A classifica??o das interven??es de Enfermagem na prática clínica de enfermeiros brasileiros

Abstract: objective: to identify evidence related to the use of the nursing interventions classification in clinical practice, as identified in nursing research within brazil. methods: integrative review of literature was conducted between march and april 2009, using electronic databases. the sample included research demonstrating the use / research of nic, conducted by brazilian nurses, published in portuguese, english or spanish, in national or international journals, for all available publication years. results: ten articles were selected which met level 5 evidence, allowing the categorization of four lines of study: association with other classifications, application to specific diagnoses, use in specific clinical situations, and validation protocol / guiding nursing care. conclusion: the difficulty in the managing nic, lack of knowledge and professional autonomy were the main findings. there are knowledge gaps, lack of validation studies on interventions, and a lack of analysis of results from the use of nic in clinical practice
Systematization of nursing care to the patient with abdominal compartment syndrome: integrative review study

Abstract: Objetives: to raise in the literature the main human responses in the abdominal compartment syndrome and building a plan of nursing care using the NANDA International Diagnosis Classification, and the Nursing Interventions Classification and the Nursing Outcomes Classification. Methods: it’s an integrative review, descriptive, with analysis publications available in the databases LILACS, MEDLINE, Cochrane Library, SciELO and BDENF held from April to June 2008. Were included full article, published in Portuguese, English or Spanish, in national journal, regardless of the period of realization, whose description has shown potential for identification of human responses in the syndrome, was selected seven articles from these criteria. Results: were raised 17 human responses, which subsidized the construction of a plan of care with six diagnoses, one result and a nursing intervention with two activities for each. Conclusion: in an environment full of tension and challenges, where the critical patient requires attention and actions of emergency care, the systematized care provide the nurse of effective tools to identify the signs and symptoms arising from the abdominal compartment syndrome, promoting the reduction of mortality rates related to this complication.
O idoso doador de sangue e o cuidado da enfermagem hemoterápica

Abstract: O estudo destaca o cuidado da enfermagem hemoterápica aos idosos candidatos à doa o de sangue, que é um ato responsável e representativo de autonomia, maturidade, e participa o social a ser cultivado por toda a vida. Especial aten o e incentivo deve ser oferecido ao idoso, hígido, cidad o, que pode doar sangue, em uma popula o que galga a longevidade de forma saudável. O objetivo da reflex o proposta neste estudo é discutir sobre o cuidado da enfermagem hemoterápica ao idoso doador de sangue. As enfermeiras que realizam a Triagem Clínica em Hemoterapia est o envolvidas no cuidado de enfermagem aos doadores de sangue, em uma área de saúde especializada, e este cuidado envolve aspectos culturais, sociais, econ micos, de cidadania, da fisiopatologia, da qualidade de vida e do processo saúde–doen a, devendo atender às diretrizes do Sistema único de Saúde, do Ministério da Saúde, e estar articulado às Necessidades de Saúde dos candidatos a doadores de sangue idosos em seu desenvolvimento e envelhecimento.

Abstract: RESUMO A importancia das micro, pequenas e médias empresas brasileiras (MPME) para o desenvolvimento econ mico e gera o de empregos n o tem encontrado paralelo no que tange à produ o de teorias no campo da estratégia, consideradas as limita es da capacidade de a o das MPMEs e das idiossincrasias de seus líderes. Este artigo busca contribuir para a compreens o dos processos de forma o das estratégias nessas empresas segundo a natureza do pensamento estratégico dos líderes-estrategistas e com base no comportamento declarado, adotando como referência a taxonomia das estratégias genéricas e dos processos cognitivos de tomada de decis o e pensamento estratégico. Tem como objetivo propor uma tipologia que permita compreender a natureza do pensamento estratégico desses gestores, identificando suas principais dimens es características e propondo instrumento que permita sua categoriza o e compara o. A aplica o de survey com amostra intencional formada por 70 estrategistas permitiu o uso de análise fatorial para constru o das categorias e escala, e a observa o que, no universo de estrategistas pesquisados no que tange a sua forma de pensar, a intui o é perspectiva complementar, n o predominante, o caráter coletivo é acentuado na forma o das estratégias, bem como a valoriza o do formalismo e a cren a na importancia de planejar e registrar as inten es. ABSTRACT The importance of micro, small and medium size companies (MSMC) to the economical development and job creation in the country hasn’t found parallel with respect to the production of theories in the strategy field, even considering the limited capacity of action of the MSMCs and the idiosyncrasies of their leaders. This article seeks to contribute to the understanding of the strategy formation processes in these companies by the nature of the strategic thinking of their strategists, having as reference the taxonomy of generic strategies and the cognitive processes of decision making and strategic thinking. It aims at proposing a typology to comprehend the nature of strategic thinking of these managers, identifying their main characteristics dimensions and proposing an instrument that allows its categorization and comparison. Winery Wastewater Treatment Applying Aerated Submerged

Abstract: The winery wastewater usually shows conditions of low pH, high organic loads and concentrations of carbon, nitrogen and phosphorus that are inappropriate for biologic treatment. The purpose of this research was to apply the technology of aerated submerged biofilter (ASB) for the winery effluent treatment during the harvest (ASB 1) and non harvest (ASB 2) at lab scale. Therefore, two up flow biofilter built on glass (5 liters volume) were installed. The nutrient balance of the winery wastewater was adjusted and the correction of the pH was done by oyster shell used as filter material. The efficiency removal (COD) for the harvest reactor was 90% while for the non harvest was 82%. The oyster shells contributed to an increase on average of 180 mg/L of alkalinity to the BAS 1 and 318 mg/L for the BAS 2. As regards the metals, the average values in the treated effluent to meet iron and zinc is permitted by the environmental standards of Santa Catarina. Under the experimental conditions applied in this research, this kind of reactor has presented potential for the treatment of winery wastewater. However, operational improvements would be required in the reactors to adequate them to the specific management into the wineries.
Epworth's sleepiness scale in outpatients with different values of arterial blood pressure

Abstract: objective: to compare sleepiness scores of the epworth scale in patients with different levels of arterial pressure when undergoing outpatient monitoring within the context of clinical evaluation. methods: a total of 157 patients selected for outpatient monitoring of arterial pressure during hypertension evaluation were divided into 3 groups: group 1 - normotensive; group 2 - hypertensive; group 3 - resistant hypertensive. for analysis, values 311 were considered as associated with respiratory disturbances during sleep. results: seventeen (10.8%) patients in group 1, 112 (71.3%) in group 2, and 28 (17.8%) in group 3, which was composed of aged, more severely hypertensive individuals, were analyzed. groups were similar relative to sex and body mass index, but different in relation to systolic and diastolic pressure levels and age. despite an absolute difference, no statistically significant difference occurred between epworth scores and in the proportion of patients with values 311 (5.9% vs. 18.8% vs. 212.4%; p=0.37). despite the positive association between degree of sleepiness measured with the scale and the severity of the hypertension, no statistical significance occurred following control by age (p=0.18). conclusion: a positive correlation exists between degree of sleepiness and hypertension severity. the absence of a statistical significance shown in the present study could be due to a beta type of error. instruments that render this complaint into an objective finding could help in the pursuit of an investigation of respiratory disturbances during sleep in more severely hypertensive patients, and should therefore be studied better.
PCR - based diagnosis to evaluate the performance of malaria reference centers

Abstract: although the giemsa-stained thick blood smear (gts) remains the gold standard for the diagnosis of malaria, molecular methods are more sensitive and specific to detect parasites and can be used at reference centers to evaluate the performance of microscopy. the description of the plasmodium falciparum, p. vivax, p. malariae and p. ovale ssrrna gene sequences allowed the development of a polymerase chain reaction (pcr) that had been used to differentiate the four species. the objective of this study was to determine plasmodium species through pcr in 190 positive smears from patients in order to verify the quality of diagnosis at sucen's malaria laboratory. considering only the 131 positive results in both techniques, gts detected 4.6% of mixed and 3.1% of p. malariae infections whereas pcr identified 19.1% and 13.8%, respectively.
Síndrome da morte súbita do lactente: aspectos clínicos de uma doen?a subdiagnosticada

Abstract: objectives: the aim of this study was to establish the socioepidemiologic characteristics of the victims of sudden infant death syndrome and to calculate its prevalence during the first year of life. methods: observational, retrospective, descriptive study based on the review of autopsy protocols and questionnaires applied to families that had cases of infants death during 1997 and 1998 in porto alegre. results: from 335 infants that died, 21 were included in sids criteria although the death certificate did not register this cause. the infants had no previous apnea or any acute disease that could justify the decease. death occurred predominantly in cold months (winter and autumn) and none of the infants were usually put to sleep in supine position. families had low income (3 minimum salaries / month). 71% of the mothers were smokers and 42% used alcohol during pregnancy. sids prevalence was estimated as 6.3% from the total cases of decease, and the specific mortality coefficient for sids was 4.5:10,000 live births. conclusions: the infant’s risk profile identified resembles very much that of the international literature. however as sids is misdiagnosed in our community, it has not been included in the official rates of infant mortality. as a consequence, no risk reducing campaigns have ever been established.
Clinical presentation and survival of smear-positive pulmonary tuberculosis patients of a University General Hospital in a developing country

Abstract: from january 1995 to august 1997 we evaluated prospectively the clinical presentation, laboratory findings and short-term survival of smear-positive pulmonary tuberculosis (tb) patients who sought care at our hospital. after providing informed, written consent, the patients were interviewed and laboratory tests were performed. information about survivorship and death was collected through september 1998. eighty-six smear-positive pulmonary tb patients were enrolled; 26.7% were hiv-seropositive. seventeen hiv-seronegative pulmonary tb patients (19.8%) presented chronic diseases in addition to tb. in the multiple logistic regression analysis a cd4+ cell count £ 200 cell/mm3 was independently associated with hiv seropositivity. in the cox regression model, fitted to all patients, hiv seropositivity and age 3 50 years were independently associated with decreased survival. among hiv-seronegative persons, the presence of an additional disease increased the risk of death of almost six-fold. use of antiretroviral drugs was associated with a lower risk of death among hiv-seropositive smear-positive pulmonary tb patients (rh = 0.32, 95% ci 0.10-0.92). in our study smear-positive pulmonary tb patients had a low short-term survival rate that was strongly associated with hiv infection, age and co-morbidities. therapy with antiretroviral drugs reduced the short-term risk of death among hiv-seropositive patients after tb diagnosis.
Raquianestesia para cesariana: estudo comparativo entre bupivacaína isobárica e hiperbárica associadas à morfina

Abstract: background and objectives: bupivacaine preparations, plain or with glucose, are frequently used in the clinical practice. blockade upper level is determined by local anesthetic spread in the csf. this study aimed at comparing isobaric and hyperbaric bupivacaine in patients submitted to spinal anesthesia for cesarean section. methods: in this prospective, randomized and double-blind study 60 patients submitted to spinal anesthesia for cesarean section were distributed in two groups: ib - (0.5% isobaric bupivacaine, 12.5 mg) and hb - (0.5% hyperbaric bupivacaine, 12.5 mg). after monitoring, venous puncture was performed followed by hydration with lactated ringer?s solution. spinal puncture was paramedially performed at l3-l4 interspace with 27g quincke needle. following the csf dripping, morphine (100 μg) and bupivacaine were separately injected at the speed of 1 ml.15 s-1. with the patient back to supine position, two parameters were recorded: onset time (absence of sensitivity in l3) at 1-minute intervals as well as motor and sensory block after 20 minutes. all patients were kept with preanesthetic blood pressure levels until umbilical cord clamping, and if necessary, ephedrine was administered. neonates were evaluated by apgar?s score at 1 and 5 minutes. sensory and motor blocks were also evaluated at pacu 120 minutes after local anesthetic injection. results: groups were homogeneous. onset time: group ib (1?, 50") and hb (1?,33"), with no statistical difference. motor and sensory block at twenty minutes showed no significant difference. ephedrine consumption: ib (11.83 mg) and hb (14.17 mg), showed also no statistical difference. pacu motor block evaluation showed significant differences. conclusions: we concluded that 12.5 mg isobaric and hyperbaric bupivacaine associated to morphine (100 μg) in spinal anesthesia for cesarean section in term pregnant women are effective and present similar profiles.
