Insuficiência respiratória aguda: quando o tempo conta?

Abstract: total anomalous pulmonary venous connection (tapvc) is a rare congenital heart disease in which all the pulmonary veins from both lungs fail to connect to the left atrium. the classification of tpavc is related with the local of connection. the obstructive variant of tapvc represents a severe cause of pulmonary hyper-tension and refractory cyanosis, with rapidly fatal evolution in the absence of emergent surgical repair. we present the clinical case of a newborn that initiated respiratory difficulty with continuous grunting and need for supplemental oxygen at fifteen minutes of life. he was transferred to the neonatal intensive care unit presenting acute hypoxic respiratory failure, initiated mechanical ventilation with subsequent installation of persistent pulmonary hypertension. prostaglandin e1 was then added to the treatment. the chest x-ray showed pulmonary edema. the echocardiogram revealed a severe obstructive supracardiac tapvc. he was transferred to a specialized neonatal heart surgical center and was submitted to corrective surgery within the first 24 hours of life. today, he is a twelve months? healthy child, with normal growth and development. obstructive tapvc is one of the rare true surgical cardiac emergencies, and should be considered in any newborn with precocious refractory cyanosis, signs of pulmonary hypertension and low cardiac output.
HIV testing among pregnant women in Brazil: rates and predictors

Abstract: objective: to assess rates of offering and uptake of hiv testing and their predictors among women who attended prenatal care. methods: a population-based cross-sectional study was conducted among postpartum women (n=2,234) who attended at least one prenatal care visit in 12 cities. independent and probabilistic samples were selected in the cities studied. sociodemographic data, information about prenatal care and access to hiv prevention interventions during the current pregnancy were collected. bivariate and multivariate analyses were carried out to assess independent effects of the covariates on offering and uptake of hiv testing. data collection took place between november 1999 and april 2000. results: overall, 77.5% of the women reported undergoing hiv testing during the current pregnancy. offering of hiv testing was positively associated with: previous knowledge about prevention of mother-to-child transmission of hiv; higher number of prenatal care visits; higher level of education and being white. hiv testing acceptance rate was 92.5%. conclusions: the study results indicate that dissemination of information about prevention of mother-to-child transmission among women may contribute to increasing hiv testing coverage during pregnancy. non-white women with lower level of education should be prioritized. strategies to increase attendance of vulnerable women to prenatal care and to raise awareness among health care workers are of utmost importance.
Dietary Sugars Analysis: Quantification of Fructooligossacharides during Fermentation by HPLC-RI Method

Abstract: In this work, a simple chromatographic method is proposed and in-house validated for the quantification of total and individual fructooligossacharides (e.g., 1-kestose, nystose, and 1F-fructofuranosylnystose). It was shown that a high-performance liquid chromatography with refractive index detector could be used to monitor the dynamic of fructooligossacharides production via sucrose fermentation using Aspergillus aculeatus. This analytical technique may be easily implemented at laboratorial or industrial scale for fructooligossacharides mass-production monitoring allowing also controlling the main substrate (sucrose) and the secondary by-products (glucose and fructose). The proposed chromatographic method had a satisfactory intra- and inter-day variability (in general, with a relative standard deviation lower than 5%), high sensitivity for each sugar (usually, with a relative error lower than 5%), and low detection (lower than 0.06 ± 0.04 g/L) and quantification (lower than 0.2 ± 0.1 g/L) limits. The correct quantification of fructooligossacharides in fermentative media may allow a more precise nutritional formulation of new functional foods, since it is reported that different fructooligossacharides exhibit different biological activities and effects.

Abstract: trust is recognized as an essential element for the establishment of therapeutic relationship and has a major impact on treatment adherence. however, there are few studies and theoretical models which allow its conceptualization and measurement within this context. this research aims to study the development of trust in therapeutic communities and its impact on treatment adherence. it was adopted a mixed metho-dology and 231 subjects spread over nine therapeutic communities participated of the study. the results show the existence of two levels of trust (with the technical staff and with the institutions) and present it as a key element for developing therapeutic relationship and treatment adherence.
H1N1pdm09 Adjuvanted Vaccination in HIV-Infected Adults: A Randomized Trial of Two Single versus Two Double Doses

Abstract: Background Since human immunodeficiency virus (HIV)-infected individuals are at increased risk of severe disease from pandemic influenza A (H1N1pdm09), vaccination was recommended as a prevention strategy. The aim of the present study was to evaluate the safety, immunogenicity and persistence of the immune response after vaccination against pandemic influenza A (H1N1pdm09) with an adjuvanted vaccine in human immunodeficiency virus (HIV)-infected adults using two single and two double doses. Methodology/Principal Findings Open label, randomized trial to evaluate the immune response following H1N1pdm09 vaccination in HIV-infected participants compared to HIV-negative controls (NCT01155037). HIV-infected participants were randomized to receive 2 single (3.75 μg hemagglutinin) or 2 double (7.5 μg hemagglutinin) doses of the vaccine, 21 days apart. Controls received one dose of the vaccine. The primary endpoint was seroconversion as measured by hemagglutination inhibition assay. Two hundred fifty six HIV-infected participants (129 and 127 randomized to single and double doses, respectively) and 71 HIV-negative controls were enrolled. Among HIV-infected participants, seroconversion increased from 46.7% and 51.7% after the first dose to 77.2% and 83.8% after the second dose of the vaccine using single and double doses, respectively. Participants aged >40 years showed higher seroconversion compared to younger participants. Seroconversion among HIV-infected women and those with nadir CD4<200 cells/mm3 was significantly higher with double doses. Persistence of protective antibodies six months after vaccination was achieved by 80% and 89.9% of the HIV-infected participants who received single and double doses, respectively. Conclusions/Significance Our results support the recommendation of two double doses of adjuvanted H1N1pdm09 vaccine for HIV-infected individuals, particularly women, and those aged >40 years or with nadir CD4<200 cells/mm3, to achieve antibody levels that are both higher and more sustained. Abstract: background: sickle cell disease is the most common monogenic hereditary disease in brazil. although strokes are one of the main causes of morbidity and mortality in these patients, the use of transcranial doppler to identify children at risk is not universally used. objective: to develop brazilian guidelines for the use of transcranial doppler in sickle cell disease children and adolescents, so that related health policies can be expanded, and thus contribute to reduce morbidity and mortality. methods: the guidelines were formulated in a consensus meeting of experts in transcranial doppler and sickle cell disease. the issues discussed were previously formulated and scientific articles in databases (medline, scielo and cochrane) were carefully analyzed. the consensus for each question was obtained by a vote of experts on the specific theme. results: recommendations were made, including indications for the use of transcranial doppler according to the sickle cell disease genotype and patients age; the necessary conditions to perform the exam and its periodicity depending on exam results; the criteria for the indication of blood transfusions and iron chelation therapy; the indication of hydroxyurea; and the therapeutic approach in cases of conditional transcranial doppler. conclusion: the brazilian guidelines on the use of transcranial doppler in sickle cell disease patients may reduce the risk of strokes, and thus reduce the morbidity and mortality and improve the quality of life of sickle cell disease patients.

Abstract: to characterize macronutrient deficiencies in black pepper plants (piper nigrum l.), an experiment was conducted using pepper cuttings, "guajarina"1 cultivar, grown in pots containing silex and the following nutrient solutions: -n, -p, -k, -ca, -mg, -s and complete. the nutrient deficiencies were described at 90 and 150 days after omission of nutrients. the most absorbed nutrients were potassium and nitrogen, followed by ca, mg, s and p. the omission of n was the treatment which most afected plant growth, followed by ca and k. the analytical indices in the leaves of complete and omission treatments were respectively: n = 1.89 and 1.39%, p = 0.12 and 0.06%, k = 2.19 and 1.22%, ca = 1.04 and 0.73%, mg = 0.35 and 0.14%, s = 0.18 and 0.12%.
Scanning electron microscopic and profilometric study of different sharpening stones

Abstract: scaling and root planing contribute to the recovery of periodontal health. all periodontal instruments loose their fine cutting angle after use. to maintain this angle, correct sharpening is required using specifically designed stones. the characteristics of sharpening stones can be compared to the blade of the instruments and also transported to root surface during instrumentation. root smoothness is related to the quality of the blade. therefore, the purpose of this study was to evaluate the characteristics of 9 sharpening stones by scanning electron microscopic and profilometric analyses. ceramic and neumar stones were very fine and both may be recommended to maintain the sharpness of the instruments. arkansas, thompson and ce stones presented greater roughness with very regular and round particles, and are suitable for maintenance of the cutting angle. in addition, these stones may be indicated for the routine sharpening of the instruments that are partly dull. oxide aluminum, carborundum and jon stones were the coarsest with large irregular particles and may be indicated for initial sharpening of totally dull instruments with completion of sharpening with finer stones.

Abstract: homocystinuria, an autosomal recessive genetic disease, is characterized by deficiency of enzimes related to the methionine metabolism. in the present communication we present two brothers who were hospitalized due to severe high blood pressure. based on the lens dislocation, slowness in body growth, both plasmatic and urinary homocysteine levels, classic homocystinuria was diagnosed. the treatment of the metabolic disorder was b6 vitamin 500 mg/day, folic acid 5.0 mg/day and a low methionine diet. after three years of this treatment, favorable changes in their general aspect and their articular symptoms were observed. severe high blood pressure episodes have not been repeated.
Behavior prediction models and control charts for safety control of concrete dams

Abstract: this paper presents a study for improving measurement procedures on concrete dams based on the development of behavior prediction models using multiple regression and control charts. the series of displacements of the ta-2 concrete block crest of the tucuruí hydroelectric power plant, the largest brazilian hydroelectric power plant, was taken as an example of application. the methodology that was used generated a behavior prediction model that is in good agreement with the measured displacements. two types of control charts were tested, both of which identified the change in structural behavior of the block.
