The identification of various risk factors of vertical human immunodeficiency virus (HIV) transmission resulted in the development of strategies whose aim was to decrease the mother's viral load, to reduce her child's exposure to it during delivery, and to avoid the subsequent viral exposure due to breastfeeding. The administration of antiretroviral treatment during pregnancy, delivery and to the neonate (PACTG 076) proved to be useful. At a first stage, zidovudine was used. A triple combination therapy was then administered. Initially, the viral transmission in mothers who were enrolled in protocols for vertically transmitted HIV prophylaxis was reduced to 9.5%, whereas the last measurement carried out between 1998 and 2005, the initial figure was brought down to 2%. Nevertheless, the delivery of infected children whose mother's HIV status was unknown is still considered likely to happen. The main step to be taken to reduce HIV infection among children is to perform universal HIV tests during pregnancy, so that HIV positive pregnant patients conveniently receive proper prophylaxis. We look forward to achieving this by following the new prevention guidelines of vertically-transmitted HIV infection, developed by the Comi

Breast Cancer Research , 2004, DOI: 10.1186/bcr790
Abstract: We prospectively studied three female cats with clinical symptoms of IMC, identified over a period of 3 years. Clinicopathological and immunohistochemical evaluations of Ki-67, and oestrogen, progesterone and androgen receptors were performed. All three animals presented with secondary IMC (postsurgical) characterized by a rapid onset of erythema, severe oedema, extreme local pain and firmness, absence of subjacent mammary nodules, and involvement of extremities. Rejection of the surgical suture was observed in two of the cats. Histologically, highly malignant papillary mammary carcinomas, dermal tumour embolization of superficial lymphatic vessels, and severe secondary inflammation were observed. The animals were put to sleep at 10, 15 and 45 days after diagnosis. Metastases were detected in regional lymph nodes and lungs in the two animals that were necropsied. All tumours had a high Ki-67 proliferation index and were positive for oestrogen, progesterone and androgen receptors. Our findings in feline IMC (very low prevalence, only secondary IMC, frequent association of inflammatory reaction with surgical suture rejection, steroid receptor positivity) indicate that feline IMC could be useful as an animal model of human inflammatory breast cancer, although the data should be considered with caution.Inflammatory mammary carcinoma (IMC) is a special type of locally advanced mammary cancer that is associated with particularly aggressive behaviour and poor prognosis in women (in which case it is termed 'inflammatory breast carcinoma' [IBC]) [1-3] and in the dog (in which case it is termed IMC) [4,5]. Until now these were the only species in which spontaneous inflammatory carcinoma of the mammary gland had been reported [6].Clinically, IBC and IMC typically present with a rapid onset of symptoms resembling an inflammatory process (erythema, firmness and warmth in the skin of the mammary gland), often without an underlying mass. In fact, this condition can be misdiagnosed a

Abstract: the viability of an escherichia coli o157:h7 strain in cottage-industry guayanes cheese was determined by evaluating several isolation protocols based on immunomagnetic separation (ims). cheese units (25 g) were inoculated with 25 and 250 cel/g of this pathogen and stored at 4°c. the pieces were analized at 0, 2, 6, 8 and 10 days post-inoculation through several ims protocols including two enrichment broths: buffered peptone water without inhibitors (bpw-wi) and buffered peptone water with vancomicyn, cefixime and telurite (bpw-vct) and two immunoseparation isolation agars: macconkey-sorbitol agar (msa) and macconkey-sorbitol agar with cefixime and telurite (msa-ct). results demonstrated the viability of the pathogen for up to 10 days post-inoculation, and during this time, for some of the schemes applied on the ims base, an increase in recovery percentages was achieved, indicating that the number of inoculated cells increased with time. in terms of the utility of ims for recovering the pathogen, variations in the isolation percentages were observed in terms of the enrichment broth and the level of inoculated cells. the biggest recovery percentages were obtained in pieces inoculated with 250 cel/g, with ranges between 35 and 85% (days 0 and 10 respectively) in the best ims scheme (bpw-wi/ims/msa), while, at levels of 25 cel/g, in the best case (bpw-wi/ims/msa), 15% was not surpassed during the first six days. the best performing enrichment broth was bpw-wi (p<0.05) and differences in the recovery percentages (p>0.05) were not observed in relation to the agars (msa and msa-ct) used for sowing the immunoseparator.

We carried out a research study based on the bibliography review and updating of the use of antibiotics in the most frequent dental diseases requiring the use of such drugs in our country. We grouped diseases by the affected site in the cranial-cervical-facial system. The concept of noxa and some references on its ethiopathogenesis, selection of microbial treatment, and alternative drugs, doses, ways of administration and duration of treatment are present. Reference is made to unwanted effects of antibiotics in each treated disease. The annexes comprise the microbiological composition of mouth flora and the present classification of available antibiotics worldwide by their biochemical structure. The reviewed bibliography for each analyzed disease is added. We present this final report in a manual format so as to facilitate the use of this document as a reference for medical personnel which requires so.

Abstract: Objective: To evaluate the effectiveness of a program of psychological preparation for invasive procedures in patients with heart disease, candidates for cardiac surgery, to reduce emotional morbidity and improve perception of health related quality of life. Methods: A Quasi-experimental design was used. The study included a non-probabilistic randomized sample of 110 patients treated in the Division of Cardiac Surgery Centro Medico Nacional Siglo XXI Instituto Mexicano del Seguro Social IMSS. They were divided in two groups: Experimental group (n = 44) and Control group (n = 66). Instruments: CHIP Coping Questionnaire, HADS Anxiety and Depression Hospital Scale, Health Related Quality of Life Questionnaire and a Visual Analogue Scale (VAS) to measure postoperative pain. Results: The experimental group showed an increase in instrumental coping style compared to pre (M = 24.55) and post evaluation (M = 25.93); statistically significant differences were found in the variables of stress (p = 0.042), anxiety (p = 0.001), depression (p = 0.056) and health related quality of life (p = 0.000); while in the control group, the distraction coping style (pre M = 25.80, post M = 26.73) is the one that increased; there were no statistically significant differences in the variables stress (p = 0.274), anxiety (p = 0.671) and depression (p = 0.850), except for the health related quality of life (p = 0.000). This study is the first in Mexico to manage emotional comorbidity in this type of population.

(The study examined strategies for written synthesis of various sources employed by experienced readers/writers in English as a foreign language. Think-aloud protocols were collected as fifteen subjects read several texts onthe same topic and then wrote a synthesis. In a subsequent (recorded) interview, the subjects answered a semi-structured questionnaire aimed at discovering more about strategy use for written synthesis. Data sources were constituted by transcriptions of the think-alouds and interviews as well as the written syntheses, both in rough copies and final versions. Fifteen strategies were identified and described. Frequency of strategy used per subject and category was also measured.)
