pirineo aragonés (provincia de Huesca). Esta cita oscense se sitúa en el límite SW de su área de distribución endémica. Además, comentamos algunos aspectos sobre su autoecología y conservación.SUMMARY: Arabis soyeri Reuter & Huet subsp. soyeri is confirmed for the flora of the Aragonese Pyrenees (Huesca province, Spain). Moreower, this new station is located on the south-western border of its endemic range. Some aspects on its autecology and conservation are discussed as well.
