Social capital and quality of life in adolescent apprentices in Brazil: An exploratory study  [PDF]

Abstract:

Background: Adolescents who rely on social aid networks tend to be more socially competent and enjoy higher levels of quality of life than those who interact only with their own limited groups of family and friends. In this light, the present study aimed to measure social capital and analyse its possible relation to adolescents’ quality of life. Methods: This is a cross-sectional analytical study with 363 adolescents from the Brazilian NGO. The instruments used included a short Portuguese version of the Integrated Questionnaire to Measure Social Capital (SC-QI) from the World Bank and its abbreviated Portuguese version from the World Health Organization Quality of Life Bref (WHOQoL-Bref) used to measure the quality of life. To measure social capital, the present study chose to use a segmentation analysis, along with the Two-Step Cluster method, applying Euclidian distance and the criteria of centroid aggregation. The Mann-Whitney test was used to compare the scores of WHOQoL-Bref domains among the clusters, at a significance level of 5%. Results: The segmentation analysis resulted in two clusters, with 160 (44.1%) adolescents classified as Low Social Capital (LSC) and 178 (49%) classified as High Social Capital (HSC). Participation in activities for the good of the community (p < 0.001), opinion regarding the differences between people in the community (p = 0.002), and number of community meetings (p < 0.001) constituted the decisive variables for determining the separation of the clusters. When quality of life among the newly formed clusters was compared, the highest average score in the HSC cluster was found in the Psychological Domain (75.4 ± 11.9), whereas in the LSC cluster, the highest average score was found in the Physical Domain (74.6 ± 12.6). Lower quality of life scores was found in the environmental domain for both the LSC clusters (51.3 ± 14.0) and the HSC clusters (54.7 ± 15.5). Curva de estimativa das interna es hospitalares nas regi es metropolitanas brasileiras: um estudo de série histórica

Abstract: O objetivo deste estudo foi investigar as varia es temporais na distribui o das interna es hospitalares realizadas no Sistema único de Saúde (SUS) em todas as Regi es Metropolitanas brasileiras entre os anos de 1995 e 2006. Trata-se de estudo ecológico de série histórica com dados secundários referentes às 36 regi es metropolitanas brasileiras. Realizou-se a análise descritiva dos dados, correla o de Spearman e curva de estimativa. Os resultados indicam que o Teste F foi significativo em ordem decrescente para os modelos linear (p=0,001), quadrático (p=0,002) e cúbico (p=0,004). Por outro lado, os valores de R2 apresentaram ordem crescente de valores – 0,277; 0,307 e 0,338, respectivamente. O modelo cúbico mostrou que o percentual médio de interna es hospitalares em 2006 apresentou tendência de queda quando comparado ao percentual médio de interna es no ano de 1995. Conclui-se que o percentual de interna es hospitalares realizadas no SUS nas regi es metropolitanas brasileiras decresceu durante o período analisado.

Abstract: Objective: to understand the meaning of the ethical process of taking care, under the group of nurses’ perception at a public hospital in Minas Gerais, Brazil Methods: a qualitative study, of phenomenally approach which the following guiding question: What is your perception, while nurses care of the ethical process of care? The data of that research were collected in the period from January to February of 2008. Starting from the analysis of the data, obtained through depositions, it was possible to make the elaboration of three categories: the team’s humanization: influence in ethical taking care; the respect to the patient, family and team; the need of space to discussions on taking care and the ethics. Results: for the nurses, in the nursing team there’s a fault in the ethical process, and in many times in the care act. The need is noticed of fomenting the ethical discussions in the process of taking care, with the participation of all that are involved so much directly as indirectly. In this perspective, taking care will be seen in all your dimensions in consonance with the caretakers' dignity and of being taken care. Conclusions: it is essential to remember that the awareness that a relationship with another is needed, the ability to be sensitive, to live with the differences, are attitudes of solidarity. It represents a value, it encourages the creative potential, the critical spirit and interaction nurse and patient. Materiais e Métodos: estudo qualitativo com inspira o fenomenológica. A pesquisa foi conduzida por uma quest o norteadora: Qual a sua percep o enquanto enfermeiro assistencial do processo ético do cuidar? Os dados dessa pesquisa foram coletados no período de janeiro a fevereiro de 2008. Toothaches in the Daily Lives of Brazilian Adults

Abstract: The aim of the present study was to estimate the prevalence of toothaches and to evaluate its effects on the daily lives of adults living in an industrialised region of southeastern Brazil. A questionnaire was administered to a sample of 744 individuals. The variables related to toothache were grouped into three components: access to dental service, pain severity, and social/functional impacts. The present study found that 68.0% of the subjects had limited access to oral health care, 39.7% presented high toothache severity, and 47.3% reported that toothache greatly affected their daily lives. Nervousness (87.2%) and chewing difficulty (72.6%) were the most commonly reported toothache-related effects. Through correspondence analysis, four groups with separate profiles for toothache and associated factors were identified. Two groups reported greater effects of toothaches in their daily lives. One group consisted of individuals who had less access to dental services (women and individuals who were multiracial, married, had a middle school education, or a low family income). The other group consisted of individuals who reported a high toothache severity and high degree of social/functional impacts (individuals who were 40 to 44 years old, married or widowed, black or multiracial, and had a middle school education). The other two groups were those whose daily lives were less affected by toothaches. One group consisted of individuals who had greater access to dental services (men and individuals who were divorced, had a college degree, or had incomes greater than R$ 300.01). The final group consisted of individuals who had low toothache severity and a low degree of associated social/functional impacts (individuals who were 35 to 39 years old, white, single, or had a high school education).
Social and health indicators as a measure of access to primary heathcare in Brazil

Abstract: the aim was to investigate the influence of social and health indicators on access to primary healthcare in the belo horizonte metropolitan area, minas gerais state, brazil. secondary data of 34 municipalities were analyzed; the dependent variables were three basic healthcare indicators: coverage of the family health strategy; coverage of scheduled first dental appointment and mean annual number of appointments in medical specialties per inhabitant. all independent variables were analyzed using principal component analysis, with varimax rotation and kaiser normalization. student's t-test was used to compare the components originated by the factor analysis in relation to the primary care indicators with a significance of 5%. the development conditions had significantly greater weight in cities with low fhs coverage (p=0.022). the socioeconomic conditions were weighted significantly higher in municipalities with high dental appointment coverage (p=0.030) and with greater mean number of medical appointments (p=0.022). socioeconomic and development conditions may be crucial to the identification of municipalities with the best and worst primary care indicators.

Abstract: Objective: To analyze the profile and the nutritional knowledge of nurse students specializing in Family Health in a University of Minas Gerais. Materials and Methods: Cross sectional study with nurses (n=18) from the postgraduate program at a university in Minas Gerais, Brazil. Data collection was performed trough questionnaire application. The first one was composed of socio-demographic information. The second instrument contained 20 questions on general knowledge in nutrition, selected by the authors through a specific literature review on the subject. In the nutrition questionnaire analyses, we carried out test-retest reliability and Cronbach's alpha test for two professional nutritionists (α=0.95). The construction of the database and descriptive analysis was performed in SPSS version 17. Results: The sociodemographic variables that contributed most to the formation of the clusters were: marital status (p=0.040) and the presence of a nutritionist on staff (p=0.001). The first cluster was formed by female nurses (76.5%), not married (66.7%), living in the county (90.9%), working in the family health strategy (69.2%) and that had no nutritionist in the team (100.0%). The association between the number of correct answers about nutrition knowledge and the clusters was performed by the Mann-Whitney test. The mean score was 10.9 (±1.5) in cluster 1 and 13.8 (±1.9) in cluster 2, and the difference was statistically significant (p=0.021). Conclusion: We concluded that nurses who have nutritionists on staff had adequate knowledge related to different aspects of nutrition. Abstract: Objective: to understand the meanings attributed to distanasia and how the nursing staff dealing with the patient’s suffering. Methodology: descriptive and exploratory study from qualitative approach, in a public hospital in a small village from Minas Gerais, Brazil. They were eight nurses who participated in this study. Data collection was from February to May 2008. The researcher was to find evidence of the subject, his talk spontaneously, without prior interpretations, with a guiding question: What do you understand by distanasia? How do you deal with the patient’s suffering? Results: after analyzing the data, emerged three categories: questions about the limits of intervention, palliative care and humane, the medical technology at the end of life. Conclusion: the concept of the term distanasia is not clear, but in the routine of work in complicated situations related to the process of dying. The nurses’ talk showed uncertainty about the limits of intervention.
Nurses’ knowledge of the postgraduate program in family’s health on dental avulsion

Abstract: Objectives: to investigate the knowledge, the attitudes and practices of the nurses students' in the specialization in Family Health, of the Institute of Continuous Education in PUC Sete Lagoas city, Belo Horizonte, Brasil. Methods: transversal, descriptive, study developed with 17 nurses, students of the Family Health Specialization, with volunteered to participate in the study. It was conducted toward the application of modified questionnaire with the thematic dental avulsion. The data were analyzed by descriptive statistical methods. Results: most of the nurses (71%) never heard before the term “dental avulsion”. Five nurses (29%) would not know which providences is necessary in case of dental avulsion, six (35%) would store the tooth and conduct the person to the dentist, three (18%) just direct the person to the dentist, one nurse (6%) would conduct the person to do the restoration, the other (6%) would store the tooth in milk and would give the tooth to the rescuer, and only one (6%) of them would make the replantation immediately. Conclusions: we can affirm that the analyzed nurses present inadequately knowledge when they were questioned about different aspects of urgency procedures in cases of dental avulsion.
Hiperalgesia visceral e dor abdominal cr?nica: abordagem diagnóstica e terapêutica

Abstract: background and objectives: chronic abdominal pain is one of the most frequent reasons for medical consultation. there is, however, no well-established protocol for its diagnostic approach and, most of the times, investigation becomes an expensive and invasive medical practice. this review aimed at explaining visceral pain pathophysiology and establishing diagnostic and therapeutic goals for these patients, based on specific criteria. contents: chronic nonspecific or functional abdominal pain is a complex interaction among impaired motility, visceral hypersensitivity and inadequate neuroendocrine and psychosocial responses. peripheral and central nociception mechanisms seem to be involved in visceral hyperalgesia. diagnosis requires detailed history and clinical evaluation, taking into consideration rome ii criteria. based on known or assumed pathophysiological mechanisms, new drugs have been researched, and some have been more recently used to control abdominal pain, such as 5-ht4 receptor agonists and sodium channel blockers. conclusions: chronic pathophysiological abdominal pain mechanisms are still not well understood. therapy and diagnosis require the understanding of such mechanisms, as well as of rome ii criteria. on the other hand, good patient-physician relationship and multidisciplinary team?s performance seem to be critical in improving treatment response and patients? quality of life.

Abstract: JUSTIFICATIVA E OBJETIVOS: A dor abdominal cr nica é uma das raz es mais freqüentes para consulta médica. N o existe, entretanto, um protocolo bem estabelecido para a abordagem diagnóstica e, na maioria das vezes, essa investiga o se torna uma prática médica onerosa e invasiva. A finalidade desta revis o é tentar esclarecer a fisiopatologia da dor visceral e estabelecer metas diagnósticas e terapêuticas, para portadores desta morbidade, baseada em critérios específicos. CONTEúDO: A dor abdominal cr nica inespecífica ou funcional representa uma intera o complexa entre distúrbio de motilidade, hipersensibilidade visceral e respostas neuroendócrina e psicossocial inadequadas. Mecanismos periféricos e centrais de nocicep o parecem estar envolvidos na hiperalgesia visceral. A abordagem diagnóstica requer uma avalia o minuciosa da história e exame clínico, levando em considera o os critérios de Roma II. Baseado nos mecanismos fisiopatológicos conhecidos, ou supostos, novas drogas vêm sendo pesquisadas, e algumas utilizadas mais recentemente, como os agonistas dos receptores 5-HT4 e bloqueadores dos canais de sódio, para o controle da dor abdominal. CONCLUS ES: Os mecanismos fisiopatológicos da dor abdominal cr nica, ainda n o est o esclarecidos. A abordagem terapêutica e diagnóstica requer o conhecimento de tais mecanismos, bem como dos critérios de Roma II. Por outro lado, uma boa rela o médico-paciente e a atua o de equipe intermultidisciplinar parecem fundamentais para melhorar a resposta ao tratamento instituído e a qualidade de vida do paciente.
