Abstract:
Background: Pneumomediastinum and subcutaneous emphysema are a rare and well-known complication of dental procedures. Many cases go unrecognized and resolve spontaneously, while others may require specific therapeutic management to prevent complications. Case Presentations: We report a case of cervical subcutaneous emphysema and pneumomediasitnum occurring after extraction of lower right second molar. The emphysema was detected 2 hours after dental surgery. This paper reports on the diagnosis and treatment of subcutaneous emphysema and pneumomediastinum. Conclusion: In order to prevent this complication, air turbine high speed drills should be used only in necessary cases.

Abstract: Background: Colchicine intoxication is uncommon but potentially life threatening because of the high toxicity of the drug. Colchicine binds to the intracellular protein tubulin, causing disturbed mitosis in all tissues followed by multi-organ failure. Case Presentation: We report a case of a 23-year-old woman admitted to emergency room 12 hours after a suicide attempt by ingestion of approximatively 40 mg of colchicine. Few hours after consumption, first symptoms were a hemodynamic shock and ARDS, leading to her death due to multi-organ failure. Conclusion: The colchicine intoxication has strong morbidity for doses higher than 0.5 mg/kg. The treatment is symptomatic. The immunotherapy using anti-colchicine allows improving critical form’s prognosis.

Dislocation of temporo-mandibular joint - an uncommon circumstance of occurrence: vaginal delivery

Abstract: Dislocation of temporo-mandibular joint (TMJ) is an infrequent disease but still almost spectacular. This disease consists of a permanent, to some extent complete disruption of the temporo-mandibular joint. These dislocations often occurs in a context of yawning, and less frequently after a burst of laughing or relatively mild facial trauma (slap, punch on the chin).We report a case of TMJ occurring in an uncommon circumstance : vaginal delivery. A young woman aged 24-years with no special past medical history; primipara was admitted in the Department of Maternity of the University Hospital Hassan II of Fez for an imminent delivery of a twin pregnancy. Obstetrical analgesia was not possible so the parturient cried in a strong manner during labour. Ten minutes after admission, the patient delivered vaginally with episiotomy. She gave birth to twins weighing 2800g and 2400g. During labour, and effort of crying, the patient presented a sudden and immediate loss of function of the temporo-mandibular joint, with difficulty of speaking, the mouth permanently opened, with the chin lowered and thrown forward. The examination found an emptiness of the glenoid fossa of the temporo-mandibular joint in both sides. The diagnosis of dislocation of the TMJ has established. Performance of special radiologic screening to study the TM was technically not possible. A CT scan of facial bones has been achieved so objectifying a bilateral dislocation of TMJ. The reduction of this dislocation was performed in the operating room under sedation
Intoxication aigu sévère par les pesticides organophosphorés: à propos de 28 cas

Abstract: Les pesticides organophosphorés (POP) sont des pesticides organiques de synthèse, largement utilisés en agriculture essentiellement comme insecticide, nemacide ou acaricide. Ce sont les produits agricoles, les plus incriminés dans les intoxications dans notre contexte. L’objectif de ce travail était de déterminer les caractéristiques cliniques, paracliniques, et évolutives de cette intoxication en milieu de réanimation. étude rétrospective portant sur les cas admis en réanimation (2003-2010). Les critères d’inclusion étaient d’ordre clinique, para clinique, thérapeutique et évolutif. 28 cas ont été recensés : 19 femmes et 9 hommes, age moyen = 24,5 plus or minus 11 ans. La tentative de suicide était le principal motif d’intoxication (19cas). Le Glasgow coma score était en moyenne de 11 plus or minus 4. Le syndrome central, était présent chez 78% de nos patients, suivi du syndrome muscarinique 71% et le syndrome nicotinique dans 53% des cas. La prise en charge thérapeutique a consisté à la ventilation mécanique dans 50% des cas, l’usage des drogues vasoactives dans 14% des cas et l’administration d’un traitement antidotique dans 64% des cas. La mortalité globale était de 25%. Les pesticides organophosphorés sont les toxiques agricoles, le plus souvent incriminés dans notre contexte. Les sympt mes résultent d’une importante accumulation d’Acétyle-choline (Ach) dans l’organisme ; responsable de l’apparition des trois syndromes caractéristiques. Le diagnostic biologique se fait par le dosage de l’activité cholinestérasique dans le plasma. Le traitement associe des mesures symptomatiques qui reposent essentiellement sur la réanimation respiratoire et neurologique au traitement antidotique. L’évolution clinique dans ce type d’intoxication, est généralement favorable sous traitement avec régression des signes en quelques jours. Le décès est essentiellement, le fait d’une insuffisance respiratoire de mécanismes multiples: encombrement bronchique réalisant une véritable noyade interne, bronchoconstriction, paralysie des muscles respiratoires, dème pulmonaire d’évolution gravissime
Intoxication par la paraphényléne-diamine (takaout) au Maroc: à propos de 24 cas

Abstract: La paraphénylène-diamine (PPD) est une amine aromatique dérivée de l’aniline, utilisée depuis 1863 par les femmes dans un but cosmétique comme teinture capillaire noire ou adjuvant de henné dans plusieurs pays d’Afrique et de Moyen Orient. Le but de notre travail était de décrire les caractéristiques cliniques, paracliniques et évolutives de nos patients et de les comparer avec les données de la littérature. Il s’agissait d’une étude rétrospective portant sur les cas admis en réanimation (2003-2010). Les critères d’inclusion étaient d’ordre clinique, paraclinique, thérapeutique et évolutif. Durant la période de l’étude 24 patients ont été inclus provenant de la région de Fés-Boulmane. L’intoxication à la (PPD) représentait 26% de l’ensemble des intoxications admises au cours de la même période. L age moyen était de 23,6 plus or minus 11 ,6 ans. Il existait une prédominance féminine avec un sex-ratio de 4,7. L’intoxication était volontaire dans 82,6 %, accidentelle dans 8,6 %, et criminelle dans 4,3%. Le syndrome de rhabdomyolyse caractéristique de cette intoxication était retrouvé chez 60% de nos patients, l’atteinte respiratoire chez 56,5%, l’atteinte cardiaque était présente dans 30% des cas et 17,4% des patients avaient présentés une insuffisance rénale. La prise en charge thérapeutique se basait sur l’apport volémique massif, alcalinisation des urines ainsi que l’administration de cortico des et de diurétiques. Le recours à une trachéotomie de sauvetage était nécessaire chez 7 patients. Trois de nos patients avaient bénéficié d’une épuration extra rénale. L’évolution était fatale chez 47,8% des cas. La PPD représente ainsi la principale cause de mortalité toxique dans notre contexte. L’intoxication à la PPD, représente la première cause de rhabdomyolyse toxique dans notre contexte. Elle est responsable d’une mortalité très élevée. Ce qui implique une réglementation du commerce de la PPD, une information du corps soignant et une prise en charge précoce
Safe Bounds in Semidefinite Programming by Using Interval Arithmetic  [PDF]

Abstract: Efficient solvers for optimization problems are based on linear and semidefinite relaxations that use floating point arithmetic. However, due to the rounding errors, relaxation thus may overestimate, or worst, underestimate the very global optima. The purpose of this article is to introduce an efficient and safe procedure to rigorously bound the global optima of semidefinite program. This work shows how, using interval arithmetic, rigorous error bounds for the optimal value can be computed by carefully post processing the output of a semidefinite programming solver. A lower bound is computed on a semidefinite relaxation of the constraint system and the objective function. Numerical results are presented using the SDPA (SemiDefinite Programming Algorithm), solver to compute the solution of semidefinite programs. This rigorous bound is injected in a branch and bound algorithm to solve the optimisation problem.
On the Resonant Spin Flavor Precession of the Neutrino in the Sun

Abstract: This work deals with the possible solution of the solar neutrino problem in the framework of the resonant neutrino spin-flavor precession scenario. The event rate results from the solar neutrino experiments as well as the recoil electron energy spectrum from SuperKamiokande are used to constrain the free parameters of the neutrino in this model. We consider two kinds of magnetic profiles inside the sun. For both cases, a static and a twisting field are discussed.
Magnetic resonance imaging tracking of human adipose derived stromal cells within three-dimensional scaffolds for bone tissue engineering

Abstract: For bone tissue engineering, human Adipose Derived Stem Cells (hADSCs) are proposed to be associated with a scaffold for promoting bone regeneration. After implantation, cellularised scaffolds require a non-invasive method for monitoring their fate in vivo. The purpose of this study was to use Magnetic Resonance Imaging (MRI)-based tracking of these cells, labelled with magnetic agents for in vivo longitudinal assessment. hADSCs were isolated from adipose tissue and labelled with USPIO-rhodamine (Ultrasmall SuperParamagnetic Iron Oxide). USPIO internalisation, absence of toxicity towards hADSCs, and osteogenic differentiation of the labelled cells were evaluated in standard culture conditions. Labelled cells were then seeded within a 3D porous polysaccharide-based scaffold and imaged in vitro using fluorescence microscopy and MRI. Cellularised scaffolds were implanted subcutaneously in nude mice and MRI analyses were performed from 1 to 28 d after implantation. In vitro, no effect of USPIO labelling on cell viability and osteogenic differentiation was found. USPIO were efficiently internalised by hADSCs and generated a high T2* contrast. In vivo MRI revealed that hADSCs remain detectable until 28 d after implantation and could migrate from the scaffold and colonise the area around it. These data suggested that this scaffold might behave as a cell carrier capable of both holding a cell fraction and delivering cells to the site of implantation. In addition, the present findings evidenced that MRI is a reliable technique to validate cell-seeding procedures in 3D porous scaffolds, and to assess the fate of hADSCs transplanted in vivo.

Abstract: We report the structural, optical and electrical properties of Graphene-Vanadium oxide nanoparticles (rGO/VO-NPs) nanocomposites prepared via a hydrothermal method on glass substrates. The samples have been characterized by X-ray diffraction (XRD), transmission electron microscopy (TEM), Fourier transform infrared spectroscopy (ATR-FTIR), Raman spectroscopy, ultraviolet-visible spectra (Uv-Vis) (absorbance/reflectance) and electrical conductivity. Our results are revealing a remarkable effect on the morphology and structure of vanadium oxide nanoparticles. Hence, the graphene layers improved their electrical conductivity and highly influenced their optical properties. Therefore, the obtained results may lead to better performance for a large field of applications.
