Repensar o aprender: desafio do 3o milênio

Abstract: Diante da complexa situa o educacional, na atual sociedade do conhecimento, insistir em priorizar somente a quest o pedagógica, deixando de lado a perspectiva sócio-econ mico-política, na qual a mesma encontra-se envolvida, pode ser considerada uma vis o unilateral. Com base em estudos recentes desenvolvidos pelas Ciências Cognitivas, fortalecer-se-á a concep o de que uma mudan a efetiva na educa o só ocorrerá quando se for capaz de compreender o que ocorre no cérebro quando em atividade de conhecimento - denominada atividade cognitiva. Repensar o aprender como um desafio do 3o milênio implica rever o que realmente significa aprender. Optou-se assim, por situar essa discuss o num contexto pósmoderno, impregnado por uma sociedade do conhecimento. Rethinking learning: challenge of the third millenium The complex educational situation, on the current knowlegde society, insisting on prioritizing only the pedagogical question, setting aside the social, political and economical perspectives, in which it is implied, can be considered a unilateral view. Based on recent studies, developed by Cognitive Sciences, we can reinforce the conception that an effective change in Education will only happen when we are able to understand what occurs in the brain when it is in learning process — it is named Cognitive Activity. Rethinking learning as a challenge of the Third Milenium means to review what “learning” really represents. We chose, then, to situate our consideration in a post-modern context, permeated by a knowledge society.

Abstract: OBJETIVO: investigar a aquisi o das fricativas coronais por crian as de creches/escolas públicas da Regi o Metropolitana do Recife. MéTODOS: 40 crian as frequentadoras de quatro creches/escolas, com idades entre 2 a 6 anos e 11 meses, sendo 20 do sexo masculino e 20 do sexo feminino. O instrumento utilizado foi constituído por figuras que visavam à nomea o de 83 palavras que possuíam os fonemas-alvos: /s/ (32), /z/ (11), /∫/ (21), / / (19), selecionadas a partir das variáveis linguísticas tonicidade e estrutura silábica. As crian as foram divididas em 10 grupos, de acordo com a faixa etária. Considerou-se o percentual de 80% de produ o como indicativo de aquisi o do fonema-alvo. RESULTADOS: o início da produ o das fricativas coronais deu-se na faixa etária de 2:0-2:5 anos. Observou-se que os fonemas /∫/, / / e /z/ foram adquiridos aos 3:0-3:5 anos e o fonema /s/ aos 2:6-2:11 anos. Os processos fonológicos mais comumente encontrados foram substitui o e omiss o. Também foi observado com bastante frequência a troca semantica. CONCLUS ES: a aquisi o das fricativas coronais das crian as frequentadoras de escolas e creches públicas da Regi o Metropolitana do Recife ocorre mais tardiamente quando comparada à literatura nacional e sofre influência de variáveis linguísticas. é importante salientar que há uma diversidade linguística e sócio-cultural no território brasileiro, de forma que os dados observados em algumas regi es n o s o necessariamente correspondentes em todas as regi es do Brasil. Esse aspecto deve ser considerado em estudos que pretendem fixar parametros de avalia o fonológica, sobretudo se houver fins de diagnóstico de desvios fonológicos. Abstract: background: little is known about the outcome of patients with chagas cardiomyopathy in comparison to that of patients with idiopathic dilated cardiomyopathy in the contemporary era. objective: to compare the outcome of chagasic patients with chronic systolic heart failure secondary to chagas cardiomyopathy with that observed in patients with idc in the contemporary era. methods: a total of 352 patients (246 with chagas cardiomyopathy, 106 with idiopathic dilated cardiomyopathy) prospectively followed at our institution from january, 2000 to january, 2008 were included. all patients received standard contemporary medical therapy. results: in cox proportional hazards model multivariate analysis, digoxin use (hazard ratio=3.17; 95% confidence interval 1.62 to 6.18; p=0.001), need of inotropic support (hazard ratio=2.08; 95% confidence interval 1.43 to 3.02; p<0.005), left ventricular ejection fraction (hazard ratio=0.97; 95% confidence interval 0.95 to 0.99; p<0.005), and chagas cardiomyopathy etiology (hazard ratio=3.29; 95% confidence interval 1.89 to 5.73; p<0.005) were positively associated with mortality, whereas beta-blocker therapy (hazard ratio=0.39; 95% confidence interval 0.26 to 0.56; p<0.005) was negatively associated with mortality. survival probability for patients with chagas cardiomyopathy at 8, 24, and 49 months was 83%, 61%, and 41%, respectively, and for patients with idiopathic dilated cardiomyopathy 97%, 92%, and 82%, respectively (p<0.005). conclusion: in the current era of heart failure therapy, patients with chagas cardiomyopathy have a poorer outcome in comparison to patients with idiopathic dilated cardiomyopathy.

Abstract: purpose: to investigate the acquisition of the consonantal meetings in children speakers of the non-standard portuguese in recife, considering linguistic and social variables. methods: 100 children of public schools and nurseries took part in the study, being 50 boys and 50 girls, with ages between 2:0 and 6: 11-year old, divided in 10 groups with intervals of six months. we applied a socioeconomic questionnaire and a phonological evaluation test. results: discrete differences at the beginning of production and domain of complex onset between the sexes were found, having a trend for an earlier acquisition from girls. about the ages of beginning for production and domain, we observed the beginning of production from 2:0 - 2:5-year old, and the domain from 4:0 - 4:5-year old, earlier than the described in most of the literature. we also observed that the meeting composed by obstruent + /r/ was acquired before the composed by obstruent + /l/. regarding to linguistics variables, we verified influence of the tonicity, the syllabic structure and the preceding and consequent linguistic context on the phonogical acquisition. regarding social variables we did not observe important correlations between the maternal education level or familiar income with the phonological acquisition. conclusion: regional differences were observed on the phonological acquisition of the brazilian portuguese and those differences should be considered during the phonological evaluation process, mainly if this evaluation is to have purposes of diagnosis as for the phonological disorders, under penalty to commit incorrect and biased diagnostic from the sociolinguistic point of view.

Abstract: OBJETIVO: investigar a aquisi o dos encontros consonantais em crian as falantes do português n o padr o da Regi o Metropolitana do Recife, considerando variáveis linguísticas e sociais. MéTODOS: participaram do estudo 100 crian as de creches/escolas públicas, sendo 50 meninos e 50 meninas, na faixa etária de 2:0-6:11 anos, divididas em 10 grupos etários com intervalos de seis meses. Foi aplicado um questionário sócio-econ mico e uma prova de avalia o fonológica. RESULTADOS: foram encontradas diferen as discretas no início de produ o e domínio do onset complexo entre os sexos, havendo uma tendência a uma aquisi o mais precoce pelas meninas. Quanto às idades de início de produ o e domínio, observou-se o início de produ o aos 2:0-2:5 anos e o domínio aos 4:0-4:5 anos, faixa etária mais precoce que a descrita na maioria da literatura pesquisada. Também se observou que os encontros constituídos por obstruinte + /r/ foram adquiridos antes dos constituídos por obstruinte + /l/. Quanto às variáveis linguísticas, verificou-se a influência da tonicidade, da estrutura silábica e dos contextos linguísticos precedente e consequente na aquisi o fonológica. Quanto às variáveis sociais, n o foram observadas correla es importantes entre escolaridade materna ou renda familiar com a aquisi o fonológica. CONCLUS O: diferen as regionais s o observadas na aquisi o fonológica do português brasileiro e essas diferen as necessitam ser consideradas durante a avalia o fonológica, sobretudo se essa avalia o tiver fins de diagnóstico de desvios fonológicos, sob pena de se cometer diagnósticos incorretos e preconceituosos do ponto de vista sociolinguístico. Alveolar Capillary Dysplasia with Misalignment of Pulmonary Veins (ACD/MPV): A Case Series

Abstract: Alveolar capillary dysplasia with misalignment of pulmonary veins (ACD/MPV) is a rare, fatal, developmental lung disorder, which usually presents as persistent pulmonary hypertension of the newborn (PPHN) unresponsive to treatment. The authors present their own experience with three cases admitted during the last 15 years. 1. Introduction Alveolar capillary dysplasia with misalignment of pulmonary veins (ACD/MPV; OMIM number 265380) is a rare, fatal, developmental lung disorder, which usually presents as persistent pulmonary hypertension of the newborn (PPHN) unresponsive to treatment [1, 2]. The majority of the reported cases have been associated with other multiple congenital nonlethal anomalies, most frequently involving the cardiovascular, gastrointestinal, urogenital, and musculoskeletal systems [1]. Increasing awareness of this clinical entity may prevent the use of more invasive and futile treatments, including extracorporeal membrane oxygenation (ECMO). We present three cases of ACD/MPV associated with gastrointestinal and urological malformations. All the newborns had an overwhelming course, with PPHN and hypoxemia refractory to treatment. The diagnosis of ACD/MPV was established by autopsy. 2. Cases Report 2.1. Case Report 1 Full-term female newborn admitted to our NICU (Neonatal Intensive Care Unit) in 1997 was diagnosed with severe bilateral hydronephrosis. She was the second daughter of a young healthy, unrelated couple. On prenatal ultrasounds severe bilateral hydronephrosis and oligoamnios were detected. Delivery occurred by C section at 38-week gestational age. The Apgar score was 5/8 and birth weight 3170?g. During the first hours of life patient developed increasing respiratory distress, with hypoxemia and bradycardia, and was intubated and ventilated. Chest radiograph showed a mild haziness pattern and echocardiogram excluded structural heart disease and demonstrated signs of PPHN. At hour 30 of life a rapid clinical deterioration was observed, with refractory hypoxemia and persistent severe metabolic acidosis, despite ventilatory optimization. Antibiotic therapy with ampicillin and gentamicin was administered since admission to NICU and was later adjusted to ampicillin, cefotaxime, and amicacin. Septic workup was negative, including blood, urine, and cerebrospinal fluid cultures. The planned renal ultrasound and voiding cystourethrography (VCUG) were not preformed since the patient’s clinical condition deteriorated rapidly and newborn died on the second day of life. Abstract: Linoleic (LA) and α-linolenic acids (ALA) are considered essential fatty acids (EFA) because they are not produced by the human body. This way, EFAs sources must come from the diet. The primary dietary source of n-3 fatty acids is ALA, found in seeds and seed oils. Other important sources are fish oils such as tuna, salmon and herring. Currently, numerous studies suggest possible benefits of essential fatty acids in human health, such as in cardiovascular, cognitive and eye health, and also during pregnancy and childhood. This paper also discusses the impact of fatty acids in human metabolism, and the available evidence regarding its risks. It addresses the relevant debate regarding a general ban of trans fatty acids (TFA) from the world food market, because of the cardiovascular risks associated with its consumption.

Abstract: advances in science have resulted in increased longevity and reflect the growing number of elderly individuals in society today. the increase in chronological age brings progressive loss of functionality, and changes the process of providing information on body awareness, resulting in the misinterpretation of body parameters. studies have shown that movement and physical activity are able to positively influence body perception. this study aimed to evaluate the effects of perceptual body stimulation activities on the body schema of the elderly. the evaluation was performed with the imp (image marking procedure), capable of measuring the degree of distortion between the actual and perceived image in different parts of the body. measurements of body size perception were taken before and after perceptual body stimulation programs. the study results showed that the body intervention program can bring benefits to the participants, but it is necessary that stimulation have a more specific focus, according to the perceptive characteristics of each individual.
Rela??o entre enfermeiros e médicos em hospital escola: a perspectiva dos médicos

Abstract: the conflict between physicians and nurses, historically the two main groups of professionals responsible for patient care, is originated from the combination of several factors ranging from the establishment of the multidisciplinary team to salary issues. this study aims to determine, in the view of physicians, the existence of conflict in the relationship between them and nurses at the hospital das clinicas, universidade federal de goiás (hc / ufg) and what factors are associated with it. to this goal, 30 doctors completed a questionnaire with demographic data and questions about the variables that affect the relationship, using a lickert scale. among the results, the average of age is 42.7 years, 50% of participants are female and 93.3% of the respondents worked elsewhere. the average of years of working in the hc is 16.3. concerning to the conflict, we highlighted four factors of conflict prevention (pc), with middle ranking (rm) greater than 3.0, and two others generators of conflict (2gc). these factors are: interprofessional communication (rm=3.03); autonomy in team (rm=3.63); interprofessional relationship (rm=3.36); working conditions (rm=2.26), influence of hospital as a school (rm=2.83) and patient benefits (rm = 3,93). we concluded that in the analyzed reality the conflict is considered non-existent, because there is a favorable ratio of protective factors (4pc:2gc), but it is imminent, due to the persistence of factors that can unleash it ("working conditions" and "influence of hospital as a school "). moreover, there is power struggle with nursing, which can unbalance the situation and create ethical problems.
