Current management of gastric cancer

Abstract: gastric cancer is a disease with high incidence and mortality in our population. the prognosis of patients with this disease is closely related to the neoplasm stage at diagnosis, including the following characteristics of the tumor: extension into the gastric wall thickness, spread to locoregional lymph nodes and the ability to generate distant metastases, as described by the tnm classification. for localized tumors characterized only by invasion of mucosa or submucosa at diagnosis, survival at 5 years is between 70 and 95% with exclusive surgical management; however, when extension into the gastric wall is higher and/or there is locoregional nodal involvement, survival decreases to 20-30% at 5 years. currently, at high-volume centers, the extent of gastrectomy is individualized based on several parameters, which in an increasing number of cases allows a total gastrectomy with d2 lymphadenectomy and preservation of the spleen and pancreas. this improved procedure increases the chance of r0 surgery and improves the relationship between resected and affected lymph nodes, resulting in a decreased risk of the long-term locoregional recurrence. to improve these results, different therapeutic strategies combining chemotherapy or chemoradiotherapy with surgery have been tested. previously, the intergroup 0116 clinical trial, published in 2001, which changed clinical practice in the united states, showed that adjuvant chemoradiotherapy improved survival (from 26 to 37 months overall survival) of these patients. in europe, perioperative chemotherapy has been considered the standard treatment, since the publication of two randomized phase iii trials showed an increase at 5 years survival in the group treated with chemotherapy.

Abstract: The electromagnetic radiation of nanoemitters placed into a multilayered microsphere with dispersive left-handed (LH) layers included is studied numerically. It is found that in the frequency range where LH layers have a negative refraction index the field frequency spectrum consists of a series of narrow and well separated resonances. In the band of such peaks, the great part of the field energy is located in a LH layer and practically does not leave the microsphere.
State-Space Dynamics Distance for Clustering Sequential Data

Abstract: This paper proposes a novel similarity measure for clustering sequential data. We first construct a common state-space by training a single probabilistic model with all the sequences in order to get a unified representation for the dataset. Then, distances are obtained attending to the transition matrices induced by each sequence in that state-space. This approach solves some of the usual overfitting and scalability issues of the existing semi-parametric techniques, that rely on training a model for each sequence. Empirical studies on both synthetic and real-world datasets illustrate the advantages of the proposed similarity measure for clustering sequences.

Abstract: We numerically investigate the optical transmission through a compound spherical stack with conventional and metamaterial (MM) layers and also embedded MM defect. A formation of extremely narrow resonant peak with nearly complete transmittance in area of a band gap is found. We demonstrate that photon fields of certain frequencies can be strongly confined by a left-handed (LH) defect. The influence of a random deviation in the width of compound spherical layers as well the transit to the whispering gallery mode (WGM) is also discussed.

Introduction Controlling the electromagnetic properties of materials, going beyond the limit that is attainable with naturally existing substances, has become a reality with the advent of metamaterials [1, 2]. The range of various structured artificial structures has promised a vast variety of otherwise unexpected physical phenomena [3–5] among which the experimental realization of a negative refractive index has been one of the main advantage. A typical metamaterial combines continuous metal films (about 50?nm) with nanostrip magnetic resonators that finally yield a negative-index material. The film can be formed with a mixture of dielectric (e.g., silica) and metal, such as silver or gold, forming a semicontinuous metal film. Such structures are fabricated using techniques as the evaporation of a metal onto a dielectric substrate (see more details in review [5]). Preparation of metamaterial structure in the optical range still is an advanced task. In recent experiment [6] it has been demonstrated that the incorporation of the gain material (rhodamine 800) in the metamaterial makes it possible to fabricate an extremely low-loss and an active optical negative-index metamaterial (NIM) that is not limited by the inherent loss in its metal constituent. In this experiment the optical (NIM) structure (silver layers + rhodamine 800) is the fishnet with periodicity about 300?nm. Epoxy doped with rhodamine 800 (Rh800) and dye is used as a gain medium. The loss compensation mechanism in the sample is straightforward to understand. When Rh800 is excited by a pump pulse with sufficiently high power, a population inversion is formed inside the dye molecules. This provides amplification for a properly delayed probe pulse whose wavelength is coincident with the stimulated emission wavelength of the dye molecules (see details in [6]). Natural extension of such directions is the analysis of a compacted spherical multilayered system with embedded metamaterial layers. It is well known that except for the whispering gallery mode (WGM)

Abstract: The reactions of Cl atoms with a series of unsaturated aldehydes have been investigated for the first time using a relative method. In order to obtain additional information for a qualitative structure versus reactivity discussion, we have also determined the rate coefficients for the reactions of atomic chlorine with their respective saturated aldehydes. These relative measurements were performed at room temperature and atmospheric pressure of air and N2, by using ethane, propene and 1-butene as reference compounds. The weighted average relative rate constants obtained, kCl±2σ (in units of cm3 molecule 1 s 1) were: trans-2-pentenal (1.31±0.19)×10 10; trans-2-hexenal (1.92±0.22)×10 10; trans-2-heptenal (2.40±0.29)×10 10; n-pentanal (2.56±0.27)×10 10; n-hexanal (2.88±0.37)×10 10; n-heptanal (3.00±0.34)×10 10. Finally, results and atmospheric implications are discussed and compared with the reactivity with OH and NO3 radicals. Spatial influence and oceanic thermal response to Santa Ana events along the Baja California Peninsula

Abstract: meteorological data were recorded at eight stations located along the coast of the pacific ocean and three along the coast of the gulf of california, aimed to assess the spatial influence of santa ana weather conditions in the baja california peninsula. february 2002 featured two santa ana events: one from the 9 to the 12 and another from the 21 to the 22. the first santa ana event had the strongest winds, however relative humidity and temperature behaved similarly on both events at some stations. data from the pacific ocean showed typical santa ana condition patterns: wind speed and temperature increase opposed to decreased relative humidity values. data from the gulf of california did not show the typical temperature rise of a santa ana condition, but there was a decrease on the amplitude of the diurnal variability of air temperature and relative humidity as well as a marked increase on wind strength. wind direction during the santa ana events on the pacific side was ne and nw on the gulf of california. ne winds are associated to the shift on the position of the north pacific high pressure center, which moves towards the continent. data suggest that relative humidity may be the best parameter to monitor both occurrence and length of santa ana conditions on the pacific side. normal weather conditions show a negative air-sea temperature difference, but during both santa ana events this difference was positive and higher than 10 °c. latent and sensible heat fluxes drastically increased during both events, reaching values more than three times higher than those for normal conditions, which is due to the presence of strong winds combined with a drier and hotter air mass over the ocean.

Abstract: occasionally hepatectomy for metastases of ampulla of vater carcinoma can result in a better survival and quality of life of patients. we report a 67 years old male subjected to a duodenopancreatectomy for a carcinoma of the ampulla of vater that required afterwards a right hepatectomy for metastases. twenty one months after the second operation and 42 months after the first operation, that patient is asymptomatic and without evidences of relapse.

Abstract: oculopharyngeal muscular dystrophy (opmd) is a rare myopathy that is characterized by ocular and pharyngeal muscle involvement. opmd typically presents with ptosis, dysarthria, and dysphagia. it can also be associated with proximal and distal extremity weakness. we report two patients with the disease. a 79 years old female presenting with ptosis, dysphagia and a history of three aspiration pneumonias. the patient was subjected to a myotomy of the cricopharyngeal muscle of 4.5 cm of length. the patient had a symptomatic improvement and is in good conditions five months after the operation. a 75 years old male presenting with dysphagia and ptosis. he was operated, performing a myotomy of the cricopharyngeal muscle of 3.5 cm of length. two and a half months after operation the patient is devoid of dysphagia.

Abstract: Propósito: Determinar el efecto de la terapia fotodinámica con el uso de verteporfino en el tratamiento de pacientes con proliferación angiomatosa retiniana (PAR) y la incidencia de esta patología retiniana en nuestro medio. Métodos: Se presenta un estudio retrospectivo de 11 casos diagnosticados de PAR y que fueron tratados con terapia fotodinámica con verteporfino. Resultados: La incidencia de PAR en el grupo de ojos con neovascularización coroidea subfoveal mínimamente clásica u oculta fue del 8%. El seguimiento medio fue de 15 meses. La AV mejoró en 3, permaneció igual en 4, y disminuyó en 4. 4 pacientes presentaban una lesión bilateral. Conclusiones: La proliferación angiomatosa retiniana no tiene un tratamiento demostrado. Estos resultados sugieren que TFD puede ser una terapia útil en pacientes con PAR y parece reducir el riesgo de pérdida visual. Purpose: To determine the effects of phothodynamic therapy using verteporfin in the treatment of patients with retinal angiomatous proliferation (RAP) and the incidence of this retinal disease in our area. Methods: We performed a retrospective study of 11 cases of RAP who were treated with photodynamic therapy using verteporfin (PDT). Results: The incidence of RAP in the group of eyes with minimally classic or occult subfoveal choroidal neovascularization was 8%. The mean follow-up time after treatment was 15 months. The visual acuity improved in 3, remained the same in 4 , and decreased in 4. In 4 patients, angiomatous lesions were observed in both eyes. Conclusions: There is no method proven to be effective for the treatment of retinal angiomatous proliferation. Our results suggest that PDT may be useful therapy in patients with RAP as it appeared to reduce the risk of visual loss.
