Valoración clínica y cefalométrica del aparato de distracción de García? para el tratamiento de la mordida abierta anterior en pacientes atendidos en la Facultad de Odontologóa,Universidad de Antioquia, Medellín

Abstract: introduction: this study aims to determine radiographic and clinical changes obtained with the garcia’s distractor appliance? and its craniofacial, dental and facial cosmetic effects in patients with anterior open bite. methods: a prospective clinical quasi-experimental study in 9 subjects, 2 men and 7 women, 7 of them in a phase of active growth with an average age of 11.7 years and 2 adults with an average age of 30 years, in which cephalic radiographs were obtained at the beginning, third and sixth month of treatment. the clinical effect was assessed by measuring monthly the overbite and overjet with a digital gaugediscover?. in the statistical analysis, descriptive measures were obtained using anova test for repeated measurements to determine changes over time, with a level of significance of 0.05. results: the radiographic findings showed an increase in the goniac angle in the growing patients; also, an increase in the overbite and a decrease in the overjet were observed, these changes are explained by the intrusion of the upper molars and retro inclination of the upper and lower incisors; facially, there was a decrease in the angle of convexity in the soft tissues. conclusions: the garcia’s distractor appliance? used for a period of six month in growing patients improved the open bite by intrusion of upper molars and retro inclination of upper and lower incisors.

Abstract: INTRODUCCIóN: esta investigación pretende determinar los cambios radiográficos y clínicos obtenidos con el distractor de García en cuanto a sus efectos craneofaciales, dentales y de cosmética facial en pacientes con mordida abierta anterior. MéTODOS: se hizo un estudio clínico prospectivo cuasi experimental en nueve sujetos dos hombres siete mujeres, siete de ellos en crecimiento con un promedio de edad de 11,7 a os y 2 adultos con un promedio de edad de 30 a os; en quienes cuales se obtuvieron radiografías cefálicas laterales al inicio, tercero y sexto mes de tratamiento. El efecto clínico se valoró mensualmente midiendo con un calibrador digital Discover la sobremordida vertical y horizontal. En el análisis estadístico se obtuvieron medidas descriptivas y se utilizó la prueba ANOVA de medidas repetidas para determinar los cambios en el tiempo, con nivel de significancia de 0,05. RESULTADOS: los hallazgos muestran en los pacientes en crecimiento, a nivel craneofacial incremento en el ángulo goniaco, a nivel dentoalveolar incremento en la sobremordida vertical acompa ada con disminución en la sobremordida horizontal explicada por intrusión del molar superior y retroinclinación de incisivos superiores e inferiores, y a nivel facial disminución en el ángulo de la convexidad de tejidos blandos. CONCLUSIóN: el aparato distractor de García utilizado durante seis meses en los pacientes en crecimiento de esta muestra mejoró la mordida abierta, tanto por intrusión de molares superiores como por retroinclinación de incisivos superiores e inferiores. Abstract: introduction. primary anastomosis is a feasible technique in the management of severe secondary peritonitis in critically ill patients; however, its use has been limited due to the risk of complications and death. materials and methods. we selected patients with severe secondary peritonitis that required resection of an intestinal segment and managed with temporary intestinal ligature, open abdomen, elective repeat laparotomies, and ulterior deferred primary anastomosis. primordial success was labeled in those patients that had primary anastomosis and no leakage or fistulae. results. twenty six patients were included in the study, with a mean apache ii score of 15.3. there were 6 anastomoses in the small bowel, 5 in the large bowel, 4 of the ileum to the large bowel, and in 3 patients an anastomosis could not be performed. a mean of 4 scheduled relaparotomies were registered, starting 24 hours after the anastomosis. primordial success was achieved in 20 patients (77%), 28-day survival was 88.3%; 23 patients left the hospital alive, and only 3 (11.5%) died in the icu; these deaths were independent of the procedure. discussion. damage control surgery was feasible and secure in patients with severe secondary peritonitis, with a primary success rate of 77%; fistulae developed in 11.5%, and mortality was 11.5%.

Abstract: objective: to report our experience with the application of the damage control technique in exanguinating patients with severe abdominal trauma, and to evaluate the temporary ligature of the the small and large bowel and an intestinal anastomosis as a delayed procedure in the performance of this type of staged surgery. material and methods: all consecutive patients that underwent damage control technique in the period 1999-2004 were studied. parameters of morbidity and mortality, uci stay, and complications were analyzed. results: the study included 40 patients with average age 34 ± 12.7 years, of which 90% were men; 75% of patients presented open trauma; apache ii = 15 (4-29), ati = 32 (7-90), with 75% of patients presenting an ati score >25. average uci stay was 8.5 ±11.8. packing was used in 38 (95%) of patients. sepsis developed in 32.5% and multiple organ failure in 45% of all patients. overall mortality was 45% (ci 95%, 29.6-60.4), 72.3% mortality within the first 72 hours. thirteen intestinal anastomoses were performed in 12 patients (30%). univariate analysis showed mortality associated with coagulopathy, acidosis, and multiple organ failure. conclusions: overall mortality in this group was 45%, seemingly associated with coagulopathy, acidosis, and multiple organ failure. delayed primary anastomosis was carried out in 30% of patients, appearing as viable and secure when performed within the context of the staged damaged control operative technique.

Abstract: background: several studies evaluating colorectal cancer (crc) screening with fecal occult blood testing (fobt) have reported a significant reduction in mortality from this tumor. the study aims to describe the results of the first round of the crc screening program in medium-risk population of valencia and assessing their feasibility and acceptability. methods: colorectal cancer screening pilot study using biennial fobt. all residents (106,000 inhabitants) between 50 and 69 years old (a subgroup of 70-74 years), from 3 selected health departments (04, 07 and 13) were invited by mail to participate. the individuals with fobt positive were referred to hospital for colonoscopy. the following rates were calculated as indicators of program quality: the participation rate, positive fobt rate, acceptance rate of colonoscopy, and detection rate of low and high-risk adenomas and invasive cancers, and positive predictive value of fobt. results: the participation was 35,231 individuals (35.70%). the number of fobt positive was 579 (1.64% of total analyzed test), 548 people agreed colonoscopy (acceptance rate: 94.69%). 61 crc were detected, 61 and 153 high-risk and low-risk adenomas (rates of 1.77, 1.77 and 4.46% per 1,000 participants). the positive predictive values for cancer, low-risk and high risk adenomas were 10.95%, 10.95%, and 27.46% respectively. conclusion: the program indicators suggest that a population program of colorectal cancer is feasible in our community as the process indicators obtained are similar to those given in other programs in spain and europe.

Abstract: a study was conducted to describe the use of lycopersicum esculentum plant in oral mucous for which a semi-structural survey was administered to 286 patients in may, 2004, who had been seen at emergency clinics and at comprehensive adult care clinic of the odontology school of university of antioquía. it was found that 51,2% of the surveyed people had sometimes used this plant because their families and friends had encouraged to do so, mainly for management of oral swellings (83,9%). leaves are the most used part of this plant (94,4%) for mouthwash (97,01%), boiled preparations and infusions to be used 3 times a day as an average. the acceptance of alternative medicine favours its use due to its therapeutic qualities for treating infections and swellings of oral mucous. it was recommended to prove by means of future clinical studies the therapeutic effectiveness of the plant so that it can be legally accepted as a medicinal plant in colombia.

León XIII supported to Regent and asked to bishops, clergy and Spanish Catholics who were accepting the legality. Tras la muerte de Alfonso XII se abría un periodo de incertidumbre. El peligro de una nueva guerra civil y de levantamientos republicanos amenazaba la regencia de María Cristina. León XIII apoyó a la regente y pidió a obispos, clero y católicos espa oles que aceptasen la legalidad.
