Increased expression levels of the pvcrt-o and pvmdr1 genes in a patient with severe Plasmodium vivax malaria

Abstract: Two different patients presented at the Hospital Clinic in Barcelona with P. vivax malaria episodes. One patient had severe symptoms and the other mild symptoms. Both patients traveled through the Brazilian Amazon (Manaus) in 2007. For both patients the current diagnosis of malaria was the first. Two other patients with mild symptoms presented to the "Centro de Pesquisa em Medicina Tropical", also in the Brazilian Amazon (Rond?nia) in 2000.To exclude the possibility that the patient's severe symptoms were due to Plasmodium falciparum, a nested PCR was performed. A magnetic method was used to purify P. vivax free of human leukocytes. Quantitative real-time PCR was performed to compare the transcript levels of two main transporters likely to be involved in chloroquine resistance in P. vivax, namely the P. vivax chloroquine resistance transporter, pvcrt-o, and the P. vivax multidrug resistance transporter, pvmdr 1.Results demonstrated that the severe clinical symptoms were exclusively due to P. vivax. The patient presented acute respiratory conditions requiring admission to the intensive care unit. The magnetic method showed highly purified infected-reticulocytes with mature stages. In addition, it was found that parasites obtained from the severe patient had up to 2.9-fold increase in pvmdr1 levels and up to 21.9-fold increase in pvcrt-o levels compared to expression levels of parasites from the other patients with mild symptoms.This is the first clinical case of severe disease exclusively associated with vivax malaria in Spain. Moreover, these findings suggest that clinical severity could be associated with increased expression levels of parasite genes likely involved in chloroquine resistance. Abstract: Se presentaron 4 casos de carcinoma papilar quístico en el período de 4 a os. Se realizó examen físico de todos los casos; se encontró una masa cervical firme en la cara anterior del cuello y en 2 de ellos, se palparon adenopatías cervicales. Se aplicaron técnicas de imagen (ecografía cervical y gammagrafía tiroidea); se completó el estudio con BAAF, que tuvo valor diagnóstico en el 75 % de los casos. Se realizó un estudio anatomopatológico de todas las piezas obtenidas. Se hizo la resección del quiste, biopsia intraoperatoria y si se confirmó el diagnóstico de carcinoma papilar quístico, se le realizó una tiroidectomía total y si fue necesario, linfadenectomía. Se completó el tratamiento con I131 a dosis ablativa y tratamiento hormonal supresor de la TSH. Se destacó el papel de la BAAF en el estudio preoperatorio. 4 cases of cyst papillary carcinoma in a period of 4 years were presented. All of them underwent physical examination. The treatment was concluded with I131 at ablative doses and TSH -supressing hormonal treatment. The role of FNAB in the preoperative study was stressed.

Abstract: This work describes a protocol of a multicentric prospective observational study with one year follow up, to describe the women's reasons and preferences to choose the way of administration (oral vs topical) in the treatment of not complicated candidal vaginitis. The number of women required is 765, they are chosen by consecutive sampling. All of whom are aged 16 and over with vaginal discharge and/or vaginal pruritus, diagnosed with not complicated vulvovaginitis in Primary Care in Madrid.The main outcome variable is the preferences of the patients in treatment choice; secondary outcome variables are time to symptoms relief and adverse reactions and the frequency of recurrent vulvovaginitis and the risk factors. In the statistical analysis, for the main objective will be descriptive for each of the variables, bivariant analysis and multivariate analysis (logistic regression).. The dependent variable being the type of treatment chosen (oral or topical) and the independent, the variables that after bivariant analysis, have been associated to the treatment preference.Clinical decisions, recommendations, and practice guidelines must not only attend to the best available evidence, but also to the values and preferences of the informed patient.Vaginitis is the most frequent reason for gynecology consultation in primary health care services. It is estimated that 75% of women experience at least one episode of vulvovaginal candidiasis throughout her life and 40-50% of them have at least one recurrence [1].The most frequent cause of vulvovaginal inflammation is infective, beeing the main organisms: Gardnerella vaginalis (15-50%), Candida (C) (20-25%) and Trichomonas vaginalis (5-50%) species, with a frequency distribution that depends on the populations studied [2]. Abstract: an odontogenic myxoma is a uncommon benign neoplasm of a gelatinous and solid consistency, of lengthy growing with a infiltration potential producing an cortex expansion of with and extend bone destruction and a high rate of relapse. its origin is ectomesenchymal, probably derived from dental organ. there is not a particular sexual preference, but a slight one by be present in mandible and generally asymptomatic. this is the case of a female white patient aged 16, with health backgrounds referred to maxillofacial consultation due to an of right hemifacial increase without another accompanying symptomatology. at physical examination there is an increase in volume of right maxillary region. in computerized axial tomography (cat) a hyperdense lesion occupying all right maxillary sinus with areas of higher density without affecting the orbit floor bone. under nasotracheal general anesthesia a hemimaxillotomy was carried out, tumor removal and obturator immediate prosthesis placing. at a year of clinical course and radiography definitive prosthesis was placed without tumor relapse.

Abstract: las enfermedades pulpares y periapicales son alteraciones que atiende con mucha frecuencia el estomatólogo general integral. se realizó un estudio descriptivo y trasversal a una muestra de 270 trabajadores que acudieron al servicio de estomatología del hospital ramón pando ferrer durante el período septiembre de 2006 a junio de 2007, a los que se les realizó un examen bucal para determinar la presencia de enfermedades pulpares y periapicales. entre los principales resultados se encontró que la frecuencia de las enfermedades pulpares fue de 19,6% y en las periapicales fue de 4,8%, el grupo de edad más afectado fue el de 35-59 a?os con 52,8% para las enfermedades pulpares y 61,5% para las periapicales, el sexo femenino con 79,2% para las enfermedades pulpares y 76,9% para las periapicales.

Abstract: El mixoma odontogénico se considera una neoplasia benigna, poco frecuente, de consistencia firme y gelatinosa, de crecimiento lento con potencial infiltrativo que produce expansión de la cortical con extensa destrucción ósea y alto índice de recidiva. De origen ectomesenquimático, probablemente derivado del órgano dentario. No existe una predilección particular por sexo, y si una ligera preferencia por presentarse en mandíbula, generalmente asintomático. Se presenta una paciente femenina de dieciséis a os de edad, color de la piel blanca, con antecedentes de salud, que acude a consulta de cirugía máxilo facial por aumento de volumen de hemicara derecha, sin otra sintomatología acompa ante. Al examen físico se observa aumento de volumen en región maxilar derecha. Al realizar tomografía axial computarizada se observó lesión hiperdensa que ocupaba todo el seno maxilar derecho, con áreas de mayor densidad, que respetaba el hueso que conforma el piso de la órbita. Bajo anestesia general naso traqueal se le realizó hemimaxilectomia, retirada de tumor y colocación de prótesis inmediata obturadora. Después de un a o de evolución clínica y radiográfica se colocó la prótesis definitiva, sin presencia de recidiva tumoral. Abstract: La pizarra digital interactiva permite una progresiva innovación en las prácticas docentes, una mejora en la motivación y la atención de los alumnos, y la disponibilidad de nuevas herramientas para atender a la diversidad de los alumnos, especialmente a aquellos alumnos con discapacidad o dificultades severas o moderadas para el aprendizaje. En esta experiencia, se ha desarrollado una metodología de participación activa, participativa, dinámica y comunicativa. Las conclusiones a las que hemos llegado en nuestra experiencia de utilización conjunta de la pizarra digital interactiva (P.D.I.) y el sistema interactivo de evaluación, han sido las siguientes: La utilización de la P.D.I. promueve un comportamiento activo por parte del alumno; El sistema de votación facilita la evaluación continua de los alumnos; El sistema de votación potencia la motivación y la atención del alumnado en general; Los alumnos han participado más en clase al utilizar el sistema de votación; Su concentración, atención es mayor al utilizar en clase el sistema de votación.
