Abstract: competition integrates abilities, attitudes and knowledge; characterize the capability to accomplish a task efficiently, to practice a profession, to resolve professional problems of autonomous and flexible form, to collaborate at the professional surroundings and in the organization of work. when changes labor space and the science develop, the modeling of competitions varies, because the fringe benefit of services demands a preparation that respond to new knowledge and introduction of technologies that the doctor will have to apply. each science contributes with knowledge that hold work at the clinic and health practice that graduate will execute. in this work we appraise the contribution of immunology to the quality of medical graduate relating to his professional competitions and the present-day deficiencies in the formation. in order to make competitions perfect, the teaching of immunology can do contributions, but requires changes in his conception, their curricular position and in the methods used for his teaching, considering the principal problems of health implicating immune system in etiopatogenia. this teaching will guide the manners of action and of performance of the doctor in the community

Abstract: La competencia integra habilidades, actitudes y conocimientos caracteriza la Ccapacidad para ejercer una profesión eficientemente y resolver problemas profesionales de forma autónoma y flexible. Cuando cambia el ámbito laboral y se desarrolla la Ciencia, el modelado de competencias varía, porque la prestación de servicios exige una preparación que responda a nuevos conocimientos e introducción de tecnologías que el médico deberá aplicar. Cada ciencia aporta conocimientos que sustentan el trabajo en la práctica de salud que ejecutará el egresado. Valoramos la contribución de la Inmunología a la calidad del egresado de Medicina en relación con sus competencias profesionales y las deficiencias actuales en la formación. Para perfeccionar las competencias, la ense anza de la Inmunología puede hacer aportes, pero requiere cambios en su inserción en la Carrera y en los métodos para su ense anza, partiendo de los principales problemas de salud que poseen una etiopatogenia en la que está involucrado el sistema inmune, orientada a los modos de acción y actuación del médico en la comunidad. Abstract: health-related quality of life (hrql) is an assessment concept significant to clinical decisions. we examine bioethical aspect in multi-items questionnaires and clinical practice by documental review of 7 hrql generic validate instrument and by 15 experts opinions expressed in opinion poll and 5 interview. in opinion poll we estimate the affirmative answer frequency about the existence of bioethical domains in health-related quality of life instrument and medical practice. interviews enquire about bioethics dilemmas in hrql study. bioethical domains lack in all revised instrument, in agree with experts opinions. knowledge about bioethics of health-related quality of life is lacking in medical care practice, in issues surrounding informed consent, dilemmas in end-of-life care, individual autonomy/beneficence balance, circumstances and consequences of health care medical decisions among others. experts think that stressful life and socioeconomic conditions, equality and inequity and sustainable environment dilemmas, must be include in study of hrql.
Evaluación bioquímico-inmunológica de infectados crónicos por el virus de la hepatitis B: Comportamiento de la respuesta inmune en sus contactos familiares

Abstract: chronic infection by hbv is a clear and important cause of morbidity and mortality worldwide. an important problem, for the control and eradication of this disease is the existance of chronic hbv carriers and their family contacts. a biochemical and immunological study of chronic hbv reservoirs were conducted to determine the relation between the concentrations of hbv surface antigen, detected and quantified by the elisa method, and the serum viral dna concentrations; as well as the correlation between the levels of antigenemia and the transaminase values (alat). a direct linking was proved between viral dna and hbsag levels and also between antigen concentration and alat values. a significant percent of carriers with high viral charges and normal enzymatic levels was observed. the performance of immune response to hbv in those persons living with the chronic hbv-infested subjects was analyzed and the frequency of occurence of immune, sensitive and infected persons was estimated resulting in a high number of infected persons and persons living with the chronic patients without protection. qualitative and quantitative elisa were used to assess the response of antibodies to hbsag, induced by vaccines or by normal contacts of blood related and non-related persons in the family, but there was no difference among them. the hbsag concentration of the carrier was associated with levels of antibodies to hbsag; after completion of the vaccination program in blood related and non-related subjects, then the association was found to be negative in the former and the existance of shared gens linked to the control of anti-hbsag response and viral clearing were infered

Abstract: a sandwich, heterogenous, qualitative, doble antigen immunoenzimatic method was standardized to detect anti-streptokinase antibodies in pure samples of human serum. different concentrations of the recovering antigen and conjugate dilutions, as well as various times and temperatures of incubation of the samples were assayed. the cut-off value of the assay and its detection limit were established and it was proved if there were some unspecificities. 0.1 mg/ml was determined as an optimal recovering concentration for a conjugate dilution of 1:100 000. the optimum conditions of incubation of the samples were 30 min at lab temperature and there were no interferences of other analytes with the technique.the optical densities between 0.282 and 0.654 were established as normal values of the technique.
Estudio del transporte madre-hijo de anticuerpos antiestreptoquinasa

Abstract: Se estudió cómo se comporta el transporte de anticuerpos anti-estreptoquinasa entre la madre y el recién nacido y cuáles son los factores que influyen en este. Se colectaron 125 muestras sanguíneas de puérperas, por la vena antecubital y de sus respectivos hijos a partir del cordón umbilical. Se confeccionaron encuestas para la recolección de datos a partir de las historias clínicas de las madres y los recién nacidos. Se empleó un método inmunoenzimático, heterogéneo, cualitativo, tipo sandwich de doble antígeno para la detección de anticuerpos anti-estreptoquinasa. A partir de los resultados obtenidos se concluyó que existe un transporte activo de los anticuerpos anti-estreptoquinasa de la madre al feto y que este transporte se ve influenciado por la edad cronológica de la madre, la edad gestacional, el estado nutricional de la madre y el peso del recién nacido. The behavior of the transport of streptokinase antibodies from the mother to the newborn infant, as well as the factors influencing on it, were studied here. 125 blood samples were obtained from the antecubital vein of the puerperas and from the umbilical cord of their children. Surveys were done to collect data from the medical histories of the mothers and the newborn infants. An immunoenzimatic, heterogenous and qualitatitve sandwich doble antigen method was used to detect anti-streptokinase antibodies. According to the results, it was concluded that there is an active transport of anti-streptokinase antibodies from the mother to the fetus and that this transport is influenced by the chronological age of the mother, the gestational age, the mother’s nutritional status and the birth weight.

Abstract: Se procedió a estandarizar un método inmunoenzimático, heterogéneo, cualitativo, tipo sandwich de doble antígeno para la detección de anticuerpos contra la estreptoquinasa en muestras puras de suero humano. Se ensayaron diferentes concentraciones del antígeno de recubrimiento y diluciones del conjugado, así como diferentes tiempos y temperaturas de incubación de las muestras. Se estableció el valor de corte del ensayo y su límite de detección y se comprobó si existían inespecificidades. Se determinó como concentración óptima de recubrimiento 0,1 μg/mL para una dilución del conjugado de 1:100 000. Las condiciones óptimas de incubación de las muestras fueron 30 min a temperatura de laboratorio y no se comprobaron interferencias de otros analitos con la técnica. Se establecieron como valores normales de la técnica las densidades ópticas que se encuentran entre 0,282 y 0,654. Evaluación y control de un brote epidémico de hepatitis viral en escolares guineanos: Eficacia de heberbiovac HB

Abstract: Se estudiaron 448 muestras serológicas con el propósito de conocer la etiología de un brote de hepatitis aguda entre jóvenes guineanos que estudiaban en Cuba. Se determinaron los anticuerpos de clase IgM contra el virus de la hepatitis A (anti-VHA IgM), los anticuerpos contra la proteína central del virus de la hepatitis C (anti-VHC), el antígeno de superficie del virus de la hepatitis B (AgsHB) y su anticuerpo (anti-AgsHB) y los anticuerpos dirigidos contra el virus de la hepatitis Delta (anti-VHD). Se encontró positividad para el AgsHB en 124 sueros (27,67 %). A 86 sueros AgsHB positivos se les determinó la presencia del antígeno asociado con la replicación viral en la infección por virus B (AgsHB) y la detección de los anticuerpos totales (IgG e IgM) contra la proteína central del virus de la hepatitis B (anti-AgcHB total) se hizo en 50 de esas muestras. Los estudiantes que no tuvieron marcadores de infección por el virus B se vacunaron con la vacuna cubana antihepatitis B, con el empleo de 3 dosis de 20 mg mediante un esquema de 30 d interdosis. No se encontraron positivos el anti-VHA IgM ni el anti-VHC. La cuantificación del AgsHB permitió clasificar como agudos con buen pronóstico a 3 de los 14 jóvenes que presentaron síntomas de hepatitis y como crónicos a los 121 restantes. De los sueros de 3 fallecidos con un cuadro de hepatitis fulminante, 2 fueron AgsHB positivos y 1 anti-AgsHB positivo, y el tiempo de vida media estimado para el antígeno fue de 48 h, a diferencia del obtenido para los agudos con buena evolución, que resultó de 7 d aproximadamente; mientras que en los crónicos fue como promedio de 60 d. En los reservorios crónicos se detectó una elevada antigenemia (59 % con cifras de AgsHB entre 10 y 100 mg/mL) en asociación con la presencia del AgeHB, que fue positivo en 31 % de los sueros B. Se encontraron anticuerpos anti-VHD en 15 % de los sueros AgsHB positivos analizados, con 66 % en los fallecidos. La cuantificación del AgsHB permitió establecer criterios de clasificación pronóstica en los infectados. La asociación de la presencia del AgeHB en los reservorios crónicos con altas cifras de AgsHB puso de manifiesto el valor de la cuantificación del antígeno de superficie como indicador indirecto de infecciosidad. La circulación del virus Delta se consideró un factor contribuyente para la aparición del brote y la mortalidad. Abstract: La infección crónica por el virus de la hepatitis B (VHB) es una clara e importante causa de morbilidad y mortalidad en el mundo. Un importante problema para el control y la erradicación de la enfermedad es la exisniveles de anti-AgsHB, después del esquema completo de vacunación en consanguíneos y no consanguíneos, resultó ser de tipo negativa en los primeros e infirió la existencia de genes compartidos relacionados con el control de la respuesta anti-AgsHB y con el aclaramiento viral Chronic infection by HBV is a clear and important cause of morbidity and mortality worldwide. An important problem, for the control and eradication of this disease is the existance of chronic HBV carriers and their family contacts. Inmunogenicidad de la vacuna cubana recombinante heberbiovac HB en modelos experimentales y aplicados al humano

Abstract: one of the fundamental pillars in the control, elimination and possible eradication of b-d viral hepatitis is the use of the cuban-made hepatitis b vaccine. experimental models as well as models for humans were developed to study immunogenicity using various routes of administration, doses and schedules. balb/c mice were used on experimental basis to which 1 mg/dose were administered by intramuscular(im) and intraperitoneal (ip) within three different vaccination schedules, i,e, at 0, 1, and 2 weeks; 0, 1 and 4 weeks, and 0, 4 and 8 weeks. immunogenicity was stufied through quantification of antibodies to surface antigens by using an immunoenzymatic method. seroconversion was present in all animals at 0, 1 and 4 and 0, 1, and 8 weeks schedule. however, in 0, 1 and 2 schedule, ip route of administration was more immunogenic than im, with 60% and 35% seroconversion respectively. ip route had a lower variation coefficient than 0, 1 and 4 week and 0, 4 and 8 schedules were compared, but anyway, it was high even for balb/c animals. medical and nursing students were immunized with 20 mg/dose im and 2mg/dose intradermally within 0, 1 and 4 week and 0,4 and 8 w schedules. it was showed that seroprotection and hyperesponse were greater in 0, 4 and 8 week vaccination schemes regardless of the route of administration and that 0, 1 and 4 w schedule does not bring required immunogenicity, therefore, it should be used exceptionally. national antibody standards were obtained to be applied in anti-hbsag quantification methods both in experimental and human-based studies
