Uso de los análogos de la insulina durante el embarazo

Abstract: Los análogos de la insulina se han usado en el tratamiento de la diabetes asociada al embarazo, aún sin la aprobación de instancias reguladoras. Con el fin de conocer su eficacia y seguridad materno-fetal, se realizó la revisión bibliográfica sobre el tema. En cuanto a la Glulisina, no hay reporte de su uso en la gestación. Con Lispro, hay pocos estudios aleatorios donde se compare su uso con insulina humana regular, pero hay variados estudios observacionales en su mayoría retrospectivos, que no revelan aumento de riesgo. Referente a Aspart, se ha investigado en mujeres con diabetes tipo 1 durante el embarazo mediante un ensayo clínico aleatorio controlado, donde se compara con insulina humana regular, cuyos resultados sirvieron de base para la aprobación de su uso en gestantes. Con Glargina, se han llevado a cabo estudios observacionales, pero no aleatorios controlados y con Detemir, se finalizó el a o 2011 un estudio aleatorio controlado, comparando con insulina humana NPH, cuyos resultados sustentaron la reciente aprobación por la Federal Drug Administration para su uso en embarazadas. En conclusión, Lispro y Aspart constituyen las opciones de análogos de acción rápida a usar durante el embarazo cuando no sea posible controlar las elevaciones postprandiales de la glucemia o prevenir hipoglucemias nocturnas y severas con insulina humana. El análogo de acción larga Detemir podrá ser usado durante el embarazo, una vez aprobado por los organismos nacionales correspondientes. Se requieren estudios aleatorios controlados en embarazadas para Glargina y de costo-efectividad para todos los análogos de la insulina. Insulin analogues have been used in the treatment of diabetes in pregnancy and some of them off-label. To know the efficacy and safety of insulin analogues concerning mother and fetus, a literature review on the subject was carried out. Regarding Glulisine, there are no reports on its use on gestation. Abstract: objective: to know the frequency of diabetes and other cardiovascular risk factors in women with previous gestational diabetes mellitus (gdm). methods: 92 women with previous gdm, without diagnosis of diabetes, who underwent initial or annual postpartum diabetes screening during 2008, were assessed. blood pressure, abdominal circumference and body mass index were measured. they were tested for an oral glucose tolerance test (ogtt) and determination of plasma insulin and lipid levels. metabolic syndrome (ms) was diagnosed according atp iii criteria and the homa-r model was used for the insulin resistance index calculation. student?s t test was used for variables association. results: the average age was 34.7 ± 6.6 years and postpartum period, 3.5 ± 2.6 years. 8.7% was diagnosed with diabetes, 32.6 % with impaired glucose tolerance and 12.0 % with impaired fasting glucose. 62.1 % showed abdominal obesity, 62.5 % presented some type of dyslipidemia and 23.8 %, high blood pressure. 46.4 % met the criteria for ms, and 29.6 % showed a homa-r index > 2.5. abdominal circumference, body mass index, blood pressure, triglycerides, basal glucose, 2-h post-load glucose and 2-h post-load insulin values were higher (p< 0.05) in women with abnormal glucose tolerance than those with normal glucose tolerance. conclusions: women with previous gdm show a high frequency of clinical and metabolic abnormalities that point towards a potential increase of cardiovascular risk in a relatively young population.
Alteraciones clínico-metabólicas en mujeres con diabetes gestacional previa

Abstract: Objetivo: Conocer la frecuencia de diabetes y otros factores de riesgo cardiovascular en mujeres con diabetes mellitus gestacional (DMG) previa. Métodos: Se evaluaron 92 mujeres con DMG previa, a quienes no se les había diagnosticado diabetes, que asistieron a la pesquisa posparto inicial o anual durante 2008. Se determinó presión arterial, circunferencia abdominal (CA) e índice de masa corporal (IMC). Se practicó prueba de tolerancia oral a la glucosa, medición de niveles de insulina y lípidos séricos. Se estableció la presencia de síndrome metabólico (SM), aplicando los criterios ATP III e índice de resistencia a la insulina mediante el modelo Homa-R. Se usó t Student para la asociación de variables. Resultados: La media de edad fue 34,7 ± 6,6 a os y del período posparto, 3,5 ± 2,6 a os. En 8,7 % se hizo el diagnóstico de diabetes, 32,6 % intolerancia a la glucosa y 12,0 %, glucemia alterada en ayunas. 62,1 % presentaba obesidad abdominal, 62,5 %, algún tipo de dislipidemia y 23,80 %, elevación de la presión arterial. 46,4 % reunía los criterios de SM y 29,6 % mostró un índice HOMA-R > 2,5. Las cifras de CA, IMC, triglicéridos, glucemia basal, glucemia 2 horas post-carga, e insulina 2 horas poscarga fueron más altas (P< 0,05) en mujeres con anormalidades de la tolerancia a la glucosa que en aquellas con tolerancia normal. Conclusiones: Las mujeres con DMG previa muestran una elevada frecuencia de alteraciones clínicas y metabólicas que representan un potencial incremento del riesgo cardiovascular en un grupo poblacional relativamente joven. Abstract: the research intended to evaluate the effectiveness of an educational program for self care of mellitus diabetes in pregnant diabetics. at the same time, existing relationships among these variables and the results of educational program were found: clinic characteristics, social and economical conditions, and educational level of the patients. the design was quasi-experimental (with only a group of pre and posttest) correlational, longitudinal and prospective. a descriptive analysis was used; and also the statistic "t student" for analysis of the differences of independent and paired group averages. sixty-four patients were under study for two years. gestational diabetes was found in 46.9% of them, 61.6% belonged to the social levels iv and v, and 31.3% had an incomplete elementary highschool-level education. the findings show the effectiveness of the educational program for the self care of mellitus diabetes applied to the patients of the diabetes and pregnancy unit of the ciudad hospitalaria dr. enrique tejera, valencia-venezuela.

Abstract: La investigación objetiva a comparar la educación que poseen las diabéticas embarazadas antes y después de recibir el Programa Educativo para el Autocuidado de la Diabetes Mellitus en sus factores: conocimientos teóricos, capacitación práctica, y actitudes frente a la Diabetes. Asimismo se determinó la relación existente entre las variables: características clínicas, socioeconómicas, y nivel de instrucción de las embarazadas con los resultados de la medición de los conocimientos de dicho Programa. El dise o fue de tipo cuasi-experimental (con un solo grupo de pre y post test), correlacional y prospectiva. Se utilizó el análisis descriptivo y el estadístico t de student. Se estudiaron 64 pacientes en dos a os, donde 46.9% tenían Diabetes Gestacional; el 61.6% pertenecían a los estratos sociales IV y V y 33.1% tenían primaria completa o secundaria incompleta. Los hallazgos se alan la efectividad del Programa Educativo aplicado a las pacientes de la Unidad de Diabetes y Embarazo - Hospital Dr. Enrique Tejera de Valencia-Venezuela
Efectos de la educación para el autocuidado en el control metabólico de pacientes diabéticas embarazadas

Abstract: Unit - Hospital City "Dr. Enrique Tejera", Valencia - Carabobo State. The purpose of the investigation wasto determine the effects of Self-care Education on the following factors: theoretical knowledge, practicaltraining and attitude toward the illness and metabolic control of pregnant diabetic patients. This is a quasiexperimentalfield investigation. Descriptive analysis, arithmetic mean and standard deviation were used,as well as Student’s t. 40 pregnant diabetic patients were studied. 67.5% had Gestational Diabetes.Results showed statistically significant differences (P <0.001) in theoretical knowledge, practical trainingand attitude for self-care and metabolic control of pregnant diabetic patients after receiving education.Before receiving education, the average score was 18.1% and after applying the educational program, theaverage score rose to 62.5%. In general, for the variable under study: Effects of self-care education onmetabolic control, the percentage obtained before receiving education was 31.9% and after havingapplied the program it was 108.8%. From these results it can be concluded that the application of theeducational program was highly effective on the metabolic control of pregnant diabetic patients.Key words: INTRODUCCIóN

Abstract: El fin de la llamada cultura moderna coincide en el siglo XX con el surgimiento del nuevo “espacio público” configurado desde los “medios de comunicación”. El ecosistema mediático se convierte en lugar privilegiado donde detectar las se ales de una situación de cambio social. La situación invita a revisar la función que corresponde a la comunicación pública equilibrando su carácter de mediación y su carácter activo; así cómo a se alar las claves para interpretar su función representativa, en un momento en que las tendencias sociales indican propuestas para un cambio de época.The end of the modern culture coincides in the 20th century with the emergence of a new public space formed from the mass media. The media ecosystem turns into privileged place where to detect the indicators of the social change. The situation invites to check the function that corresponds to the public communication. It is necessary to reflect about its character of mediation and its active character. This way could to indicate the keys to interpret the representative function, in a moment in which the social trends indicate offers for a change of epoch.

Abstract: canned tuna is one of the most consumed products in the country because of its easy acquisition, preparation and excellent nutritional properties. the objective of this research was to determine proximate composition and histamine presence in three commercial brands of canned tuna preserved in water and oil, expended in maracaibo city, zulia state, venezuela. 30 cans of each brand were collected, 15 in oil and 15 in water. sampling was performed five times, in 15 days intervals. sample acquisition was made at supermarkets. in moisture, protein and ashes determination were used official methods of analysis reported by association of official analytical chemist (aoac), and for fat analysis bligh and dyer methodology was used. histamine presence was measured by means of high performance liquid chromatography (hplc).differences in proximate composition depending on commercial brand and presentation. moisture values for tuna canned in water were found between 71.98-75.59%, protein between 21.26-25.22%, fat 1.62-2.66% and ashes 1.48-1.57%. values for tuna canned in oil were 66.43-67.12% for humidity, 23.44-25.84% for protein, 6.45-8.68% for fat and 1.11-1.46% for ash content. all analyzed canned tuna samples presented histamine, in some cases histamine concentrations were close to limit established by food and drug administration (fda) of 50 ppm, because of this constant monitoring in canned tuna produced in venezuela is suggested.

Abstract: violence is still on the increase even though the world is no longer divided. with the fall of the berlin wall most of us were expecting the end of wars, something that never happened. on the other hand, internal conflicts in the third world have become more and more frequent. today, homicide is the first cause of death in colombia, being men aged from 15 to 44 the principal victims. violence is the major public health problem in colombia nowadays. it is impossible to understand this issue from a unique point of view. to address this problem, as it occurs with bioethics, inter-disciplinary actions must be taken. it is a phenomenon related to distributive justice and the autonomy of men and nation's, in a context where the intention to inflict harm (maleficence) prevails. there exist, therefore, an enormous domain for bioethics, especially for those of us who deem necessary to remove it from its clinical atmosphere in order to collaborate in the searching for solutions to the urgent problems this side of the world is facing
Semi-classical Scar functions in phase space

Abstract: We develop a semi-classical approximation for the scar function in the Weyl-Wigner representation in the neighborhood of a classically unstable periodic orbit of chaotic two dimensional systems. The prediction of hyperbolic fringes, asymptotic to the stable and unstable manifolds, is verified computationally for a (linear) cat map, after the theory is adapted to a discrete phase space appropriate to a quantized torus. Characteristic fringe patterns can be distinguished even for quasi-energies where the fixed point is not Bohr-quantized. Also the patterns are highly localized in the neighborhood of the periodic orbit and along its stable and unstable manifolds without any long distance patterns that appears for the case of the spectral Wigner function.
