Carcinoma diferenciado de tiroides después de irradiación: a propósito de un caso

Abstract: the differentiated thyroid carcinoma is a rare disease in children. in patients at pediatric ages and among adolescents, it has a good prognosis and a high survival rate. the case of a patient diagnosed in our hospital that received irradiation of the head and neck due to a non-hodgkin lymphoma of right cervical localization 6 years before, was presented. a nodule in the right lobe of the thyroid was detected in the follow-up. the diagnosis of thyroid papillary carcinoma was made by biopsy. as the result of the treatment (surgery and radioiodine) was good, it was concluded that the early diagnosis and the current treatment allowed a complete remission in the differentiated thyroid carcinoma.

Abstract: de los 168 casos ingresados en el servicio de oncopediatría del hospital pediátrico docente ?william soler? desde marzo de 1995 hasta noviembre de 2003, se diagnosticó tumor germinal maligno de ovario a 9 ni?as, que han sido tratadas según el protocolo de tumores malignos (tgm-90) de la sociedad francesa de oncología pediátrica (sfop). de este grupo de ni?as, 3 tenían disgerminomas y 6, teratoma inmaduro. todas en la actualidad se hallan bajo seguimiento y en remisión completa.

Abstract: the ultrasonographic screen-assisted aspiration biopsy or the laparoscopy-assisted biopsy have been introduced by some medical centers to diagnose from the histological point of view different types of kidney tumors in children. 15 patients diagnosed at "william soler" pediatric teaching hospital from march 1995 to march 2003 were included in the study. all of them had clinical and imaging diagnoses of malignant kidney tumors. this study was aimed at differentiating the histological variant of the tumor before applying any type of treatment and at reducing this way preoperative morbidity.

Abstract: La biopsia en los tumores renales ya sea por biopsia aspirativa guiada bajo pantalla ultrasonográfica o por medio de la laparoscopia ha sido introducida por algunos centros para realizar el diagnóstico de los diferentes tipos de tumores renales de la infancia, desde el punto de vista histológico. Se ha incluido en este estudio a 15 pacientes diagnosticados en el Hospital Pediátrico Universitario "William Soler", desde marzo de 1995 hasta marzo de 2003, todos con diagnósticos clínicos e imagenológico de tumores renales malignos. Y el objetivo fue diferenciar la variante histológica del tumor previo a cualquier tipo de tratamiento, y reducir así la morbilidad preoperatoria. Abstract: El carcinoma diferenciado de tiroides es una enfermedad rara en el ni o. En pacientes de edad pediátrica y en adolescentes tiene un buen pronóstico y una elevada sobrevida. Presentamos el caso de un paciente diagnosticado en nuestro hospital quien había recibido, 6 a os atrás, irradiación de cabeza y cuello a causa de un linfoma no Hodgkin de localización cervical derecha. En el seguimiento se detectó nódulo del lóbulo derecho del tiroides y mediante biopsia se realizó el diagnóstico de carcinoma papilar de tiroides. El resultado del tratamiento realizado (cirugía y aplicación de yodo radioactivo) fue bueno, por lo que se concluye que el diagnóstico temprano y el tratamiento actual logran una remisión completa en el carcinoma diferenciado de tiroides. Abstract: the soft parts sarcomas appear in any age, although are more frequent in the fourth decade of life. are called in this way, those tumors originating in structures supporting the body or surrounding the organs and tissues and it is the rhabdomyosarcoma the more frequent soft parts tumor present in children. it is uncommon the irritability whereas the early and late relapses are frequent. this is the case of a patient diagnosed in the newborn stage with a poor response to treatment and progression during its course (something unusual), since generally, there is a good initial response and afterwards may be relapses.

Abstract: the pleuropulmonary blastoma (ppb) ia a rare and aggressive tumor affecting children at early ages. the case of a 3-year-old male patient that was diagnosed ppb of the left lung in our hospital (july, 1995) and required 5 operations during the 6 years of survival due to the trend to relapse of this type of tumor, which is a challenge for the pediatric surgeon, is presented. pediatricians and specialists in general comprehensive medicine are given information on the therapeutic conducts and the current trends in a child with cancer of long time of evolution.

Abstract: introduction: the aim of present paper was to report cased detected in our study of structural anomalies of thyroid gland in patients underwent radiotherapy for treatment of malignant head and neck tumors. methods: a cross-sectional and descriptive study was carried out to determine prevalence of structural alterations of thyroid gland in 43 patients survival of head-neck tumors underwent radiotherapy admitted in "william soler" and "juan manual márquez" teaching hospitals during 10 years (1990 to 2000). results: more frequent structural alteration of thyroid gland includes glandular fibrosis (18,5%) followed by thyroid carcinoma, and of simple cysts of that gland (11,1% respectively). there were 2 cases of differential carcinoma of thyroid gland (7, 4%), and one of adeno-follicular mixed injury (3, 7%). fine-needle aspirate cytology allows detection of most of these injuries and also the intra-parenchymatous guided-echography of this gland. treatment of injuries founded was surgical and we performed a total thyroidectomy in two patients presenting with differential carcinoma of thyroid gland, subtotal thyroidectomy in t here patients with cysts of that gland, and thyroid adenomas. in cases of glandular fibrosis there was a concomitant thyroid hypofunction, thus, we administered substitute treatment using levothyroxine. conclusions: fine-needle aspiration cytology by means of echography of thyroid gland allows diagnosing of structural alterations no detected in physical examination. more irradiated malignant pathology was non-hodgkin lymphomas. more doses of irradiation, more is thyroid pathology.

Abstract: Los sarcomas de partes blandas aparecen a cualquier edad, aunque son más frecuentes en la cuarta década de la vida. Se denominan así a aquellos tumores que se originan en las estructuras que soportan el cuerpo o envuelven los órganos y tejidos, y es el rabdomiosarcoma el tumor de partes blandas más frecuente en edad pediátrica. Es rara la condición de intratabilidad, mientras que son frecuentes las recaídas tempranas y tardías después del tratamiento. Presentamos un caso de una paciente diagnosticada en etapa de recién nacida con una mala respuesta al tratamiento, con progresión en el curso de este (algo no habitual), pues, generalmente, existe una buena respuesta inicial y después puede haber recaídas.
