Quantitative ColourDopplerSonography Evaluation of Cerebral Venous Outflow: A Comparative Study between Patients with Multiple Sclerosis and Controls

Abstract: Background Internal Jugular Veins (IJVs) are the principle outflow pathway for intracranial blood in clinostatism condition. In the seated position, IJVs collapse, while Vertebral Veins (VVs) increase the venous outflow and partially compensate the venous drainage. Spinal Epidural Veins are an additional drainage pathway in the seated position. Colour- Doppler-Sonography (CDS) examination is able to demonstrate IJVs and VVs outflow in different postural and respiratory conditions. The purpose of this study was to evaluate CDS quantification of the cerebral venous outflow (CVF) in healthy subjects and patients with multiple sclerosis (MS). Methodology/Principal Findings In a group of 27 healthy adults (13 females and 14 males; mean age 37.8±11.2 years), and 52 patients with MS (32 females and 20 males; mean age 42.6±12.1 years), CVF has been measured in clinostatism and in the seated position as the sum of the flow in IJVs and VVs. The difference between CVF in clinostatism and CVF in the seated position (ΔCVF) has been correlated with patients' status (healthy or MS), and a number of clinical variables in MS patients. Statistical analysis was performed by Fisher's exact test, non-parametric Mann-Whitney U test, ANOVA Kruskal-Wallis test, and correntropy coefficient. The value of ΔCVF was negative in 59.6% of patients with MS and positive in 96.3% of healthy subjects. Negative ΔCVF values were significantly associated with MS (p<0.0001). There was no significant correlation with clinical variables. Conclusions/Significance Negative ΔCVF has a hemodynamic significance, since it reflects an increased venous return in the seated position. This seems to be a pathologic condition. In MS patients, a vascular dysregulation resulting from involvement of the autonomous nervous system may be supposed. ΔCVF value should be included in the quantitative CDS evaluation of the cerebral venous drainage, in order to identify cerebral venous return abnormalities.

Abstract: Objective: This review focuses on syncope in diabetic patients who suffer from hypoglycemia. Clinically, transient loss of consciousness during hypoglycemia appears similar to vasovagal syncope. Research Design and Methods: Current understanding of this problem is based on physicians’ personal experiences as well as on published case reports. It is difficult to explain a temporary loss of consciousness as a result of hypoglycemia. Demonstration that hypoglycemia can be transient, with the patient suffering from neuroglycopenia without autonomic symptoms due to delayed counterregulation, might be a first step in confirming that a diabetic patient suffered from a transient loss of consciousness with spontaneous recovery. Results: Hypoglycemic syncope is uncommon, affecting 1.9% of diabetic patients using insulin therapy. It is characterized clinically by brief periods of unconsciousness with slow recovery and without loss of postural muscle tone. The difficulty in correlating loss of consciousness to hypoglycemia arises from mismatching symptoms, that is, there may be mental symptoms such as confusion, loss of memory or consciousness, in the absence of autonomic manifestations such as sweating or blurred vision. There are currently no established glucose values that define the level of hypoglycemia that causes loss of consciousness. Conclusion: Hypoglycemic syncope should be suspected in older diabetic patients with preserved postural tone, usually but not always using insulin therapy, who show a slow recovery from transient loss of consciousness with persisting neurological impairment and low blood glucose levels.

Abstract: This review examines the pathogenesis of Carotid Sinus Hypersensitivity (CSH) by describing the different hypotheses that exist in literature. One hypothesis is that CSH is not a disease of the heart but of the central and/or peripheral nerv- ous system where the heart is the victim. CSH is associated with cognitive disturbance, vasovagal syncope and ortho- static hypotension. Another hypothesis is that the CSH is considered a syndrome or a symptom associated with different diseases so as to require a diagnostic framework. This Hypothesis is the basis of frequent recurrences that are associated with the pacemaker implant therapy only.
Effect of Tantalum Doping on TiO2 Nanotube Arrays for Water-Splitting  [PDF]

Abstract: This work is intended to define a new possible methodology for TiO2 doping through the use of electrochemical deposition of tantalum directly on the titanium nanotubes obtained by a previous galvanostatic anodization treatment in an ethylene glycol solution. This method does not seem to cause any influence on the nanotube structure, showing final products with news and interesting features with respect to the unmodified sample. Together with a decrease in the band gap and flat band potential of the TiO2 nanotubes, the tantalum doped specimen reports an increase of the photo conversion efficiency under UV light.
Efficiency of Xist-mediated silencing on autosomes is linked to chromosomal domain organisation

Abstract: Our results show that the organisation of the chromosome into large gene-rich and L1-rich domains is a key determinant of silencing efficiency. Specifically genes located in large gene-rich domains with low L1 density are relatively resistant to Xist-mediated silencing whereas genes located in gene-poor domains with high L1 density are silenced more efficiently. These effects are observed shortly after induction of Xist RNA expression, suggesting that chromosomal domain organisation influences establishment rather than long-term maintenance of silencing. The X chromosome and some autosomes have only small gene-rich L1-depleted domains and we suggest that this could confer the capacity for relatively efficient chromosome-wide silencing.This study provides insight into the requirements for efficient Xist mediated silencing and specifically identifies organisation of the chromosome into gene-rich L1-depleted and gene-poor L1-dense domains as a major influence on the ability of Xist-mediated silencing to be propagated in a continuous manner in cis.Classical studies on X; autosome rearrangements have demonstrated that X inactivation propagates in cis from a single locus on the X chromosome, the X inactivation centre (Xic), and additionally highlighted that autosomal genes in cis with the Xic are inactivated less efficiently than normal X-linked genes [1-4]. This latter observation was suggested to be due to inefficient propagation or maintenance of X inactivation. More recently it has been shown that chromosome coating by X inactive specific transcript (Xist) RNA is the primary cis-acting trigger for X inactivation [5-7], and moreover that expression of Xist transgenes randomly integrated on mouse autosomes leads to coating and chromosome-wide silencing [8,9]. Abstract: Nowadays the utilization of vegetable oils, particularly the palm oil, as fuel in the power plant, has made a significant development in the last few years. This vegetable oil can be used in particular in slow marine-derived diesel internal combustion engine (ICE), with an electric efficiency of about 40%. The efficiency is connected to the size of the power plant. Moreover, the considerable amount of the required vegetable oil to feed the system forces, especially in Italy, makes it necessary to import the fuel. This is the most critical element as the palm oil is subject to continuous variations in prices. Due to this variation it is difficult to stabilize a convenient fuel supply for a long period. The paper aims to evaluate and estimate the economic, technical and environmental feasibility of a 20 MW plant for the stationary power generation fed with palm oil, enlightening the system solution (technical constructive aspects) and the economic appraisal, on the basis of variations in oil prices. A comparison with a cogenerative power plant has been also carried out and, at last, the economic sensibility analysis based on the fuel cost and the European mechanisms of biomass incentives is performed.
Relativity Current Paradigm with Unresolved Anomalies  [PDF]

When a theory, as the general relativity, linked to special relativity, is foundation of a scientific paradigm, through normal science and academy, scientifics, professionals, professors, students and journals of that scientific community, the paradigm, it self-sustains and reproduces. Thus, the research is obligated and limited to apply the model existent of the paradigm to formulate problems and solve them, without searching new discoveries. This self-protection of the paradigm causes it to end its cycle of life, only after a long time, until that arise unresolved anomalies, some presents since origin, that they finally cause its change by other new paradigm. In this work, we study the most important anomalies that are part of the foundations of general relativity with the goal of promoting the call period of transition that is previous to scientific revolution. We use critical analysis method for rereading the general relativity, from the perspective of the history of science and philosophy of science. We find, the structural and complex anomaly of general relativity based on metaphysical spacetime that produces the metaphysics replaces physics. Also, two internal anomalies their direct consequences. These are: matter curves metaphysical spacetime, and metaphysical spacetime determines the geodesic motion of physical matter. We conclude that general relativity has no valid physical concept of spacetime, therefore of gravity. For these reasons, a new paradigm is needed.

Abstract: Energetic complementarity has been studied in recent years and can be an important tool for managers to decide on the design and operation of hybrid systems based on renewable energy resources. Complementarity is an ability presented by two or more energy resources to complement each other over time. Complementarity can be verified in one place or at different places. This second case can be termed as spatial complementarity and is more complex than the complementarity in the same place, requiring a specific approach for its quantification This paper discusses concepts related to energetic complementarity and presents the basis for a method to evaluate energetic time-complementarity across space, applying the concepts presented to the northern coast of the state of Rio Grande do Sul, the southernmost state of Brazil.
