Single Incision Laparoscopic Surgery-An Overview and Current Status  [PDF]

Abstract: Single incision laproscopic surgery is an alternative to conventional multiport laproscopy. Natural orifice transluminal endoscopic surgery term coined by a Consortium in 2005 remains a research technique with few clinical cases. Single incision surgery offers advantage of better cosmesis, reduced incisions, lesser hernias, decreased pain and infections. Long learning curves and cost of instrumentation are the difficulties encountered in its propagation. Single incision laproscopic surgery is an evolving technique for advanced laproscopic centers. This article outlines the overview of devices and instruments and the techniques, feasibility of single incision laproscopic surgery.

Abstract: Objective: Maxillofacial injuries are one of the commonest injuries encountered. Roentgenographic evaluation of maxillofacial trauma is of prime importance for diagnosis and treatment of these injuries. Study Design: Forty patients were evaluated in the prospective four-year study. We studied and evaluated the demography and diagnostic efficacy of clinical, plain radiography, and computed scan in maxillofacial trauma. Result: Road traffic accidents were the commonest cause of maxillofacial injuries. Patients having multiple fractures, mandibular fractures were the commonest. Conclusion: Computed tomography proved a useful adjunct in midfacial trauma.

Abstract: In general, a supplier/retailer frequently offer trade credit to stimulate their respective sales. The main purpose of this paper is to investigate the optimal supplier/retailer’s replenishment decisions under two levels of trade credit policy within the economic order quantity (EOQ) framework. This paper deals with the supplier/retailer’s inventory replenishment problem under two levels of trade credit in one replenishment cycle. A different approach of two levels of trade credit is used, which give more freedom to the supplier/retailer in business. In addition, the easy-to-use procedure is developed to efficiently find the optimal cycle time for the retailer under minimizing annual total relevant cost. Finally, a numerical example is given to illustrate these results.

Abstract: Many previous studies showed that seam appearance as well as performance depends on the interrelationship of fabrics, threads, and the stitch/seam selection and lastly on the sewing conditions, which include the needle size, needle thread tension, stitch density, and the appropriate operation and maintenance of the sewing machine. Present investigation elucidates the effect of blend composition, sewing thread size, and sewing needle parameters on garment seam characteristics (i.e., seam strength, seam strength efficiency, seam puckering, seam stiffness, and drape coefficient). The seam quality characterization is studied through the L9 orthogonal design methodology. In good quality apparels, compatibility of the seams with the functional requirement is very important for serviceability and life of the apparel. Reversibility as well as repairing of seams in the apparel is very limited in the condition of seam failure. Seam strength efficiency is higher for the uniform fiber matrix in the structure of fabrics. The polyester dominated suiting fabrics give minimum seam stiffness because polyester component has low flexural rigidity. The cotton dominated suiting fabrics have less seam puckering due to increase in fabric stiffness. 1. Introduction The fabric quality influences not only the quality of the garment but also the ease with which a shell structure can be produced out with flat fabric. The specifications of fabrics for apparel manufacturing can be considered in terms of primary and secondary quality characteristics. The primary quality characteristics are static physical dimensions and secondary characteristics are the reactions of the fabric to an applied dynamic force. The apparel manufacturer is primarily interested in the secondary characteristics of the fabric and focus on the seam quality during the fabrication and production of apparel [1, 2]. On the other hand, apparel consumers are mainly paying attention to appearance, comfort, and wearability of fabric and evaluate seamquality based on the seam appearance and its mechanical intactness after wear and care procedures of apparel. The production of garments from high quality fabrics not only gives comfort to the wearer but also helps in the smooth working of manufacturing processes and leads to almost defect-free garments. Further, the primary function of a seam is to provide uniform stress transfer from one piece of fabric to another, thus preserving the overall integrity of the fabric assembly. Influence of nebivolol on anticonvulsant effect of lamotrigine

Abstract: Objective: The present study describes the effect of nebivolol (NBV) either alone or in combination with lamotrigine (LTG) using increasing current electroshock seizures (ICES) model in mice. Materials and Methods: Male albino mice of Swiss strain each weighing 18-30 g were used. Lamotrigine (Lamitor tablets, Torrent; 1.5 and 3.0 mg/kg) and NBV (Nebicard tablets, Torrent; 0.25 and 0.5 mg/kg) were suspended in 0.25% of carboxy methyl cellulose (CMC) in 0.9% saline and administered orally in volumes of 10 mg/kg. Control animals received an equivalent volume of 0.25% CMC in 0.9% saline suspension. The anticonvulsant effects of the drugs were measured using ICES model whereas cognitive behavior was measured by the spontaneous alternation behavior and grip-strength test. The biochemical estimation was done by measuring the lipid peroxidation and reduced glutathione (GSH). Results: Both NBV and LTG produced significantly enhanced seizure threshold (ST), decreased grip strength, inhibited lipid peroxidation, and increased brain GSH levels in acute and chronic dosages likened to control group, whereas there was no significant effect on alternation scores. The combination of NBV with LTG significantly potentiated the ST when compared to LTG. Conclusion: Nebivolol showed antiepileptic effects in addition to its reported antihypertensive effect, which could be attributed to action of the two drugs through different mechanisms or due to drug interaction that may be pharmacodynamic or pharmacokinetic needing elucidation.
Five year clinico pathological study of breast cancer.

Abstract: Breast cancer is a prototype for exploring variations in pathological diagnosis and etiology. BASIC PROCEDURE: We analysed the various risk factors, age, Tamoxifen treatment and pathological diagnosis for Breast cancer. MAIN FINDINGS: We present a review of 105 cases of breast cancers diagnosed and treated at Dr Ram Manohar Lohia Hospital in a five years prospective study between 1997 and 2002. Breast cancer was most commonly found in age group 30-40 years and infiltrating carcinoma of breast was the commonest occurring in 90% of the cases. IMPLICATIONS: We demonstrated the utility of such study of ensuring greater equity and understanding in etiology. The risk factors and pathological diagnosis in cancer of breast were studied and need for comprehensive screening programme were outlined.
A Framework for Prefetching Relevant Web Pages using Predictive Prefetching Engine (PPE)

Abstract: This paper presents a framework for increasing the relevancy of the web pages retrieved by the search engine. The approach introduces a Predictive Prefetching Engine (PPE) which makes use of various data mining algorithms on the log maintained by the search engine. The underlying premise of the approach is that in the case of cluster accesses, the next pages requested by users of the Web server are typically based on the current and previous pages requested. Based on same, rules are drawn which then lead the path for prefetching the desired pages. To carry out the desired task of prefetching the more relevant pages, agents have been introduced.
Supernumerary eumorphic mandibular incisor in association with aggressive periodontitis

Abstract: According to the literature, the prevalence of supernumerary teeth is 1% to 4% of permanent dentitions; and among these, the presence of fifth mandibular incisor - a supernumerary eumorphic tooth - has rarely been described in literature, and its association with localized aggressive periodontitis is an even more rare entity. This paper reports a very rare case of unusual association of supernumerary eumorphic fifth mandibular incisor with aggressive periodontitis in a Muslim individual, so that these findings generate curiosity and inspire others to carry out further studies and investigations.
